Abstract Aim The study aims to assess effectiveness of a three-hour suturing skills course for healthcare professionals at a tertiary hospital in London. Intended outcomes were improvement in confidence and proficiency at simple interrupted sutures. Method Four suturing sessions were delivered over five months to healthcare workers new to the clinical setting. The session included lecture, video, and practical skills. Participants completed a pre- and post-course survey to measure confidence levels in suturing using Likert scale. Participants performed simple interrupted sutures for 10 minutes in a pre- and post-course assessment. Performance was assessed using a suturing proficiency proforma. Pre- and post-course data was compared to assess improvement. Results Fifty participants attended the course. 93% had previous suturing teaching. Pre-course confidence in simple interrupted suture was 3.1 (SD = 1.2) and post course was 4.8 (SD = 0.2). One tailed T score was 14.7, and the difference was significant (p < 0.05). Pre- and post-course assessment demonstrated improvement in the following parameters and participant proportions: handling of the needle driver (50%, n = 25), adequate placement of needle driver (68%, n = 34), appropriate needle angle entering skin (60%, n = 30), following needle curve (36%, n = 18), non-touch technique (64%, n = 32), surgical knot tying (56%, n = 28). Average number of sutures completed in 10 minutes increased by 1.9 times. Conclusions The majority of participants had previously been taught suturing in other settings; however, data demonstrated improvement in confidence and performance. Ad-hoc suturing skill teaching is a valuable tool to increase confidence of healthcare workers at early stages in their careers.

The difference (D) between a person's Verbal IQ (VIQ) and Performance IQ (PIQ) has for some time been considered clinically meaningful ( Kaufman, 1976 , 1979 ; Matarazzo, 1990 , 1991 ; Matarazzo & Herman, 1985 ; Sattler, 1982 ; Wechsler, 1984 ). Particularly useful is information about the degree to which a difference (D) between scores is “abnormal” (i.e., deviant in a standardization group) as opposed to simply “reliable” (i.e., indicative of a true score difference) ( Mittenberg, Thompson, & Schwartz, 1991 ; Silverstein, 1981 ; Payne & Jones, 1957 ). Payne and Jones (1957) proposed a formula to identify “abnormal” differences, which has been used extensively in the literature, and which has generally yielded good approximations to empirically determined “abnormal” differences ( Silverstein, 1985 ; Matarazzo & Herman, 1985 ). However applications of this formula have not taken into account the dependence (demonstrated by Kaufman, 1976 , 1979 , and Matarazzo & Herman, 1985 ) of Ds on Full Scale IQs (FSIQs). This has led to overestimation of “abnormality” of Ds of high FSIQ children, and underestimation of “abnormality” of Ds of low FSIQ children. This article presents a formula for identification of abnormal WISC-R Ds, which overcomes these problems, by explicitly taking into account the dependence of Ds on FSIQs.

The working environment of healthcare workers (HCW) exposes them to sharp injuries. This communication attempts to examine the injury registers, incidence of sharps injuries and blood splash exposures, and the post-exposure prophylaxis status of employees in a tertiary care hospital. Analysis included records form 54 locations of two units of a tertiary hospital attached to a Medical College. Maintenance of the injury register overall was highly satisfactory in both units. Two hundred and nine injuries were recorded from both units of the hospital. The majority of injuries (60.5%) occurred in the age group of 20-30 years with 70% among females. Waste handlers were at increased risk during waste management procedures. Thirty two percent of sharps injury injuries occurred in wards. Of the ward nursing staff, 25.3% received sharps injuries. Post-exposure prophylaxis for Hepatitis B (primary dose) was given to 25 HCWs; 11 received booster doses. The basic regimen for HIV post-exposure prophylaxis was given to 4 HCWs. Awareness about records maintenance, regular documentation, awareness and training, and implementation of appropriate preventive measures can reduce the incidence of injuries. Summary141 patients with an ICD-9 diagnosis of schizophrenic psychosis were followed up prospectively for 3 years in 7 Italian centres, representative of the different degrees of application of the psychiatric reform law (Law 180) passed in 1978. It was agreed that each centre would treat the patients according to its routine, and that all contacts with the patients and each intervention performed would be carefully recorded in an ad hoc schedule. The baseline evaluation of psychosocial adjustment was performed by the Disability Assessment Schedule (DAS), and this assessment was then repeated every 6 months during the follow-up period. At the end of this period, the rating on the DAS section 5 was taken as a global measure of patients’ psychosocial outcome. On a stepwise logistic regression analysis, 2 variables were found to be significantly predictive of psychosocial outcome, that is the use of social and/or vocational skills training (associated with a favourable outcome) and the number of days/year of full hospitalization (associated with a poor outcome). Trieste and Arezzo were the only centres in which a significant improvement of the score on some DAS subscales (namely, “occupational role, interest” and “social withdrawal”) was detected. In the whole patient sample, the difference between the final and the baseline score on these subscales correlated significantly with the number/year of outpatient contacts and of home visits. These findings confirm the favourable impact of the community-oriented care provided in some Italian centres on the Psychosocial outcome of schizophrenic patients.

Research on stress related to the COVID-19 pandemic has been dominated by the cases of healthcare workers, students, patients, and their stress during the COVID-19 pandemic. This study examined the relationship between the amount of stress change under the COVID-19 pandemic and demographic factors (age, sex, occupation, etc.) in residents of a large city and a rural area of Japan. A total of 1331 valid responses were received in June 2020 from residents of Tokyo, Osaka, and Nagano registered with a private research firm. We were able to identify 15 statistically significant variables out of 36 explanatory variables, which explained the significant increase in stress compared to the pre-pandemic period. Multiple-factor analysis showed that the relationship with people is a more significant explanatory variable for the level of increase in stress than the difference in environment between big cities (Tokyo, Osaka) and rural areas (Nagano), the type of housing, and the decrease in income compared to the pre-pandemic period.

5G communications have become an enabler for the creation of new and more complex networking scenarios, bringing together different vertical ecosystems. Such behavior has been fostered by the network function virtualization (NFV) concept, where the orchestration and virtualization capabilities allow the possibility of dynamically supplying network resources according to its needs. Nevertheless, the integration and performance of heterogeneous network environments, each one supported by a different provider, and with specific characteristics and requirements, in a single NFV framework is not straightforward. In this work we propose an NFV-based framework capable of supporting the flexible, cost-effective deployment of vertical services, through the integration of two distinguished mobile environments and their networks: small sized unmanned aerial vehicles (SUAVs), supporting a flying ad hoc network (FANET) and vehicles, promoting a vehicular ad hoc network (VANET). In this context, a use case involving the public safety vertical will be used as an illustrative example to showcase the potential of this framework. This work also includes the technical implementation details of the framework proposed, allowing to analyse and discuss the delays on the network services deployment process. The results show that the deployment times can be significantly reduced through a distributed VNF configuration function based on the publish–subscribe model.

AbstractMulti-dimensional arrays (also known as raster data or gridded data) play a key role in many, if not all science and engineering domains where they typically represent spatio-temporal sensor, image, simulation output, or statistics “datacubes”. As classic database technology does not support arrays adequately, such data today are maintained mostly in silo solutions, with architectures that tend to erode and not keep up with the increasing requirements on performance and service quality. Array Database systems attempt to close this gap by providing declarative query support for flexible ad-hoc analytics on large n-D arrays, similar to what SQL offers on set-oriented data, XQuery on hierarchical data, and SPARQL and CIPHER on graph data. Today, Petascale Array Database installations exist, employing massive parallelism and distributed processing. Hence, questions arise about technology and standards available, usability, and overall maturity. Several papers have compared models and formalisms, and benchmarks have been undertaken as well, typically comparing two systems against each other. While each of these represent valuable research to the best of our knowledge there is no comprehensive survey combining model, query language, architecture, and practical usability, and performance aspects. The size of this comparison differentiates our study as well with 19 systems compared, four benchmarked to an extent and depth clearly exceeding previous papers in the field; for example, subsetting tests were designed in a way that systems cannot be tuned to specifically these queries. It is hoped that this gives a representative overview to all who want to immerse into the field as well as a clear guidance to those who need to choose the best suited datacube tool for their application. This article presents results of the Research Data Alliance (RDA) Array Database Assessment Working Group (ADA:WG), a subgroup of the Big Data Interest Group. It has elicited the state of the art in Array Databases, technically supported by IEEE GRSS and CODATA Germany, to answer the question: how can data scientists and engineers benefit from Array Database technology? As it turns out, Array Databases can offer significant advantages in terms of flexibility, functionality, extensibility, as well as performance and scalability—in total, the database approach of offering “datacubes” analysis-ready heralds a new level of service quality. Investigation shows that there is a lively ecosystem of technology with increasing uptake, and proven array analytics standards are in place. Consequently, such approaches have to be considered a serious option for datacube services in science, engineering and beyond. Tools, though, vary greatly in functionality and performance as it turns out.

Allometric equations use easily measurable biometric variables to determine the aboveground and belowground biomasses of trees. Equations produced for estimating the biomass within Canadian forests at a large scale have not yet been validated for eastern Canadian boreal open woodlands (OWs), where trees experience particular environmental conditions. In this study, we harvested 167 trees from seven boreal OWs in Quebec, Canada for biomass and allometric measurements. These data show that Canadian national equations accurately predict the whole aboveground biomass for both black spruce and jack pine trees, but underestimated branches biomass, possibly owing to a particular tree morphology in OWs relative to closed-canopy stands. We therefore developed ad hoc allometric equations based on three power models including diameter at breast height (DBH) alone or in combination with tree height (H) as allometric variables. Our results show that although the inclusion of H in the model yields better fits for most tree compartments in both species, the difference is minor and does not markedly affect biomass C stocks at the stand level. Using these newly developed equations, we found that carbon stocks in afforested OWs varied markedly among sites owing to differences in tree growth and species. Nine years after afforestation, jack pine plantations had accumulated about five times more carbon than black spruce plantations (0.14 vs. 0.80 t C·ha−1), highlighting the much larger potential of jack pine for OW afforestation projects in this environment.

Abstract Background Occupational hazards like sharp injury and splash exposure (SISE) are frequently encountered in health-care settings. The adoption of standard precautions by healthcare workers (HCWs) has led to significant reduction in the incidence of such injuries, still SISE continues to pose a serious threat to certain groups of HCWs. Materials and Methods This was a retrospective study which examined the available records of all patients from January 2015 to August 2019 who self-reported to our emergency department with history of sharp injury and/or splash exposure. Details of the patients, mechanism of injury, the circumstances leading to the injury, status of the source (hepatitis B surface antigen, human immunodeficiency virus, and hepatitis C virus antibody status), and the postexposure prophylaxis given were recorded and analyzed. Data were represented in frequency and percentages. Results During the defined period, a total of 834 HCWs reported with SISE, out of which 44.6% were doctors. Majority of the patients have SISE while performing medical procedures on patients (49.5%), while 19.2% were exposed during segregation of waste. The frequency of needle stick injury during cannulation, sampling, and recapping of needle were higher in emergency department than in wards. More than 80% of HCWs received hepatitis B vaccine and immunoglobulin postexposure. Conclusion There is need for periodical briefings on practices of sharp handling as well as re-emphasizing the use of personal protective equipment while performing procedures.
