Abstract Objectives: The objective of this study is to compare the amount of extrusion of bacteria beyond the apical foramen after instrumentation with Crown down and Step-back techniques using a manual and engine driven nickel-titanium instruments Materials and Methods: Seventy-five mandibular premolars with similar dimensions were used for the study. Access cavities prepared and root canals contaminated with a suspension of Enterococcus faecalis. The contaminated teeth were then divided into three experimental groups. Group 1(Crowndown group) divided into two: Group 1–A Hand files: root canals were instrumented using K-files and Group 1B – Rotary files: root canals were instrumented using ProTaper instruments. Group II (Step-back group) divided into two: Group II A– Hand files: root canals were instrumented using K-files and group II B–Rotary files: the root canals were instrumented using Light Speed LSX instruments. Group III (control group): no instrumentation was done.Bacteria were extruded after preparation were collected into vials, microbiological samples were incubated in culture media for 24hrs. The CFUs were determined for each sample. The data obtained was analyzed using the Kruskal-Wallis one way analysis of variance and Mann-Whitney U-tests. Result: There was a significant difference in the amount of bacteria extruded by both Crowndown and Step-back. The Step- back hand method extruded significantly more bacteria when compared with Crowndown hand technique. Conclusion: All instrumentation techniques extruded intracanal bacteria apically. There was a significant difference in both the engine driven instrumentation techniques, while the hand instrumentation by Step-back extruded more bacteria.

Objective: All instrumentation techniques and instruments are associated with apical extrusion during chemomechanical preparation, and this causes postoperative pain and flare-up. However, it is controversial whether reciprocal systems or rotary systems cause more apical extrusion. The objective of this in vitro study was to determine the differences in the amounts of apically extruded bacteria (AEB) associated with nickel-titanium rotary and reciprocating systems when used in oval-shaped root canals. Material and Methods: Seventy human mandibular premolar teeth with oval-shaped canals were randomly assigned to four experimental groups (15 teeth in each group) and one control group (10 teeth). The root canals were contaminated with Enterococcus faecalis and instrumented using two full-sequence rotary instruments (ProTaper Universal [PTU] and ProTaper Next [PTN]) and two reciprocating single-file instruments (Reciproc [R] and WaveOne [WO]). A 0.9% NaCl solution was used as an irrigant, and the bacterial extrusion was quantified as the number of colony-forming units for each sample. The results were statistically analyzed using the Kruskal-Wallis one-way analysis of variance and the Mann-Whitney U test, and the statistical significance level was set at p <0.05. Results: The R system was associated with the highest amount of bacterial extrusion (p <0.05). The PTU system caused more bacterial extrusion than the PTN and WO systems (p <0.05). There was no significant difference between the PTN and WO systems (p >0.05). Conclusions: All instrumentation techniques caused apical bacterial extrusion. The instrument design and preparation techniques affect the number of extruded bacteria.KEYWORDS Apical extrusion; Bacteria; Endodontics; Root canal preparation.

ABSTRACT Aim The purpose of this study was to evaluate the fracture strength of roots instrumented with the self-adjusting file (SAF; ReDent-Nova, Ra'anana, Israel) and the Reciproc reciprocating file and that were and were not obturated using the warm vertical lateral compaction technique. Materials and methods In total, 75 mandibular premolar teeth were sectioned at or below the cementoenamel junction to obtain roots 13 mm in length. The roots were balanced with respect to buccolingual and mesiodistal diameters and weight. They were distributed into four experimental groups and one control group (n = 15): No instrumentation (group I), instrumentation with SAF files but no obturation (group II), instrumentation with SAF files and obturated with warm vertical lateral compaction (group III), instrumentation with Reciproc File but no obturation (group IV), and instrumentation with Reciproc File and obturated with warm vertical lateral compaction (group V). AH Plus sealer (Dentsply DeTrey, Konstanz, Germany) was used along with gutta-percha points. One week later, a vertical load was applied to the specimen's canal until fracture occurred. Data were statistically analyzed using one-way analysis of variance (p = 0.05). Results The mean fracture load was 312.83 N for group I, 297.35 N for group II, 359.15 N for group III, 231.51 N for group IV, and 275.81 N for group V. Conclusion The fracture resistances exhibited a statistically significant difference between all the groups. Teeth instrumented by SAF exhibited a better fracture resistance. This study evaluated in vitro the capacity of debris removal from the apical third of flattened root canals, using different final irrigation protocols. Thirty human mandibular central incisors with a mesiodistal flattened root were prepared using rotary instrumentation by Endo-Flare 25.12 and Hero 642 30.06, 35.02, 40.02 files, irrigated with 2 mL of 1% NaOCl after each file. The specimens were randomly distributed into 5 groups according to the final irrigation of root canals: Group I: 10 mL of distilled water (control), Group II: 10 mL of 1% NaOCl for 8 min, Group III: 2 mL of 1% NaOCl for 2 min (repeated 4 times), Group IV: 10 mL of 2.5% NaOCl for 8 min, and Group V: 10 mL of 2.5% NaOCl for 2 min (repeated 4 times). The apical thirds of the specimens were subjected to histological processing and 6-μm cross-sections were obtained and stained with hematoxylin-eosin. The specimens were examined under optical microscopy at ×40 magnification and the images were subjected to morphometric analysis using the Scion image-analysis software. The total area of root canal and the area with debris were measured in square millimeters. Analysis of variance showed no statistically significant difference (p>0.05) among the groups GI (2.39 ± 3.59), GII (2.91 ± 2.21), GIII (0.73 ± 1.36), GIV (0.95 ± 0.84) and GV (0.51 ± 0.22). In conclusion, the final irrigation protocols evaluated in this study using the Luer syringe presented similar performance in the removal of debris from the apical third of flattened root canals.

Objective: Endodontic treatment should be both conservative and effective. Endodontic instruments with a greater taper are used for coronal flaring, for proper debridement with efficient irrigation. However, increased taper of an instrument can remove a larger amount of pericervical dentin, compromising the strength of the tooth. The aim of this study was to determine the effect of hand files, ProTaper Universal, ProTaper Next, and V Taper rotary instrument systems on the fracture resistance of teeth. Materials and Methods: In total, 60 extracted human maxillary first premolars were divided into four groups—Group I (Hand Files; HF), Group II (ProTaper Universal; PT), group III (ProTaper Next; PTN) and Group IV (V Taper; VT) (N = 15). Each group was instrumented with the respective instrument system, irrigated, obturated, restored, and mounted in cold cure acrylic. A universal load-testing machine (Shimadzu, Japan) was used to apply a vertical compressive load. The maximum force was recorded in Newton. Analysis of variance (ANOVA) and Independent t-tests were applied to compare the maximum mean force required to fracture the tooth. Results: There was a statistically significant difference in fracture resistance between Group I (HF) and Group II (PT) and between Group II (PT) and Group IV (VT) (p < 0.001). Similarly, a significant difference was observed between Group II (PT) and Group III (PTN) (p < 0.01). Furthermore, a significant difference was observed between Group I (HF) and Group III (PTN), and between Group III (PTN) and Group IV (VT) (p < 0.05), too. However, there was no statistically significant difference between Group I (HF) and group IV (VT) (p > 0.05). Conclusion: Rotary files with more taper seem to remove more pericervical dentin than traditional manual and rotary files with less taper, thus altering the strength of the tooth.

ABSTRACTObjectives: The purpose of this in vitro study was to evaluate the effect of multiple consecutive adhesive coatings of a one-step self-etch adhesive on microleakage of Class V cavities.Methods: Standardized box shaped Class V cavities were prepared onto the buccal side of forty five non-carious human premolar teeth. The teeth were randomly divided into three groups of fifteen teeth in each and restored as: Group I- one-step self-etch adhesive resin (Clearfil S3, Kuraray Co. Ltd., Osaka, Japan) was applied according to the manufacturer�s instructions, Group II- two consecutive same one-step self-etch adhesive application was performed, and Group III- three consecutive same one-step self-etch adhesive application was performed. After the adhesive applications light curing unit was activated for 20 seconds and the cavities were restored with a composite resin. The restorations were finished with aluminum oxide discs and the specimens were stored in water at room temperature for 24 hours before they were immersed in 2% methylene blue for 48 hours. The dye penetration was examined under a stereomicroscope and the asymtotic significance were analysed with Kruskall Wallis and Mann Whitney U tests and dentin-enamel margins were compared with each other with Wilcoxon Signed Rank test.Results: The microleakage at the dentinal margins of Class V cavities were significantly decreased with two (Group II) and three (Group III) consecutive adhesive applications (P<.05). There was no significant difference between Group I, Group II and Group III at the enamel margins (P<.05). The microleakage at the dentinal margins were significantly higher than the enamel margins in Group I and Group II but in Group III there was no statistically significant difference (P<.05).Conclusions: Within the limitations of this in vitro study, it may be concluded that three consecutive applications of the one-step self-etch adhesive resin provided better sealing than the one coat of adhesive resin at the dentinal margins of Class V cavities. Background. This research aimed to investigate and compare the amount of apically extruded debris after root canal preparation using ProTaper Next, ProTaper Gold, and TruNatomy systems. Methods. Forty-five extracted mandibular premolar teeth with single canals with similar lengths were used. The root canals were prepared using ProTaper Next (Dentsply Maillefer, Ballaigues, Switzerland), ProTaper Gold (Dentsply Maillefer, Ballaigues, Switzerland), or TruNatomy (Dentsply Maillefer, Ballaigues, Switzerland) files. Apically extruded debris during preparation was gathered into preweighed Eppendorf tubes. Then the Eppendorf tubes were incubated at 70°C for five days. The Eppendorf tubes were weighed again to determine their final weight plus the extruded debris. Results. The TRN system resulted in significantly less debris extrusion than the PTN system (P<0.05). There was no significant difference between the PTN and PTG groups and between the PTG and TRN groups (P>0.05). Conclusion. All the instrumentation systems caused apical extrusion of debris. However, the TRN system resulted in significantly less debris extrusion than the other systems.

ABSTRACT Aim To evaluate the accuracy of Root ZX-II (J Morita Corp) apex locator in enlarged root canals with different root canal irrigants. Materials and methods 48 freshly extracted single rooted mandibular premolar teeth were used. The apical enlargement was done up to #45 K-file as the master apical file (MAF). The teeth were randomly divided into 4 groups and mounted in an experimental apparatus. The following irrigants were used during electronic canal measurements: group 1: saline; group 2: 3% NaOCl; group 3: 2% chlorhexidine; group 4: 17% EDTA. The canal measurement was done with Root ZX-II apex locator using #10 and #45 K-file. Data were analyzed by two way ANOVA and Bonferroni multiple comparison test. Results Statistical analysis showed that there was statistically significant difference between #10 and #45 K-file for group 1 (p < 0.01). Conclusion Root ZX-II was accurate in the presence of 3% NaOCl and 17% EDTA when measured with smaller and larger files. Aim: To evaluate the surface detail reproduction of dental stone this is immersed in different disinfectant solution and studied under stereomicroscope. Methodology: Total number of 30 specimens of dental stone (Type III) were made with measurements of 1.5cm diameter and 1cm height .This samples are divided in to 3 groups group A,B,C. were A is immersed in Distilled water which was taken as control group ;B is immersed in 2% Glutaraldehyde and C is immersed in 5%sodium hypochlorite. Each specimen were immersed in the disinfectant solution for 15 minutes and dried under room temperature for 24 hrs. After 24 hrs each specimens are studied under stereomicroscope for surface details. Result: The results showed no significant difference in the surface irregularities and porosities for a group 1 and group 2 except group 3 which showed significant increase in the porosities, surface irregularities and erosions after disinfection with 5% NaHOCl by immersion method. Conclusion: The surface detail reproduction capacity of die stone was adversely affected when 5% Sodium hypochlorite was used as disinfectant solution when compare d to control group and 2% Glutaraldehyde

ABSTRACT Objective:  To evaluate the proliferation and morphology of human osteoblasts cultured on two brands of mini-implants after 24, 48, and 72 hours, in addition to the chemical composition found on their surface. Materials and Methods:  Two brands of mini-implant (Morelli and Neodent) were evaluated; polystyrene was used as a control group (n  =  3). Osteoblasts were cultured on the surface of sterilized mini-implants in a CO2 incubator at different time periods (24, 48, and 72 hours). Osteoblast proliferation was quantified by scanning electron microscopy using up to 5000× magnification, and cell morphology was analyzed by a single observer. For the chemical analysis, spectroscopy X-ray fluorescence was used to identify and quantify chemical components on the surface of the mini-implants. Results:  Two-way ANOVA showed no significant interaction between the factors studied (P  =  0.686). A Tukey test revealed no significant difference in osteoblast proliferation between the mini-implants at all studied periods; however, a difference in cell proliferation was detected between the Neodent and the control group (P  =  .025). For all groups, time had a direct and positive effect on osteoblast proliferation (P &lt; .001). The significant elements present in both brands of mini-implants were titanium, aluminum, vanadium, and iron. Conclusions:  Osteoblast proliferation was present on the mini-implants studied, which increased over time; however, no significant difference between brands was observed. No difference was seen between the mini-implants evaluated in terms of chemical composition. Cell adhesion after 72 hours suggests that areas of bone remodeling can be achieved, thus initiating the process of mini-implant anchorage.

Aim. The purpose of this in vitro study was to assess the antimicrobial efficacy of Biopure MTAD against E. faecalis in contaminated root canals. Materials and Methods. Forty-two single rooted extracted human teeth were inoculated with E. faecalis and incubated for four weeks. The samples were divided in two control and five experimental groups irrigated with 1.5% sodium hypochlorite solution (NaOCl); 3% NaOCl; BioPure MTAD; 1.5% NaOCl/17% EDTA; or 3% NaOCl/17% EDTA. After a one-week incubation, complete disinfection was confirmed by the absence of turbidity in the incubation media. Dentin shavings were taken from samples with no turbidity to verify whether E. faecalis was present in dentin tubules. Results were analyzed statistically using Fisher's exact test, with the level of significance set at . Results. Statistical analysis of the data obtained at Day 7 and after dentin shaving analysis showed that BioPure MTAD had significantly greater antibacterial activity than 1.5% NaOCl, 1.5% NaOCl/17% EDTA and 3% NaOCl/17% EDTA. No significant difference was detected between MTAD and 3% NaOCl. Conclusions. These findings suggest that BioPure MTAD possesses superior bactericidal activity compared with NaOCl and EDTA against E. faecalis.
