Background: Heart failure has the highest rate of Rehospitalization with 20-33% readmissions within 1 to 3 months of discharge from the hospital. we decided to find out the frequency of different causes of hospitalization in patients with heart failure during 2017 to 2019. Methods: This study was a cross-sectional descriptive study. A total of 120 patients with heart failure who were referred to Afshar Hospital of Yazd during 2017 to 2019 were enrolled. The random sampling method was used.The required information was collected from heart failure patients’ registry project. datum were collected and were analyzed by statistical tests and SPSS version 18. Results: The mean age of patients was 53.53±12.36 years. Of the 120 patients under study,41.7% were women and 58.3% were male. The results showed that, 14.2% did not follow the recommended diet,14.2% had not regular use of drugs,14.2% had renal dysfunction,9.2% had miscellaneous causes,8.3% had pulmonary disease and 40% had idiopathic cause(where the cause of the condition is not known). There was no significant difference among the distribution of various causes of hospitalization in terms of the variables in the study. Conclusions: It can be concluded that the most common reason for hospitalization of patients with heart failure is idiopathic cause.

Objective: Marital satisfaction is considered as satisfaction with a marital relationship on which the presence of a child has different effects. Concerns about a childfree life and its effect on marital satisfaction in infertile couples are very critical. Therefore, this study was intended to characterize and compare concerns about a childfree lifestyle and the need for parenthood and their relationship with marital satisfaction in infertile couples. Method: A total of 200 men and 200 women who referred to fertility centers in Tehran participated in this cross-sectional study. Convenience sampling method was used for sampling. Demographic survey, ENRICH Marital Satisfaction Scale, and Fertility Problem Inventory were used for data collection. The resulting data were analyzed using descriptive and analytical statistical tests (Pearson Correlation Coefficient and Stepwise Regression). Results: The mean scores for concern about a childfree lifestyle and the need for parenthood in women were significantly higher than in men. The variables rejection of a childfree lifestyle and the need for parenthood were respectively predictors of marital satisfaction in women and men. Conclusion: Since marital satisfaction in infertile couples is affected by their feelings about having a child and becoming a parent, it is therefore suggested that appropriate counseling be provided in supportive healthcare programs for infertile couples to promote their marital satisfaction.

Introduction: Limits of movement in knees is one of the most common complications following trauma and surgery. With early diagnosis and appropriate treatment choices, knee replacement and performance improvement are expected in many patients. However, limits of movement in knees is unpleasant and should be corrected as soon as possible. In this study, we decided to evaluate the results of arthroscopic release in motion limits of knees. Methods: This was a descriptive cross-sectional study.14 patients who referred to the Shahid Sadoughi Hospital of Yazd from 2014 to 2016 and diagnosed with knee limitation (flexion or extension restriction of more than 10 degrees) were selected. Patients were undergone arthroscopic release and followed-up for 6 months. Lysholm knee score, degrees of flexion and extension, and patient satisfaction were evaluated. The results were analyzed by SPSS-18 and statistical tests Paired T-test as well as Spearman correlation. Results: The mean age of the patients was 28.92±10.99 years. The most common type of trauma in the patients was femoral distal fracture (46.7%). The frequency of the patients' satisfaction with arthroscopy was as followed: 50% (7 patients), had moderate satisfaction, 28.6% (4 ones), high satisfaction, and 21.4% (3 ones) had low satisfaction.The mean difference in Lysholm Knee Score, improvement in degree of extension, improvement in degree of flexion and pain score before and after arthroscopy were 26.21±14.11,64.78±26.13,3.57±2.92 and 4.13±1.65,respectively.There was a significant difference between the mean values of these factors before and after arthroscopy. Conclusion: According to the results obtained, we can conclude that arthroscopic release is an effective and low-complication method in motion limits of knees.

Abstract: Loss of teeth can be caused by various diseases such as caries and periodontal disease. Losing teeth can lead people to emotional impact as well as impaired functions of speaking, chewing, and aesthetics. The use of denture to replace missing teeth is important to avoid these impacts. This study aimed to determine patients’ perception as users of removable acrylic based denture in Kotamobagu. This was a descriptive study with a cross sectional design. Population were 203 users of removable acrylic based denture at dentist services in Kotamobagu. Samples were 67 respondents obtained by using Solvin formula and simple random sampling method. In this study we used questionnaire consisted of 25 questions. The results showed that the patient’s perception was in good category based on competence, access, needs, time, and budget.Keywords: patient’s perception, removable denture, dentist serviceAbstrak: Kehilangan gigi dapat disebabkan oleh berbagai penyakit seperti karies dan penyakit periodontal. Kehilangan gigi dapat menimbulkan dampak emosional serta terganggunya fungsi bicara, pengunyahan, dan estetika. Penggunaan gigi tiruan untuk menggantikan gigi yang hilang penting dilakukan untuk menghindari dampak tersebut. Penelitian ini bertujuan untuk mengetahui persepsi pasien pengguna gigi tiruan lepasan berbasis akrilik yang menggunakan jasa dokter gigi di Kotamobagu. Penelitian ini bersifat deskriptif dengan desain potong lintang. Populasi yaitu pasien pengguna gigi tiruan lepasan berbasis akrilik yang menggunakan jasa dokter gigi di Kotamobagu yang berjumlah 203 jiwa. Pengambilan sampel menggunakan rumus Slovin menghasilkan 67 sampel, dan metode pengambilan sampel dilakukan dengan simple random sampling. Studi ini menggunakan kuesioner yang berjumlah 25 pertanyaan. Hasil penelitian menunjukkan berdasarkan kompetensi, akses, kebutuhan, waktu, dan biaya persepsi pasien termasuk kategori baik.Kata kunci : persepsi pasien, gigi tiruan lepasan, jasa dokter gigi

Abstract: Tooth extraction is a mostly performed treatment in dental practice because most patients come with bad tooth condition that cannot be taken care anymore. The obstacle of tooth extraction is the society’s knowledge. Lack of knowledge causes doubt about going to the dentist. This study aimed to determine the overview of the knowledge and attitude levels of the people in North Molompar Village, South East Minahasa about tooth extraction. This was a descriptive study with a cross sectional design. There were 87 samples obtained by using the Slovin formula with random sampling method. Data presented in the form of a diagram based at the frequency distribution. The results showed that the knowledge level of tooth extraction in North Molompar was 55% good, obtained from scoring result of 481, and the attitude level of tooth extraction was 69% good, obtained from scoring result of 604. Conclusion: Levels of knowledge and attitude of the people in North Molompar Village, South East Minahasa, about tooth extraction were categorized as good. Keywords: tooth extraction, knowledge, attitude.     Abstrak: Pencabutan gigi merupakan tindakan yang sering dilakukan dalam praktek kedokteran gigi karena kebanyakan pasien datang dengan keadaan gigi yang sudah tidak  bisa dirawat lagi. Hambatan yang dialami dalam upaya pencabutan gigi ialah pengetahuan masyarakat mengenai hal-hal yang menyangkut pencabutan gigi. Pengetahuan yang kurang memadai membuat masyarakat ragu untuk berobat ke dokter gigi. Penelitian ini bertujuan untuk mengetahui gambaran tingkat pengetahuan dan sikap masyarakat tentang pencabutan gigi di Desa Molompar Utara Kabupaten Minahasa Tenggara. Penelitian ini bersifat deskriptif dengan desain potong lintang. Jumlah sampel sebanyak 87 responden diperoleh dengan menggunakan rumus Slovin dan penarikan sampel berupa acak sederhana. Data disajikan dalam bentuk diagram berdasarkan distribusi frekuensi. Hasil penelitian menunjukkan tingkat pengetahuan masyarakat tentang pencabutan gigi di Desa Molompar Utara yaitu 55% dapat dikatakan baik (hasil skoring 481) dan sikap masyarakat tentang pencabutan gigi di yaitu 69% dapat dikatakan baik (hasil skoring 604). Simpulan: Tingkat pengetahuan dan sikap masyarakat di Desa Molompar Utara terhadap pencabutan gigi tergolong baik. Kata kunci: pencabutan gigi, pengetahuan, sikap.

ABSTRAK Latar Belakang : Diabetes melitus merupakan penyakit metabolik dengan karakteristik hiperglikemia sebagai akibat kelainan sekresi insulin maupun kerja insulin. Diabetes sebagai salah satu penyebab dislipidemia sekunder, sehingga pengelolaan glukosa darah merupakan pencegahan primer timbulnya komplikasi penyakit kardiovaskular. Hasil Riskesdas tahun 2018, prevalensi diabetes melitus yang didiagnosis dokter pada penduduk di semua umur sebesar 3,4% di Kota Surabaya.Tujuan : Penelitian ini untuk mengetahui perbedaan profil lipid pada level HbA1C normal, prediabetes dan diabetes melitus.Metode : Penelitian cross sectional, teknik pengambilan sampel dengan simple random sampling dari data rekam medis pasien rawat jalan di Rumah Sakit Islam Surabaya  dari 1 Januari tahun 2018 sampai dengan 31 Desember 2019 berusia 35-80 tahun dan mendapat pemeriksaan HbA1c, kolesterol, trigliserida dan LDL-kolesterol pada waktu yang sama dan terdokumentasi lengkap pertama sekali sehingga diperoleh besar sampel 73 data pasien. Uji Anova one way digunakan untuk mengetahui perbedaan rata-rata kolesterol, trigliserida dan LDL-kolesterol berdasarkan HbA1C.Hasil : Hasil penelitian menunjukkantidakada perbedaan yang signifikan rata-rata kolesterol, dan LDL-kolesterol dengan tingkatan HbA1C (p>0,05), ada perbedaan yang signifikan rata-rata trigliserid dengan HbA1C normal, prediabetes, dan diabetes (p=0,01). Hasil multiple comparison dengan metode Tukey HSD menunjukkan perbedaan signifikan rata-rata trigliserid  pada HbA1C normal dengan diabetes (p=0,039) dan prediabetes dengan diabetes (p=0,044).Kesimpulan :Perbedaan rata-rata trigliserida signifikanpada HbA1Ckategorinormal dan prediabetes dengan diabetes, pentingnya mengendalikanglukosa darah untuk mencegahkomplikasi kardiovaskuler pada penderita diabetes melitus yang dapat dilakukan melalui pemantauan mandiri glukosa darah, pola hidup sehat, aktivitas fisik secara teratur, terapi nutrisi medis sesuai kebutuhan, menurunkan berat badan bagi yang mengalami obesitas, tidak merokokdan intervensi obat anti hiperglikemia jika dibutuhkan.Kata Kunci : diabetes, HbA1C, kolesterol, trigliserid, LDL-kolesterol. ABSTRACT  Background :Diabetes melitus is a metabolic disease characterized by hyperglicemia as a result of abnormal insulin secretion and insulin action. Riskesdas 2018 said that the prevalence of doctors' diagnosed diabetes in the population at all age 3.4% in Surabaya.Objective : This study is to find out the difference in lipid profiles on normal HbA1Clevels, pre-diabetes and diabetes mellitusMethod: Cross-sectional study, the sampling technique used was simple random sampling fromoutpatient medical recordsthe Surabaya Islamic hospital's from 1st of January 2018 to 31st December 2019 aged 35-80 years and checked for HbA1C, cholesterol, triglyceride and LDL-cholesterol at the same and firsttime documented. Sample sizes of 73 data analized with One Way Anova test was used to identify differences in mean cholesterol, triglyceride and LDL-cholesterol based Hba1C.Results :The results showed that there was no significant difference mean cholesterol and mean LDL-cholesterol with HbA1C levels (p> 0.05), there were significant differences mean the triglyceride with normal HbA1C levels, pre-diabetes, and diabetes (p= 0.01). Multiple comparason results using Tukey HSD methods showed that there was significant differences mean the triglycerid on normal HbA1C levels with diabetes (p= 0.039) and the mean triglyceride ebetween hba1c prediabetesand diabetes (p= 0.044).Conclusions: The mean difference trigliseride signifnificant in normal HbA1C levels and pre-diabetes with diabetes.The importantce of controlling blood glucose to prevent cardiovasculer complication in people with diebetes mellitus can be done through education on independent monitoring of blood glucose, healthy lifestyle, reguler physical activity, medical nutrition therapy according to the needs, lost weight for those who are obese, do not smoke and  anti-hyperglicemia drug intervention if needed.

Abstract Objective: To evaluate the intensity and discomfort of perioperative thirst and related factors during anesthesia recovery. Method: This is a quantitative, cross-sectional, descriptive study. Results: Of the 203 participants, 182 (89.6%) reported they were thirsty. The mean intensity of thirst was 6.9 measured using a verbal numerical scale of 0 to 10 and discomfort was 7.3 on a scale of 0 to 14. All attributes evaluated by the scale were cited including dry mouth and desire to drink water (87.3%), dry lips (79.1%), thick tongue feeling (43.4%), thick saliva (56.5%), dry throat (75.2%) and bad taste in the mouth (63.1%). There was a positive correlation between the intensity of thirst and discomfort assessed by the scale (Spearman coefficient: 0.474; p-value: <0.05). No correlation was found between age, length of fasting and use of opioids with the intensity of thirst and discomfort. Conclusion and implication in the clinical practice: Discomfort arising from the attributes of thirst is evidenced as the intensity of thirst increases.

Background & Aim: Due to the chronic nature of heart failure, it is necessary to observe and maintain self-care behaviors accordingly. Tele-monitoring using smartphone applications can be effective in this regard. This study aimed to determine the effect of using smartphone applications on self-care behaviors in patients with heart failure. Methods & Materials: This randomized controlled clinical trial was conducted between August and March in 2018. Using consecutive sampling, 120 patients were selected and were allocated to the two groups by a random quadruple block approach. The participants in both groups completed the European heart failure self-care questionnaire before and after the intervention. Patients in both groups received routine hospital care; however, patients in the intervention group used an Android smartphone application for three months daily as well. Via the application, there were ongoing interactions between patients and researchers, depending on the needs of patients and their conditions. The data were analyzed using SPSS V.16 software. Results: There was a statistically significant difference between groups in terms of the mean score of self-care after the intervention where the mean score in the intervention group was lower (p<0.001), which indicates better self-care. Based on the results, the intervention effect was reported at 0.787. Conclusion: The findings of this study showed that using a smartphone application can improve self-care in patients with heart failure. Therefore, it is recommended to health care providers to use this mobile application to care and monitor remote patients with HF.

Objective: To assess the role of the complement components (C3 and C4) in the pathogenesis and severity of bronchial asthma (BA) among Yemeni patients. Methods: This cross-sectional, clinic-based study was conducted in Sana’a city, Yemen in the period from March to June 2012. It included 100 BA patients, where demographic and clinical data were collected using a pre-designed, structured questionnaire. Blood samples then collected by venipuncture, and sera were then separated and tested for the levels of C3 and C4 by immunoturbidimetry assay. Data were analyzed using suitable statistical tests using IBM SPSS Statistics, version Results: Of 100 patients, 53% were males and 29% were older than 40 years. The mean age of patients was 34.7± 17.5 years (range: 6–80). The majority of patients had active asthma (81.0%) and experienced asthma for more than two years (66.0%). Dyspnea (81.0%) and cough (65.0%) were the most frequent clinical features, while edema (11.0%) was the least frequent clinical feature among asthmatic patients. The mean level of serum C3 among BA patients was elevated (127.9 ± 21.3 mg/dL) and was significantly higher among females, patients aged 40 years or older and those with active BA, but there was no statistically significant difference with respect to the duration of asthma. In contrast, the mean level of serum C4 was normal (27.6 ± 8.8 mg/dL) but was significantly higher among patients aged 40 years or older and those having asthma for more than two years. Wheezing was significantly associated with the mean level of serum C3 among BA patients. Similarly, the mean level of serum C4 was significantly higher among patients having cough. Conclusions: The majority of Yemeni patients with BA have elevated levels of serum C3, which can be significantly higher in females, patients older than 40 years and those with active asthma. On the other hand, the levels of serum C4 are normal among the majority of Yemeni patients with BA, but these can be elevated among those with severe clinical features or long-term asthma. ABSTRACT CONTEXT AND OBJECTIVE: The association of serum triglycerides plus waist circumference seems to be a good marker of cardiovascular risk and has been named the “hypertriglyceridemic waist” phenotype. The aim of our study was to investigate the association between the hypertriglyceridemic waist phenotype and HDL-cholesterol among patients with heart failure. DESIGN AND SETTING: Cross-sectional study in a tertiary-level hospital in southern Brazil. METHODS: We included patients with heart failure aged > 40 years. Anthropometric assessment (weight, height, waist and hip circumferences) was performed; body mass index (BMI) and waist-hip ratio were calculated and lipid measurements (serum total cholesterol, LDL-cholesterol, HDL-cholesterol and triglycerides) were collected. In men and women, respectively, waist circumference ≥ 94 cm and ≥ 80 cm, and triglycerides ≥ 150 mg/dl were considered abnormal and were used to identify the hypertriglyceridemic waist phenotype. Analyses of covariance were used to evaluate possible associations between levels of HDL-cholesterol and the hypertriglyceridemic waist phenotype, according to sex. RESULTS: 112 participants were included, of whom 62.5% were men. The mean age was 61.8 ± 12.3 years and the mean ejection fraction was 40.1 ± 14.7%. Men and woman presented mean HDL-cholesterol of 40.5 ± 14.6 and 40.9 ± 12.7 mg/dl, respectively. The prevalence of the hypertriglyceridemic waist phenotype was 25%. There was a significant difference in mean HDL-cholesterol between men with and without the hypertriglyceridemic waist phenotype (32.8 ± 14.2 versus 42.1 ± 13.7 mg/dl respectively; P = 0.04), even after adjustment for age, body mass index, type 2 diabetes mellitus, use of statins and heart failure etiology. CONCLUSIONS: The hypertriglyceridemic waist phenotype is significantly associated with lower HDL-cholesterol levels in men with heart failure.

<p>Communication, as a key element in providing high-quality health care services, leads to patient satisfaction and health. The present Cross sectional, descriptive analytic study was conducted on 70 nurses and 50 patients in two hospitals affiliated to Alborz University of Medical Sciences, in 2012. Two separate questionnaires were used for nurses and patients, and the reliability and validity of the questionnaires were assessed. In both groups of nurses and patients, nurse-related factors (mean scores of 2.45 and 2.15, respectively) and common factors between nurses and patients (mean scores of 1.85 and 1.96, respectively) were considered the most and least significant factors, respectively. Also, a significant difference was observed between the mean scores of nurses and patients regarding patient-related (p=0.001), nurse-related (p=0.012), and environmental factors (p=0.019). Despite the attention of nurses and patients to communication, there are some barriers, which can be removed through raising the awareness of nurses and patients along with creating a desirable environment. We recommend that nurses be effectively trained in communication skills and be encouraged by constant monitoring of the obtained skills.</p>
