Spectrum of liver diseases occurring solely in pregnancies carrying male fetuses: A case series

Liver disease affects nearly 3% of pregnancies. It is a challenging topic for obstetricians as it can be fatal for both, the mother and the fetus. They carry a high mortality rate reaching 25%. Sex-specific effect on the expression of pregnancy-related liver diseases is largely unknown. We present several cases of familial pregnancy-related liver diseases that were expressed solely when the mothers had male fetuses, ending with deleterious outcomes. Liver disease was not expressed when they had female fetuses. These cases provide a novel insight into the diversity of pregnancy-related liver disease complications expression among sex.

Summary Objective: In liver diseases, hyperferritinemia (HYF) is related to injured cells in acquired and genetic conditions with or without iron overload. It is frequent in patients with nonalcoholic fatty liver disease (NAFLD), in which it is necessary to define the mean of HYF to establish the better approach for them. The present study evaluated the significance of elevated ferritin in patients with NAFLD and steatohepatitis (NASH). Method: The review was performed using search instruments of indexed scientific material, including MEDLINE (by PubMed), Web of Science, IBECS and LILACS, to identify articles published in Portuguese, English and Spanish, from 2005 to May, 2016. Studies eligible included place and year of publication, diagnose criteria to NAFLD, specifications of serum ferritin measurements and/or liver histopathologic study. Exclusion criteria included studies with patients with alcohol consumption ≥ 20 g/day and other liver diseases. Results: A total of 11 from 30 articles were selected. It included 3,564 patients and they were cross-sectional, retrospective, case series and case-control. The result's analyses showed in 10 of these studies a relationship between ferritin elevated serum levels and NAFLD/NASH with and without fibrosis and insulin resistance. Conclusion: Hyperferritinemia in patients with NAFLD/NASH is associated more frequently with hepatocellular injury than hemochromatosis. These data suggest the relevance to evaluate carefully HYF in patients with NAFLD/NASH to establish appropriate clinical approach.

Alcohol-related liver disease (ALD) is caused by over-consumption of alcohol. ALD can develop a spectrum of pathological changes in the liver, including steatosis, inflammation, cirrhosis, and complications. Autophagy is critical to maintain liver homeostasis, but dysfunction of autophagy has been observed in ALD. Generally, autophagy is considered to protect the liver from alcohol-induced injury and steatosis. In this review, we will summarize novel modulators of autophagy in hepatic metabolism and ALD, including autophagy-mediating non-coding RNAs (ncRNAs), and crosstalk of autophagy machinery and nuclear factors. We will also discuss novel functions of autophagy in hepatocytes and non-parenchymal hepatic cells during the pathogenesis of ALD and other liver diseases.

<p class="abstract">Rhino-orbital-cerebral mucormycosis is an invasive disease associated with high mortality ranging from 25-62%. There is an increase in the incidence of mucormycosis in post COVID-19 infection patients. We have come across 10 such patients. On retrospective analysis of the patient’s records, we found that 60% patients had received steroids and majority had co-morbidities. All the patients received similar treatment with IV amphotericin B and local debridement and the mortality rate was as high as 44%. We conclude that patients with COVID 19 infection are susceptible to mucormycosis because of impairment of barrier defense, dysfunction phagocytes and lymphocytes and the use of immunosuppressive medications such as steroids and tocilizumab.</p>

Abstrack. Demam berdarah dengue (DBD) adalah infeksi virus ditemukan pada negara-negara tropistermasuk Indonesia. Indonesia termasuk dalam katagori A dalam perkembangan penyakit virus tersebut yang mengindikasikan tingginya angka kematian. Angka insiden terus meningkat pada anak maupun dewasa dengan tingginya angka mortalitas. Tidak ada pencatatan lengkap untuk angka insidensi pada kehamilan, meskipun mortalitas cukup tinggi. Kami melaporkan empat kasus kehamilan dengan penyakit yang disebabkan oleh vektor Aedes aegypti. Pada keempat kasus tersebut kami telah diagnosis berdasarkan kriteria demam yang khas, trombositopenia, hemokonsentrasi dan pemeriksaan serologi. Terdapat penatalaksanaan yang berbeda dari berbagai kasus tersebut termasuk indikasi terminasi. Satu pasien kami rawat konservatif dan tiga pasien dilakukan terminasi berdasarkan indikasi obstetri. Pertimbangan cara terminasi sangat bergantung pula dengan indikasi obstetri. Seksio sesarea tidak secara mutlak dilakukan pada kehamilan dengan demam berdarah dengue. Kata kunci: Demam berdarah dengue, kehamilan, penatalaksanaan Abstract. Dengue hemorrhagic fever (DHF) is a viral infection found in tropical countries including Indonesia. Indonesia is included in category A in the development of viral diseases that's the high mortality rate. The numbers keep increasing on both adult children with high mortality rates. No complete logging for the incidence rate in pregnancy, although mortality is quite high. We reported four cases pregnancy with diseases caused by the Aedes aegypti vector. In four cases We had diagnosed based on criteria that met the specific criteria, thrombocytopenia, hemoconcentration and serology. There are different treatments for these cases includes the term indication. One patient was treated conservatively and three patients were admitted termination based on obstetric indications. The consideration on how to terminate is also very dependent with obstetric indications. Caesarean section is not absolutely necessary in pregnancy with dengue hemorrhagic fever. Keywords: Dengue hemorrhagic fever, pregnancy, management

Sarcopenia is defined as a loss of muscle strength, mass and function and it is a predictor of mortality. Sarcopenia is not only a geriatric disease, but it is related to several chronic conditions, including liver diseases in both its early and advanced stages. Despite the increasing number of studies exploring the role of sarcopenia in the early stages of chronic liver disease (CLD), its prevalence and the relationship between these two clinical entities are still controversial. Myosteatosis is characterized by fat accumulation in the muscles and it is related to advanced liver disease, although its role in the early stages is still under researched. Therefore, in this narrative review, we firstly aimed to evaluate the prevalence and the pathogenetic mechanisms underlying sarcopenia and myosteatosis in the early stage of CLD across different aetiologies (mainly non-alcoholic fatty liver disease, alcohol-related liver disease and viral hepatitis). Secondly, due to the increasing prevalence of sarcopenia worldwide, we aimed to revise the current and the future therapeutic approaches for the management of sarcopenia in CLD.

Background: Rocky Mountain Spotted Fever (RMSF) is a disease with a high mortality rate, caused by Rickettsia rickettsii, a bacteria transmitted to humans by infected ticks. In 2008 there was a Rocky Mountain Spotted Fever (RMSF) outbreak in the city of Mexicali, México, resulting in an increased mortality rate amongst the area population. Methods: Case-series study of patients admitted to the General Hospital of Mexicali between 2014 and 2019 with a confirmed diagnosis of RMSF. Mortality was compared dividing the population on those ≤20 and younger than ˃21 years of age. Results: A total of 129 patients’ records during a 5-year period whose diagnosis was RMSF confirmed with PCR were included. Mortality was compared among patients admitted who were younger than ≤20 years of age with that among patients who were older than ˃20 years of age (61 versus 68 respectively), the latter being higher with an OR 4.2 (p<0.0001). Conclusion: RMSF in hospitalized patients has a high mortality rate in spite of early treatment in all age groups, without showing any predominance in gender. However, patients older than 20 years of age had a higher mortality rate than those younger than 20 years, without any predominance in gender.

The liver is one of the most important organs, playing critical roles in maintaining biochemical homeostasis. Accordingly, disease of the liver is often debilitating and responsible for untold human misery. As biochemical nexus, with kinases being master regulators of cellular biochemistry, targeting kinase enzymes is an obvious avenue for treating liver disease. Development of such therapy, however, is hampered by the technical difficulty of obtaining comprehensive insight into hepatic kinase activity, a problem further compounded by the often unique aspects of hepatic kinase activities, which makes extrapolations from other systems difficult. This consideration prompted us to review the current state of the art with respect to kinome profiling approaches towards the hepatic kinome. We observe that currently four different approaches are available, all showing significant promise. Hence we postulate that insight into the hepatic kinome will quickly increase, leading to rational kinase-targeted therapy for different liver diseases.
