The eloquent statement on the status of Negro medical care and education in the United States by the eminent anatomist, Dr. W. Montague Cobb (Brown America's Medical Diaspora: A Paradox of Democracy, in The Pediatrician and The Public, Pediatrics 3:854, 1949) requires the attention of all physicians interested in the distribution of medical care. Although pediatricians cannot begin to assume responsibility for this entire problem, it is possible to demonstrate leadership in the same manner in which the Academy study of infant and child health services provided leadership to the profession and the public. We refer specifically to an extension of training facilities in pediatrics for Negro physicians. Certainly 15 certified Negro pediatricians in a country with 14,000,000 Negro people represents a serious discrepancy in the distribution of training facilities. Admittedly most of the problem has its origin in the distribution of training facilities for undergraduate students and the basic problems responsible for this situation. However, we have observed—as has Dr. Cobb—that many Negro physicians desiring training in pediatrics (as well as other specialties) are discouraged from applying for training because of what seems to be a dearth of positions open to them. It has been our impression, however, that many centers would consider Negroes for training appointments if qualified applicants applied. Would it not be advisable, therefore, for the American Board of Pediatrics to circularize the approved training centers in pediatrics in order to establish a roster of those centers which would consider Negro applicants for training positions?

A physician may face substantial penalties for refusing to treat a patient with HIV. Even the referral of a patient with HIV to a specialist in the disease may constitute a discriminatory act. The law provides certain guidelines which, if followed, may help keep the physician out of the courtroom.

Background Medical crowdfunding has emerged as a growing field for fundraising opportunities. Some environmental trends have driven the emergence of campaigns to raise funds for medical care. These trends include lack of medical insurance, economic backlash following the 2008 financial collapse, and shortcomings of health care regulations. Objective Research regarding crowdfunding campaign use, reasons, and effects on the provision of medical care and individual relationships in health systems is limited. This study aimed to explore the nature and dimensions of the phenomenon of medical crowdfunding using a visual analytics approach and data crawled from the GoFundMe crowdfunding platform in 2019. We aimed to explore and identify the factors that contribute to a successful campaign. Methods This data-driven study used a visual analytics approach. It focused on descriptive analytics to obtain a panoramic insight into medical projects funded through the GoFundMe crowdfunding platform. Results This study highlighted the relevance of positioning the campaign for fundraising. In terms of motivating donors, it appears that people are typically more generous in contributing to campaigns for children rather than those for adults. The results emphasized the differing dynamics that a picture posted in the campaign brings to the potential for medical crowdfunding. In terms of donor’s motivation, the results show that a picture depicting the pediatric patient by himself or herself is the most effective. In addition, a picture depicting the current medical condition of the patient as severe is more effective than one depicting relative normalcy in the condition. This study also drew attention to the optimum length of the title. Finally, an interesting trend in the trajectory of donations is that the average amount of a donation decreases with an increase in the number of donors. This indicates that the first donors tend to be the most generous. Conclusions This study examines the relationship between social media, the characteristics of a campaign, and the potential for fundraising. Its analysis of medical crowdfunding campaigns across the states offers a window into the status of the country’s health care affordability. This study shows the nurturing role that social media can play in the domain of medical crowdfunding. In addition, it discusses the drivers of a successful fundraising campaign with respect to the GoFundMe platform.

The aim is to identify parents’ (legal representatives’) satisfaction with the quality of medical care provided to their disabled children to improve this type of service further. Materials and methods. A sociological survey was conducted of 506 legal representatives of minors (aged from birth to 17 years) with the status of a disabled child. The study design is single-center, non-randomized, uncontrolled. Results. An analysis of the living conditions of a disabled child in the family, the parents’ assessment of his health status, the problems arising in the registration of disability, in the provision of medical and rehabilitation assistance, and issues of medical and social support made it possible to determine the position of this part of the child population in current legal and medical and social conditions. The main problems were the collection of a large number of documents when registering a disability, a long wait for the day of examination, the remoteness of the medical and social examination bureau, the lack of specialist doctors, problems with obtaining subsidized drugs, the lack of consideration of the individual needs of the child when conducting unique rehabilitation programs, the need to apply at the same time to various organizations and departments, violation of rights in the provision of medical services to a disabled child. Conclusion. The obtained information is very significant for further improving the provision of medical and social assistance to disabled children and children with disabilities. The main task today is to develop mechanisms for implementing the declared rights and freedoms of persons with disabilities, the obligations assumed by the State concerning them.

In recent years the language and logic of medical care have moved from providing medical services to marketing product lines. Analysis in this article examines this task transformation and its implications for transformation of the nonprofit sector and of the state. The authors argue that these transformations are essential explanatory elements to account for the origins of medical services in the nonprofit sector, the early exclusion of capitalist organizations from hospital care, and the changes that fostered corporate entry. To wit, medical care tasks have undergone a two-stage transformation. The first transformation changed open-ended, ill-defined services with uncertain funding into more highly organized and codified services with stable funding, attracting both capitalist enterprises and capitalist logic into the nonprofit sector. The second transformation standardized medical care tasks into product lines, a process that also challenged the status of the nonprofit organizations performing these tasks. In an analysis of the second transformation, the authors argue that this challenge is in the process of turning back upon itself, undermining the conditions that fostered capitalist entry into medical care delivery in the first place.

IS THERE to be another EMIC program? This question is heard again in Washington, in various state health departments, in gatherings of interested persons, and among those physicians primarily concerned: pediatricians, obstetricians and general practitioners. It is not our purpose to argue for or against an EMIC rebirth, but rather to report the status of the proposals which have already appeared, bearing in mind that the scene may have shifted much or little between the present date (May 28) and the appearance of this column. As the tempo of the Korean War and war mobilization increased during the early months of this year, it was hardly surprising that consideration should again be given to medical care for the wives and infants of service men. In February there were informal discussions between the Children's Bureau staff and the AMA. In early March, the Committee on Maternal and Child Care of the AMA Council on Medical Services met and discussed the possibility of revival of EMIC. Those attending this meeting included Dr. Lull and others from the AMA, Miss Lenroot, Dr. Blain and Dr. Daley from the Children's Bureau and Drs. Barba, Webb and Christopherson from the Academy. A questionnaire to elicit comments on the EMIC had been sent out to District and State Chairmen of the Academy, to all State Medical Societies, to individual pediatricians and obstetricians and to others who had been vocal about the old EMIC.

AbstractIntroduction:In December 2003, the residents of Bam, Iran experienced an earthquake that measured 6.6 on the Richter scale and destroyed more than 90% of the city.Problem:The purpose of this study was to assess the status of the rescue, evacuation, and transportation of the casualties during the early stages following the earthquake.Methods:A cross-sectional study of 185 casualties who were transferred to and hospitalized in the university hospital during the first week period following the earthquake was conducted. Information regarding different places of settlement after being removed from the rubble, initial medical care, and the means of transportation was obtained by reviewing medical records and interviewing the victims.Results:The mean value of the duration of times taken for the first rescuers to reach the scene and remove the casualties from the rubble was 1.7 ±2.7 and 0.9 ±1.1 hours, respectively. Sixty-nine (37.7%) of the patients stayed within the area immediately surrounding their home for average times of 8 ±10 hours. The majority of casualties (57.6%) were transferred manually to a first place of settlement; 45.8% were taken to a second place of settlement using blankets. Of the patients studied, 159 (85.9%) did not receive any basic medical care at the first place and intravenous fluid therapy was the most common treatment provided for 24 (13%) patients at the second place of settlement. Patients received medical care at the first place of settlement for a mean time of 16.8 ±13.5 hours after escaping the rubble.Conclusions:These findings indicate that the emergency medical service system in Bam was destroyed and not able to respond adequately. In order to reduce the negative effects of such disasters in the future, there is an essential need for a comprehensive disaster management plan and improvement of hospital structures, healthcare facilities, and communication between the different governmental departments for better coordination and planning.
