Abstract   Dysphagia is a common adverse effect of fundoplication and in some patients, there is no clear identifiable cause despite extensive investigation. Subtle anatomical features of anti-reflux surgery may relate to dysphagia. In this study multiple observers examine gastro-esophageal junction (GEJ) anatomy using objective measures of video-fluoroscopy swallow study (VFSS) to explore possible correlates with post-fundoplication dysphagia. Methods Thirty-one patients underwent structured VFSS 6–12 months after laparoscopic total (TotLF) or partial anterior (PAntLF) fundoplication recording: standing AP, 2x standing oblique (SOb), 2x prone oblique (POb), and prone oblique with continuous drinking (PObCont). Post-operative dysphagia was evaluated (Dakkak & Bennett Score: range 0–45; troublesome ≥12). Three observers (Obs1–3) independently measured: GEJ displacement anteriorly; degree of axis deviation of esophagus & GEJ (E-GEJ); posterior distal esophageal angle (PDEA); and GEJ opening diameter cf. maximal distal esophageal diameter (ME-GEJ). Correlations between measurements and dysphagia by operation type were assessed using linear regression analysis and linear mixed-effects models. Results Post-operatively, 5/18 TotLF and 4/13 PAntLF patients reported troublesome dysphagia. Three observers independently found: patients with troublesome dysphagia after TotLF had greater anterior displacement of the GEJ (SOb, range 0.61 cm–1.18 cm, Obs1 p = 0.04), and larger axis deviation of E-GEJ (POb, range 3.28°-13.07°, Obs2 p = 0.03) compared to patients with no/mild dysphagia. There was a trend for greater PDEA in patients with troublesome dysphagia after TotLF (POb, range 0.46°-2.12° and PObCont, range 3.37°-13.4°), but this trend did not reach statistical significance. Following PAntLF, all observers recorded a reduction in ME-GEJ for each unit of worsening dysphagia (PObCont, range 0.03 cm–0.04 cm, Obs1 p = 0.02, Obs2 p = 0.02). Conclusion Multiple observers concur that anterior GEJ displacement, the angle between the esophagus axis—GEJ axis, and posterior distal esophageal angulation are anatomical factors associated with troublesome dysphagia after total fundoplication. After partial anterior fundoplication, a small reduction in GEJ opening diameter relative to the distal esophagus related to worsening dysphagia. To reduce post-fundoplication dysphagia, attention to operative techniques affecting angulation and luminal diameter at the GEJ by fundoplication and hiatal repair is warranted.

Background. This prospective study was conducted to identify a suitable alternative to birth weight and establish its cutoff point to facilitate the identification of low-birth-weight (LBW) infants in Enugu, Southeast Nigeria. Methods. The study involved newborn babies within the first 48 hours of life. Five anthropometric measurements (head, chest, mid-arm and calf circumferences, as well as abdominal girth) were taken using a tape measure while supine length was measured with an aluminum infantometer. Birth weight was also recorded. Linear regression analysis was done to identify the measurement with the highest coefficient of determination with birth weight while its cutoff point was defined using a receiver operating characteristic curve. Standard statistical tests were used to determine the statistical significance of the findings. Results. The LBW prevalence for the study population was 21.41%. Chest circumference had the highest R2 value of 0.83 for the general study population and 0.72 for the LBW infants. The identified cutoff point for chest circumference is ⩽30 cm. Conclusion. Chest circumference is the best alternative to birth weight in identifying LBW babies within the first 48 hours of life in this environment.

Abstract. The paper presents changes and mathematical models of autofluorescence of reduced nicotinamide adenine dinucleotide (NADH) and flavin adenine dinucleotide (FAD) and their redox ratio (RR) in the rat skeletal muscle during the first 24 hours after death. Aim. To establish a pattern of change in the fluorescence intensity of NADH and FAD coenzymes, as well as their relationship in the skeletal muscle during the first 24 hours after death using mathematical modeling. Material and methods. The experiment was performed on Sprague Dawley rats. The fluorescence intensity of FAD and NADH coenzymes in skeletal muscle was measured in situ in living rats under general anesthesia, 5 minutes after euthanasia, and at intervals of 1.5-3 h during the 24-hour postmortem period. To estimate the fluorescence intensity of NADH and FAD coenzymes and to calculate the RR, the Lasma MC-3 fluorescence measuring device with special software was used. The obtained data were analyzed by non-linear regression analysis. The summary, accuracy estimation and significance of the regression equation coefficients were assessed using SigmaPlot 10.0 software. The significance of the regression model was tested using the Fisher F-criterion. Results. During the first 3 hours of the postmortem period, an increase in the mean values of RR and NADH fluorescence was detected, starting from 4.5 and till 24 hours post-mortem their gradual decrease was observed. The relationship between NADH, RR and time after death is characterized by the Weibull equation. The statistical significance of NADH and RR models, based on the obtained equations was considered high. Conclusion. The analysis of the obtained data allowed to create mathematical models describing the relationship between RR and NADH fluorescence intensity and time after death, which confirms non-randomness and regularity of the discovered patterns.time of death; NADH; FAD; auto-fluorescence; redox ratio; early post-mortem changes

Based on the premise that elevated blood pressure and low bone mass have both been associated with poor Ca nutriture and disturbances in Ca metabolism, a cross-sectional study was employed to determine if blood pressure and dietary Ca intake were significantly related to bone mass. Forty-seven men between 24–77 years of age with blood pressure values ranging from normal to mildly elevated comprised the study group. Blood pressure was measured with a random-zero sphygmomanometer. Bone mineral content (BMC) and density (BMD) of the hip, spine and total body were measured with dual-photon absorptiometry. Dietary intake and physical activity were also assessed. Multiple linear regression analysis was used for statistical analysis. After adjusting for known confounding variables (age, BMI, Ca intake, and others) diastolic blood pressure was negatively related to BMC (P ≤ 0·05) and BMD (P ≤ 0·01) of the total body, trochanteric region (P < 0·01) and Ward's triangle (P < 0·05), and to BMC of the femoral neck (P < 0·05) and lumbar spine, although the latter was just shy of statistical significance (P = 0·058). Systolic blood pressure was negatively related to trochanteric BMD (P = 0·04) and BMC (P = 0·06). Ca intake was positively related to total body BMD (P = 0·005), and BMC of the lumbar spine (P = 0·05). In this population of men, Ca intake was a positive predictor, and blood pressure was a negative predictor of regional measures of bone mass. These findings support the concept that independent of age, BMI and Ca intake, elevated blood pressure varies indirectly with bone mass and density, known predictors of osteoporotic fractures. Future studies are needed to determine whether elevated blood pressure is causally related to the development of low bone mass, and what role dietary Ca plays in that pathway.

Objectives: The objective of this study was to describe patterns of antimicrobial resistance to gentamicin (Gen) and amikacin (Ak) among Gram-negative aerobic bacteria during 1-year period and to determine the association between antibiotic resistance and the consumption of Gen.Methods: Aminoglycosides consumption at Soba University Hospital wards was measured and susceptibility of Gram-negative bacteria for the same period was evaluated. Consumption data were converted to defined daily doses (DDDs)/100 bed days based on DDD/anatomical therapeutic chemical the WHO system. The association between the frequency of strains resistant to Gen and Ak and their consumption was assessed by linear regression analysis using Spearman’s correlation. The level of statistical significance was set at p<0.05.Results: A total of 973 Gram-negative isolates were identified and tested for antimicrobial susceptibility to Gen and Ak. Resistance to Gen alone was found to be 19.42%; n=189, resistance to Ak alone was found to be 3.08%; n=30, and resistance to Gen plus Ak was found to be 5.24%; n=51. Pseudomonas aeruginosa was the most resistant pathogen to Ak plus Gen (2.26%; n=22). A positive correlation between the increases in the use of Gen and the prevalence of bacterial resistance among hospital wards was found (correlation coefficient r=0.6; p=0.04).Conclusion: Gen and Ak are still highly active antimicrobial agents for the treatment of aerobic Gram-negative bacteria at times of intensified resistance to other antimicrobial agents. Monitoring the use of aminoglycosides is very important too.

Ζoonotic parasitic diseases that can occur through animal contact pose risks to pets, their owners and to their bond. This study aims to assess the level of knowledge about zoonoses, specifically echinococcosis and toxocariasis, among cat/dog owners and non-pet owners in Greece. Multiple-choice questionnaires were designed to obtain data regarding the knowledge of pet and non-pet owners on echinococcosis and toxocariasis, including signs and symptoms of these zoonoses, ways of transmission and precautions that need to be taken into account in order to avoid it. A total of 185 questionnaires were retrieved and data was expressed as absolute (Ν) and relative frequencies (%). Associations between pet ownership, residence and outcome variables were evaluated using the Fisher exact test and Chi-squared test, respectively. Multifactorial linear regression analysis was used to investigate the cross-sectional association between demographic characteristics and the awareness of helminthic zoonoses. All tests were two-sided and statistical significance was set at p < 0.05. Our study revealed a disturbing lack of awareness of echinococcosis and toxocariasis (mean zoonotic knowledge score 8.11 ± 3.18) independently of pet ownership. Surprisingly, in some cases the ignorance of pet owners exceeded that of non-pet owners. Given the progressive impact of toxocariasis in public health and the high prevalence of echinococcosis in the Mediterranean region, measures should be taken to inform people about zoonoses and eliminate their putative transmission.

Postoperative handover of patients has been described as a complex work process challenged by interruptions, time pressure and a lack of supporting framework. The purpose of this study was to investigate involved personnel’s experiences with the quality of patient handovers between the operating room and the postoperative anesthesia care unit (PACU) before and after implementation of a structured tool for communication. The study was conducted in a hospital in South-eastern Norway. Personnel completed a questionnaire before (n=116) and after (n=90) implementation of the Identification-Situation-Assessment- Recommendations (ISBAR)- tool. Analysis included summative statistics, t-tests and generalized linear regression analysis. Statistical significance assumed at P<0.05. The overall impression of quality in handovers improved significantly after implementation of the ISBAR (P=0.001). Personnel’s experiences were improved in relation to that handovers followed a logical structure, available documentation was used and all relevant information was communicated (P<0.001). Moreover, personnel found it easier to establish contact at the beginning of the handover, ambiguities were resolved and documentation was more complete (P=0.001). Profession was associated with seven of the statements, relating to whether relevant information is clearly communicated, whether possible risks and complications are discussed, contact easily established, and to completeness of documentation and information. In addition, findings indicate significantly more negative experiences among receiving personnel both pre- and post-implementation. Implementation of a structured tool for communication in patient handovers, may improve quality and safety in patient handovers between the operating room and the PACU. Research is needed to define optimal patient handovers and to determine the effect of handover quality on patient outcomes.

Abstract In 212 thalassemia major patients, repeated assessments for cardiac and hepatic iron (LIC) assessed by Magnetic Resonance Imaging (MRI – T2*) have been performed. The chelation regimes were either desferrioxamine (DFO), deferiprone (DFP), combination of DFO and DFP (Comb) or deferasirox (DFX). In general over the last few years, tailoring of chelation therapy has been principally guided by the cardiac iron loading. As many patients had been found to have excess cardiac iron, the majority (48%) had been placed on Comb. Patients were grouped according to the degree of siderosis. A T2* of &lt;1.6ms was regarded as heavy LIC, between 1.6–4.0 moderate, 4.1–9.0 mild and &gt; 9.1 acceptable. Taking into account that the change in T2* is not necessarily linear with respect to time and as the overall time of exposure to DFO, DFP and Comb regimes was significantly greater than that with DFX it was unjustified to perform comparative analysis using the total time period of the patients who were on any of the non DFX regimes. Therefore, to compare the efficacy of the four regimes on LIC, we performed an analysis using student T test to assess the rate of change only between the first and second MRI in patients with comparable LIC according to each chelation regime with adjustment for overall time of exposure (Table 1). Using the same data and applying linear regression analysis (Table 2) we compared the effect of DFO to the other three regimes in the annual rate of increasing hepatic T2*. Only Comb is effective at all levels of hepatic iron loading in reducing the iron content. DFX is effective in the mildly iron loaded patients and for the moderately iron loaded patients, its efficacy approaches statistical significance. DFP does not seem to significantly decrease LIC at any level of hepatic iron load however the numbers of patients in that group are very small. Interestingly DFO seems the least effective at all levels of hepatic iron loading and particularly in the heavy loaded patients. This factor may be related to poor compliance to its use as the patients who have reached such levels of iron load are more often those who are not compliant. In the comparison analysis to DFO, only Comb is significantly better and DFP and DFX are equivalent to it. In addition Comb is more effective than DFX and DFP in that over 12 months it would increase the T2* by 3.8ms (p &lt;0.001) and 3.9ms (p 0.012) respectively. DFX and DFP are similar in efficacy in that they maintain the liver iron at the same levels (DFP vDFX 0.009ms p=0.95). In patients with hepatic T2* &gt;9ms, 4 of 11 on DFO, 5 of 6 on DFX, 7 of 11 on DFP and 3 of 22 on Comb fell below 9. It is of note that DFO only maintains LIC and that a number of patients in the normal range increased LIC. Taking this data into account the DFX and DFP results are compatible with those seen both in the clinic and in trials. It is apparent however that combination therapy is the most effective regime for reducing hepatic iron significantly. As with cardiac iron loading, by knowing the degree of hepatic iron loading by the non-invasive T2* measurement and being able to manipulate patients chelation regimes, it seems possible to be able to have patients free of excess hepatic iron and potentially reduce other iron related morbidities as well. AbstractIn pursuit of better CRTh2 receptor antagonist agents, QSAR studies were performed on a series of 2,4-disubstituted phenoxyacetic acid derivatives. Stepwise multiple linear regression analysis was performed to derive QSAR models which were further evaluated for statistical significance and predictive power by internal and external validation. The best QSAR model was selected; having the correlation coefficient R = 0.904, standard error of estimation SEE = 0.456 and the cross validated squared correlation coefficient Q 2 = 0.739. Predictive ability of the selected model was also confirmed by the leave one out cross validation method and by leave 33 % out Q 2 = 0.688. The QSAR model indicates that the descriptors (logP, SI3, LM, and DVZ) play an important role in the CRTh2 receptor antagonist activities. Results of the present study may be useful in the designing of more potent 2,4-disubstituted phenoxyacetic acid derivatives as CRTh2 receptor antagonist agents.

This study aims to investigate (1) the ecotourism perception of tourists visiting Monkey Forest Ubud; (2) the visitor satisfaction level on Monkey Forest Ubud attraction; and (3) the influence of ecotourism perception on visitor satisfaction level at Monkey Forest Ubud. The result of this research carried out descriptive statistic by using an importance-performance analysis (IPA) and inferential statistic by using a simple linear regression analysis. The technique of determining sample size is incidental sampling technique by distributing questionnaires to 170 tourists visiting Monkey Forest Ubud. The result showed that tourists which visit Monkey Forest Ubud strongly agree on the implementation of ecotourism concept. On the other hand, the calculation results of concordance rate showed 89.59% which means that the overall tourist is satisfied with the Monkey Forest Ubud attraction. Moreover, based on the hypothesis testing by using t-test statistical significance showed that there is a significant influence of independent variable (perception of ecotourism) on the dependent variable (tourist satisfaction).

Abstract BACK GROUND Caring behaviors are actions concerned with the wellbeing of the patients such as sensitivity, comforting, attentive listening, honest and nonjudgmental acceptance. Nurses have an opportunity to convey caring and feeling being cared through their behavior. Behavior associated with caring has a paramount role in linking nursing interaction to the client in experiences but, the concept is ambiguous and elusive toward different scholars to reach on common understanding which is myth in dealing with caring behavior. Only a few studies have been done on the caring behavior and associated factors globally, and no study was done in this study area. OBJECTIVE To assess caring behavior and its associated factors among nurses working in Jimma University specialized hospital, south west Ethiopia. METHODS An institutional based cross-sectional study design was conducted on a sample of 224 nurses working in Jimma university specialized hospital from March 20-April 20, 2019. Data was collected by self-administered questionnaire. Descriptive statistics including frequency table, mean, standard deviation and percentage were employed. Bivariate and multiple linear regression analysis were used with regression coefficient (β), coefficient of determinant (R2), CI 95% and p<0.05 were used for statistical significance. RESULTS The overall proportion of nurses caring behavior was 80.3% which was mostly measured in terms of professional –technical (82.9%) and psychosocial (81.3%) dimension. Job satisfaction as personal satisfaction (beta=1.12, p=0.00), professional satisfaction, (beta=1.07, p=0.00), joint participation in caring process (beta=0.58, p=0.00,) satisfaction with nurse management (beta=0.85, p=00) were significantly associated with caring behavior. CONCLUSION The proportion of nurse’s who had high perception of caring behavior was found to be lower. Thus, all predictors have their own effect on enhancing job satisfaction, improving and creating conducive management and working environment to increase caring behavior. Further comparative studies involving multidisciplinary and patient point of view were recommended.
