Zieve syndrome, a scarcely reported case is a triad of jaundice, haemolytic anaemia and hyperlipidaemia that develops secondary to alcohol-induced liver injury. Herein, a case of 23-year-old young male with history of steatohepatitis is reported who presented with diminution of vision in both eyes. On fundoscopy, there were extensive retinal haemorrhages involving fovea. Workup was consistent with haemolytic anaemia with no source of active bleeding. The patient was managed with supportive treatment. No specific ocular treatment was given. However, the vision gradually improved during follow-up. A rare association of zieve syndrome with haemorrhagic retinopathy is described here. Moreover, zieve syndrome has been described in the literature, mostly in non-English language case studies and is still under-recognised and under-reported. Diagnosis should be made quickly to avoid unnecessary invasive diagnostic interventions.

Objective: To report a case of young male with stroke and bilateral internal carotid artery (ICA) dissection. Background: Cervical Artery Dissection in Stroke Study trial has provided some insight on management of patients with ICA dissection. However, there is a need to modify the management strategies as per specific clinical scenario. Design/Methods: Case report and literature review. Results: A 45-year-old male presented with 1 month old history of acute onset numbness of right half of the body with slurring of speech. Computed tomography angiography showed complete occlusion of left cervical ICA just beyond origin with presence of fusiform dilatation and spiral flap in right extracranial cervical ICA. The patient was started on antiplatelets and taken for endovascular procedure using 2-mesh-based carotid stents. Patient was discharged after 3 days on antiplatelet therapy. At 1-year follow-up, there were no fresh symptoms. Conclusion: This case emphasizes the role of successful endovascular management of carotid dissection in a young male. These clinical situations may not be fully represented in trials, and a case-based approach is required.

Abstract Background: In October 2019, 94 patients were admitted into Alexandria Poison Center (APC) with a history of ingestion of Feseekh (salted fish). As a trial to allocate the resources, not all patients were given Heptavalent botulinum antitoxin (HBAT) immediately.The current study aimed to portray the clinical characteristics of the cases, explore the possible relation between these characteristics and necessity of HBAT administration, explore the reliability of MLT, and to establish a clinical guide for management with preservation of resources.Subject and Method: the current prospective study included 94 patients who were admitted to Alexandria Poison Center (APC) in the period from 29 th September to 27 th October 2019. The patients' data was recorded using a checklist that includes: personal data, past medical history, clinical assessment, investigations, treatment and the outcome. The checklist was carried out to assess and follow up each patient. Hospitalized patients were categorized according to symptoms consistent with botulism. The equine HBAT, made by Emergent BioSolutions Canada Inc. (formerly Can gene Corporation) was used in the treatment.Results: HBAT was given to (36.2%) patients only out of the total admission. However, 87.2% of patients were completely cured, whereas 10.6% of patients were discharged with mild neurological sequelea and death occurred only in two cases (2.2%).Conclusion: 63.8% of cases with suspected foodborne botulism toxicity could be managed by supportive treatment only with no need for HBAT.

A 31-year-old male presented to our facility with complaints of shortness of breath and left-sided chest pain. On record review, it was revealed that he had been seen in 2014 for an almost identical presentation and had been found to have haemolytic anaemia with warm autoantibodies. Following his acute treatment during that hospital admission, he was lost to follow-up. During his subsequent admission, 5 years later, he was found to have a systemic autoimmune disorder with a superimposed acute bacterial infection leading to a second case of haemolytic anaemia and at this time with both cold and warm antibodies present. While his diagnosis was initially difficult to make due to both derangements in expected laboratory values and the mixed pattern of the haemolytic anaemia, he was promptly treated with intravenous immune globulin and steroids and was able to make a full recovery.

Resumen Objetivo: El síncope es frecuente en la población general y relativamente raro en la población de deportistas. La seguridad y la actividad física en deportistas que han presentado un episodio sincopal no está aún bien establecida en la actualidad. Método: Varón joven, futbolista de élite, que presenta cuadro sincopal mientras realizaba actividad física. No presenta antecedentes personales ni familiares de interés. Tras presentar ECG patológico, se plantea la suspensión de la actividad deportiva. Resultados: En estudio cardiológico completo, se objetivan múltiples hallazgos dudosos que pueden ser considerados fisiológicos en el corazón de atleta, por lo que se autoriza la reanudación de actividad física de alta intensidad con seguimiento favorable. Discusión: El objetivo principal de la evaluación del deportista con síncope es determinar si existe enfermedad cardíaca subyacente y si puede continuar con la actividad deportiva con seguridad. Abstract Objective: Syncope is frequent in general population and relatively rare in the athlete population. Safety and physical activity in athletes who have presented a syncope is not yet well established. Method: Young male, elite soccer player, who presents a syncope while performing physical activity. He has no personal or family history of interest. After presenting pathological ECG, the suspension of the sport activity is considered. Results: In a complete cardiologic study, several dubious findings that can be considered physiological in the athlete's heart were identified. It allows the resumption of high intensity physical activity with favourable follow-up. Discussion: The main objective of the evaluation of the athlete with syncope is to determine if there is underlying heart disease and if he can continue with the sport activity safely. Resumo Objetivo: a síncope é freqüente na população em geral e relativamente raro na população de atletas. Segurança e atividade física em atletas que apresentaram síncope ainda não está bem estabelecida. Método: jovem masculino, jogador de futebol de elite, que apresenta síncope ao realizar atividades físicas. Ele não tem histórico pessoal ou familiar de interesse. Após a apresentação do ECG patológico, considera-se a suspensão da atividade esportiva. Resultados: Em um estudo cardiológico completo, foram identificados vários achados duvidosos que podem ser considerados fisiológicos no coração do atleta. Permite a retomada da atividade física de alta intensidade com acompanhamento favorável. Discussão: O principal objetivo da avaliação do atleta com síncope é determinar se há doença cardíaca subjacente e se ele pode continuar com a atividade esportiva com segurança.

This review essay critically engages with recent works by Mark Granovetter, an American sociologist whose articles about social networks are among the most cited in the history of the discipline, and Thomas Piketty, a French economist whose early, illustrious career turned to worldwide public recognition with the publication of le Capital au XXieme siècle in 2015. The contrast is therefore between a well-known sociologist, one of the foremost scholars in economic sociology in the English language, and an economist of international renown who has challenged mainstream economics with his research on economic inequality and his call for participatory public involvement in economic decision-making. Both insist on the economic as social, but in distinctive ways. In this essay, first, I consider Granovetter’s long-awaited Society and Economy: Framework and Principles and Piketty’s recent Capital et idé ologie, a follow-up to his earlier, best-selling book, on their own terms. Second, I think through how each scholar might understand and critique the other, pointing to complementarities and important differences in their approaches. I conclude that although both Granovetter and Piketty challenge mainstream economistic accounts of economic life, while emphasizing human agency and so the contingency of given economic relationships, they differ significantly in their respective analytical and political-normative focus. For Granovetter, what matters is that economic life is a social fact, while for Piketty it is a social struggle.

Journal of Stomatology, Oral and Maxillofacial Surgery is a bimonthly peer-reviewed French publication which exists since 1874 [3]. The purposes of this paper are: 1) to analyze the publication history of the core French journal from its foundation in 1894 till nowadays, 2) to study a transition period of changing the Journal`s language from French to English, 3) to understand the arguments for the language transition, and 4) to analyze the immediate positive results of the transition in a 2-year follow-up period.

Background: Although foam sclerotherapy of varicose tributaries is common in daily practice, scientific evidence for the optimal sclerosant-concentration and session-frequency is still low. This study aimed to increase the knowledge on foam sclerotherapy of varicose tributaries and to evaluate the efficacy and safety of foam sclerotherapy with 0.5 % polidocanol in tributaries with 3-6 mm in diameter. Patients and methods: Analysis of 110 legs in 76 patients. Injections were given every second or third day. A maximum of 1 injection / leg and a volume of 2ml / injection were administered per session. Controls were performed approximately 6 months and 12 months after the start of therapy. Results: 110 legs (CEAP C2-C4) were followed up for a period of 14.2 ± 4.2 months. Reflux was eliminated after 3.4 ± 2.7 injections per leg. Insufficient tributaries were detected in 23.2 % after 6.2 ± 0.9 months and in 48.2 % after 14.2 ± 4.2 months, respectively. Only 30.9 % (34 / 110) of the legs required additional therapy. In 6.4 % vein surgery was performed, in 24.5 % similar sclerotherapy was repeated. Significantly fewer sclerotherapy-sessions were required compared to the initial treatment (mean: 2.3 ± 1.4, p = 0.0054). During the whole study period thrombophlebitis (8.2 %), hyperpigmentation (14.5 %), induration in the treated region (9.1 %), pain in the treated leg (7.3 %) and migraine (0.9 %) occurred. One patient with a history of thrombosis developed thrombosis of a muscle vein (0.9 %). After one year there were just hyperpigmentation (8.2 %) and induration (1.8 %) left. No severe adverse effect occurred. Conclusions: Foam sclerotherapy with injections of 0.5 % polidocanol every 2nd or 3rd day, is a safe procedure for varicose tributaries. The evaluation of efficacy is difficult, as it can hardly be said whether the detected tributaries in the controls are recurrent veins or have recently developed in the follow-up period. The low number of retreated legs indicates a high efficacy and satisfaction of the patients.

AbstractReports of incidental pneumomediastinum in infants secondary to inflicted trauma are limited. A retrospective review of infants with pneumomediastinum and history of inflicted trauma was performed. A comprehensive literature review was performed. Three infants presented with pneumomediastinum associated with inflicted trauma. Mean age was 4.6 weeks. All patients underwent diagnostic studies, as well as a standardized evaluation for nonaccidental trauma. All patients with pneumomediastinum were resolved at follow-up. Review of the literature identified other cases with similar presentations with related oropharyngeal injuries. Spontaneous pneumomediastinum in previously healthy infants may be associated with inflicted injuries. Clinicians should be aware of the possibility of an oropharyngeal perforation related to this presentation.
