Introduction: Microleakage is a major concern in composite restorations, especially with margins located on dentin or cementum of the tooth. This study aimed to review the available literature investigating in vitro the efficacy of different tooth lining materials on the marginal seal of composite restorations extended below the Cemento-Enamel Junction (CEJ) of the tooth. Materials and Methods: Different combinations of the review terms were used to electronically search PubMed, Google Scholar, Cochrane Library, and Ebscohost databases for the last fifteen years. The records were initially screened for relevancy based on title and abstract. Articles that seemed to meet the criteria for inclusion were selected for full-text assessment.  Studies deemed eligible were in vitro studies conducted on human teeth with cavities extending below the CEJ and restored using different tooth lining materials under the composite restorations.Every included study was evaluated using the Cochrane risk of bias assessment tool. Results: The initial search identified 619 publications. After removing duplicated records, the remaining 421 records are screened at the level of titles and abstracts to meet our inclusion criteria. 62 records were designated for full-text methodological assessment, and 15 studies were found eligible for qualitative synthesis. The sealing ability of different tooth lining materials under composite restorations was investigated in the chosen studies, with varying outcomes. Microleakage in the tooth restorations was determined by examining the extent of dye penetration under a microscope. Conclusion: Despite the reports of less dye penetration with specific tooth liners in some in-vitro studies, there is no consistent evidence supporting that such tooth liners could reduce microleakage in composite restorations of the tooth.

Background: The effect of cruciferous vegetable intake on breast cancer survival is controversial at present. Glucosinolates are the naturally occurring constituents found across the cruciferous vegetables. Isothiocyanates are produced from the hydrolysis of glucosinolates and this reaction is catalysed by the plant-derived enzyme myrosinase. The main isothiocyanates (ITCs) from cruciferous vegetables are sulforaphane, benzyl ITC, and phenethyl ITC, which had been intensively investigated over the last decade for their antibreast cancer effects. Objective: The aim of this article is to systematically review the evidence from all types of studies, which examined the protective effect of cruciferous vegetables and/or their isothiocyanate constituents on breast cancer. Methods: A systematic review was conducted in Pubmed, EMBASE, and the Cochrane Library from inception to 27 April 2020. Peerreviewed studies of all types (in vitro studies, animal studies, and human studies) were selected. Results: The systematic literature search identified 16 human studies, 4 animal studies, and 65 in vitro studies. The effect of cruciferous vegetables and/or their ITCs intake on breast cancer survival was found to be controversial and varied greatly across human studies. Most of these trials were observational studies conducted in specific regions, mainly in the US and China. Substantial evidence from in vitro and animal studies was obtained, which strongly supported the protective effect of sulforaphane and other ITCs against breast cancer. Evidence from in vitro studies showed sulforaphane and other ITCs reduced cancer cell viability and proliferation via multiple mechanisms and pathways. Isothiocyanates inhibited cell cycle, angiogenesis and epithelial mesenchymal transition, as well as induced apoptosis and altered the expression of phase II carcinogen detoxifying enzymes. These are the essential pathways which promote the growth and metastasis of breast cancer. Noticeably, benzyl ITC showed a significant inhibitory effect on breast cancer stem cells, a new dimension of chemoresistance in breast cancer treatment. Sulforaphane and other ITCs displayed anti-breast cancer effects at variable range of concentrations and benzyl isothiocyanate appeared to have a relatively smallest inhibitory concentration IC50. The mechanisms underlying the cancer protective effect of sulforaphane and other ITCs have also been highlighted in this article. Conclusion: Current preclinical evidence strongly supports the role of sulforaphane and other ITCs as potential therapeutic agents for breast cancer, either as adjunct therapy or combined therapy with current anti-breast cancer drugs, with sulforaphane appeared to display the greatest potential.

Some authors have been proposing the use of cavity disinfectants in order to reduce, or even eliminate, the effect of the microorganisms present in a dental cavity before a restoration is placed. The aim of this study was to evaluate the effect of different cavity disinfectants on bond strength and clinical success of composite and glass ionomer restorations on primary teeth. The research was conducted using Cochrane Library, PubMed/MEDLINE, SCOPUS, and Web of Science for articles published up to February 2021. The search was performed according to the PICO strategy. The evaluation of the methodological quality of each in vitro study was assessed using the CONSORT checklist for reporting in vitro studies on dental materials. Sixteen in vitro studies and one in situ study fulfilled the inclusion criteria and were analyzed. Chlorhexidine was the most studied cavity disinfectant, and its use does not compromise dentin bonding. Sodium hypochlorite is a promising alternative, but more research on its use is required to clearly state that it can safely be used as a cavity disinfectant for primary teeth. Although other disinfectants were studied, there is a low-level evidence attesting their effects on adhesion, therefore their use should be avoided.

SummaryBackground/Aim: Molar incisor hypomineralization (MIH) is a qualitative defect of systemic origin, affecting permanent first molars and often permanent incisors. The treatment modalities can include, amongst others, fissure sealants for prevention of dental caries and composite restorations. Both require adhesion to tooth structure. The aim of this study was to review the literature on the adhesion to enamel affected by MIH.Material and Methods: A search of PupMed/Medline, ResearchGate and Google Scholar was performed and limited between 2003, when the judgement criteria for MIH were set, and 2016. Thirty-three papers were considered relevant to the subject including five in vivo and six in vitro studies. Studies involving less than ten teeth were excluded.Results: A four-year clinical trial showed that the application of a total-etch 2-step adhesive system prior to sealant placement is superior to the etch-seal technique. Despite the high success rate of composite restorations shown in three clinical longitudinal studies, there are conflicting results over self-etch being superior to total etch adhesive systems. Pretreating the enamel surface, prior to the adhesive system, with fluoride preventive solutions could reduce the mikroleakage under orthodontic brackets. Three in vitro studies provide inconsistent data about NaOCl pretreating potentials to improve adhesion of composite restorations. Resin infiltration, prior to resin restorations, could improve the microhardness of defected enamel, which may lead to increased bond strength, especially in combination with NaOCl pretreatment.Conclusions: Adhesion to enamel affected by molar incisor hypomeralization is inferior compared to normal enamel. Sealants applied with the etch-bond-seal technique have greater retention than with the etch-seal technique. Further research is required to provide evidence of the effectiveness of the adhesive system and pretreatment to achieve optimal bonding to MIH.

Background and objectives: An area on the surface of tooth, apical to CEJ, where root caries are bound to occur undergoes clinically apparent dissolution of the calcified tissues. When dentin bonding agent is applied, it forms finger like resin tags protruding into patent dentin tubules When this resin penetrates the superficially demineralized collagen matrix, it forms a hybrid layerand prevents root surface lesions. Aim: To evaluate the caries protective effect of three recent DBAs on demineralized root surface. Materials and methods: 88 single rooted extracted human teeth were selected, root surfaces apical to CEJ were polished .Acid resistant nail paint wascoated over root surfaces exposing a rectangular window on the buccal surface. Three experimental groups made: Group 1 (XENO V PLUS), Group 2 (TETRIC N BOND), Group 3 (ONE COAT 7 UNIVERSAL). These groups were divided into 2 Subgroups (A and B). Adhesive was applied and light cured with air thinning. Over this second layer of adhesive was applied, light cured with /without air thinning. Each tooth samples were sliced. A light microscope was used to examine the caries like lesions.The statistical analysis was carried out. Results:All control group specimen showed lesions with a mean depth of 177.74μm. Subgroup B results were better than A and C. Summary and Conclusion: Demineralization on root surface can be impeded by DBA tested TetricN Bond which showed maximum cariesprotective effect Key words:Cementum, demineralization, dentine bonding agent

Background. Monolithic restorations made of translucent yttria-stabilized tetragonal zirconia polycrystal (Y-TZP) have become popular over the past few decades. However, whether aging affects the color and translucency of monolithic translucent Y-TZP is unclear. Objective. The aim of this systematic review and meta-analysis of in vitro studies was to evaluate the effects of aging on the color and translucency of monolithic translucent Y-TZP ceramics. Materials and Methods. This systematic review/meta-analysis was reported according to the PRISMA statement and registered in the OSF registries (https://osf.io/5qjmu). Four databases including Medline via the PubMed, Embase, and Web of Science databases and the Cochrane Library were searched using no publication year and language limits. The last search was executed on November 20, 2020. In vitro studies comparing the translucency and/or color of monolithic translucent Y-TZP ceramics before and after simulated aging were selected. Meta-analyses were performed using Review Manager software (version 5.3, Cochrane Collaboration, Oxford, UK) with random-effects models at a significance level of 0.05. A risk-of-bias assessment was also performed for the included studies. Results. Of the 188 potentially relevant studies, 13 were included in the systematic review. The hydrothermal aging duration ranged from 1 to 100 h at relatively similar temperatures (~134°C). In the general meta-analyses, the aged Y-TZP ceramics exhibited similar translucency parameter (TP), L ∗ , and b ∗ values compared with the nonaged controls ( P = .73 , P = .49 , and P = .62 , respectively). Moreover, there was a significant difference between the aged and nonaged Y-TZP ceramics in the a ∗ value ( P = .03 ; MD = − 0.26 ; 95% CI = − 0.51   to − 0.02 ), favoring the nonaged Y-TZP ceramics. The subgroup analyses showed that the duration of aging contributed to changes in the translucency and color of the Y-TZP ceramics. Conclusions. The optical properties of monolithic translucent Y-TZP ceramics were stable after hydrothermal aging at 134°C and 0.2 MPa for ≤20 h. Moreover, clinically unacceptable changes in the translucency and color of monolithic translucent Y-TZP ceramics were found after hydrothermal aging for >20 h.

OBJECTIVE: The purpose of this study was to evaluate the root surface roughness after instrumentation with hand curette and diamond-coated sonic and universal ultrasonic tips. MATERIALS AND METHODS: Forty root surfaces of human teeth were randomly assigned to four treatment groups: control group (without instrumentation), curette instrumentation, ultrasonic instrumentation with universal tip and sonic instrumentation with diamond-coated tip. Each sample was instrumented with fifteen strokes. Before and after instrumentation, surface roughness was measured. In addition, the root surface topography was examined after treatment under the scanning electron microscope. RESULTS: Significant statistical differences (p <0.05) were observed when comparing the control group (0.48±0.07mm) to the treated groups (hand - 1.246±0.279mm, ultrasonic - 1.468±0.177mm and sonic instrumentation - 1.576±0.20mm). The highest roughness was produced by diamond-coated sonic tip and by ultrasonic universal tip (p >0.05). CONCLUSION: The diamond-coated tip with sonic scaler instrumentation and ultrasonic instrumentation produce similar root surface roughness, higher than curette instrumentation.

The surface smoothness of composite restorations affects not only their esthetic appearance but also other properties. Thus, rough surfaces can lead to staining, plaque accumulation, gingival irritation, recurrent caries, abrasiveness, wear kinetics, and tactile perception by the patient. The aim of this study was to evaluate the influence of irrigation during the finishing and polishing of composite resin restorations. A systematic search of the PubMed, Cochrane Library, EMBASE, Web of Science, and Clinical Trials databases was conducted. Papers published up to 11 February 2021 were considered. The quality of each study was assessed using the modified Consolidated Standards of Reporting Trials checklist for reporting in vitro studies on dental materials. No clinical studies were identified. Six in vitro studies were included, reporting changes in physical and esthetic properties. After performing a methodological quality assessment of the studies, some limitations were identified, the main limitation being the heterogeneous methodology across studies. The evidence resulting from this systematic review did not favor either wet or dry finishing/polishing procedures. There is a clear need for well-designed studies focusing on the comparison of dry/wet finishing/polishing with standard protocols to evaluate the differences among different materials and methods.

Introduction: Coronal microleakage is one of the most important factors for the success and longevity of endodontically treated teeth. Aim: The aim of this study was to evaluate in vitro the coronal microleakage in carious and intact teeth obturated with lateral compaction of gutta-percha using the dye penetration method. Materials and Methods: Forty extracted human teeth were allocated to two groups: Group I consisted of 20 intact teeth (10 single-rooted and 10 multi-rooted) and Group II comprised 20 carious teeth (10 single-rooted and 10 multi-rooted). Root canal preparation was performed using the Step-back technique and copious irrigation with 2.5% NaOCl. All root canals were obturated using lateral compaction and AH Plus endodontic sealer. The teeth were coronally filled with either CAVIT only or the combination of GIC and CAVIT, coated with 2 layers of nail varnish and immersed in 50% AgNO3. Afterwards, the teeth were sectioned mesio-distally using a diamond disc and linear dye penetration was evaluated using a stereo loupe with a micrometer scale at x6 magnification. Results: The results showed dye penetration in 70% of teeth filled with CAVIT and in 10% of teeth filled with GIC+CAVIT. The difference was statistically significant (p<0.05). Conclusion: Based on the obtained results, it may be concluded that the least coronal microleakage was observed in intact teeth temporarily filled with the GIC+CAVIT combination.

Objective: An in vitro comparison of coronal microleakage of three root canal sealers was performed using the dye leakage method. Methodology: Ninety-eight single-rooted human teeth were instrumented and randomly allocated to two groups. In the first group, root canals were treated with EDTA to remove the smear layer. The roots in both groups were obturated using laterally condensed gutta-percha and Sealapex, AH-26 or Ketac-Endo as the sealer. The teeth were suspended in 1% methylene blue dye for 6 days. The teeth were then rinsed, sectioned perpendicular to the long axis and evaluated for coronal dye penetration. Results: In general, Ketac-Endo root canal sealer showed greater dye penetration than Sealapex and AH-26. A statistically significant difference in the coronal penetration of the dye was found in the group with intact smear layer. Conclusion: Highly significant differences between AH-26 and Ketac-Endo (p < 0.001) and Sealapex and Ketac Endo (p < 0.001) were found.
