AbstractIdiopathic pulmonary fibrosis (IPF) is a chronic lung condition with poor survival times. We previously published a genome-wide meta-analysis of IPF risk across three studies with independent replication of associated variants in two additional studies. To maximise power and to generate more accurate effect size estimates, we performed a genome-wide meta-analysis across all five studies included in the previous IPF risk GWAS. We utilised the distribution of effect sizes across the five studies to assess the replicability of the results and identified five robust novel genetic association signals implicating mTOR signalling, telomere maintenance and spindle assembly genes in IPF risk.

Asthma is a chronic inflammatory lung condition characterised by variable respiratory symptoms (wheeze, shortness of breath, cough, and chest tightness) and variable expiratory airflow limitation, usually associated with airway inflammation. It affects 1-in-11 people in the UK and is the cause of over 75 000 hospital admissions per year. Given its prevalence, and the fact that patients are mainly cared for in the community, this article aims to highlight the need for a thorough annual asthma review and what that review entails. When carried out effectively, an asthma review will improve quality of life for those living with the condition, reduce their likelihood of hospital admission and reduce the cost to the NHS as a whole.

Spontaneous bilateral pneumothorax is a rare condition occurring only in 1,3% of cases of spontaneous pneumothorax. Although spontaneous pneumothorax is recognized complication of COVID it is usually associated with severe cases of COVID pneumonia with massive lung involvement and a high level of inflammatory cytokines – so called "inflammatory storm". Large percentage of this patients requires life support with mechanical ventilation and pneumothorax is usually accompanied with pneumomediastinum. Nevertheless occurrence of spontaneous pneumothorax in non-intubated patients is very low. Therefore the presented case is exceptional in two ways – spontaneous pneumothorax occurred in non-intubated patient and is presented bilaterally. Early diagnosis of spontaneous bilateral pneumothorax can pose a diagnostic dilemma as it mimics progression of lung involvement in the course of COVID, pulmonary embolism or myocardial infarction. In our case apart from COVID pneumonia, patient had typical clinical presentation of myocardial infarction and known coronary artery disease, which initially mislead clinicians toward diagnosis of myocardial infarction (MI). However after exclusion of MI, chest radiograph was requested which revealed bilateral pneumothorax. The exact mechanism of pneumothorax development in non-intubated patients is not clear. Lung inflammation in COVID causes formation of pneumatocele, which can predispose to spontaneous pneumothorax. It is important to note that our patient developed pneumothorax without evident pneumatocele, underlying lung condition or history of trauma. This case shows that bilateral pneumothorax in COVID-19 can develop also in patients with only mild clinical course of COVID pneumonia. Early diagnosis and proper management is essential and can prevent life-threatening complications.

Lung diseases can complicate pregnancy, but little is known about the experiences of pregnancy among women living with them. This survey aimed to understand the experiences of women with a lung condition before and during pregnancy, in childbirth and post-partum. The survey was translated into 9 languages and hosted online between March and May 2018. This paper reports on 327 women who had asthma, cystic fibrosis (CF), lymphangioleiomyomatosis (LAM) and sarcoidosis as a sole or primary lung condition. Women with CF and LAM were most likely to report that their condition influenced their decision to have children. Those with CF and LAM who did become pregnant reported greater satisfaction with their healthcare during pregnancy and gave more consideration to factors such as location and type of birth; they were also more concerned about the impact of the pregnancy on their health than women with other diseases. Women with sarcoidosis reported receiving conflicting advice as to both the impact of their condition on pregnancy and how becoming pregnant might impact their health. Women with asthma reported not always being able to access the information they needed from healthcare professionals. The results suggest that healthcare providers should be having dialogues with affected women early on, from before conception, throughout the pregnancy and after giving birth, and training should be provided to healthcare staff to equip them with the information they need to do this.

A 52-year-old man was re-admitted two weeks after recovering from severe COVD-19 following a 3-days history of cough and worsening shortness of breath. The chest radiograph showed a large right-sided pneumothorax. The first attempt at drainage, performed through a large bored tube, failed. Due to the large dimension of the pneumothorax, and the lung condition (extensive consolidation and diffuse bullous dystrophies), the only thoracic surgical approach prospected was a pneumonectomy. Willing to preserve the lung, the pulmonology team attempted a multi-phase medical-oriented strategy based on medical thoracoscopy. Therefore, the patient underwent 5 chest tube insertions, 2 talc pleurodesis, and an intrapleural blood patch. Air leakage resolution was progressively achieved, and the patient became asymptomatic.             We strongly encourage a medical thoracoscopic approach for the patient presenting with recurrent pneumothorax in order to ensure complete lung re-expansion and preserve lung parenchyma.

ABSTRAKPenyakit COVID-19 dapat timbul karena berbagai faktor sebab dan akibat, sehingga penyakit ini memiliki efek buruk bagi penderita. Pencitraan CT-Scan memiliki keunggulan dalam memproyeksikan kondisi paru-paru pasien penderita, sehingga dapat membantu dalam mendeteksi tingkat keparahan penyakit. Dalam studi ini, penelitian dilakukan untuk mendeteksi penyakit COVID-19 melalui citra CT-Scan menggunakan metode Filter Gabor dan Convolutional Neural Networks (CNN) dengan Hyperparameter Tuning. Data yang digunakan yaitu citra CT-Scan SARSCoV-2 berjumlah 2481 gambar. Sebelum melatih model, dilakukan preprocessing data, seperti pelabelan, pengubahan ukuran, dan augmentasi gambar. Pengujian Model dilakukan dengan beberapa skenario uji. Hasil terbaik diperoleh pada skenario untuk model Filter Gabor dan CNN dengan Hyperparameter Tuning mendapatkan akurasi sebesar 97,9% dan AUC sebesar 99% dibandingkan dengan model tanpa Hyperparameter Tuning dan Filter Gabor.Kata kunci: COVID-19, CNN, Filter Gabor, Hyperparameter Tuning, COVID-19 Classification ABSTRACTCOVID-19 disease can arise due to various causal and causal factors, so it has an adverse effect on patients. CT-Scan imaging has an advantage in projecting the lung condition of patients with the patient, so it can help in detecting the severity of the disease. In this study, research was conducted to detect COVID-19 disease through CT-Scan imagery using Gabor Filter method and Convolutional Neural Networks (CNN) with Hyperparameter Tuning. The data used is CT-Scan SARSCoV-2 imagery amounting to 2481 images. Before training the model, preprocessing data is performed, such as labeling, resizing, and augmentation of images. Model testing is performed with multiple test scenarios. The best results were obtained in scenarios for The Gabor Filter model and CNN with Hyperparameter Tuning getting 97.9% accuracy and AUC by 99% compared to models without Hyperparameter Tuning and Gabor Filter.Keywords: COVID-19, CNN, Filter Gabor, Hyperparameter Tuning, COVID-19 Classification
