Modeling of Gait Adaptations to Minimize Plantar Tissue Strain During Walking

Computational models often represent the most cost-effective approach to predict the behavior of musculoskeletal systems. Historically, dynamic musculoskeletal simulations have simplified representations of soft tissue structures, which makes it impossible to investigate the relationship between neuromuscular control and tissue loading. It is possible to overcome this limiting assumption by coupling a finite element model, e.g. of a foot, directly with a musculoskeletal model, e.g. of the lower extremity [1]. The goal of the current study was to apply this concept to the control of human gait and demonstrate that it is possible to have a gait pattern that minimizes internal foot deformation while satisfying an overall movement goal, e.g. minimal deviations from normal gait. Successful implementation will have wide-ranging implications such as finding therapeutic and rehabilitative movement patterns that relieve localized tissue loading.

The biomechanical analysis allows to understand the normal and pathological gait, the mechanics of neuromuscular control, and last but not least, allows the visualisation of the effects of footwear on human gait or feet. Biomechanical analyses are very important for the footwear development process, as they can identify the incorrect loading of the foot or the incorrect gait pattern, thus avoiding the occurrence of deformations. This paper aims to create an average representative model of barefoot loading based on an extended group of participants by applying an optimal procedure for measuring biomechanical parameters. The variation of four basic biomechanical parameters, namely force, pressure, contact time and contact area, was measured using a pressure platform and a specialised software system. The data was collected from 32 healthy females, without particularities regarding foot health and the practice of performance sports, aged between 18 and 30 years, divided into three size groups – 36, 37 and 38. The T-Student test was applied to verify if there are significant differences between the left and right foot. Statistical indicators for each parameter were calculated, in order to characterize and establish the degree of variation of the obtained values, as follows: mean, standard deviation, minimum and maximum values, the amplitude of variation and coefficient of variation (CV). The study results confirm that the obtained mean values can be used as input data to load the foot and perform virtual simulations of footwear products.

Background Postpartum haemorrhage (PPH) is a leading cause of maternal mortality and severe morbidity globally. When PPH cannot be controlled using standard medical treatments, uterine balloon tamponade (UBT) may be used to arrest bleeding. While UBT is used by healthcare providers in hospital settings internationally, their views and experiences have not been systematically explored. The aim of this review is to identify, appraise and synthesize available evidence about the views and experiences of healthcare providers using UBT to treat PPH. Methods Using a pre-determined search strategy, we searched MEDLINE, CINAHL, PsycINFO, EMBASE, LILACS, AJOL, and reference lists of eligible studies published 1996–2019, reporting qualitative data on the views and experiences of health professionals using UBT to treat PPH. Author findings were extracted and synthesised using techniques derived from thematic synthesis and confidence in the findings was assessed using GRADE-CERQual. Results Out of 89 studies we identified 5 that met our inclusion criteria. The studies were conducted in five low- and middle-income countries (LMICs) in Africa and reported on the use of simple UBT devices for the treatment of PPH. A variety of cadres (including midwives, medical officers and clinical officers) had experience with using UBTs and found them to be effective, convenient, easy to assemble and relatively inexpensive. Providers also suggested regular, hands-on training was necessary to maintain skills and highlighted the importance of community engagement in successful implementation. Conclusions Providers felt that administration of a simple UBT device offered a practical and cost-effective approach to the treatment of uncontrolled PPH, especially in contexts where uterotonics were ineffective or unavailable or where access to surgery was not possible. The findings are limited by the relatively small number of studies contributing to the review and further research in other contexts is required to address wider acceptability and feasibility issues.

Chapter 7 concludes by highlighting the three biggest messages from the analysis presented in this book: (1) the global gag rule has failed to achieve its goal of reducing abortions; (2) restrictive legislation is associated with more unsafe abortions; and (3) the expanded global gag rule is likely to have negative repercussions across a range of health outcomes for women, children, and men. They are simple but powerful messages that should be heard by policymakers over the voices calling for an ideologically based policy that fails to achieve its desired outcome. The chapter closes with a more constructive and cost-effective approach for US family-planning assistance that targets integrated reproductive health services.

Background: A major disadvantage of current spacers for two-stage revision total knee arthroplasty (R-TKA) is the risk of (sub-) luxation during mobilization in the prosthesis-free interval, limiting their clinical success with detrimental consequences for the patient. The present study introduces a novel inverse spacer, which prevents major complications, such as spacer (sub-) luxations and/or fractures of spacer or bone. Methods: The hand-made inverse spacer consisted of convex tibial and concave femoral components of polymethylmethacrylate bone cement and was intra-operatively molded under maximum longitudinal tension in 5° flexion and 5° valgus position. Both components were equipped with a stem for rotational stability. This spacer was implanted during an R-TKA in 110 knees with diagnosed or suspected periprosthetic infection. Postoperative therapy included a straight leg brace and physiotherapist-guided, crutch-supported mobilization with full sole contact. X-rays were taken before and after prosthesis removal and re-implantation. Results: None of the patients experienced (sub-) luxations/fractures of the spacer, periprosthetic fractures, or soft tissue compromise requiring reoperation. All patients were successfully re-implanted after a prosthesis-free interval of 8 weeks, except for three patients requiring an early exchange of the spacer due to persisting infection. In these cases, the prosthetic-free interval was prolonged for one week. Conclusion: The inverse spacer in conjunction with our routine procedure is a safe and cost-effective alternative to other articulating or static spacers, and allows crutch-supported sole contact mobilization without major post-operative complications. Maximum longitudinal intra-operative tension in 5° flexion and 5° valgus position appears crucial for the success of surgery.

One of the basic conditions for the successful implementation of energy demand-side management (EDM) in smart grids is the monitoring of different loads with an electrical load monitoring system. Energy and sustainability concerns present a multitude of issues that can be addressed using approaches of data mining and machine learning. However, resolving such problems due to the lack of publicly available datasets is cumbersome. In this study, we first designed an efficient energy disaggregation (ED) model and evaluated it on the basis of publicly available benchmark data from the Residential Energy Disaggregation Dataset (REDD), and then we aimed to advance ED research in smart grids using the Turkey Electrical Appliances Dataset (TEAD) containing household electricity usage data. In addition, the TEAD was evaluated using the proposed ED model tested with benchmark REDD data. The Internet of things (IoT) architecture with sensors and Node-Red software installations were established to collect data in the research. In the context of smart metering, a nonintrusive load monitoring (NILM) model was designed to classify household appliances according to TEAD data. A highly accurate supervised ED is introduced, which was designed to raise awareness to customers and generate feedback by demand without the need for smart sensors. It is also cost-effective, maintainable, and easy to install, it does not require much space, and it can be trained to monitor multiple devices. We propose an efficient BERT-NILM tuned by new adaptive gradient descent with exponential long-term memory (Adax), using a deep learning (DL) architecture based on bidirectional encoder representations from transformers (BERT). In this paper, an improved training function was designed specifically for tuning of NILM neural networks. We adapted the Adax optimization technique to the ED field and learned the sequence-to-sequence patterns. With the updated training function, BERT-NILM outperformed state-of-the-art adaptive moment estimation (Adam) optimization across various metrics on REDD datasets; lastly, we evaluated the TEAD dataset using BERT-NILM training.

Cadmium pollution threatens food safety and security by causing health issues and reducing farmland availability. Engineering genetic changes in crop plants to lower Cd accumulation can be a cost-effective approach to address this problem. Previously, we reported that a rice line, 2B, which expresses a truncated version of OsO3L2 had reduced Cd accumulation throughout the plant, including in seed. However, downstream events caused by expression of this gene were not known. In this study, RNA-seq was used to identify differentially expressed genes between the wild type and 2B rice with or without Cd treatment, leading to the study of an ABC transporter gene, OsABCG48 (ATP-Binding Cassette transporter G family member 48). Heterologous expression of OsABCG48 conferred tolerance to Cd in Schizosaccharomyces pombe, Arabidopsis and rice. Moreover, overexpressing OsABCG48 in rice lowered root Cd accumulation that was associated with more extensive lateral root development. These data suggest that OsABCG48 might have applications for engineering low-Cd rice.
