Abstract Hypothesis Patients with hemianopic field defects (HFD) might benefit from reading text in vertical orientation if they place the text in the seeing hemifield along the vertical midline. Methods We assigned 21 patients with HFD randomly to either vertical or horizontal reading training. They trained reading single lines of texts from a computer screen at home for 2 × 30 min/day, 5 days/week, for 4 weeks. The main outcome variable was reading speed (RS) during reading standardized paragraphs of printed text (IReST) aloud. RS was assessed before training (T1), directly after training (T2) and 4 weeks later (T3). Quality of life (QoL) was assessed by Impact of Visual Impairment (IVI) questionnaire. Results Vertical training improved RS in the vertical direction significantly. Only patients with right HFD benefited. Horizontal training improved RS in horizontal diection significantly, but much more in patients with left than in those with right HFD. Both effects remained stable at T3. RS during training at the computer improved highly significantly and correlated strongly with RS of printed text (Pearson r= > 0.9). QoL: Vertical training showed a statistically significant improvement in the complete IVI-score, patients with right HFD in the emotional IVI-score. Conclusions The improvements of RS were specific for the training. The stable effect indicates that the patients can apply the newly learned strategies to everyday life. The side of the HFD plays an essential role: Left-HFD patients benefitted from horizontal training, right-HFD patients from vertical training. However, the vertical RS did not reach the level of horizontal RS. The study was registered in the German Clinical Trials register (DRKS-ID: DRKS00018843).

This article introduces Cognitive Health Counseling 40+ (CH.CO40+), an individualized intervention that is conceptually based on the orchestration model of quality-of-life management ( Martin & Kliegel, 2010 ) and aims at improving satisfaction with cognitive health in adults aged 40 years and older. We describe the theoretically deduced characteristics of CH.CO40+, its target group, its multifactorial nature, its individualization, the application of subjective and objective measures, the role of participants as agents of change, and the rationale for choosing participants’ satisfaction with their cognitive health as main outcome variable. A pilot phase with 15 middle-aged and six older adults suggests that CH.CO40+ attracts, and may be particularly suitable for, subjective memory complainers. Implications of the pilot data for the further development of the intervention are discussed.

PurposeThe purpose of this study was to identify the long-term developmental effects of a NICU music therapy intervention, Multimodal Neurologic Enhancement, provided to preterm infants in the NICU.DesignProspective randomized controlled study with one control group and one experimental group.SampleParticipants were medically stable preterm infants with a birth age of 31 and 6/7 weeks or less, admitted to a level-III NICU. A total of 84 participants were enrolled, and 48 completed the study.Main Outcome VariablePost-discharge developmental scores on the Mullen Scales of Early Learning: AGS Edition.ResultsThe experimental group performed significantly better than the control group on the Visual Reception and Early Learning Composite scores.

Purpose: To explore the relationships between stress, coping, duration of tracheostomy, and quality of life (QOL) of parental caregivers who care for a child with a tracheostomy at home.Design: A cross-sectional correlational design was used to study parents who care for a child with a tracheostomy at home. Family Inventory of Life Events, Family Crisis Oriented Personal Evaluation Scale, and Psychological General Well-Being Index were used to measure stress, coping, and QOL, respectively. Data were collected using both online and paper-pencil format.Main Outcome Variable: quality of life.Results: Parents who care for their infants/toddlers with a tracheostomy at home were found to be in moderate distress. Mean age of the participants was 33 years (N = 71), and the average duration of the child’s tracheotomy was 18.22 (SD = 9.59) months. Multivariate analysis showed a significant inverse relationship between QOL and stress. QOL was significantly associated with coping, B = 19.91, β = .43, p<.001.

Background. Expressed emotion (EE) is a measure that has been used to assess the quality of the relationship between patient and their key relative. It has been shown to be strongly predictive of clinical outcome in a range of psychiatric and medical disorders. This study investigated the effect of EE on treatment outcome in chronic post-traumatic stress disorder (PTSD).Methods. A prospective design was adopted. The key relatives of 31 PTSD patients participating in a treatment trial comparing imaginal exposure with cognitive therapy were interviewed and rated on EE prior to treatment allocation. The effect of EE on post-treatment clinical outcomes was assessed.Results. Sixteen patients (52%) had high EE and 15 (48%) low EE relatives. Patients with high EE relatives showed lesser change scores on the main outcome variable of the trial, the total CAPS score, and on all the secondary outcome variables than those with low EE relatives. Using different multiple regression models the EE scales of criticism and hostility predicted just under 20% of the outcome variance. These two scales were highly correlated and criticism marginally predicted the greatest variance (19·7%).Conclusions. The results highlight the importance of the quality of the patient's social environment in influencing their response to cognitive and behavioural treatments.

This study investigated reading comprehension, reading speed, and the quality of eye movements while reading on an iPad, as compared to printed text. 31 visually-normal subjects were enrolled. Two of the passages were read from the Visagraph standardized text on iPad and Print. Eye movement characteristics and comprehension were evaluated. Mean (SD) fixation duration was significantly longer with the iPad at 270 ms (40) compared to the printed text (p=0.04) at 260 ms (40). Subjects’ mean reading rates were significantly lower on the iPad at 294 words per minute (wpm) than the printed text at 318 wpm (p=0.03). The mean (SD) overall reading duration was significantly (p=0.02) slower on the iPad that took 31 s (9.3) than the printed text at 28 s (8.0). Overall reading performance is lower with an iPad than printed text in normal individuals. These findings might be more consequential in children and adult slower readers when they read using iPads.

As a person read, the eyes move to extract information from the printed text. Reading became disturbed when obstructed by the scotoma. The effect of reading and eye movement were investigated between different locations of simulated central vision scotoma. The time to read the text and the eye movements were recorded and tracked among eleven participants. The reading speed showed a significant difference at different locations of central scotoma simulation. The presence of central scotoma affects the quality of reading as the eye moved slowly during reading in order to comprehend the text.

Introduction: The treatment of avascular necrosis of the femoral head (AVNFH) is based on invasive (e.g., core decompression) and non-invasive methods (e.g., hyperbaric oxygen therapy – HBO2). The purpose of the present study is to evaluate the effect of HBO2 on the quality of life (QoL) of patients with AVNFH. Methods: This was a prospective observational non-controlled study of patients with AVNFH treated by HBO2. It was conducted, with the use of Steinberg scale, on 73 patients with AVNFH Stage I or II who were treated with HBO2. Patients’ QoL was assessed with EuroQol-5D-5L (EQ), Harris Hip Score (mHHS), MAHORN (MHOT), and VAS, in three different phases: before HBO2; after the completion of the first phase (20 HBO2 sessions, up to two months); and after the completion of the second phase (20 HBO2 sessions, up to two months after the first phase). A reassessment was made on the completion of each phase. Ratings were also made after the completion of each phase, over the first five months of follow-up. Results: All 73 patients (67.1% males, 32.9% females, mean age: 40.34, SD ± 9.99) participated in the study. Steinberg scale, mean EQ (F (1, 57) = 25.18, η2 = .306 and F (1, 43) = 43.402, η2 = .502); mHHS (F (1, 61) = 67.13, η2 = .524) and F (1, 43) = 31.84, η2 = .425); MHOT (F (1, 61) = 11.68, η2 = .161) and F (1, 43) = 98.01, η2 = .695); and VAS (F (1, 53) = 24.11, η2 = .313) and F (1, 39) = 45.61, η2 = .539), improved between the first and second measurements and between the second and third measurement accordingly (p < .01). Conclusions: HBO2 treatment does not induce alteration of quality of life and is well tolerated and accepted by patients.

Purpose: Biofeedback training (BT) was adapted to idiopathic infantile nystagmus syndrome (IINS) cases to enhance visual functions and quality of life (QoL). Methods: 10 patients (age 9 ± 3.2 years) treated with the audio-visual BT module of the MAIA microperimeter (Centervue, Padova, Italy) were assessed in two baseline visits and 1week post-BT (BT 80 min in total). The outcomes were distance and near binocular best corrected visual acuity (BBCVA), fixation stability, reading speed, contrast sensitivity, stereopsis and Children’s Visual Function Questionnaire. One-way repeated measured ANOVA and paired t-tests were used. Results: Distance BBCVA improved from 0.46 ± 0.21 and 0.43 ± 0.18 pre-BT to 0.33 ± 0.2 logMAR post-BT ( F (2,27) = 13.75, p = 0.0002). Post-BT was better than baseline ( p = 0.0001) and pre-BT ( p = 0.001). Near BBCVA improved from 0.23 ± 0.09 and 0.21 ± 0.14 pre-BT to 0.04 ± 0.08 post-BT (F (2,27) = 22.12, p = 0.000014), post-BT was better than baseline ( p = 0.0001) and pre-BT ( p = 0.0006). Stereopsis improved from 283 ± 338″ to 39 ± 32.2″ ( p = 0.04), contrast sensitivity from 0.26 ± 0.17 to 0.08 ± 0.12 log units ( p = 0.01), and reading speed improved from 74.7 ± 51.2 wpm to 104.7 ± 53.6 wpm ( p = 0.0006). Fixation stability improved from 33.6 ± 28.1 to 14.3 ± 10.1 sq. QoL increased from 23.8 ± 2.2 to 26.3 ± 2.3 units ( p = 0.001). Conclusion: BT benefited all visual functions and QoL in this pilot study, heralding a new possibility for Low Vision Rehabilitation in IINS.
