Summary: Background: Two-dimensional (2D) perfusion angiography is useful for the evaluation of foot perfusion in patients with critical limb-threatening ischemia (CLTI). Iloprost is a synthetic prostacyclin analogue presenting vasodilating properties. Aim of this study was to demonstrate the utility of 2D perfusion angiography as quantitative method to evaluate iloprost effect on foot circulation. Patients and methods: Between January 2020 and June 2020 25 patients with CLTI underwent below-the-knee (BTK) endovascular revascularization, intra-arterial administration of iloprost, and assessment of foot perfusion by 2D perfusion angiography. Iloprost was administered as an intra-arterial bolus of 3 μg over 1–3 minutes immediately after BTK revascularization. The 2D perfusion angiography was performed in a standardized manner with a 5-F catheter placed into the popliteal artery. A wide region of interest (ROI) was identified to assess the foot perfusion. Time–density curves were calculated by the perfusion software. Changes of the overall time-density curves before and after the administration of iloprost were evaluated. Results: Endovascular revascularization was successful in all cases. The mean reduction of systolic pressure value after iloprost administration was 23.1 mmHg. Eight patients (32%) experienced a minor complication (6 cutaneous rush, 2 symptomatic hypotension >40 mmHg). In 20 patients the time-density curves under ROI increased after the intra-arterial administration of iloprost (+31.6%, range from +4.9% to +78.7%). Five patients had no modification or a slight decrease of foot perfusion after iloprost administration (non-responders patients). Conclusions: Patients undergoing intra-arterial administration of iloprost accounted for a not negligible rate of minor complications. 2D perfusion angiography was valuable as quantitative method to evaluate the iloprost effect on foot circulation. This technique could be useful to classify the patients in responders or non-responders to iloprost therapy.

The aim of this study was to demonstrate the association between 2-dimensional (2D) perfusion angiography and wound healing rate in patients with combined femoro-popliteal and below-the-knee lesions in critical limb-threatening ischemia (CLTI) and foot wounds undergoing isolated femoro-popliteal endovascular revascularization. Between January and June 2019, 24 patients with multilevel CLTI and foot wounds underwent isolated femoro-popliteal endovascular revascularization. In all of them, an assessment of foot perfusion by 2D perfusion angiography was performed. To evaluate the foot perfusion, a region of interest was identified, and time–density curves were calculated. Changes of the overall time–density curves were evaluated together with transcutaneous oximetry (TcPO2) using bivariate correlation (Pearson correlation coefficient) and associated with 6-month wound healing. The mean increase of time–density curves was 212.2% (range from +9.8% to +1984.9%) and the mean increase of TcPO2 was 116.4% (range from −4.7% to 485.7%). No significant association between time–density curves and TcPO2 values (Pearson correlation coefficient: −0.24) was observed ( P = .3). At 6 months, wound healing occurred in 15 of 24 (62.5%) patients. In conclusion, this preliminary experience confirmed that 2D perfusion angiography associates with wound healing rate in CLTI patients with ischemic foot wounds and combined femoro-popliteal and below-the-knee lesions who are undergoing isolated femoro-popliteal endovascular revascularization. No association between time–density curves and TcPO2 values was observed.

BackgroundCarotid webs (CaWs) are associated with ischemic strokes in younger patients without degrees of stenosis that are traditionally considered clinically significant.ObjectiveTo compare the hemodynamic parameters in the internal carotid artery (ICA) bulbar segment in patients with CaW with those in patients with atherosclerotic lesions using time–density curve (TDC) analysis of digital subtraction angiography (DSA) images.MethodsWe retrospectively assessed DSA images of 47 carotid arteries in 41 adult patients who underwent ICA catheter angiography for evaluation after ischemic stroke. Hemodynamic parameters, including full width at half maximum (FWHM) and area under the time–density curve (AUC) as proxies for increased flow stasis, were calculated using TDC analyses of a region of interest (ROI) in the ICA bulb immediately rostral to the web/atherosclerotic plaque, relative to a standardized ROI in the ipsilateral distal common carotid artery (eg, relative FWHM (rFWHM)). Hemodynamic parameters were compared using non-parametric Kruskal-Wallis tests. Logistic regression was used to predict CaW versus mild/moderate atherosclerosis for each hemodynamic parameter, adjusting for degree of stenosis.ResultsMean age of patients was 56.0±13 years, with 22 (53.7%) women. 17 CaWs, 22 atherosclerotic plaques (15 mild/moderate and 7 severe), and eight normal carotid arteries were assessed. Significant between-group differences were present in the relative total AUC (p<0.001), relative AUC at wash out (p=0.031), and relative FWHM (p=0.001). Logistic regression to predict CaW versus mild/moderate atherosclerosis showed that rAUC total had the highest predictive value (pAUC=0.96, 95% CI 0.90 to 1.00), followed by rFWHM (0.87, 95% CI 0.74 to 1.00), and rAUC WO (0.74, 95% CI (0.57 to 0.91).ConclusionCaW results in larger local hemodynamic disruption, characterized by flow stasis, than mild/moderate carotid atherosclerotic lesions, suggesting that CaWs may produce larger regions of thrombogenic flow stasis.

The aim of this study was to evaluate the incidence of fluid responsiveness (FR) to a fluid challenge (FC) in normotensive dogs under anaesthesia. The accuracy of pulse pressure variation (PPV), systolic pressure variation (SPV), stroke volume variation (SVV), and plethysmographic variability index (PVI) for predicting FR was also evaluated. Dogs were anaesthetised with methadone, propofol, and inhaled isoflurane in oxygen, under volume-controlled mechanical ventilation. FC was performed by the administration of 5 mL/kg of Ringer’s lactate within 5 min. Cardiac index (CI; L/min/m2), PPV, (%), SVV (%), SPV (%), and PVI (%) were registered before and after FC. Data were analysed with ANOVA and ROC tests (p < 0.05). Fluid responsiveness was defined as 15% increase in CI. Eighty dogs completed the study. Fifty (62.5%) were responders and 30 (37.5%) were nonresponders. The PPV, PVI, SPV, and SVV cut-off values (AUC, p) for discriminating responders from nonresponders were PPV >13.8% (0.979, <0.001), PVI >14% (0.956, <0.001), SPV >4.1% (0.793, <0.001), and SVV >14.7% (0.729, <0.001), respectively. Up to 62.5% of normotensive dogs under inhalant anaesthesia may be fluid responders. PPV and PVI have better diagnostic accuracy to predict FR, compared to SPV and SVV.

OBJECTIVE: Interest has focused recently on the influences of the polypeptide factors inhibin and activin on the selective regulation of the pituitary secretion of gonadotropins. DESIGN: Measurement of the concentrations of inhibin-related proteins in relation to the changes in pituitary gonadotropin (FSH, LH) parameters, after GnRH stimulation with a bolus injection of 100 microg gonadorelin, in 19 women with ovulatory disturbances. METHODS: Serum levels of inhibin A and B, activin A, and pro alpha-C were measured using sensitive ELISA kits. RESULTS: Within 60 min after GnRH stimulation, FSH values doubled from 5 to 10 mU/ml (P < 0.001). LH increased 12-fold from 2 to 24 mU/ml (P < 0.001). Activin A showed a significant decrease from 0.47 to 0.36 ng/ml (P < 0.001), whereas pro alpha-C increased from 127 to 156 pg/ml (P = 0.039). The median inhibin A concentration did not show a significant change between baseline and the 60 min value, whereas inhibin B was characterized by a minor, but not significant, increase in the median from 168 to 179 pg/ml (P = 0.408). A significant inverse correlation (P = 0.014) with a mean coefficient of correlation of 0.5516 was found, demonstrating a strong relationship between high inhibin B baseline levels and a small increase of FSH after 60 min. CONCLUSION: Our results show an interesting correlation between the baseline inhibin B and the change in FSH before and after GnRH stimulation. A high baseline inhibin B implies only a minor increase of FSH after 60 min.

The contractile history of muscle can potentiate electrically evoked force production. A link to voluntary force production, related in part to an increase in reflex excitability, has been suggested.Purpose:Our purpose was to quantify the effect of postactivation potentiation on voluntary force production and spinal H-reflex excitability during explosive plantar fexion actions.Methods:Plantar flexor twitch torque, soleus H-reflex amplitudes, and the rate of force development of explosive plantar fexion were measured before and after 4 separate conditioning trials (3 × 5 s maximal contractions).Results:Twitch torque and rate of force production during voluntary explosive plantar flexion were significantly increased (P < .05) while H-reflex amplitudes remained unchanged. Although twitch torque was significantly higher after conditioning, leading to a small increase in the rate of voluntary force production, this was unrelated to changes in reflex excitability.Conclusion:We conclude that postactivation potentiation may result in a minor increase in the rate of voluntary isometric force production that is unrelated to neural excitability.

Purpose: Evaluate the effect of addition of commonly consumed fructose containing sugars to the usual diet on liver fat content. Background: Fructose induced triglyceride synthesis has been argued to be augmented when accompanied by glucose. As the most common sources of fructose, high fructose corn syrup HFCS) and sucrose, also contain glucose such a response would be particularly detrimental to metabolic health due to the known role of excess fat in the liver in the induction of abnormalities in glucose homeostasis, insulin resistance and the subsequent development of type II diabetes. Methods: For ten weeks, sixty-eight individuals (42.16 ± 11.66 years) consumed low-fat milk sweetened with either HFCS or sucrose such that the added sugar matched the 25th, 50th and 90th percentile population consumption levels of fructose. Fat content of the liver was obtained before and after the ten week intervention. Imaging was performed on a Philips 64-slice CT scanner. Region of interest measurements in the four sectors of the liver delineated by the hepatic veins were obtained in all patients, and converted to hepatocellular fat content percentages. Fat content of the liver remained unchanged (13.32 ±10.49 vs 13.32 ± 10.75%, p > 0.05). Group assignment did not affect the result interaction p > 0.05). Conclusions: These data suggest that ectopic fat storage in the liver is not promoted when fructose is consumed as part of a normal diet, even in amounts as high as the 90th percentile consumption level.

The focus of this review is on information practical to the practicing nephrologist and internists managing patients with chronic kidney disease (CKD), with an emphasis on the quantitative aspects of risk, diagnosis, treatment, and prognosis. Consequently, anemia associated with non–dialysis-associated CKD is emphasized, with special attention to the role of erythropoiesis-stimulating agents and intravenous (IV) iron in treating the anemia of CKD, as well as sections on uremic bleeding and anticoagulation in CKD patients. Figures show a patient before and after a minor infusion reaction, an algorithm outlining grading and management of acute hypersensitivity reactions to IV iron infusions, and an algorithm for the management of uremic platelet dysfunction. Tables list Food and Drug Administration-recommended dose adjustments for novel oral anticoagulant (NOACs) in CKD patients, evidence for preprocedural withholding of NOACs, and management guidelines for anticoagulation in nonvalvular atrial fibrillation and venous thromboembolism. The study aims at evaluating health-generating function of humor therapy in a hospital ward hosting children suffering from respiratory pathologies. The main scope of this study is to investigate possible positive effects of the presence of a clown on both the clinical evolution of the on-going disease, and on some physiological and pain parameters. Forty-three children with respiratory pathologies participated in the study: 21 of them belonged to the experimental group (EG) and 22 children to the control group (CG). During their hospitalization, the children of the EG interacted with two clowns who were experienced in the field of pediatric intervention. All participants were evaluated with respect to clinical progress and to a series of physiological and pain measures both before and after the clown interaction. When compared with the CG, EG children showed an earlier disappearance of the pathological symptoms. Moreover, the interaction of the clown with the children led to a statistically significant lowering of diastolic blood pressure, respiratory frequency and temperature in the EG as compared with the control group. The other two parameters of systolic pressure and heart frequency yielded results in the same direction, without reaching statistical significance. A similar health-inducing effect of clown presence was observed on pain parameters, both by self evaluation and assessment by nurses. Taken together, our data indicate that the presence of clowns in the ward has a possible health-inducing effect. Thus, humor can be seen as an easy-to-use, inexpensive and natural therapeutic modality to be used within different therapeutic settings.

Rencana redenominasi rupiah di Indonesia menjadikan pengalaman beberapa negara yang telah menerapkan redenominasi mata uang penting untuk ditinjau dari sisi makro ekonomi saat sebelum dan sesudah redenominasi sehingga menjadi dasar pertimbangan bagi Indonesia mengambil keputusan dalam rencana penerapan redenominasi rupiah. Tujuan penelitian ini untuk mengetahui ada tidaknya perbedaan kondisi inflasi, nilai tukar, ekspor, penanaman modal asing (PMA) dan pertumbuhan ekonomi 1 tahun sebelum dan 1 tahun sesudah menerapkan redenominasi pada 20 negara yang telah menerapkan redenominasi. Penelitian ini menggunakan metode penelitian kuantitatif dengan alat analisis uji beda dua rata-rata sampel berpasangan. Hasil analisis menunjukkan bahwa pada variabel nilai tukar terdapat perbedaan antara sebelum dan sesudah redenominasi yaitu nilai tukar semakin lemah setelah penerapan redenominasi, sedangkan pada variabel inflasi, ekspor, PMA, dan pertumbuhan ekonomi tidak terdapat perbedaan sebelum dan sesudah redenominasi.   Rupiah redenomination which planned by Indonesia make the experience of countries that have implemented the currency redenomination is important to be reviewed in terms of macroeconomic before and after the redenomination so that it becomes a basic consideration for Indonesia to take decisions in the plan of implementation rupiah redenomination. The purpose of this study is to determine whether there is difference in the conditions of inflation, exchange rate, exports, foreign direct investment (FDI) and economic growth in the current 1 year before and 1 year after implementing the redenomination in the 20 countries that have implemented the currency redenomination. This study used a quantitative method, with analysis dependent sample t-test which showed that there is a difference between before and after redenomination in exchange rate variable is getting weaker after the implementation of the redenomination, while there is no difference before and after the redenomination in inflation, exports, FDI, and economic growth.

This study aims to determine the effect of Pegadaian Digital Service (PDS) on the financial inclusion of sharia business segments. The study was conducted using secondary data and analyzed by quantitative and qualitative methods. Quantitative method used the t-test to determine the significance of the effect of PDS on the financial inclusion of sharia business segments. The t-test was conducted two years before the release of PDS (2016-2017) and the two years after released of PDS (2018-2019). Sharia business segments in this study are sharia pawn bussiness (rahn) and micro fiducia sharia business (Arrum and Amanah). Furthermore, qualitative analysis is to find the right modeling for optimizing PDS in increasing financial inclusion in sharia business segments. The results showed that in the two years after the released of PDS the turnover, outstanding loan, and mu'nah revenue had a very significant increasing. There were strong correlations and significant differences in the achievement of turnover, outstanding loans, and mu’nah revenue between before and after the PDS, both in sharia pawn business (Rahn), Arrum and Amanah products. The results of empirical testing proved that PDS was supporting the acceleration of financial inclusion programs, especially in the sharia business segment at PT. Pegadaian. Optimizing PDS in increasing financial inclusion in sharia business segments can be done by developing a product based PDS optimization model.
