1. Introduction. This paper is concerned with collision risk reduction through the identification, measurement and analysis of contributing factors and the evaluation of risk control measures appropriate to the approaches to the Strait of Juan de Fuca. Risk factors were identified through a traffic survey and collision probabilities were estimated and modified by a simulated modification to traffic routeing. Marine collision risk analyses have tended to describe the circumstances of collisions without comparison to the conditions encountered by marine traffic. In the absence of any test of significance, such an analysis is revealing yet provides an incomplete basis for traffic management decisions. This study examines the importance of contributing factors through such a comparison.

Objective: This study reports current status of knowledge and challenges associated with the emergency vehicle (police car, fire truck, and ambulance) crashes, with respect to the major contributing risk factors. Background: Emergency vehicle crashes are a serious nationwide problem, causing injury and death to emergency responders and citizens. Understanding the underlying causes of these crashes is critical for establishing effective strategies for reducing the occurrence of similar incidents. Method: We reviewed the broader literature associated with the contributing factors for emergency vehicle crashes: peer-reviewed journal papers; and reports, policies, and manuals published by government agencies, universities, and research institutes. Results: Major risk factors for emergency vehicle crashes identified in this study were organized into four categories: driver, task, vehicle, and environmental factors. Also, current countermeasures and interventions to mitigate the hazards of emergency vehicle crashes were discussed, and new ideas for future studies were suggested. Conclusion: Risk factors, control measures, and knowledge gaps relevant to emergency vehicle crashes were presented. Six research concepts are offered for the human factors community to address. Among the topics are emergency vehicle driver risky behavior carryover between emergency response and return from a call, distraction in emergency vehicle driving, in-vehicle driver assistance technologies, vehicle red light running, and pedestrian crash control. Application: This information is helpful for emergency vehicle drivers, safety practitioners, public safety agencies, and research communities to mitigate crash risks. It also offers ideas for researchers to advance technologies and strategies to further emergency vehicle safety on the road.

The System Engineering principles are applied to identify the risk factors of navigation environment of dock waters adjacent to fairway. The Evaluation Index System for risks of navigation environment of such a dock is established and the relative risk degrees among each index are reflected quantitatively by weights of risk factors obtained by adopting the method of AHP. Based on the identified risk factors, the risk control measures for navigation environment of dock waters adjacent to fairway are systematically put forward. One dock in Yangpu Port, Hainan Province, is cited as an example to analyze, and the risk control measures are proposed accordingly.

Abstract Background Recent conflict and war in Yemen lead to collapse of the health system, decrease of immunization coverage and spread of many outbreaks. On May 22, 2018, the surveillance officer in Shabwah governorate reported an increased number of suspected measles. On May 24, 2018, a team from Yemen-Field Epidemiology Training Program was sent to investigate. The aims were to describe the outbreak, determine the risk factors for measles infection and recommend control measures. Methodology A descriptive followed by case-control study design (1:2 ratio) were performed. National Measles Surveillance Program case definition and predesigned questionnaire were used to collect data from 73 cases and 146 controls. Attack rate (AR), adjusted odds ratios (aOR) and 95% confidence intervals (95%CI) were calculated. P value < 0.05 was considered as the cut point for significant. Epi info version 7.2 was used. Results A total of 73 suspected cases were found. Almost 53% were from Habban district, 63% were males and 56% were among age group < 5 years. The overall AR was 82/100,000 population. Measles was significantly associated with contact with case (aOR = 27.3, 95% CI:1.3–551.7), malnourished children aged 6–60 months (aOR = 24.9, 95% CI;1.9–329.6) and unvaccinated children (aOR = 17.2, 95% CI:2.9–100.7). The six collected blood samples found to be positive for measles IgM. Conclusions Measles outbreak in Ataq and Habban districts was confirmed. Contact with measles cases, malnutrition and un-vaccination were the potential contributing factors of measles outbreak in Shabwah governorate. An urgent vaccination campaign with health education interventions are highly recommended. Reactivation of the outreach immunization services and strengthening surveillance and response systems are top priority to take place at district and governorate levels.

For the approval process of offshore platforms and offshore wind farms, risk analyses that consider collisions between ships and stationary offshore structures are helpful and sometimes necessary. Ship traffic, meteorological and hydrological data are important input information for the investigated sea area. Possible collision scenarios with powered ships sailing into the structures and disabled ships drifting into the structures have to be investigated to determine the collision probabilities and the consequences. Furthermore, risk control measures have to be examined and assessed in such risk analyses. In recent years, Germanischer Lloyd has developed and applied suitable methods and procedures to evaluate the collision risk, including the influence of risk control measures. For these analyses, Germanischer Lloyd developed an analysis software that uses Monte Carlo simulations. For the evaluation of different risk control measures, Bayesian nets are used to determine the reduction of the collision probabilities for powered and drifting vessels and the corresponding risk. In this paper, the methods and processes that were developed are presented together with results of a sample collision risk analysis. Moreover, experience in the practical application of collision risk analyses is discussed, as part of the approval process for offshore wind farms.

Strategies to control occupational risk factors face various challenges in terms of their application and effectiveness, even more so when it comes to providing risk-control solutions under informal working conditions. Purpose: explore opportunities presented by the use of social innovation strategies and methodologies derived from research-creation to establish educational control measures for occupational risk factors, for recovery and recycling workers in Bogotá (Colombia). Methods and procedures: There were two main phases for this research: one aimed to understand the different risk posed to informal labor communities based on workplace inspection and risk assessment norms NTC4114 and GTC45, the other one was aimed to develop a research-creation and social innovation-based strategy, to co-create innovative solutions to address risk factors with the informal workers New Results: Risk conditions were measured for the first time with this group of informal workers and a social innovation creation was made to control risk factors and to dignify working conditions Conclusions: Informal work has precarious and uncontrolled risk. In addition, the systematic exclusion of informal workers from public health labor systems organized by national governments means that protective measures for this group of people are often ineffective or inadequate, or in the worst case, non-existent.

Background: Measles is highly contagious vaccine preventable disease (VPD), and a major public health problem considered as leading cause of morbidity and mortality in developing countries like Pakistan. An outbreak of measles was reported in Sharifabad Islamabad on 15th of April 2017, and an investigation was launched to assess the magnitude of outbreak, evaluate risk factors and recommend control measures. Methods: A comprehensive house to house active case search along with vaccine coverage survey was conducted from April 19-22, 2017. A case was defined as "onset of maculopapular rash with fever in a resident of Sharifabad with at least one of the following signs/ symptoms, Coryza, Conjunctivitis, Cough, Otitis media or Pneumonia present in between 19 March to 22nd April 2017". Four age & sex matched controls were selected from the neighborhood. Data was collected through interview method using structured questionnaire and vaccination coverage was determined by using Epi survey form. Blood samples were sent for laboratory confirmation. Results: A total of eight cases were identified through active case finding while three were reported by local practitioner. Mean age of cases were 20 months (range 8-36 months). Severely affected age-group was 1-2 years with attack rate of 46%. Around two-third (64%) of cases and a few (16%) of controls were unvaccinated against measles. Contact with measles patient [OR 25.2, CI 3.9-160.1, P=0.00], unvaccinated children [OR 9.2 CI 2.12-40.4, P=0.000], social misconception regarding vaccination [OR 7.8 CI 1.42-42.6, P=0.00], and distance from healthcare facility [OR 5.7 CI 1.15-28.35, P=0.02] were significant risk factors. Vaccine efficacy was 90%. Conclusion: Main reasons of the outbreak were contact with the cases, and low vaccination status. We recommended comprehensive measles vaccination and community awareness sessions. On our recommendations district health authority Islamabad carried out mop up of whole area.

Abstract Background Gestational weight gain (GWG) can have implications for the health of both mother and child. However, the contributing factors remain unclear. Despite the advantages of using a biopsychosocial approach, this approach has not been applied to study GWG in the UK. This study aimed to investigate the risk factors of excessive GWG in a UK population, employing a biopsychosocial model. Methods This study utilised data from the longitudinal Grown in Wales (GiW) cohort, which recruited women in late pregnancy in South Wales. Specifically, data was collected from midwife recorded notes and an extensive questionnaire completed prior to an elective caesarean section (ELCS) delivery. GWG was categorised according to Institute of Medicine (IOM) guidelines. The analysis was undertaken for 275 participants. Results In this population 56.0% of women had excessive GWG. Increased prenatal depression symptoms (Exp(B)=1.10, p=.019) and an overweight (Exp(B)=4.16, p<.001) or obese (Exp(B)=4.20, p=.010) pre-pregnancy BMI, consuming alcohol in pregnancy (Exp(B)=.37, p=.005) and an income of less than £18,000 (Exp(B)=.24, p=.043) and £25–43,000 (Exp(B)=.25, p=.002) were associated with excessive GWG. Conclusion GWG is complex and influenced by a range of biopsychosocial factors, with the high prevalence of excessive weight gain in this population a cause for concern. Women in the UK may benefit from a revised approach toward GWG within the National Health Service (NHS), such as tracking weight gain throughout pregnancy. Additionally, this research provides evidence for potential targets for future interventions, and potentially at-risk populations to target, to improve GWG outcomes.

AbstractA cross-sectional study was carried out to determine the seroprevalence of Toxoplasma gondii and associated risk factors in pigs in the largest pork-producing region in Cuba. Serum samples from 420 pigs, including 210 sows and 210 post-weaning pigs, were tested for antibodies against T. gondii using a commercial indirect enzyme-linked immunosorbent assay. Anti-T. gondii antibodies were detected in 56 animals (13.3%, 95% CI: 10.1–16.6). A generalized estimating equations model revealed that the risk factors associated with higher seropositivity in pigs were altitude (higher in farm’s location < 250 m above sea level (masl) versus ≥ 250 masl) and age (higher in sows compared to post-weaning pigs). The results indicated that this protozoan parasite is widely distributed on pig farms in the study area, which is a public health concern since the consumption of raw or undercooked pork meat products containing tissue cysts is considered one of the main routes of T. gondii transmission worldwide. Control measures should be implemented to reduce the risk of exposure to T. gondii in pigs in Cuba.

In recent years, conflicts between crossing pedestrians and right-turning vehicles have become more severe at intersections in China, where right-turning vehicles are usually not controlled by traffic signals. This study proposes a quantitative method for evaluating the conflict risk between pedestrians and right-turning vehicles at intersections based on micro-level behavioral data obtained from video detection. A typical intersection in Shanghai was selected as the study site. In total, 670 min of video were recorded during the peak hours from 7:30 a.m. to 9:30 p.m on one day. After processing the video information, vehicle and pedestrian tracking data were obtained, including the velocity, acceleration, deceleration, time, and location coordinates. Based on these data, several conflict indicators were proposed and these indicators were extracted automatically using MATLAB to identify pedestrian–right-turning vehicle conflicts and to determine the severity of the conflicts identified. This process identified 93 examples of such conflicts. The conflict risks were quantitatively classified using the K-means fuzzy clustering method and all of the conflicts were assigned to five grades. The characteristics of the conflict distribution and the severity of different types of conflict were also analyzed, which showed that conflicts on different areas on the crosswalk differed in their severity. Based on the conclusions, practical traffic management and control measures are proposed to reduce the risk on pedestrian crossings.

AbstractThis cross-sectional study aimed to determine the prevalence and risk factors of S. stercoralis infection among 1142 Orang Asli primary schoolchildren in six different states of Peninsular Malaysia. Fecal samples were examined using direct smear, formalin-ether sedimentation (FES), agar plate culture (APC) and PCR techniques. Overall, 15.8% of the children were found to be infected with S. stercoralis. The prevalence was 0.2, 1.3, 15.2 and 13.7% by direct smear, FES, APC and PCR, respectively. Multivariate analysis showed that an age of >10 years, being male, belonging to a Proto-Malay tribe, belonging to the Senoi tribe, indiscriminate defecation, using an unimproved water source for drinking water and not wearing shoes when outside were the significant risk factors of infection among these children. In conclusion, we provide new evidence on the occurrence of S. stercoralis in Malaysia to show that there is a relatively high prevalence of infection among Orang Asli schoolchildren. Therefore, the use of specific methods for detecting S. stercoralis should be considered when screening these children for intestinal parasites. Moreover, prevention and control measures specific to S. stercoralis should be integrated into the intestinal parasitic infections control programme in Malaysia.
