Introduction: Prior meta-analysis has reported mortality rates among post-operative bariatric patients, but they have not considered psychiatric factors like suicide contributing to mortality. Objectives: The current meta-analysis aims to determine the pooled prevalence for mortality and suicide amongst cohorts using reported suicides post bariatric surgery. It is also the aim of the current meta-analytical study to determine moderators that could account for the heterogeneity found. Results: In our study, the pooled prevalence of mortality in the studies which reported suicidal mortality was 1.8% and the prevalence of suicide was 0.3%. Mean body mass index (BMI) and the duration of follow-up appear to be significant moderators. Conclusions: Given the prevalence of suicide post bariatric surgery, it is highly important for bariatric teams to consider both the medical and psychiatric well-being of individuals pre- and post-operatively.

Gifted youth frequently experience perfectionism, which for some can affect their emotional well-being. This study evaluated the effectiveness of a 6-week mindfulness intervention for 42 middle school students who were identified as Gifted and Talented. Participants’ levels of self-oriented and socially prescribed perfectionism, as well as their use of mindfulness practices, were evaluated pre- and post- intervention, as well as at a 5-week follow-up to assess changes in perfectionism ratings and use of mindfulness. Three Bonferroni-adjusted, one-way, repeated measures analyses of variance were performed, and the results indicated a significant decrease in self-oriented perfectionism and an increase in mindfulness at both the end of the intervention and at follow-up. These findings support the use of mindfulness as an appropriate intervention to reduce self-imposed forms of perfectionism among gifted youth.

ObjectivesWe conducted an individual participant meta-analysis to test the hypothesis that cortisol patterns indicative of dysregulated hypothalamic–pituitary–adrenal axis functioning would be prospectively associated with poorer well-being at follow-up.SettingFour large UK-based cohort studies.ParticipantsThose providing valid salivary or serum cortisol samples (n=7515 for morning cortisol; n=1612 for cortisol awakening response) at baseline (age 44–82) and well-being data on the Warwick Edinburgh Mental Wellbeing Scale at follow-up (0–8 years) were included.ResultsWell-being was not associated with morning cortisol, diurnal slope or awakening response though a borderline association with evening cortisol was found. Adjusting for sex and follow-up time, each 1 SD increase in evening cortisol was associated with a −0.47 (95% CI −1.00 to 0.05) point lower well-being. This was attenuated by adjustment for body mass index, smoking and socioeconomic position. Between-study heterogeneity was low.ConclusionsThis study does not support the hypothesis that diurnal cortisol is prospectively associated with well-being up to 8 years later. However, replication in prospective studies with cortisol samples over multiple days is required.

BACKGROUND There is a need for regularly updating the evidence base on the effectiveness of online mindfulness-based interventions (MBIs), especially considering how fast this field is growing and developing. OBJECTIVE The current study presents an updated meta-analysis of randomised controlled trials assessing the effects of online MBIs on mental health, as well as the potential moderators of those effects. METHODS A systematic literature search was conducted in PsycINFO, PubMed and Web of Science up to July 19th, 2019. Seventy trials totalling 91 comparisons were included. Pre-to-post and pre-to-follow-up between-group effect sizes (Hedges’ g) were calculated for depression, anxiety, stress, well-being and mindfulness, using a random effects model. RESULTS The findings revealed statistically significant moderate pre-to-post effects on stress (g=0.41) and mindfulness (g=0.43) and small effects on depression (g=0.31) and anxiety (g=0.23). For well-being, a significant effect was found only when omitting low quality studies (g=0.34). Significant but small follow-up effects were found for depression (g=0.25), anxiety (g=0.17) and stress (g=-0.11). Subgroup analyses revealed that online MBIs resulted in higher effect sizes for stress when offered with guidance. For stress and mindfulness, studies that used an inactive control condition yielded larger effects. CONCLUSIONS Our findings do not only demonstrate that the field of online MBIs is booming, but also corroborate previous evidence that online MBIs are beneficial for improving mental health outcomes in a broad range of samples. To advance the field of online MBIs, future trials should pay specific attention to methodological quality, adherence and long-term follow-up measurements.

Abstract This study aimed to analyze an intervention on Strengths Based Character Education for the Elderly (Educafi) and its effects on the variables character strengths, psychological well-being, life satisfaction and depressive symptoms. Forty-one elderly people participated in the research, divided into three groups: experimental, active control and control. Participants answered the same questionnaires in three moments: pretest, post-test and follow up. Statistically significant results were obtained for depressive symptoms, but not for the positive constructs assessed. Elderly people who participated in Educafi decreased the levels of these symptoms from the pretest to the post-test. However, in the follow up they returned to the initial levels. The results are discussed and suggestions for Educafi and other positive interventions for the elderly are presented.

The integration of mindfulness and character strengths is an emerging area of research and practice. The evidence-based, 8-week program, Mindfulness-Based Strengths Practice (MBSP) represents a unique approach in that the starting point and focus is on what is best in human beings – their character strengths. These qualities are then leveraged to improve one’s meditation practice or area of mindful living (referred to as “strong mindfulness”) and in turn, mindfulness is used as a lens for deepening awareness and use of strengths (referred to as “mindful strengths use”). While early controlled studies find MBSP to elicit well-being and reduce stress, and find it to be superior to popular mindfulness programs for different outcomes, we are not aware of any published qualitative analyses examining the MBSP participant’s experience. This study offers insights from a large international sample and extends the empirical data on MBSP, including the novel finding of benefit for building positive relationships, as well as confirming other findings such as the most common obstacles people confront in mindfulness practices. Additional areas discussed using qualitative and quantitative findings include the most beneficial mindfulness/character strengths practices, the use of inward and outward-oriented practices, cognizance of the integration of mindfulness and character strengths, and self-reported positive outcomes, including substantial benefits to stress and problem management and boosts to meaning, purpose, engagement, accomplishment, and sense of self.

Background and AimsWeight gain is a major adverse effect of antipsychotic medication, negatively affecting physical and mental well-being. The objective of this study was to explore if dose reduction, discontinuation, switch to a partial agonist, or switch from polypharmacy to monotherapy will lead to weight loss.MethodsControlled and uncontrolled studies reporting the effects of discontinuation, dose reduction, switch to a partial agonist, or switch from polypharmacy to monotherapy on weight were included. Primary outcome was difference in weight compared to maintenance groups based on controlled studies. Secondary outcome was change in weight from initiation of one of the included interventions until follow-up in a pre-post analysis.ResultsWe identified 40 randomized controlled trials and 15 uncontrolled studies including 12,279 individuals. The effect of the interventions, i.e. dose reduction, drug discontinuation, or switch to a partial agonis, reduced the weight with 1.5 kg (95% CI −2.03 to −0.98; P < 0.001) compared to maintenance treatment. The weight change from pre to post was a reduction of 1.13 kg (95% CI −1.36 to −0.90; P < 0.001).ConclusionWe found a significant but small reduction in weight, suggesting that antipsychotic-induced weight gain can be reversed to some degree. Only a few studies were designed to address the question as primary outcome, which limits the generalizability of our findings.

Abstract. Background: Little is known about the role of resilience in the likelihood of suicidal ideation (SI) over time. Aims: We examined the association between resilience and SI in a young-adult cohort over 4 years. Our objectives were to determine whether resilience was associated with SI at follow-up or, conversely, whether SI was associated with lowered resilience at follow-up. Method: Participants were selected from the Personality and Total Health (PATH) Through Life Project from Canberra and Queanbeyan, Australia, aged 28–32 years at the first time point and 32–36 at the second. Multinomial, linear, and binary regression analyses explored the association between resilience and SI over two time points. Models were adjusted for suicidality risk factors. Results: While unadjusted analyses identified associations between resilience and SI, these effects were fully explained by the inclusion of other suicidality risk factors. Conclusion: Despite strong cross-sectional associations, resilience and SI appear to be unrelated in a longitudinal context, once risk/resilience factors are controlled for. As independent indicators of psychological well-being, suicidality and resilience are essential if current status is to be captured. However, the addition of other factors (e.g., support, mastery) makes this association tenuous. Consequently, resilience per se may not be protective of SI.
