AbstractPomegranate (Punica granatum), a polyphenol-rich fruit, has been suggested to reduce cardiovascular risk due to its antioxidant properties. Hypertension and obesity are the most preventable cardiovascular risk factors. Few studies on blood pressure and/or body-weight status have been conducted in human subjects. Previous investigations have tended to focus on pomegranate juice. The aim of the present study was to investigate the effect of pomegranate extract (PE) on blood pressure and anthropometric measures in adults with no symptomatic disease. A total of fifty-five participants enrolled in a randomised double-blinded placebo-controlled clinical trial where they were assigned to either PE capsules or placebo capsules for 8 weeks. Blood pressure, body weight, waist circumference, waist:hip ratio (WHR) and body composition (lean body mass, body fat) were measured at baseline, week 4 and week 8. Results showed a significant decrease in diastolic blood pressure after 8 weeks (by 2·79 (sd 5·32) mmHg; P < 0·05), while the decrease in systolic blood pressure did not reach statistical significance (2·57 (sd 7·4) mmHg; P > 0·05). Body fat percentage, lean body mass, waist circumference and WHR did not significantly differ between groups at the end of the intervention. Results suggest that PE may reduce blood pressure and possibly prevent hypertension in the normotensive population. Further large trials are required to elucidate this effect.

Background: Barriers to physical activity for obese women include overheating, sweating, fatigue, exhaustion, and rapid heart rate. Adipose tissue acts as a thermal insulator, promoting a greater heat load on the nonfat tissues, reducing heat tolerance; exercise causes a rise in body temperature with an inability to dissipate heat contributing to reduced exercise tolerance. With difficulties of thermoregulation in the sedentary obese population, the aspect of attenuating the discomfort thus associated may encourage continuation of exercise. A heat sink applied to palmar surfaces extracts heat and cools the venous blood, reducing thermal strain by enhancing the volume of cooled venous return. We hypothesized that palmar cooling using a rapid thermal exchange device (RTX) during exercise would attenuate the thermal discomfort of exercise of sedentary obese women, improving exercise tolerance. Methods: To examine whether palmar cooling would impact exercise tolerance in obese women, 24 healthy women aged 30–45 years, with no history of long term structured exercise, a body mass of 120–135% above ideal and/or BMI between 30 and 34.9 were recruited. Women were randomized into a cooling (RXT with 16°C water circulating) or a control (RTX with 37°C water circulating) group and attended 3 exercise sessions a week for 3-months (12 weeks). Each session was comprised of 10 min body weight exercises, 25–45 min treadmill walking at 70–85% HRR with the RTX device, and 10 min of core strengthening exercises. The performance marker was a 1.5 mi walk for time; conducted on the first and last days of the intervention. Mixed models were used to model each of the outcomes as a function of thermal strain, time and treatment with covariates of speed, heart rate, distance, and the interaction of the main effects included in the model. Results: Groups were matched at baseline for key variables (time for 1.5 mile walk test, resting and exercising heart rate [HR], blood pressure [BP], waist circumference [WC], body weight, body mass index [BMI]). Among the cooling group, time to complete the 1.5mile walk test was significantly faster (31.6 ± 2.3 vs. 24.6 ± 2.5 min, pre vs. post, P< 0.01). A greater average exercising HR was observed (136 vs. 154 bpm, pre vs. post, P <0.001), with a significant reduction in WC (41.8 ± 3.1 vs. 39.1 ± 2.2 inches, pre vs. post, P< 0.01) and resting BP (139/84 ± 124/70 mmHg, pre vs. post, P < 0.025). There were no significant differences observed in the control group. Conclusion: Results indicate that exercise tolerance in obese women improved with cooling during exercise, more so than those women who did not have cooling. An improvement in blood pressure, heart rate, waist circumference, and overall aerobic fitness was observed. These findings suggest that by reducing thermal discomfort during exercise, tolerance increases, thus improving cardiovascular parameters of obese women.

AbstractThe association between dietary patterns and metabolic cardiovascular risk factors has long been addressed but there is a lack of evidence towards the effects of the overall diet on the complex net of biological inter-relationships between risk factors. This study aimed to derive dietary patterns and examine their associations with metabolic cardiovascular risk factors following a theoretic model for the relationship between them. Participants included 417 adults of both sexes, enrolled to the cross-sectional population-based study performed in Brazil. Body weight, waist circumference, high-sensitivity C-reactive protein, blood pressure, total cholesterol:HDL-cholesterol ratio, TAG:HDL-cholesterol ratio, fasting plasma glucose and serum leptin were evaluated. Food consumption was assessed by two non-consecutive 24-h dietary recalls adjusted for the within-person variation of intake. A total of three dietary patterns were derived by exploratory structural equation modelling: ‘Traditional’, ‘Prudent’ and ‘Modern’. The ‘Traditional’ pattern had a negative and direct effect on obesity indicators (serum LEP, body weight and waist circumference) and negative indirect effects on total cholesterol:HDL-cholesterol ratio, TAG:HDL-cholesterol ratio and fasting plasma glucose. The ‘Prudent’ pattern had a negative and direct effect on systolic blood pressure. No association was observed for the ‘Modern’ pattern and metabolic risk factors. In conclusion, the ‘Traditional’ and ‘Prudent’ dietary patterns were negatively associated with metabolic cardiovascular risk factors among Brazilian adults. Their apparent protective effects against obesity and high blood pressure may be important non-pharmacological strategies for the prevention and control of obesity-related metabolic disorders and CVD.

Abstract Background Abnormal fetal growth is associated with increased cardiovascular risk in adulthood. We investigated the effect of fetal programming on arterial health and morphology during early childhood. Methods We examined 90 children (median age 5.81 years, interquartile range: 5.67; 5.95), born small for gestational age with fetal growth restriction, large or appropriate for gestational age (SGA, N = 23, LGA, N = 19, AGA N = 48). We measured body composition, anthropometrics, blood pressure, pulse wave velocity (PWV), lipids, glucose and inflammatory markers, and assessed carotid, brachial, radial and femoral arterial morphology and stiffness using very-high resolution ultrasound (46–71 MHz). Results LGA showed increased anthropometry, lean body mass and body mass index. SGA displayed decreased anthropometry and lean body mass. Blood pressure, PWV, carotid artery stiffness and blood work did not differ groupwise. Differences in lumen diameters, intima-media thicknesses (IMT) and adventitia thicknesses disappeared when adjusted for lean body mass and sex. In multiple regression models arterial dimensions were mainly predicted by lean body mass, with birth weight remaining associated only with carotid and brachial lumen dimensions, and not with IMTs. Carotid-femoral PWV was predicted by height and blood pressure only. No independent effect of adiposity was observed. Conclusions Arterial dimensions in childhood associate with current anthropometrics, especially lean body mass, and sex, explaining differences in arterial layer thickness. We found no signs of fetal programming of cardiovascular risk or arterial health in early childhood.

Abstract Steamed ginger ethanolic extract (SGE) is a product with a high 6-shogaol contents and is thought to be more potent than other ginger products. We conducted a 12-week, randomized, double-blind, placebo-controlled clinical trial to determine the effects of SGE on weight and body fat loss. Eighty healthy obese participants were recruited and randomly divided into the SGE and placebo groups. The outcome measures comprised indicators of efficacy (body weight, body mass index, body composition, and blood markers) and safety. Following the supplementation period, mean body weight, body mass index, and body fat level were significantly lower in the SGE group than in the placebo group. No clinically significant changes were observed for any safety parameter. These results suggest that SGE is a potent anti-obesity agent that does not cause significant side effects. Therefore, SGE supplementation combined with lifestyle modification could be effective in the management of body weight and fat mass.

Objectives. The study aimed to assess the relationship between body fat and each of body mass index (BMI) and waist circumference (WC), and to test the effectiveness of fat mass (FM), percent of body fat (PBF), BMI, and WC in predicting high levels of fasting blood glucose (FBG).Methods. A total of 167 adolescents aged 11–17 years were recruited from Rabat region. BMI and WC were determined using standard equipments. FM and PBF were derived from isotope dilution technique. FBG was determined by the hexokinase method.Results. Regardless of the weight status, BMI showed a strong positive correlation with FM and PBF in both genders. WC was significantly correlated with FM in boys and girls, and with PBF in different groups of girls and boys of the study sample. However, there was no significant relationship between WC and PBF in normal weight and overweight-obese groups of boys. FBG was highly correlated with FM and PBF in girls of the study sample and in overweight-obese girls. Similar significant relationship between FBG and both BMI and WC was observed in overweight-obese girls, while there was no significant association between FBG and other variables in boys and normal-weight girls.Conclusion. BMI and WC were closely associated with FM and PBF, respectively. However, the degree of these associations depends on gender and weight status. BMI may provide a better proxy estimate of overall adiposity than WC; nevertheless, both of them would appear to be a reasonable surrogate for FM and PBF as screening tools to identify adolescents at risk of developing excess body fat and high level of FBG.

Diet mengendalikan hipertensi orang Indonesia di Jakarta (dietary approach to stop hypertension for Indonesian at Jakarta, disingkat DASHI-J) dan olahraga jalan cepat dapat mengurangi berat badan dan menurunkan tekanan darah pada pasien prahipertensi. Tujuan penelitian ini adalah mengevaluasi diet dan olahraga tersebut dengan desain penelitian clinical trial terhadap 100 laki-laki berusia 25 – 55 tahun di PT Krama Yudha Ratu Motor. Responden dialokasikan secara acak dalam 4 kelompok perlakuan meliputi kelompok diet (A), kelompok olahraga jalan cepat (B), kelompok diet dan olahraga jalan cepat (C), dan kelompok kontrol (D). Kelompok A dan C menerima diet 5 hari dalam seminggu selama 8 minggu. Kelompok diet DASHI-J diberikan makan siang dan makan malam. Setelah 2 bulan intervensi, berat badan, indeks massa tubuh, visceral fat, body fat, lingkar perut, tekanan darah sistolik dan diastolik, kolesterol serum dari semua kelompok menurun secara signifikan. Penurunan tertinggi terjadi pada kelompok diet DASHI-J dan olahraga jalan cepat dengan penurunan berat badan 4,18 kg, indeks massa tubuh 1,50 kg/m2, tekanan darah 12,00 mmHg/8,60 mmHg. Diet DASHI-J dan olahraga jalan cepat berperan menurunkan berat badan, indeks massa tubuh, serta tekanan darah sistolik dan diastolik.Dietary approach to stop hypertension for Indonesian at Jakarta (DASHI-J) and brisk walking exercise could reduce body weight and blood pressure of males prehypertension. The objective of the study was to evaluate DASHI-J by an experimental clinical trial conducted with 100 male subjects, aged 25 – 55 years divided randomly into 4 groups: DASHI-J diet group (A), brisk walking exercise group (B), DASHI-J and brisk walking exercise group (C), and control group (D). Group A and C got 5 days a week for 8 weeks diet. DASHI-J group was given lunch and dinner. After 2 months of intervension, the body weight, body mass index, body fat, visceral fat, waist circumference, systole, diastole, cholesterol serum of the groups all reduce significantly. The highest reduction of those intervention achieved by DASHI-J and brisk walking exercise group with body weight decreased 4.18 kg, Body Mass Index (BMI) 1.50 kg/m2, blood pressure 12.00/8.60 mmHg. This implies that DASHI-J and brisk walking exercise play a significant role in reducing body weight, BMI, and both sistolic and diastolic blood pressure.

Abstract Objectives To determine the effect of beef consumption as a part of the DASH diet on measures of anthropometric and muscle health in adults 65 and older. Methods Twenty-eight older adults were randomly assigned to consume either 3 oz. (n = 14) or 6 oz. (n = 14) of fresh, lean beef as a part of the DASH diet for 12-wks. Anthropometrics and muscle strength were measured at wks 0, 3, 6, 9, and 12 throughout the study. The seven-day cyclical DASH menu contained fresh lean beef. Other red meats, poultry and seafood were excluded. Breakfast, lunch and dinner were provided every day for 12-wks and all food items were purchased, prepared and weighed to the nearest gram by the research staff. The portions of beef were evenly distributed throughout each meal provided every day. Results Changes in body composition and muscle strength were observed during the 12-wk study regardless of beef intake. Significant time effects were detected for: body weight (P < 0.001); BMI (P < 0.001); waist circumference (P < 0.001); hip circumference (P < 0.001); %body fat (P < 0.001); absolute fat mass (P < 0.001); systolic blood pressure (P < 0.001); and diastolic blood pressure (P < 0.001) such that a decrease was observed over the 12-wk intervention. Time effects were detected for sit-to-stand (P < 0.001) such that an increase was observed over the 12-wk intervention. Body weight decreased (P = 0.001) by 6.7% from baseline (90 kg) to study-end (84 kg); BMI decreased (P < 0.001) from baseline (31.2) to study-end (29.4); waist circumference decreased (P < 0.001) from baseline (98.1 cm) to study-end (94.4 cm); %body fat decreased (P < 0.001) from baseline (36.1%) to study-end (34.2%); absolute fat mass decreased (P < 0.001) from baseline (33.1 kg) to study-end (29.4 kg); systolic blood pressure decreased (P < 0.001) from baseline (134 mmHg) to study-end (118 mmHg). Handgrip strength and REE were well-maintained (P > 0.05) despite the weight loss. Of the obese participants, 33% were overweight and 29% of the overweight participants were normal weight by study-end. Conclusions The results of this highly-controlled dietary intervention study indicate that daily consumption of high-quality protein as a part of the DASH diet positively influences body composition and muscle strength in older adults. These results also suggest that beef can be included in healthy dietary patterns. Funding Sources South Dakota Beef Industry Council.

CONTEXT AND OBJECTIVE: Studies have demonstrated that metabolic complications from child obesity, although silent, increase the risk of development of cardiovascular diseases in adulthood. The present paper sought to describe the prevalence of overweight/obesity and analyze the possible relationship between obesity and other cardiovascular risk factors among children and adolescents. DESIGN AND SETTING: Cross-sectional study, conducted in a university. METHODS: The study included 564 children and adolescents, aged 8 to 17 years. Body mass index and waist circumference were used to evaluate obesity. Other cardiovascular risk factors were evaluated, like systolic and diastolic blood pressure, glycemia, triglycerides and total cholesterol. Descriptive analysis was used for sample characterization, the chi-square test for categorical variables and Pearson's linear correlation for evaluating the relationship between obesity indicators and other cardiovascular risk factors. RESULTS: High prevalence of overweight/obesity was found among the schoolchildren (25.3% among the boys and 25.6% among the girls), along with abdominal obesity (19.0%). The overweight/obese schoolchildren presented higher percentages for the pressure and biochemical indicators, compared with underweight and normal-weight schoolchildren. Body mass index and waist circumference showed a weak correlation with the variables of age and systolic and diastolic blood pressure (P < 0.001), but there was no correlation between these obesity indices and biochemical variables. CONCLUSION: The high prevalence of overweight/obesity and its relationship with other cardiovascular risk factors demonstrate that it is necessary to develop intervention and prevention strategies from childhood onwards, in order to avoid development of chronic-degenerative diseases in adulthood.
