Abstract Exposure to per- and polyfluoroalkyl substances (PFAS), ubiquitous persistent environmental contaminants, has led to substantial global concern due to their potential environmental and human health effects. Several epidemiological studies have assessed the possible association between PFAS exposure and risk of metabolic syndrome (MetS), however, the results are ambiguous. The aim of this study was to assess the current human epidemiologic evidence on the association between exposure to PFAS and MetS. We performed a systematic search strategy using three electronic databases (PubMed, Scopus, and Web of Science) for relevant studies concerning the associations of PFAS with MetS and its clinical relevance from inception until January 2021. We undertook meta-analyses where there were five or more studies with exposure and outcomes assessments that were reasonably comparable. The pooled odd ratios (ORs) were calculated using random effects models and heterogeneity among studies was assessed by I2 index and Q test. A total of 12 cross-sectional studies (10 studies on the general population and two studies in the occupational settings) investigated the association between PFAS exposure and MetS. We pooled data from seven studies on the general population for perfluorooctanoic acid (PFOA) and perfluorooctanesulfonate (PFOS) and five studies for perfluorohexanesulfonate (PFHxS) and perfluorononanoic acid (PFNA). Predominately, most studies reported no statistically significant association between concentrations of PFAS and MetS. In the meta-analysis, the overall measure of effect was not statistically significant, showing no evidence of an association between concentrations of PFOA, PFOS, PFNA, and PFHxS and the risk of MetS. Based on the results of the meta-analysis, current small body of evidence does not support association between PFAS and MetS. However, due to limited number of studies and substantial heterogeneity, results should be interpreted with caution. Further scrutinizing cohort studies are needed to evaluate the association between various and less well-known PFAS substances and their mixture with MetS and its components in both adults and children in different settings.

Abstract Background Schizophrenia is associated with cognitive dysfunction as well as cardiovascular disease (CVD). A central risk factor for CVD is the metabolic syndrome (MetS), which is of special concern in schizophrenia. The prevalence of MetS in U.S. patients with schizophrenia is higher versus general population (32.5% versus 23%). The prevalence of MetS and diabetes mellitus (DM) in those with schizophrenia double that of the general population. Adverse events of some antipsychotics used to treat schizophrenia include weight gain, obesity and other MetS complications, particularly abnormal glucose and lipid metabolism. Patients with schizophrenia have low rates of treatment for MetS and its components. Furthermore, components of MetS are risk factors for cognitive impairment and dementia in the general population. Cognitive impairment is a hallmark feature of schizophrenia, and the level of community functioning is strongly correlated with the degree of cognitive impairment. Given the importance of cognitive impairment in schizophrenia, the potential role of MetS in contributing to cognitive dysfunction is important. The objective of this post-hoc analysis was to examine cross-sectional relationships between metabolic syndrome and cognitive performance in patients with schizophrenia treated with lurasidone or quetiapine XR for 6-weeks. Methods This post hoc analysis utilized data from 6-week, double-blind, placebo-controlled trial of patients with an acute exacerbation of schizophrenia who were randomized to fixed, once-daily oral doses of lurasidone 80 mg (LUR 80 n=125), lurasidone 160 mg (LUR 160, n=121), quetiapine XR 600 mg (QXR, n=120) and placebo (PBO, n=122). Patients with metabolic syndrome (MetS) at baseline were identified based on the National Cholesterol Education Program – Adult Treatment Panel III criteria (NCEP-ATP-III). Cognitive performance and functional capacity were assessed by the CogState computerized cognitive battery at baseline and 6 weeks. Results In the acute 6-week period, LUR160 was significantly superior on the cognitive composite score to PBO (p<0.05, d=0.37), while LUR 80 and QXR did not separate from PBO in the evaluable analysis sample (excluding subjects with non-evaluable composite Z-scores; n=267). A total of 45/267 (16.9%) patients met criteria for MetS. Treatment of patients with MetS group with LUR 160 (vs placebo) was associated with significantly greater week 6 improvement in the cognitive composite score (p<0.05, d=1.15), while LUR 80 and QXR did not separate from PBO. In the group without MetS, LUR dose groups and QXR did not differ from PBO in the CogState composite score. In the analysis of cognitive domain scores, LUR 80 was significantly superior to PBO on working memory in the group with MetS (p<0.05, d=1.01) and reasoning/problem solving in the group without MetS (p<0.05, d=0.46). LUR 160 was significantly superior to PBO on processing speed in the group with MetS (p<0.05, d=1.20), reasoning/problem solving (p<0.05, d=0.45) and social cognition (p<0.05, d=0.46) in the group without MetS. QXR was significantly superior to PBO on verbal learning and reasoning/problem solving in the group without MetS (p<0.05, d=0.38 and p<0.05, d=0.37, respectively). Discussion Patients with MetS responded to treatment with lurasidone with significantly improved CogState composite and domain scores. No improvement on cognition was seen in patients with MetS treated with QXR. Evaluation of potential for MetS and improvements in cognition should be important elements in the algorithm of optimization of treatment in patients with schizophrenia.

Abstract Background Metabolic syndrome is identified as a risk factor for the development of several systemic cancers, but its frequency among patients with glioblastoma and its association with clinical outcomes have yet to be determined. The aim of this study was to investigate metabolic syndrome as a risk factor for and affecting survival in glioblastoma patients. Methods A retrospective cohort study, consisting of patients with diagnoses at a single institution between 2007 and 2013, was conducted. Clinical records were reviewed, and clinical and laboratory data pertaining to 5 metabolic criteria were extrapolated. Overall survival was determined by time from initial surgical diagnosis to date of death or last follow-up. Results The frequency of metabolic syndrome among patients diagnosed with glioblastoma was slightly greater than the frequency of metabolic syndrome among the general population. Within a subset of patients (n = 91) receiving the full schedule of concurrent radiation and temozolomide and adjuvant temozolomide, median overall survival was significantly shorter for patients with metabolic syndrome compared with those without. In addition, the presence of all 5 elements of the metabolic syndrome resulted in significantly decreased median survival in these patients. Conclusions We identified the metabolic syndrome at a slightly higher frequency in patients with diagnosed glioblastoma compared with the general population. In addition, metabolic syndrome with each of its individual components is associated with an overall worse prognosis in patients receiving the standard schedule of radiation and temozolomide after adjustment for age.

Osteocalcin (OCN) was potentially associated with inflammatory factors, so we explored the metabolic role in this association in general population. Our findings suggest that OCN was positively associated with IgG while inversely associated with C3, both of which were probably mediated by obesity. Moreover, serum OCN was inversely associated with hsCRP in men with impaired fasting glucose, hyperglycemia, or metabolic syndrome, while its association with IgE was significantly observed in men with a normal metabolic profile.

Psoriatic arthritis (PsA) is associated with serious comorbidities such as increased cardiovascular risk, hypertension, depression, and reduced quality of life. Patients with psoriasis have been observed to have an increased incidence of metabolic syndrome compared with the general population; recently, this has also been observed in patients with PsA. This review focuses on the comorbidities associated with PsA, with an emphasis on risks of coronary artery disease and metabolic syndrome. We also discuss the development of a comprehensive approach for the management of comorbidities of PsA. The review summarizes a presentation at the 2010 annual meeting of GRAPPA (Group for Research and Assessment of Psoriasis and Psoriatic Arthritis).
