Biostatistics Health Assessment of Matrix Metalloproteinases in Vaginal Secretions in Late Pregnant Women

Objective: The health management of pregnant women in late pregnancy is difficult. Once vaginal infection occurs, adverse pregnancy outcomes will occur. The aim of this study was to explore the application value of biostatistics health assessment of matrix metalloproteinases (MMP) in vaginal secretions in late pregnant women with vaginal infection. Methods: A total of 120 late pregnant women with vaginal infection, including vulvovaginal candidiasis (WC) in 70 cases and bacterial vaginosis (BV) in 50 cases, were selected as the experimental group. Another 80 pregnant women with normal vaginal secretion in the same gestational week were randomly selected as the control group. Vaginal soft capsule of mycomycin was used in the treatment. The expression levels of MMP-8 and MMP-9 in vaginal secretions before and after treatment were detected by enzyme-linked immunosorbent assay (ELISA) and smear immunohistochemistry, Results: Both the ELISA and immunohistochemistry analysis showed that the levels of MMP-8 and MMP-9 in the vaginal secretion of experimental group were significantly increased than the data of control group (P < 0.05). After effective intervention, the levels of MMP-8 and MMP-9 were significantly lower than those before treatment (P < 0.05). Conclusion: The late pregnant women with vaginal infection was accompanied by the high level of MMP-8 and MMP-9 in vaginal secretions. Effective intervention will lead the reduction of MMP. Thus, the biostatistics health assessment of MMP is important for the late pregnant women with vaginal infection.

Aims: Investigation thyroid dysfunction and autoimmunity in pregnant women with gestational diabetes mellitus. Background: This article was written to evaluate the thyroid function and anti-thyroid peroxidase (anti- TPO) antibodies in pregnant women with gestational diabetes mellitus (GDM). Method: A total of 252 women with GDM and 252 healthy pregnant women were enrolled. Thyroid tests, including TSH, FreeT3, Free T4, and anti-TPO were performed for all women at 24–28 weeks of gestation. Data analysis was then carried out using SPSS ver. 22. Result: There was a significant difference between the experimental group (38.4%) and the control group (14.06%) in terms of the prevalence of subclinical hypothyroidism (p= 0.016). The frequency of anti-TPO was higher in the experimental group than the control group and positive anti-TPO was observed in 18.6% of women with GDM and 10.3% of healthy pregnant women (P= 0.008). Conclusion: Thyroid disorders are observed in pregnant women with GDM more frequently than healthy individuals and it may be thus reasonable to perform thyroid tests routinely.

Aim. To investigate formation of neutrophil extracellular traps (NETs) and their impact on fibrinolysis in patients with colon cancer.Materials and methods. The study was performed in two groups. The experimental group consisted of patients with stage 2–3 non-metastatic colon cancer (n = 17, average age – 67 years). The control group included healthy volunteers matched by sex and age (n = 30, average age – 68 years). An experimental model was created from the whole blood. It included platelet-poor plasma and an isolated culture of neutrophils, previously induced to NETosis by adding 100 nmol PMA. The samples were incubated for 4 hours, then the test tubes were centrifuged to pellet cells and their remnants, and the plasma was transferred for subsequent examination. The plasma incubated with intact neutrophils was used as a control. The levels of interleukin-8 (IL-8) and P-selectin glycoprotein ligand-1 (PSGL-1) were used to determine the degree of cell activation. NETosis was confirmed by enzyme-linked immunosorbent assay (ELISA) and fluorescent microscopy. Fibrinolysis was assessed using the thrombodynamics test. The results were compared with the levels of fibrinolytic system components measured by flow cytometry.Results. In the control group, NETosis induction contributed to pronounced neutrophil activation that was accompanied by an increase in the IL-8, PSGL-1, and plasminogen levels, a decrease in PAI-1, and enhancement of fibrinolysis, compared with the intact samples. Higher levels of IL-8, PSGL-1, plasminogen, and PAI-1 and intensified fibrinolysis were detected in the intact samples. However, PMA-induced NETosis did not result in an increase in the degree of activation and significant changes in the given parameters.Conclusion. NETosis promotes both formation and lysis of fibrin clots. However, in cancer patients, suicidal NETosis does not contribute to fibrinolysis due to intracellular protease depletion, which may be one of the mechanisms causing hypercoagulation and insufficient fibrinolysis in cancer.

Abstract BackgroundPhysical activity during pregnancy period is one of the issues with priority during pregnancy period. Researches show that women reduce their physical activity during this period and are unaware of the benefits on the health of the mother and embryology. Although researchers regarding physical activity during pregnancy have prepared many guidelines; it is not however clear why pregnant women do not perform physical activity and the effectiveness factors that facilitate the desired behavior. MethodsThe research population included all pregnant women aged 18 to 40 years with gestational age of 12-38 weeks referring to the healthcare centers in District 5 of Tehran Municipality, from 22 districts of the city who had eliminated their physical activity during pregnancy. Questionnaires of physical activity assessment questionnaire after educational intervention in pregnant women (PPAQ) and a questionnaire designed based on the results of the needs assessment and the dimensions of the PEN-3 model were used for assessment. This study is a Research Clinical Trial (RCT). Due to the nature, a quasi-experimental research design (pre-test, post-test) with the control group was used in this research. ResultsBased on the obtained results, the intervention based on E-learning has a significant effect of enabling factors to increase physical activity in pregnant women in the experimental group. In addition, the intervention based on E-learning has a significant effect on the training factors in order to increase physical activity in pregnant women in the experimental group. ConclusionBased on the research results, it can be said that there is a statistically significant difference between the experimental and control groups in the scores obtained from training factors in the pretest and the experimental group. Furthermore, it can be said that there is a statistically significant difference between the experimental and control groups in the scores obtained from the enabling factors in the pre-test and the experimental group.

Summary: Introduction: Preeclampsia is a life-threatening condition for the mother and foetus. Globally, it is diagnosed in 10 mil. women every year, which accounts for 3% to 8% of all pregnancies. Currently there is no proven effective treatment for preeclampsia. The aforesaid text actualises the issue of predicting this complication. To determine the prognostic significance of matrix metalloproteinases-2 and -9 levels as early markers of preeclampsia, the present prospective study was conducted. Materials and methods: The levels of matrix metalloproteinases-2  and -9  were assessed in 72  patients. Thirty-four  of them subsequently developed preeclampsia during pregnancy (20 patients with moderate preeclampsia, 14 patients with severe preeclampsia), and constituted the basic group; 38 patients made up the control group. Results: In pregnant women with the subsequent development of preeclampsia, the level of matrix metalloproteinase-2 at 11–13 weeks of gestation was 155 ± 73.4 ng/mL and significantly exceeded its level in pregnant women without hypertensive disorders – 75.0 ± 32.8 ng/mL. The study conducted demonstrates a significantly lower concentration of matrix metalloproteinase-9 in pregnant women with preeclampsia compared to the control – 749 ± 296 ng/mL and 1,667 ± 552 ng/mL (P < 0.001). The performed research figures that in the first trimester, the cut-off value of matrix metalloproteinase-2 for predicting the development of preeclampsia is ≥ 102 ng/mL (sensitivity 88.24% and specificity 82.76%). For matrix metalloproteinase-9, a level of ≤ 980 ng/mL in the first trimester predicts the development of preeclampsia with a sensitivity of 85.29% and a specificity of 84.48%. Conclusion: The study established the cut-off values of matrix metalloproteinases-2 and -9 for predicting the development of preeclampsia in the first trimester.

Background: Pre-eclampsia(PE) is  the  most  frequently encountered  medical  complication  during  pregnancy. In developing countries PE   is a principal cause of maternal mortality. A disturbance  in  the  angiogenic/antiangiogenic  factors  and  in  the  hypoxia/placental re-oxygenation  process,  seems  to  activate a maternal  endothelial  dysfunction.Aims and Objective: To estimate Vascular Endothelial Growth Factor ( VEGF )  level  in the cord blood of healthy and Preeclamptic ( PEc ) pregnant women and to associate this with Preeclamptic pregnancy.Material and Methods: A case-control study ofUmbilical cord serum VEGF levels from women with uncomplicated pregnancies (control group, n=60) and pregnancies complicated by Pre-eclampsia (n=40). VEGF in the cord serum was estimated by SANDWICH Enzyme Linked Immunosorbent Assay method by using ELISA Kit and then compared between the two groups.Results: The mean VEGF concentrations in the women who had pre-eclampsia  (578.62±468.3)  were lower than in the control group( 625.75±533.1) , but the difference was not statistically significant ( p= 0.8548).  Conclusion VEGF plays a key role in the instability between endothelial dysfunction and angiogenesis that occurs during Preeclampsia.  VEGF levels might be a useful tool for the early diagnosis of Pre-eclampsia.Asian Journal of Medical Sciences Vol.8(1) 2017 21-25

Objective. The aim of this study was to determine whether maternal serum matrix metalloproteinases 2, 3, 9, and 13 levels differ in early- and late-onset preeclampsia and uncomplicated pregnancies. Patients and Methods. The study was carried out in 125 pregnant women (29 with early-onset preeclampsia; 31 preeclamptic patients with late-onset preeclampsia; and 65 healthy pregnant controls). Levels of MMP-2, MMP-3, MMP-9, and MMP-13 were measured in the maternal serum using an enzyme-linked immunosorbent assay. Results. Maternal serum MMP-2 levels in both the groups of preeclamptic women were significantly higher than those in the controls. Levels of MMP-3 were significantly higher in preeclamptic patients with early-onset disease; however, the MMP-3 levels in patients with late-onset preeclampsia were similar to those observed in the control subjects. MMP-9 levels were lower whereas the levels of MMP-13 were higher in both preeclamptic groups of pregnant women than in the healthy controls, but these differences were statistically insignificant. Conclusions. One important finding of the present study was that MMP-3 appears to be involved solely in early-onset preeclampsia, but not in late-onset preeclampsia. Higher levels of MMP-2 and MMP-13 and lower levels of MMP-9 seem to be related to both early- and late-onset severe preeclampsia.

This study used a retrospective analysis method to select 1061 pregnant women from March 2012 to December 2018 to our hospital for obstetrics, and 11–11 weeks +6 days of ultrasound screening for NT values, according to inclusion criteria and exclusion. There were 60 patients who met the criteria of the experimental group and 313 who met the criteria of the control group. All of them were subjected to fetal villus puncture in early pregnancy or amniocentesis in middle pregnancy. The specimens were divided into two parts, one was cultured through cells and the chromosomes were prepared. Analysis and send a copy to a collaborative laboratory for highthroughput sequencing of chromosomes. The aim was to investigate the association between simple cervical thickening (NT) thickening and karyotype and chromosomal microdeletion/microreplication in 11–13 weeks +6 days of gestation. The results of this study show that NT thickening is associated with karyotypic abnormalities, especially with 21-trisomy syndrome; when karyotype is normal, NT thickening is associated with chromosome microdeletions/microduplication; only a single NT increase when thick, the meaning of CNVs detection is not significant.

ABSTRACT Objective: To verify if participation in educational workshops about bonding with the fetus influences the adoption of practices of interaction with the baby by pregnant women. Method: Randomized clinical trial conducted in a health clinic. The experimental group participated in educational workshops designed for the pregnant couple and the control group received usual care. The practices of interaction with the fetus were evaluated before and after the intervention. Results: 19 pregnant women participated in the study. There was a positive correlation between higher gestational age and the interaction with the fetus (p = 0.016), and between younger mothers and the practice of touching or poking the baby (p = 0.019). There was an increase in the interaction with the fetus in the control and experimental groups (p= 0.024). Conclusion: Educational workshops are strategies for sharing knowledge about fetuses' sensory skills and to foster bonding and child development since pregnancy.

Background: Alzheimer’s disease is predicted to increase dramatically in the near future. Alzheimer’s caregiving brings about severe problems for caregivers. Considering the corrosive consequences of Alzheimer’s disease on Alzheimer’s caregivers, finding an effective intervention is necessary. Thus, the present research conducted by the aim of investigating the effectiveness of cognitive-behavioral treatment for Alzheimer’s disease patients and their caregiver’s protocol on burdens of Alzheimer’s disease on caregivers and their sense of coherence (SOC). Methods: This study was a quasi-experimental research with a pretest-posttest control group design. The target population included all mild Alzheimer’s disease patients and their caregivers referred to the neurology ward of the hospitals in Rasht during summer 2018. According to the medical records and results of screening, 40 mild Alzheimer’s disease, patients and their caregivers recruited by convenience sampling method. After sampling, research participants randomly assigned into two experimental (n= 20) and waitlist control (n=20) groups. The Impact of Alzheimer’s Disease on Caregiver Questionnaire and SOC scale were used to measure the burdens of Alzheimer’s disease on caregivers and caregivers’ SOC. The CBTAC provided for the experimental group participants in 25 sessions of 90-minutes. In the end, the data analyze has done by multivariate analysis of covariance (MANCOVA) and using SPSS24 software. Results: The findings indicated that the CBTAC had a significant effect on the burdens and SOC of Alzheimer’s caregivers in the experimental group (P<0.01). In other words, the CBTAC decreases burdens and increase the SOC of Alzheimer’s caregivers significantly (P<0.01). Conclusion: Based on the research results, the CBTAC is an effective intervention in decreasing caregiving burden and the increasing SOC of Alzheimer’s caregivers. Therefore, CBTAC is a multicomponent intervention that can be used to improve the competency and mental health of Alzheimer’s caregivers.

Introduction: Preterm delivery is the delivery before 37 weeks of gestation are completed. Preterm birth is a major course of neonatal morbidity and mortality, the incidence of premature delivery in developedcountries is 5 to 9%. Aims of this study were to determine the common etiological factors for preterm delivery, most common weeks of gestation for pretern delivery, and most commom way of delivery for preterm delivery.Methods: The study included 600 patients divided into two groups, experimental group (included 300 preterm delivered pregnant women), control group (included 300 term delivered women).Results: The incidence of preterm delivery in pregnant women younger than 18 years was 4.4%, and in pregnant women older than 35 years was 14%. 44.6 % of preterm delivered women at the experimentalgroup had lower education. In the experimental group burdened obstetrical history had 29%, 17.2% had a preterm delivery, 35.6% had a premature rupture of membranes, 15% had a preterm delivery before32 weeks of gestation, 12.4% between 32-33.6 weeks of gestation, while 72.6% of deliveries were between 34- 36.6 weeks of gestation. Multiple pregnancy as an etiological factor was present in 10.07% ofcases. Extragenital diseases were present in 10.4%. In the experimental group there were 29%, while in the control group there were 15% subjects with burdened obstetrical history.Conclusions: Preterm birth more often occurs in a pregnant women younger than 18 and older than 35 years, and in a pregnant women of lower educational degree. Preterm delivery in the most commoncases was fi nished in period from 34 to 36.6 weeks of gestation. The most common etiological factor of preterm delivery in the experimental group was preterm rupture of membranes and idiopathic pretermdelivery.
