Introduction: The World Health Organization had recommended a multi-prong approach for the prevention and control of COVID-19, which has risen into a major global public health challenge. Government of nations are to disseminate information on these protective measures to the general public to mitigate the spread of the disease. However, people’s adherence to these preventive strategies is essential to guarantee success. Objective: To appraise the perception and practice of recommended measures for the prevention and control of COVID-19 by caregivers of children attending the University of Port Harcourt Teaching Hospital. Methodology: This descriptive cross-sectional study was done among parents/caregivers of children attending the Children’s Out Patient clinics, wards and emergency room of the Department of Paediatrics at the University of Port Harcourt Teaching Hospital, between August and September 2020. Information obtained using a structured investigator-administered questionnaire included bio data and perception about COVID-19 while practice of its preventive measures was observed. Data were analysed using SPSS version 25.  Results: All participants (161) have heard about COVID-19, mainly through the television and radio. The majority of them (n=141, 87.6%) knew that a combination of measures is needed for its prevention. More than half of participants (59%) washed their hands with water and soap provided, 89% of them had a facemask, a lesser proportion (58%) wore the mask and an even lesser proportion (28.6%) of them wore it properly. There was no significant difference between level of education or occupation of caregivers and proper wearing of the facemask. Conclusion: Though the awareness of COVID-19 was high, there was a gap between perception and practice of its preventive measures. Health education and innovative local strategies are recommended, to help improve the people’s acceptance of, and compliance to the stipulated multi-prong preventive measures.

Introduction: Healthcare-associated infections represent a global public health challenge and are associated with significant mortality and morbidity. Infection Prevention and Control (IPC) is a neglected area in healthcare facilities across Pakistan. The objective of our study was to elucidate the current state of infection prevention and control practices in public sector hospitals of Islamabad to underscore potential areas for improvement. Methodology: A cross-sectional survey was conducted between November and December 2019 at five public sector hospitals of Islamabad. The World Health Organization’s Infection Prevention and Control Assessment Framework (IPCAF) was used to assess the strengths and weaknesses of hospitals regarding infection prevention and control. Adapted tools derived from Centers for Disease Control and Prevention and Infection Prevention Society were used for detailed assessment of various departments. Data was analyzed using Microsoft Excel 2016. Results: In all five hospitals, the total IPCAF score was less than 200 denoting that infection prevention and control implementation is deficient and significant improvement is needed. The median IPCAF score was 117.5 with an interquartile range of 53.75. With the exception of central sterile services unit at one hospital, departments at all hospitals failed to meet even 50% of required IPC standards. Conclusions: Significant change is needed to improve the existent situation of infection prevention and control in public sector hospitals of Islamabad. This would involve establishment of functional programs, development and implementation of infection prevention and control guidelines and provision of adequate supplies.

Background The World Health Organization has defined a traditional birth attendant as a ‘person who assists the mother during childbirth and initially acquired her skills by delivering babies herself or through apprenticeship to other traditional birth attendants’. It is estimated that over 40 million neonatal deaths occur annually worldwide; some of these deaths are the result of infection, which may be attributed to the practices of traditional birth attendants. In Nigeria, maternal mortality because of puerperal sepsis occurs in 12% of cases. This study aimed to assess infection prevention and control practice among traditional birth attendants in Lagos State, Nigeria. Methods This is a descriptive study of infection prevention and control among traditional birth attendants in the three local government areas of Ikorordu, Mushin and Surulere. Simple random sampling (balloting) was used to select 144 registered traditional birth attendants. A self-structured questionnaire made up of four sections (sociodemographic characteristics, knowledge, attitude and practice) was used for data collection. The participant knowledge scores were categorised into ‘good’ (60–100%) and ‘poor’ (0–59%). Descriptive and inferential statistics were used to analyse the data and tested for significance at P=0.005. Results The mean age of the respondents was 48 ± 10.6 years. Over one-third (37.5%) of the respondents had poor knowledge of infection control. There was a statistically significant association between respondent age and practice of infection prevention and control (P=0.003). There was no significant difference between the duration of practice and infection prevention and control (P=0.688). Additionally, there was no significant association between the length of training and infection prevention and control (P=0.890). Conclusions There is a need to increase the knowledge of traditional birth attendants by organising training for them on infection prevention and control. Over 70% of the respondents identified that practice could be improved by additional training. This will reduce maternal and infant morbidity and mortality caused by infection in Lagos, Nigeria.

Abstract Background Health workers are crucial to the successful implementation of infection prevention and control strategies to limit the transmission of SARS-CoV-2 at healthcare facilities. The aim of our study was to determine SARS-CoV-2 infection prevention and control knowledge and attitudes of frontline health workers in four provinces of South Africa as well as explore some elements of health worker and health facility infection prevention and control practices. Methods A cross-sectional study design was utilised. The study population comprised both clinical and non-clinical staff working in casualty departments, outpatient departments, and entrance points of health facilities. A structured self-administered questionnaire was developed using the World Health Organization guidance as the basis for the knowledge questions. COVID-19 protocols were observed during data collection. Results A total of 286 health workers from 47 health facilities at different levels of care participated in the survey. The mean score on the 10 knowledge items was 6.3 (SD = 1.6). Approximately two-thirds of participants (67.4%) answered six or more questions correctly while less than a quarter of all participants (24.1%) managed to score eight or more. A knowledge score of 8 or more was significantly associated with occupational category (being either a medical doctor or nurse), age (< 40 years) and level of hospital (tertiary level). Only half of participants (50.7%) felt adequately prepared to deal with patients with COVD-19 at the time of the survey. The health workers displaying attitudes that would put themselves or others at risk were in the minority. Only 55.6% of participants had received infection prevention and control training. Some participants indicated they did not have access to medical masks (11.8%) and gloves (9.9%) in their departments. Conclusions The attitudes of participants reflected a willingness to engage in appropriate SARS-CoV-2 infection prevention and control practices as well as a commitment to be involved in COVID-19 patient care. Ensuring adequate infection prevention and control training for all staff and universal access to appropriate PPE were identified as key areas that needed to be addressed. Interim and final reports which identified key shortcomings that needed to be addressed were provided to the relevant provincial departments of health.

Abstract Background: Antimicrobial resistance (AMR) is increasingly pervasive due to multiple, complex prescribing and consuming behaviours. Accordingly, behaviour change is an important component of response to AMR. Little is known about the best approaches to change antibiotic use practices and behaviours. This project aims to develop a context-specific behaviour change strategy focusing on promoting appropriate prescription practices following the World Health Organization recommendations for surgical prophylaxis in an orthopaedic surgery unit in Egypt.Methods: The project included a formative qualitative research study was based on the Theoretical Domains Framework (TDF) to explore the determinants for inappropriate prescription of surgical antibiotic prophylaxis at an orthopaedic unit. The intervention was developed to following the Behaviour Change Wheel (BCW) in a knowledge co-production workshop with infection prevention and control experts that ensured that the theory based intervention was a culturally acceptable, practical and implementable intervention. Results: The prescription of surgical prophylaxis was influenced by five TDF domains including, knowledge, belief in consequences (mistrust towards infection prevention and control measures), environmental factors (lack of prescription guidelines) , professional role and reinforcement (a lack of appropriate follow up actions influenced prescription of surgical prophylaxis). The appropriate set of behaviour change functions of BCW and related activities to improve the current practices included education, enablement, persuasion, environmental restructuring and restriction. Conclusions The study showed that a theory based and context specific intervention can be created by using the TDF and BCW together with knowledge-co creation to improve the prescription of surgical prophylaxis in and Egyptian orthopaedic unit. The intervention need to piloted and scaled up.

Abstract The novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) originated in Wuhan, China in late 2019, and its resulting coronavirus disease, COVID-19, was declared a pandemic by the World Health Organization on March 11, 2020. The rapid global spread of COVID-19 represents perhaps the most significant public health emergency in a century. As the pandemic progressed, a continued paucity of evidence on routes of SARS-CoV-2 transmission has resulted in shifting infection prevention and control guidelines between classically-defined airborne and droplet precautions. During the initial isolation of 13 individuals with COVID-19 at the University of Nebraska Medical Center, we collected air and surface samples to examine viral shedding from isolated individuals. We detected viral contamination among all samples, supporting the use of airborne isolation precautions when caring for COVID-19 patients.

Abstract Background Harmonization in hand hygiene training for infection prevention and control (IPC) professionals is lacking. We describe a standardized approach to training, using a “Train-the-Trainers” (TTT) concept for IPC professionals and assess its impact on hand hygiene knowledge in six countries. Methods We developed a three-day simulation-based TTT course based on the World Health Organization (WHO) Multimodal Hand Hygiene Improvement Strategy. To evaluate its impact, we have performed a pre-and post-course knowledge questionnaire. The Wilcoxon signed-rank test was used to compare the results before and after training. Results Between June 2016 and January 2018 we conducted seven TTT courses in six countries: Iran, Malaysia, Mexico, South Africa, Spain and Thailand. A total of 305 IPC professionals completed the programme. Participants included nurses (n = 196; 64.2%), physicians (n = 53; 17.3%) and other health professionals (n = 56; 18.3%). In total, participants from more than 20 countries were trained. A significant (p < 0.05) improvement in knowledge between the pre- and post-TTT training phases was observed in all countries. Puebla (Mexico) had the highest improvement (22.3%; p < 0.001), followed by Malaysia (21.2%; p < 0.001), Jalisco (Mexico; 20.2%; p < 0.001), Thailand (18.8%; p < 0.001), South Africa (18.3%; p < 0.001), Iran (17.5%; p < 0.001) and Spain (9.7%; p = 0.047). Spain had the highest overall test scores, while Thailand had the lowest pre- and post-scores. Positive aspects reported included: unique learning environment, sharing experiences, hands-on practices on a secure environment and networking among IPC professionals. Sustainability was assessed through follow-up evaluations conducted in three original TTT course sites in Mexico (Jalisco and Puebla) and in Spain: improvement was sustained in the last follow-up phase when assessed 5 months, 1 year and 2 years after the first TTT course, respectively. Conclusions The TTT in hand hygiene model proved to be effective in enhancing participant’s knowledge, sharing experiences and networking. IPC professionals can use this reference training method worldwide to further disseminate knowledge to other health care workers.

Coronavirus disease (COVID-19) outbreak, caused by the most recently discovered coronavirus, is currently affecting a large population across the globe. World health organization (WHO) has already declared COVID-19, a pandemic, and the world is fighting to contain the COVID-19 outbreak. Nepal has taken several preventive measures to control the coronavirus outbreak. However, some additional steps are needed to prevent community transmission of the disease. This brief communication discusses the government of Nepal actions and provides recommendations for the prevention and control of COVID-19 infection in Nepal.

World Health Organization (WHO) has declared the recent outbreak of coronavirus disease (COVID-19) a Pandemic and a global public health emergency. The eventual scope and consequences of this outbreak are uncertain at present as the condition is swiftly evolving. The purpose of this article is to cumulate existing published data and researches, identify the challenges with reference to India and develop a comprehensive preventive plan to prevent the spread of this deadly pandemic. The authors explored WHO situation dashboards, the web portal of Ministry of Health and Family Welfare of India and press reports of different countries, published literature on coronavirus transmission, textbooks, scientific and medical journals and several other sources to identify ongoing challenges in the fight against COVID-19 in India. After an exhaustive literature search, 15 ongoing challenges in the fight against novel COVID-19 especially with reference to India have been identified. After a thorough analysis of all these challenges along with the lessons from the International community, a preventive plan has been established to be devised by different agencies from our personal perspectives to overcome the challenges which may assist decision-makers in the prevention and control of COVID-19 in India.

Abstract Objectives: This study explores the experiences and practices of emergency medical services (EMS) providers, as well as the motivations that underpin perceptions toward standard infection prevention and control (IPC). The current literature suggests that EMS providers have a low compliance level with preventive measures, with misperceptions about risks and self-justification of personal skills reported. Methods: The study used qualitative methods and conducted 2 distinct focus group discussions and 20 in-depth interviews with both prehospital and inter-facility EMS providers. Data were thematically analyzed using the Framework approach. Results: The participants considered respiratory infections the most significant nosocomial risks. Lack of full disclosure of medical history to EMS providers was considered a significant threat. Beliefs about low effectiveness and harmful effects of the influenza vaccine, as well as low perceptions of influenza risks, were common. While apparent misperceptions contributed largely to the inappropriate use of preventive measures, the reliance on intuition and individual experiences was attributed to the inaccessibility of appropriate guidelines, and lack of formal IPC training programs. Conclusions: There is need to address EMS doubts and fears, improve IPC practices and awareness by institutionalizing IPC training programs, and ensure the design and accessibility of simplified and well-tailored IPC guidelines for EMS providers.
