Objective. To study the effects of dexmedetomidine in combination with ropivacaine in patients undergoing craniocerebral surgery and their efficiency on cognitive function and inflammatory response of patients. Methods. 100 patients undergoing craniocerebral surgery in our hospital from November 2018 to September 2020 were randomly selected and divided into a control group and an experimental group by drawing lots, with 50 cases in each group. Patients in the control group received routine anesthesia, while those in the experimental group received 1 μg/kg of dexmedetomidine combined with 0.5% of ropivacaine for anesthesia to compare the anesthesia onset time, analgesic time, postoperative awake time, Social Disability Screening Schedule (SDSS) cognitive function score after waking, visual analogue scale (VAS) pain score, Ramsay sedation score, incidence of adverse reactions, postoperative inflammatory factor expression levels, and changes in heart rate, oxygen saturation, and blood pressure at T0, T1, T2, T3, and T4 between the two groups. Results. The anesthesia onset time, SDSS cognitive function score after waking, VAS pain score, Ramsay sedation score, incidence of adverse reactions, and postoperative inflammatory factor expression levels in the experimental group were significantly lower than those in the control group ( P < 0.05 ). The analgesic time and postoperative awake time in the experimental group were significantly longer than those in the control group, with statistical significance ( P < 0.05 ). There were no statistically significant differences in the changes of heart rate, oxygen saturation, and blood pressure at T0, T1, T2, T3, and T4 between the two groups ( P > 0.05 ). Conclusion. Dexmedetomidine combined with ropivacaine has high application value in craniocerebral surgery.

Background: To compare the analgesic effect and safety of bilateral inferior alveolar nerve block combined with parecoxib sodium analgesia and simple intravenous analgesia pump in analgesia after orthognathic surgery. Methods: Forty patients with simple ascending sagittal split osteotomy and ankle plasty were randomly divided into the experimental group and the control group, with 20 patients in each group. The experimental group received 2 ml 1% on both sides. Ropivacaine was treated with inferior alveolar nerve block anesthesia. Immediately after surgery, parecoxib sodium 40 mg was intravenously administered. The control group was given an intravenous analgesia pump for analgesia. Pain intensity (VAS pain score) and Ramsay sedation score were recorded at 2h, 4h, 8h, 24h, 48h after operation, and the incidence of postoperative adverse reactions was observed. Results: There was no significant difference in pain intensity and Ramsay sedation score between the two groups at each time point (P>0.05). During the analgesic treatment, the incidence of nausea and vomiting (P=0.046) in the experimental group was significantly lower than that in the control group (P<0.05). Conclusion: Bilateral inferior alveolar nerve block combined with parecoxib sodium analgesia and simple intravenous analgesia pump are effective for analgesia after mandibular orthognathic surgery, but the incidence of adverse reactions is significantly lower, more suitable for Analgesia after mandibular orthognathic surgery.

Introduction: Exposure to pesticides in greenhouse workers is associated with several health outcomes, such as cognitive impairment. This study aimed to investigate the association between exposure to pesticides and cognitive function in Yazd city greenhouse workers and those living in the proximity of the greenhouses. Materials and Methods: In this study, three groups of participants were selected, including the greenhouse workers, the residents in the proximity of the greenhouses, and the control group. A semi-quantitative assessment of pesticide exposure was used to calculate the subjects’ cumulative pesticide exposure as a proxy for long-term exposure to pesticides. Blood level of acetylcholinesterase (AChE) activity was measured as a biomarker of effect. Frontal Assessment Battery (FAB), and Mini–mental State Examination (MMSE) were used to assess cognitive functions (including memory, executive functions, attention, visual, and verbal functions). Results: Mean cholinesterase activity in the greenhouse workers (average = 7009.3 U/L) was lower than the two other groups.  The results of the cognitive function score for both FAB and MMSE tests did not show a significant difference between the direct exposure and indirect exposure groups, although the cognitive function score in the pesticide applicators was lower than the two other study groups (e.g. for FAB score: 13.89, 14.55, and 15.4 for the greenhouse workers, the residents in the proximity of greenhouses, and the control group). The results also showed that in the direct exposure group, those with lower levels of cholinesterase activity also had a lower cognitive function. Conclusion: The findings indicated that there is a potential link between impaired cognitive function and exposure to pesticides in the greenhouse workers.

We aimed to investigate the effect of Taekwondo training on physical fitness, mood, sociality, and cognitive function among international students in South Korea. We randomly assigned 24 international students to a control group (CG, n = 12) and experimental group (EG, n = 12). The EG performed Taekwondo training for 16 weeks, while the CG did not train. Each participant underwent a physical fitness test and sociability questionnaire before and after the intervention. We also examined changes in mood state and cognitive function, using the Korean version of the Profile of Mood State-Brief (K-POMS-B), and the Stroop Color and Word test, respectively. Regarding the physical fitness variables, sit-and-reach records in the EG significantly increased after intervention (p < 0.05). In the sub-variable of K-POMS-B, Vigor-Activity scores significantly increased (p < 0.05) after intervention, while the Fatigue-Inertia scores significantly decreased in the EG (p < 0.05). Furthermore, in the EG, peer relationship scores, a sub-variable of sociability, significantly decreased after intervention (p < 0.05). These findings suggest that Taekwondo training can not only improve flexibility among physical fitness factors, but can also be effective in improving the mood state and sociality of international students.

ABSTRACT Introduction: Difficulty falling asleep is connected to the malfunctioning of the sleep and wakefulness mechanism of the human body caused by various reasons. There are a series of adverse reactions resulting from abnormal or poor quality of sleep during sleep per se. This symptom severely affects an individual's physical condition and mental health. Objective: To explore the effect of physical exercise on patients with difficulty falling asleep. Methods: Mathematical statistics were used to analyze 60 patients with difficulty falling asleep. We divided the patients into a sports group and a control group. The patients in the sports group took sports training, while the control group did not. After two weeks of intervention and comparison, we used mathematical statistics to evaluate the groups’ cognitive function. Results: After physical exercise, the patient's sleep quality was higher and sleep disorders were reduced. Conclusion: Physical activity is a simple and popular way of exercising. It is effective in improving the cognitive function of patients with difficulty falling asleep. Level of evidence II; Therapeutic studies - investigation of treatment results.

【Abstract】Objective: Toanalyze the efficacy of combined drug therapy for elderly patients with coronary heart disease and hypertension.METHODS:Sixty-six elderly patients with coronary heart disease and hypertension were enrolled from December 2017 to November 2018. They were randomly divided into two groups, 33 patients in each group. Patients in the experimental group received nifedipine. In combination with enalapril, patients enrolled in the control group received nifedipine monotherapy.RESULTS:Compared with the control group, the total effective rate, Serum Nitric Oxide (Serum NO) after treatment, CRP after treatment, HCY after treatment, and blood pressure after treatment were significantly improved (P<0.05). Serum NO and treatment before treatment in the 2 groups. There was no significant difference in pre-CRP, pre-treatment HCY, pre-treatment blood pressure, and adverse reactions during treatment (P>0.05).Conclusion: Theelderly patients with coronary heart disease and hypertension are treated with nifedipine and enalapril.

AbstractIt was still unclear how homocysteine (Hcy) levels and cognitive deficits change in patients with schizophrenia of various ages. The present article attempts to assess the relationship between Hcy levels and cognitive deficits in patients with schizophrenia across age groups, especially in young people. Totals of 103 patients and 122 healthy controls were included. All participants were stratified into four groups according to their age: 18–29 years, 30–39 years, 40–49 years, and 50–59 years. Clinical data, plasma Hcy levels, and cognitive function score were collected. Cognitive function was evaluated using the MATRICS Consensus Cognitive Battery of tests assessing speed of processing, verbal learning and memory, visual learning and memory, working memory, and attention/vigilance. Compared with the healthy group, Hcy levels increased significantly, and all the measured cognitive function score were significantly lower in all age groups of patients with schizophrenia (p < 0.001). Hcy levels were negatively associated with speed of processing (SoP), working memory (WM), and visual learning and memory (Vis Lrng) score in 18–29 years. Further multiple regression analysis showed that SoP were independently associated with Hcy levels in patients with schizophrenia aged 18–29 years (B = 0.74, t = 3.12, p = 0.008). Based on our results, patients with schizophrenia performed worse on cognitive assessments and Hcy levels were more closely related to cognition in young patients.

Abstract Background: Previous studies reported that the association between lipid levels and cognitive function related with gender, age and specific cognitive domains, but the influence of body mass index (BMI) on this association is limited. This triggered our interest in exploring how serum lipids relate to cognitive function in different subgroups. Methods: Data was collected from 2009 wave and 2015 wave of China Health and Nutrition Survey (CHNS). Multivariable linear regression analyses examined serum lipids level as predictors of sex- and age-specific measure of cognitive function in different BMI levels, which were adjusted for nationality, BMI, systolic blood pressure (SBP), diastolic blood pressure (DBP), smoking status, alcohol consumption and education level. Results: Cognitive function score have different concentration curves in serum lipids quartile levels in different BMI categories. After adjustment for confounding factors, serum TG was positively associated with cognitive function score in underweight (β±SE: 2.06±0.88, P=0.023) and obese (β±SE: 1.44±0.71, P=0.045) male group, and serum HDL-C was positively associated with cognitive function score in overweight (β±SE: 1.89±0.92, P=0.041) and obese (β±SE: 5.04±1.62, P=0.002) female group. Serum TC was negatively associated with cognitive function score in overweight (β±SE: -2.55±1.26, P=0.043) mid-life adults, and serum HDL-C was positively associated with cognitive function score in overweight (β±SE: 2.15±0.94, P=0.022) and obese (β±SE: 5.33±2.07, P=0.011) older adults. Conclusion: The associations of serum lipids with cognitive function were related with BMI levels and differed between gender and age groups. This result indicated that better nutritional status has superior cognitive function performance.

Background: To explore the application effects of whole course high-quality nursing on patients with liver cancer during radiotherapy. Methods: One hundred and five patients with liver cancer who received radiotherapy in Liaocheng People’s Hospital, Liaocheng, China from March 2010 to February 2012 were selected and divided into a control group (42 cases with routine nursing) and an experimental group (63 cases with whole course high-quality nursing). The two groups of patients were observed and compared in terms of clinical data and the Activity of Daily Living (ADL) score before and after nursing (1 course of treatment/6 week). The Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS) were used to evaluate and compare the patients’ anxiety and depression before and after nursing. Adverse reactions during radiotherapy and nursing satisfactory of patients were observed and compared between the two groups. Results: After nursing, the ADL score and the nursing satisfaction in the experimental group was significantly higher than that in the control group (P<0.05). The SDS and SAS scores in the 2 groups before nursing were significantly higher than those after nursing (P<0.05). The total incidence of adverse reactions in the control group was significantly higher than that in the experimental group (P<0.05). Conclusion: Whole course high-quality nursing can improve the negative emotions, quality of life and nursing satisfaction, and reduce adverse reactions of patients with liver cancer during radiotherapy.

81 Background: Pain is one of the most common symptoms in cancer patients and often has a negative impact on patients’ functional status and quality of life. Cognitive dysfunction can impact the assessment and treatment of pain. Taking analgesics may worsen the cognitive dysfunction. The study of the prevalence of cognitive dysfunction and the association with analgesics in the CP patients is rare in China. This study aimed to investigate the prevalence of cognitive dysfunction in Chinese CP patients and analyze the related factors. Methods: A total of 220 Chinese CP patients in 9 first-grade hospitals received the following assessment: the functional assessment of cancer therapy-cognitive function (FACT-Cog) and numerical rating scale (NRS), Patient Health Questionnaire (PHQ-9) and Generalized Anxiety Disorder-7 (GAD-7). ANOVA and multiple linear regression analysis were performed to explore the related factors. Results: A total of 220 Chinese CP patients received the assessment: The overall incidence of cognitive dysfunction in CP patients was 33.64%, with an average cognitive function score 105.93±21.39. The total cognitive function score and each dimension were negatively correlated with the pain, depression, and anxiety levels. The analgesics did not affect the cognitive function of CP patients. Sleep and smoking history were two key factors influencing the cognitive function. Conclusions: An effective management of pain, depression, anxiety, sleep, and smoking are recommended to protect the cognitive function of patients under CP stress. It is safe and reliable to regularly use the analgesics for CP patients. Clinical trial information: NCT03641820.

This study measured cognitive and vascular responses to aerobic training in sedentary young adults. Ten adults (6 women, 4 men; 18–29 years) were randomly assigned to an experimental or no-treatment control group. The experimental group engaged in a 6-week intervention, performed on exercise cycle and treadmill, 3x/week, 50 min/session; intensity was increased over time. Outcome measures included arterial stiffness (augmentation index, AIx, and pulse pressure), cardiorespiratory fitness (), and cognitive function (attention, processing speed, working memory, episodic memory, and executive function). Participants randomized to aerobic training improved processing speed versus control (, ES = 0.55). However, no group × time effects were noted in other domains of cognitive function. AIx was reduced by approximately 16% from before to after intervention in the experimental group; however, the improvement was not statistically significant versus control (, ES = 0.22). Pulse pressure did not change between groups over time (, ES = 0.0). increased by approximately 10% in the experimental group; however, the change was not significant between groups over time (, ES = 0.27). Vascular and cognitive adaptations to aerobic training may move in parallel. Robust trials simultaneously investigating a broad spectrum of aerobic training interventions and vascular and cognitive outcomes are warranted.
