Objectives: To conduct a systematic review and meta-analysis on dental caries among the elderly in Iran. Background: The prevalence of dental caries among elderly is high worldwide, and the major burden of oral diseases is caused by dental caries. Methods: Systematic review of the published and grey literature performed. Six international and local databases used to provide the most comprehensive population-based studies. National oral health surveys, as well as national disease and health surveys considered as other primary sources of data. Quality of remained studies was assessed by a modified tool designed based on STROBE statement checklist to evaluate the cross-sectional studies. The target population was 65+-year-olds Iranian population. Results: Overall, 917 English articles who those reported dental caries among all age groups were found in international databases and 2138 Persian articles were found in Iranian databases. After quality assessment, and excluding other age groups, 46 points of data with 10411 aged people ≥ 65 years, were included in the meta-analysis. Mean pooled decayed, missing, and filled teeth among the elderly was 26.84 (26.41-27.28). This index was 26.78 (26.12-27.43) in women and 26.91 (26.32-27.50) in men. Mean number of decayed teeth was 1.48 (1.32-1.65). Mean pooled missing teeth was 24.83 (24.20-25.46), and mean pooled filled teeth was 0.14 (0.12-0.17). The majority (92%) of DMFT was related to missing teeth. Conclusion: Dental caries has a very high burden on the oral health of the elderly in Iran, mainly responded by tooth extraction.

Abstract This systematic review compared the oral health status between institutionalized and non-institutionalized elders. The following electronic databases were searched: PubMed (Medline), Scopus, Web of Science, Lilacs and Cochrane Library, in a comprehensive and unrestricted manner. Electronic searches retrieved 1687 articles, which were analyzed with regards to respective eligibility criteria. After reading titles and abstracts, five studies were included and analyzed with respect their methodological quality. Oral status of institutionalized and non-institutionalized elderly was compared through meta-analysis. Included articles involved a cross-sectional design, which investigated 1936 individuals aged 60 years and over, being 999 Institutionalized and 937 non-institutionalized elders. Studies have investigated the prevalence of edentulous individuals, the dental caries experience and the periodontal status. Meta-analysis revealed that institutionalized elderly have greater prevalence of edentulous (OR = 2.28, 95%CI = 1.68-3.07) and higher number of decayed teeth (MD = 0.88, 95%CI = 0.71-1.05) and missed teeth (MD = 4.58, 95%CI = 1.89-7.27). Poor periodontal status did not differ significantly between groups. Compared to non-institutionalized, institutionalized elders have worse dental caries experience.

<p><strong>Objective</strong>: To describe prevalence dental caries and to study the association between nutritional status and oral health based on various indices among adolescents from under privileged communities. <strong>Material and Methods</strong>: The study was based on a cross-sectional sample of 196 apparently healthy children (104 males and 92 females) in the age range of 14 to 18 years belonging to under-privileged communities. Each subject was measured for height, body weight to assess nutritional status and clinically observed for various oral health traits like dental caries, plaque, calculus and gingivitis. <strong>Results</strong>: Decayed, missing due to caries and filled teeth (DMF) index was low among adolescent children; it was 0.48 in males and 0.93 in females. Prevalence of calculus was higher among females through all age groups, while prevalence of plaque was higher among males. Sex differences were significant only for plaque index and DMF index. The inadequacy of nutrition was not a major determinant for the observed magnitude of soft deposits, plaque and calculus indices except for Oral health status index and DMF index where higher magnitude of the indices were observed in underweight children than the normal. <strong>Conclusion</strong>:  The inadequate nutritional status was not a major determinant of oral health indicating the general awareness of oral hygiene and its observance was a major factor. AbstractBackgroundA year following the onset of the COVID-19 pandemic, new infections and deaths continue to increase in Europe. Serological studies, through providing evidence of past infection, can aid understanding of the population dynamics of SARS-CoV-2 infection.ObjectivesThis systematic review of SARS-CoV-2 seroprevalence studies in Europe was undertaken to inform public health strategies including vaccination, that aim to accelerate population immunity.MethodsWe searched the databases Web of Science, MEDLINE, EMBASE, SCOPUS, Cochrane Database of Systematic Reviews and grey literature sources for studies reporting seroprevalence of SARS-CoV-2 antibodies in Europe published between 01/12/2019 - 30/09/20. We provide a narrative synthesis of included studies. Studies were categorized into subgroups including healthcare workers (HCWs), community, outbreaks, pregnancy and children/school. Due to heterogeneity in other subgroups, we only performed a random effects meta-analysis of the seroprevalence amongst HCWs stratified by their country.Results109 studies were included spanning 17 European countries, that estimated the seroprevalence of SAR-CoV2 from samples obtained between November 2019 – August 2020. A total of 53/109 studies included HCWs with a reported seroprevalence among HCWs ranging from 0.7% to 45.3%, which did not differ significantly by country. In community studies significant heterogeneity was reported in the seroprevalence among different age groups and the majority of studies reported there was no significant difference by gender.ConclusionThis review demonstrates a wide heterogeneity in reported seroprevalence of SARS-CoV-2 antibodies between populations. Continued evaluation of seroprevalence is required to understand the impact of public health measures and inform interventions including vaccination programmes.

Identification of the association between Early Childhood Caries (ECC) and Iron Deficiency Anemia (IDA) will aid pediatricians and pediatric dentists to enhance health promotion measures to reduce the related morbidity in children. This systematic review aims to determine an evidence based association between ECC and IDA. A systematic search was carried out from MEDLINE via PubMed, EMBASE, LILACS, Cochrane Oral Health Group’s Specialised Register, CINAHL via EBSCO, Web of Science and Scopus up to May 2020. Hand searching and grey literature screening were also conducted. Cross-sectional, case-control and cohort studies in English language which assessed the association was included. Two reviewers independently assessed the study quality and extracted the outcome data. A total of 1434 studies were identified. Fourteen studies qualified for qualitative review and seven of them for a meta-analysis. In comparison with children not affected by ECC, those affected had an increased likelihood of IDA (OR = 6.07 (3.61, 10.21). The meta- analysis showed no statistical difference when comparing blood parameters (Hb, MCV and Serum ferritin) in children with and without ECC. This systematic review demonstrates an association between ECC and increased odds of IDA rather than it being the cause for IDA. Further longitudinal studies with robust methodology are required to determine an evidence based association.

Abstract Background Observational studies examining the association between oral health and juvenile idiopathic arthritis (JIA) among children and adolescents have reported inconsistent findings. The aims of this systematic review and meta-analysis were to ascertain a potential difference in oral health and oral health-related quality of life (OHRQoL) among children and adolescents with JIA and healthy peers, and to assess the association of prevalence of oral diseases/conditions, temporomandibular disorders (TMD), including temporomandibular joint (TMJ) diseases, in relation to activity and severity of JIA. Method Medline Ovid, Embase, CINAHL, SweMed+ and Cochrane Library were searched up to 25 November 2018. All articles published in English, German and Scandinavian languages focusing on children and adolescents with JIA and without JIA in relation to oral health measures, were considered. Two authors independently evaluated observational studies for inclusion. The study quality was assessed using modified Newcastle Ottawa Scale. Meta-analysis was performed for studies focusing on dental caries as an outcome. Results Nineteen articles met the inclusion criteria, covering a range of oral diseases/conditions and OHRQoL. Eighteen studies had cross-sectional design. No mean difference of dmft/DMFT indices (decayed/missed/filled teeth) was observed between the JIA - and healthy group. None of the oral health measures including dental erosive wear, enamel defects, dental maturation and OHRQoL, indicated better oral health among children and adolescents with JIA compared to healthy group. However, periodontal conditions and TMD were more predominant among children and adolescents with JIA compared to healthy peers. Conclusions Based on the cross-sectional studies, periodontal diseases and TMD were found to be more frequent in children and adolescents with JIA compared to healthy peers. Furthermore, more high-quality studies with large sample size are needed before we infer any concrete conclusion regarding the association between the prevalence of oral and TMJ diseases or oral conditions in relation to activity and severity of JIA.

Abstract Background: Dental caries is the most common chronic untreated disease worldwide. The simplest and most important factor in preventing dental caries is maintaining oral hygiene and removing microbial plaque using a toothbrush. Despite the relationship between toothbrush filament wear and plaque removal effectiveness as a potentially important factor in maintaining oral health, there is little objective standard evidence as to 1) what constitutes a worn-out brush and 2) the degree of loss in plaque removal effectiveness due to brush wear. Contradictions in the results of studies on toothbrushing and the loss of its effectiveness in removing plaque based on the time spent using the toothbrush have led to conflicting recommendations for changing toothbrushes after different periods. While some studies generally question the relationship between toothbrush age and effectiveness. The lack of comprehensive evidence in this area necessitates a structured review study.Methods: We will search the electronic databases ISI, Scopus, and PubMed to find related articles. Our main inclusion criterion is Clinical trial and observational studies investigating manual toothbrush longevity in the natural toothbrush-worn model on each objective indicator of oral health (including plaque removal and gingival indices ...). All funded citations are entered into the Endnote software. the full texts of potentially relevant studies are prepared. study selection and extracting the data will be performed by two reviewers. Also, the studies quality will be assessed. The findings will be displayed using figures, summary tables and narrative summaries. If the similarity of studies and their quality is desirable, meta-analysis will be performed. We will assess the heterogeneity on the bias of the magnitude of heterogeneity variance parameter. We are also going to conduct subgroup analysis and sensitivity analysis if needed.Discussion: The final systematic review highlights the gaps in the available evidence about the effectiveness of toothbrush longevity on each oral indices to provide the best recommendation for toothbrush renewal periods. Registration: The review subject has been submitted in PROSPERO database

Malnutrition in the elderly could be tackled by addressing socioeconomic factors. This study aimed to determine the magnitude of the relationship between socioeconomic factors and the malnutrition or malnutrition risk (MR) in the elderly. The PubMed and SCOPUS databases were searched for observational studies that included assessment of malnutrition or/and MR and socioeconomic variables (educational level, living alone, marital status, income and occupational level, feeling of loneliness, place of residence, and food expenditure) in ≥60-year-old subjects, published in English among 2000–2018 (PROSPERO: CRD42019137097). The systematic review included 40 observational studies (34 cross-sectional and 4 cohort studies) and 16 cross-sectional studies in the meta-analysis (34,703 individuals) of malnutrition and MR in relation to low educational level (Odds Ratio (OR): 1.48; 95% Confidence Interval (CI): 1.33–1.64; p < 0.001), living alone (OR: 1.92; 95% CI: 1.73–2.14; p < 0.001), being single, widowed, or divorced (OR: 1.73; 95% CI: 1.57–1.90; p < 0.001), and low income level (OR: 2.69; 95% CI: 2.35–3.08; p < 0.001), and considering these four socioeconomic factors, malnutrition and MR is associated with them (OR: 1.83; 95% CI: 1.73–1.93; p < 0.001). Malnutrition and MR could be reduced by increasing economic level, supporting people living alone or being single, widowed, and divorced, and improving lifelong learning.

AbstractDetermine age-specific infection fatality rates for COVID-19 to inform public health policies and communications that help protect vulnerable age groups. Studies of COVID-19 prevalence were collected by conducting an online search of published articles, preprints, and government reports that were publicly disseminated prior to 18 September 2020. The systematic review encompassed 113 studies, of which 27 studies (covering 34 geographical locations) satisfied the inclusion criteria and were included in the meta-analysis. Age-specific IFRs were computed using the prevalence data in conjunction with reported fatalities 4 weeks after the midpoint date of the study, reflecting typical lags in fatalities and reporting. Meta-regression procedures in Stata were used to analyze the infection fatality rate (IFR) by age. Our analysis finds a exponential relationship between age and IFR for COVID-19. The estimated age-specific IFR is very low for children and younger adults (e.g., 0.002% at age 10 and 0.01% at age 25) but increases progressively to 0.4% at age 55, 1.4% at age 65, 4.6% at age 75, and 15% at age 85. Moreover, our results indicate that about 90% of the variation in population IFR across geographical locations reflects differences in the age composition of the population and the extent to which relatively vulnerable age groups were exposed to the virus. These results indicate that COVID-19 is hazardous not only for the elderly but also for middle-aged adults, for whom the infection fatality rate is two orders of magnitude greater than the annualized risk of a fatal automobile accident and far more dangerous than seasonal influenza. Moreover, the overall IFR for COVID-19 should not be viewed as a fixed parameter but as intrinsically linked to the age-specific pattern of infections. Consequently, public health measures to mitigate infections in older adults could substantially decrease total deaths.

ObjectiveWe performed a systematic review and a meta-analysis to estimate the prevalence of human papillomavirus (HPV) in ovarian cancer.MethodsA comprehensive search of the Cochrane Library, MEDLINE, CANCERLIT, LILACS, Grey literature and EMBASE was performed for articles published from January 1990 to March 2012. The following MeSH (Medical Subject Headings) terms were searched: “ovarian tumor” or “ovarian cancers” and “HPV” or “human papillomavirus.” Included were case-control and cross-sectional studies, prospective or retrospective, that evaluated clinical ovarian cancer and provided a clear description of the use of in situ hybridization, Southern blot hybridization, and polymerase chain reaction. The statistical analysis was performed using REVMAN 5.0.ResultsIn total, 24 primary studies were included in this meta-analysis. Studies from 11 countries on 3 continents contained data on HPV and ovarian cancer, including 889 subjects. Overall, the HPV prevalence in patients with ovarian cancer was 17.5 (95% confidence interval [CI], 15.0%–20.0%). Human papillomavirus prevalence ranged from 4.0% (95% CI, 1.7%–6.3%) in Europe to 31.4% (95% CI, 26.9%–35.9%) in Asia. An aggregate of 4 case-control studies from Asia showed an odds ratio of 2.48 (95% CI, 0.64–9.57).ConclusionsWe found a high prevalence of HPV-positive DNA in ovarian cancer cases, but the role of HPV in ovarian cancer remains inconclusive. Further studies are needed to control case to answer this question.

BackgroundLiterature supports a strong relationship between childhood maltreatment and mental illness but most studies reviewed are cross-sectional and/or use recall to assess maltreatment and are thus prone to temporality and recall bias. Research on the potential prospective impact of maltreatment reduction on the incidence of psychiatric disorders is scarce.MethodElectronic databases and grey literature from 1990 to 2014 were searched for English-language cohort studies with criteria for depression and/or anxiety and non-recall measurement of childhood maltreatment. Systematic review with meta-analysis synthesized the results. Study quality, heterogeneity, and publication bias were examined. Initial screening of titles and abstracts resulted in 199 papers being reviewed. Eight high-quality articles met eligibility criteria. Population attributable fractions (PAFs) estimated potential preventive impact.ResultsThe pooled odds ratio (OR) between any type of maltreatment and depression was 2.03 [95% confidence interval (CI) 1.37–3.01] and 2.70 (95% CI 2.10–3.47) for anxiety. For specific types of maltreatment and depression or anxiety disorders, the ORs were: physical abuse (OR 2.00, 95% CI 1.25–3.19), sexual abuse (OR 2.66, 95% CI 1.88–3.75), and neglect (OR 1.74, 95% CI 1.35–2.23). PAFs suggest that over one-half of global depression and anxiety cases are potentially attributable to self-reported childhood maltreatment. A 10–25% reduction in maltreatment could potentially prevent 31.4–80.3 million depression and anxiety cases worldwide.ConclusionThis review provides robust evidence of childhood maltreatment increasing the risk for depression and anxiety, and reinforces the need for effective programs and policies to reduce its occurrence.
