Abstract Background: Hypoxia and inflammation tumor microenvironment (TME) play a crucial role in tumor development and progression. Although increased understanding of TME contributed to gastric cancer (GC) progression and prognosis, the direct interaction between macrophage and GC cells was not fully understood.Methods: Hypoxia and normoxia macrophage microarrays of GEO database was analyzed. The peripheral blood mononuclear cell acquired from the healthy volunteers. The expression of CXCL8 in GC tissues and cell lines was detected by quantitative reverse transcription PCR (qRT-PCR), western-blot, Elisa and immunofluorescence. Cell proliferation, migration, and invasion were evaluated by cell counting kit 8 (CCK8), colony formation, real-time imaging of cell migration and transwell. Luciferase reporter assays and chromatin immunoprecipitation were used to identify the interaction between transcription factor and target gene. Especially, a series of truncated and mutation reporter genes were applied to identify precise binding sites.The corresponding functions were verified in the complementation test and in vivo animal experiment.Results: Our results revealed that Hypoxia triggered macrophage secreted C-X-C Motif Chemokine Ligand 8 (CXCL8), which induced GC invasion and proliferation. This macrophage-induced GC progression was CXCL8 activated C-X-C Motif Chemokine Receptor 1/2 (CXCR1/2) on the GC cell membrane subsequently hyperactivated Janus kinase 1/ Signal transducer and activator of transcription 1 (JAK/STAT1) signaling pathway. Then, the transcription factor STAT1 directly led to the overexpression and secretion of Interleukin 10 (IL-10). Correspondingly, IL-10 induced the M2-type polarization of macrophages through the Nuclear Factor kappa B (NF-κB) pathway-dependent mechanism and continued to increase the expression and secretion of CXCL8 through the transcription factor Nuclear Factor Kappa B Subunit 1 (NFKB1, p50). It suggested a positive feedback loop between macrophage and GC. In clinical GC samples, increased CXCL8 predicted a patient's pessimistic outcome.Conclusion: Our work identified a positive feedback loop governing cancer cells and macrophage in GC that contributed to tumor progression and patient outcome.

Background/Aim: This study aimed to present a rare case of fibrous dysplasia (FD) in a healthy young adult man with a concomitant osteoporotic vertebral compression fracture. FD is a benign lesion of the bone characterized by replacement of the medullary component with fibro-osseous tissue that contains abnormally arranged trabeculae of immature woven bone. Recently it has been reported that several bone tumors including FD express the receptor activator of nuclear factor-kappa B (RANK) and its ligand (RANKL). Therefore, we hypothesized that FD contributed to osteoporosis, linked by the RANK-RANKL pathway of osteoclastogenesis. Case Report: We report the case of a healthy man with monostotic femoral fibrous dysplasia (FD) with concomitant 7th thoracic vertebra compression fracture due to osteoporosis [young adult mean (YAM) was 79% in bone mineral density (BMD)]. After curettage of the FD, artificial bone grafting in the cavity, and administration of alendronate sodium, BMD improved considerably within 9 months. FD is a benign bone condition in which abnormal fibrous tissue replaces normal bone. The axis of the receptor activator of nuclear factor-kappa B (RANK) and its ligand (RANKL) has been implicated in osteoporosis pathogenesis. RANKL immunohistochemical staining was performed, and strong staining of stromal cells was observed compared to other FD cases that showed weak to moderate staining. Conclusion: The presence of FD might have contributed to the low BMD due to the RANK-RANKL axis acting as osteoclastogenesis stimulator.

Yukmijihwang-tang (YJ) has been used to treat diabetes mellitus, renal disorders, and cognitive impairment in traditional medicine. This study aimed to evaluate the anti-osteoporotic effect of YJ on ovariectomy (OVX)-induced bone loss in a rat and receptor activator of nuclear factor kappa-B ligand (RANKL)-mediated osteoclast differentiation in bone marrow macrophages (BMMs). YJ reduced the formation of tartrate-resistant acid phosphatase (TRAP)-positive multinucleated cells (MNCs) in an osteoclast/osteoblast co-culture system by regulating the ratio of RANKL/osteoprotegerin (OPG) by osteoblasts. Overall, YJ reduced TRAP-positive cell formation and TRAP activity and F-actin ring formation. Analysis of the underlying mechanisms indicated that YJ inhibited the activation of the nuclear factor of activated T cell cytoplasmic 1 (NFATc1) and c-Fos, resulting in the suppression of osteoclast differentiation-related genes such as TRAP, ATPase, H+ transporting, lysosomal 38 kDa, V0 subunit d2, osteoclast-associated receptor, osteoclast-stimulatory transmembrane protein, dendritic cell-specific transmembrane protein, matrix metalloproteinase-9, cathepsin K, and calcitonin receptor. YJ also inhibited the nuclear translocation of NFATc1. Additionally, YJ markedly inhibited RANKL-induced phosphorylation of signaling pathways activated in the early stages of osteoclast differentiation including the p38, JNK, ERK, and NF-κB. Consistent with these in vitro results, the YJ-administered group showed considerably attenuated bone loss in the OVX-mediated rat model. These results provide promising evidence for the potential novel therapeutic application of YJ for bone diseases such as osteoporosis.

Hepatocellular carcinoma (HCC) is a highly lethal type of malignancies that possesses great loss of life safety to human beings worldwide. However, few effective means of curing HCC exist and its specific molecular basis is still far from being fully elucidated. Activation of nuclear factor kappa B (NF-κB), which is often observed in HCC, is considered to play a significant part in hepatocarcinogenesis and development. The emergence of regulatory non-coding RNAs (ncRNAs), particularly microRNAs (miRNAs) and long non-coding RNAs (lncRNAs), is a defining advance in cancer biology, and related research in this branch has yielded many diagnostic and therapeutic opportunities. Recent studies have suggested that regulatory ncRNAs act as inhibitors or activators in the initiation and progression of HCC by targeting components of NF-κB signaling or regulating NF-κB activity. In this review, we attach importance to the role and function of regulatory ncRNAs in NF-κB signaling of HCC and NF-κB-associated chemoresistance in HCC, then propose future research directions and challenges of regulatory ncRNAs mediated-regulation of NF-κB pathway in HCC.

BACKGROUND: Nasopharyngeal carcinoma (NPC) is a malignancy induced by the mutation of the transcription factors nuclear factor-kappa B (NF-kB) and peroxisome proliferator-activated receptor-gamma (PPAR-gamma). There was no known of the study about the association and targeted therapy of NF-kB and PPAR-gamma-induced NPC. AIM: This study analyzed and compared the proportion of NF-kB and PPAR-gamma and its association with the clinical characteristic of various NPC patients. METHODS: This was a cross-sectional study and conducted in Adam Malik General Hospital. The samples were paraffin block tissue obtained from 58 NPC patients and underwent immunohistochemistry staining for NF-kB or PPAR-gamma overexpression in March–November 2018. Determination of overexpression was based on the immunoreactive score. The association of NF-kB or PPAR-gamma overexpression with the clinical characteristics of the patients was analyzed using Fisher’s exact test. RESULTS: This study showed a significant increase of NF-kB and PPAR-gamma (p < 0.05). Male was found common than women (3.46:1) with non-keratinizing squamous cell carcinoma as the most common form of NPC (75.9%) and the 41–60 years old is the most common age (56.9%). Overexpression of NF-kB and PPAR-gamma was found mostly in T3-T4 (66.0%; 69.6%), N+ (92.5%; 91.3%), and clinical Stage IV (67.9%; 73.9%), respectively. CONCLUSION: The number of samples overexpressed was proportional to the clinical stage of NPC. This study provides an insight into the relationship of NF-kB and PPAR-gamma to NPC, suggesting their role in the development of malignancy.

Η παρεντερική διατροφή αποτελεί αναπόσπαστο μέρος της διατροφικής υποστήριξης των νεογνών που νοσούν σε κρίσιμη κατάσταση, στις μονάδες εντατικής θεραπείας. Ως εκ τούτου, η επάρκεια, η αποτελεσματικότητα και η συνοχή της υποστήριξης με παρεντερική διατροφή είναι μεταξύ των κύριων προβληματισμών των ιατρών και των φαρμακοποιών. Η χορήγηση των συνολικών θρεπτικών συστατικών της παρεντερικής διατροφής με βάση τη χρήση ενός συστήματος υποστήριξης αποφάσεων έχει μεγάλη σημασία στην κλινική πρακτική. Ένα σύστημα υποστήριξης κλινικών αποφάσεων αναπτύχθηκε και δοκιμάστηκε για την ακρίβειά του. Στη συνέχεια αξιολογήθηκε η εφαρμογή του στην κλινική πρακτική, η οποία είχε ως αποτέλεσμα την εξασφάλιση του διαλύματος της παρεντερικής διατροφής ως προς τις φαρμακευτικές συμβατότητες και τις κατευθυντήριες οδηγίες. Η ανάπτυξη του συγκεκριμένου συστήματος βασίστηκε στην ενσωμάτωση φαρμακευτικών και θεραπευτικών πρωτοκόλλων, καθώς και σε μία τυποποιημένη φόρμα συνταγογράφησης που δημιουργήθηκε και παράλληλα βελτίωσε την επικοινωνία μεταξύ ιατρών και φαρμακοποιών. Για τη συνταγογράφηση παρεντερικής διατροφής χρησιμοποιήθηκε το σύστημα υποστήριξης κλινικών αποφάσεων στα πρόωρα νεογνά και αξιολογήθηκε η παροχή ω-3 πολυακόρεστων λιπαρών οξέων [συγκεκριμένα γαλάκτωμα λιπιδίων εμπλουτισμένο με ω-3 πολυακόρεστα λιπαρά οξέα (PUFA) (Smof) συγκριτικά με γαλάκτωμα λιπιδίων με βάση το σογιέλαιο (Intralipid]]. Η μελέτη είχε ως στόχο την εκτίμηση των πρώιμων μεταβολών των βιοδεικτών οστικής μεταβολής και της επίδρασης των δύο παρεντερικών Γαλακτωμάτων Λίπους (ΓΛ). Η μελέτη παρείχε αποδείξεις που επιβεβαιώνουν υψηλό ποσοστό οστικού μεταβολισμού στα νεογνά πολύ χαμηλού βάρους γέννησης με επικράτηση της οστικής επαναρόφησης έναντι του οστικού σχηματισμού. Τα παρατηρούμενα χαμηλότερα επίπεδα μείωσης της οστεοπροτεγερίνης [osteoprotegerin (OPG)]/διαλυτού προσδέτη του υποδοχέα ενεργοποίησης του πυρηνικού παράγoντα-κΒ [soluble Receptor Activator of Nuclear factor Kappa-B Ligand (sRANKL)] στην ομάδα Smoflipid, υποδηλώνει ότι τα εμπλουτισμένα (με ω-3 πολυακόρεστα λιπαρά οξέα) παρεντερικά γαλακτώματα λιπιδίων μπορεί να βοηθήσουν στη μείωση της πρώιμης απώλειας οστού σε νεογνά πολύ χαμηλού βάρους γέννησης. Αυτή η παρατήρηση εκφράζει την ιδέα ότι, στην κλινική πρακτική, παρεντερικά γαλακτώματα λιπιδίων εμπλουτισμένα σε με ω-3 πολυακόρεστα λιπαρά οξέα μπορεί να είναι η προτιμότερη πηγή παρεντερικού λίπους για αυτά. Επιπλέον, τα επίπεδα βιοδεικτών οστικής ανακατασκευής μπορούν να είναι χρήσιμα στην κλινική πρακτική ως προς την παρακολούθηση της κατάστασης της υγείας των οστών των νεογνών πολύ χαμηλού βάρους γέννησης, καθώς μπορούν να μετρηθούν σε κάθε εργαστήριο χρησιμοποιώντας προσβάσιμες τεχνικές, οι οποίες απαιτούν ελάχιστες ποσότητες αίματος. Η ενδοφλέβια χορήγηση γαλακτωμάτων με βάση το σογιέλαιο και η υψηλή περιεκτικότητα σε λινελαϊκό οξύ μπορεί να οδηγήσει σε φλεγμονή και υπεροξείδωση των λιπιδίων σε πρόωρα νεογνά. Ένα δεύτερο μέρος αυτής της μελέτης είχε ως στόχο τη διερεύνηση των επιδράσεων που προκαλεί ένα ενδοφλέβιο γαλάκτωμα λιπιδίων εμπλουτισμένο σε τριγλυκεριδία μέσης αλύσου (MCT) και ω-3 πολυακόρεστα λιπαρά οξέα (PUFA) στο προφίλ λιπαρών οξέων πλάσματος και στην ιντερλευκίνη-6 ορού (IL-6) ) σε πρόωρα νεογνά. Σε αυτήν την τυχαιοποιημένη διπλά τυφλή μελέτη, 92 πρόωρα νεογνά (ηλικία κύησης <32 εβδομάδες, βάρος γέννησης <1500 g) κατανεμήθηκαν σε 2 ομάδες, ώστε να λάβουν είτε ένα ενδοφλέβιο γαλάκτωμα λιπιδίων εμπλουτισμένου σε MCT/ω-3 PUFA (ομάδα παρέμβασης), είτε ένα ενδοφλέβιο γαλάκτωμα λιπιδίων με βάση το σογιέλαιο (ομάδα ελέγχου). Τα επίπεδα των λιπαρών οξέων στο πλάσμα μετρήθηκαν κατά την έναρξη (ημέρα 0) και την ημέρα 15 της παρεντερικής διατροφής με τη μέθοδο αέριας χρωματογραφίας – φασματομετρίας μάζας (GC-MS). Μετρήθηκε, επίσης, η IL-6 ορού με τη μέθοδο ELISA. Τα λιπαρά οξέα στο πλάσμα άλλαξαν σημαντικά με την πάροδο του χρόνου. Η ομάδα παρέμβασης είχε υψηλότερα επίπεδα συνολικών ω-3 PUFA, εικοσαπεντανοϊκού οξέος, καθώς και του μονοακόρεστου ελαϊκού οξέος σε σύγκριση με την ομάδα ελέγχου. Ταυτόχρονα, στην ομάδα παρέμβασης παρατηρήθηκε χαμηλότερη αναλογία ω-6/ω-3 PUFA, καθώς και μικρότερη συγκέντρωση ω-6 PUFA πλάσματος συγκριτικά με την ομάδα ελέγχου. Το λινελαϊκό οξύ ήταν υψηλότερο στην ομάδα με το ενδοφλέβιο γαλάκτωμα λιπιδίων με βάση το σογιέλαιο σε σύγκριση με την ομάδα του ενδοφλέβιου γαλακτώματος λιπιδίων εμπλουτισμένου σε MCT/ω-3 PUFA. Και οι δύο τύποι γαλακτώματος μείωσαν την ιντερλευκίνη-6 ορού σε σύγκριση με την αρχική τιμή, αλλά οι αλλαγές ήταν μη στατιστικά σημαντικές μεταξύ των ομάδων. Η χορήγηση ενδοφλέβιου γαλακτώματος λιπιδίων εμπλουτισμένου σε MCT/ω-3 PUFA σε πρόωρα νεογνά συνδέεται με ένα πιο ευνοϊκό προφίλ λιπαρών οξέων και ιντερλευκίνης-6, χαρακτηριστικά μιας εξασθενημένης φλεγμονώδους απόκρισης.
