Vitamin and mineral supplement use in medically complex, community-living, older adults

Individuals who take medications may benefit from using vitamin and/or mineral supplements (VMS) yet excess supplementation can lead to overuse (daily intake exceeding the tolerable upper intake level (UL)). This study assessed VMS use of medically complex, community-living, older adults. A chart review of 229 patients ≥50 years with new medication assessments between 2014 and 2017 indicated that 76.9% of patients used ≥1 VMS daily. The UL for one or more nutrients was exceeded by 39.8% of supplement users.

This study investigated the prevalence of inadequate micronutrient intake and the proportion of subjects who exceed Tolerable Upper Intake Levels a) with food only, and b) with food+supplements, in a population of French regular supplement users (n = 259). Assessment tools were seven-day records for supplements, three-day records for food intake, and a questionnaire about supplement use. Most subjects were recruited in retail outlets that sold supplements. They were recent users of vitamin/mineral supplements, aged over 15 years, and normo-energy reporters. The prevalence (%) of inadequate intake decreased with the inclusion of mean annual supplements, from 68.0 to 54.8 for magnesium, 55.9 to 40.7 for vitamin C, 53.4 to 43.9 for folic acid, 37.5 to 27.5 for iron, and 40.1 to 29.7 for pantothenic acid. Few subjects exceeded upper intake levels when mean annual intake of supplements was considered. When supplement consumption was considered during the studied week only, the proportion of subjects who were in excess of the upper intake levels was higher (maximum: 9.6% for magnesium). Supplement use brought a nutritional benefit for some targeted nutrients. It was not associated with excessive intake in this study, but could become hazardous if the annual frequency of use were to increase.

BACKGROUND: Little information exists on herb and vitamin—mineral supplement use in very old people and whether use varies by ethnicity. OBJECTIVE: To examine the prevalence and predictors of herb and vitamin—mineral supplement use in a triethnic sample of adults aged ⩾77 years. METHODS: In-home interviews in 1997–1998 assessed medications use and sociodemographic and health factors in community-dwelling elderly non-Hispanic white (n = 125), black (n = 112), and Hispanic (n = 128) adults. RESULTS: Of the 365 subjects (71.5% ⩾80 y old, 52.6% female), 172 reported using at least one of the preparations (vitamin—mineral supplements 132, herbs alone 21, both herbs and vitamin—mineral supplements 19). Herbal use varied by ethnicity: 12.8% in non-Hispanic whites, 16.1% in blacks, and 4.7% in Hispanics. The most commonly used herbs were garlic, Ginkgo biloba, saw palmetto, and vinegar. Use of vitamin—mineral supplements, alone or combined with herbs, also varied by ethnicity: 54.4% in non-Hispanic whites, 31.3% in blacks, and 37.5% in Hispanics. In the fully adjusted multivariate model with white ethnicity as reference, the odds ratio of vitamin—mineral supplements use for blacks was 0.37 (95% CI 0.21 to 0.65) and for Hispanics was 0.56 (95% CI 0.30 to 1.03). In bivariate analyses, female gender, black ethnicity, being born in the US, and having coronary artery disease were significantly associated with herbal use (p < 0.05); however, in the fully adjusted multivariate model, only the female gender remained significantly associated with herbal use (OR 2.14; 95% CI 1.00 to 4.59). CONCLUSIONS: Use of herbs or vitamin—mineral supplements is common in very old people and varies by ethnicity.

Micronutrient deficiencies and low dietary intakes among community-dwelling older adults are associated with functional decline, frailty and difficulties with independent living. As such, studies that seek to understand the types and magnitude of potential dietary inadequacies might be beneficial for guiding future interventions. We carried out a systematic review following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement. Observational cohort and longitudinal studies presenting the habitual dietary intakes of older adults ( ≥ 65 years) were included. Sex-specific mean (and standard deviation) habitual micronutrient intakes were extracted from each article to calculate the percentage of older people who were at risk for inadequate micronutrient intakes using the estimated average requirement (EAR) cut-point method. The percentage at risk for inadequate micronutrient intakes from habitual dietary intakes was calculated for twenty micronutrients. A total of thirty-seven articles were included in the pooled systematic analysis. Of the twenty nutrients analysed, six were considered a possible public health concern: vitamin D, thiamin, riboflavin, Ca, Mg and Se. The extent to which these apparent inadequacies are relevant depends on dynamic factors, including absorption and utilisation, vitamin and mineral supplement use, dietary assessment methods and the selection of the reference value. In light of these considerations, the present review provides insight into the type and magnitude of vitamin and mineral inadequacies.

Vitamin/mineral supplements are often used by athletes as ergogenic aids to improve performance. This paper reviews studies of the prevalence, patterns, and explanations for vitamin/mineral supplement use among athletes. Fifty-one studies provided quantitative prevalence data on 10,274 male and female athletes at several levels of athletic participation in over 15 sports. The overall mean prevalence of athletes’ supplement use was 46%. Most studies reported that over half of the athletes used supplements (range 6% to 100%), and the larger investigations found lower prevalence levels. Elite athletes used supplements more than college or high school athletes. Women used supplements more often than men. Varying patterns existed by sport. Athletes appear to use supplements more than the general population, and some take high doses that may lead to nutritional problems. Sport nutritionists should include a vitamin/mineral supplement history as part of their dietary assessment so they can educate athletes about vitamin/mineral supplements and athletic performance.

Purpose: We investigated barriers to healthful eating and vitamin/mineral supplement use among groups at risk for low nutrient intakes, particularly those with low income. Methods: Twelve focus groups (73 participants) and 11 key informant interviews were conducted in Saskatoon, Saskatchewan. Focus group participants represented a diverse population. Key informants included health professionals and personnel from community-based organizations who worked in a lowincome area. Focus group meetings and key informant interviews were audiotaped and transcribed; thematic coding was used to identify key concepts. Results: The focus groups and interviews revealed five themes on barriers to healthful eating and to the use of vitamin/mineral supplements: knowledge, income, accessibility, health, and preferences. Key informants were aware of the barriers, and were able to see not only individual and family reasons but also societal influences. Conclusions: The study results provide valuable information for focusing efforts on reducing barriers to healthful eating and to appropriate vitamin/mineral supplement use.

Abstract Background A variety of dietary supplements are commercially available. However, the efficacy and safety of dietary supplement use in children are not well established. Understanding dietary supplement use is important for developing public health policy regarding dietary supplements. This study aimed to investigate the types of dietary supplements used and characteristics of dietary supplement users among Japanese elementary school children. Method We conducted a cross-sectional web-based questionnaire study. Dietary supplement use, socio-demographics, and health-related behaviors were assessed through mother-reported questionnaire. Types of dietary supplements were identified based on ingredient using product barcodes and brand names. Multivariate logistic regression analysis was conducted to investigate the socio-demographics and health-related behaviors associated with supplement use. Results Among 4933 children, 333 (6.8%) were identified as dietary supplement users. The most common supplement was amino acids or protein (1.4%), followed by n–3 fatty acids or fish oil (1.0%), probiotics (1.0%), multivitamins (0.9%), multivitamin-minerals (0.8%), and botanicals (0.8%). Overall, any dietary supplement use was significantly associated with the highest frequency of sports participation (odds ratio [OR], 2.58; 95% confidence interval [CI], 1.65–4.02), highest household income (OR, 1.87; 95% CI, 1.13–3.10), highest maternal educational level (OR, 1.82; 95% CI, 1.31–2.52), and male sex (OR, 1.38; 95% CI, 1.09–1.75). The highest frequency of sports participation was significantly associated with higher odds of use of amino acids or protein (OR, 6.06; 95% CI, 1.78–20.6) and multivitamins (OR, 3.56; 95% CI, 1.11–11.5), compared to the lowest frequency of sports participation. Conclusion This study showed that Japanese children primarily use non-vitamin, non-mineral supplements. Non-vitamin, non-mineral supplements should thus be included in future studies aimed at monitoring dietary supplement use. We also found that dietary supplement use in children was associated with sports participation. Guidelines for dietary supplement use for children, in particular sport participants, are needed.

AbstractObjectiveTo examine overall micronutrient intake periconceptionally and throughout pregnancy in a population-based cohort of Australian women.DesignIn a prospective cohort study, micronutrient dosages were extracted from self-reported maternal supplement use, recorded pre-conception, and for each trimester of pregnancy. A food frequency scale (DQESv2) captured usual maternal diet for gestational weeks 14–26. The influence of sociodemographic and lifestyle factors associated with supplement use was examined using logistic regression, and changes in micronutrient intakes prior to and throughout pregnancy were assessed using repeated-measures ANOVA analyses.SettingMetropolitan hospital sites in Melbourne, Australia.SubjectsWomen with a viable singleton pregnancy were recruited at less than 19 weeks’ gestation (n2146).ResultsCompared with non-users, women using supplements during pregnancy were more likely to have planned their pregnancy, be >25 years old, primiparous, Caucasian, non-smokers, have a tertiary education and be consuming a folate-rich diet. Intakes of folate, Fe and Zn were significantly lower in the periconceptional period, compared with other periods (P<0·001). Intakes below Recommended Daily Intake levels were common both periconceptionally and throughout pregnancy, with 19–46 % of women not meeting the Recommended Daily Intake for folate, 68–82 % for Fe and 17–36 % for Zn. Conversely, 15–19 % of women consumed beyond the recommended Upper Limit for folate and 11–24 % for Fe.ConclusionsThe study highlights the need for improved public health education on nutritional needs during pregnancy, especially among women with lower educational achievements and income.
