Subjective experiences of dissociative and conversion disorders among adolescents in India

Dissociative/conversion disorders affect almost 31% of children and adolescents in a clinical setting. These children experience significant impairments in their academics, and daily functioning, with high chances of developing other psychiatric comorbidities such as anxiety and depression. However, there are no studies that explore the experiences of suffering from dissociative/conversion disorders from perspective of the sufferer. Therefore, the paper has aimed at exploring the subjective experiences of dissociative and conversion disorders among adolescents in the Indian context by examining their understanding about the illness and the reason they ascribe to the cause of their illness. The study used a qualitative semi-structured interview to understand their illness. In total, 10 adolescents of age group between 12 and 16 years participated. Eight out of the 10 participants were female and the mean age was 12 years. All of them were in-patients in the department of Child and Adolescent Psychiatry, NIMHANS, which is the tertiary mental health care Institute India and pioneer Institute of mental health in Asia. All participants have had at least one or more consultation history either with a traditional healer or/and physician. Thematic analysis identified vital themes such as (1) Adolescents’ attributing factors of the illness, (2) Cognitive and emotional appraisal of stressful situation/s, (3) Adolescents’ appraisal of different explanatory model, (4) Living with the Impact of the illness. This analysis about dissociative/conversion disorders from adolescents’ perspective has a major contribution in enhancing the clinical knowledge and practice in planning and managing children/adolescents diagnosed with dissociative/conversion disorders.

The emergence of a new Coronavirus Disease 2019-2022 has turned the globe down and disrupted main determinants of health. The world constitutes by more than 2.2 billion children, which is approximately 28% of the world's population. Compare to adult, this pandemic has a long -term adverse consequences among children and adolescents. The objective of this literature review was to examine the impact of COVID-19 pandemic on children and adolescent’s mental health. PubMed and Google Scholar databases were searched from 07 April 2021 to 22 April 2021. A systematic literature search was performed in accordance with PRISMA and 07 literatures were selected. Among 16,169 samples, the mean percentage of male was higher 5 by 6.11% than female of 43.89%. Depression was mentioned in three studies and the percentages were 43.7% (n=8079), 22.6% (n=2330), and 22.8% (n=3613). Anxiety was mentioned in four studies and the percentages were 37.4% (n=8079), 18.9% (n=2330), and 23.87 ± 15.79 and 29.27 ± 19.79 (n=3613) and in one study according to CAQ and NRS, the level of anxiety was 19.4% and 21.8%. The combination of anxiety and depression was 31.3%. One study mentioned that pro social behaviour score was higher in females than males. In other two studies, various psychological symptoms are mentioned such as worry, fear, helplessness, boredom, difficulty in concentration, irritability, restlessness, nervousness, feeling of loneliness and uneasiness. These psychological symptoms have long-term effect on child’s health. Therefore, as a nurse, we should take care of the child and aware that the child and their parents regarding various psychological problems and its management.

Our study aimed to investigate the immediate impact of the COVID-19 pandemic on mental health and quality of life among local Chinese residents aged ≥18 years in Liaoning Province, mainland China. An online survey was distributed through a social media platform between January and February 2020. Participants completed a modified validated questionnaire that assessed the Impact of Event Scale (IES), indicators of negative mental health impacts, social and family support, and mental health-related lifestyle changes. A total of 263 participants (106 males and 157 females) completed the study. The mean age of the participants was 37.7 ± 14.0 years, and 74.9% had a high level of education. The mean IES score in the participants was 13.6 ± 7.7, reflecting a mild stressful impact. Only 7.6% of participants had an IES score ≥26. The majority of participants (53.3%) did not feel helpless due to the pandemic. On the other hand, 52.1% of participants felt horrified and apprehensive due to the pandemic. Additionally, the majority of participants (57.8–77.9%) received increased support from friends and family members, increased shared feeling and caring with family members and others. In conclusion, the COVID-19 pandemic was associated with mild stressful impact in our sample, even though the COVID-19 pandemic is still ongoing. These findings would need to be verified in larger population studies.

Purpose Whilst there is growing evidence to suggest that the recovery college (RC) environment supports students towards their mental health recovery (Meddings et al., 2015b), students’ initial motivations for engagement, alongside factors that may hinder or support attendance, have yet to be exclusively explored. The paper aims to discuss these issues. Design/methodology/approach All new RC students were invited to take part in a semi-structured interview three months following their enrolment. Four participants completed an interview which were later analysed using thematic analysis. Findings Four themes emerged within analysis: making the effort; being “too unwell”; friendly environment; and glad I came. These are discussed alongside the literature, and it is proposed that whilst there is a substantial struggle involved in engagement with a RC, likely related to mental health and social factors, the RC environment, peer support and support of the tutors helps students to overcome the impact of this. Research limitations/implications Due to the small sample size and exploratory stance of this study, additional research into the complexities around engagement with RCs is strongly recommended. Only students who had attended at least one RC course chose to participate in this study, therefore an under-researched population of non-attendees may provide a valuable contribution to further understanding. Originality/value This is one of the first studies to qualitatively explore factors which may support, or hinder, initial and ongoing engagement with a RC. It is proposed that a greater understanding of these important issues could be used to increase RC accessibility and inclusion.

Aims and methodCrisis resolution and home treatment (CRHT) teams began operating in Edinburgh in late 2008. We ascertained service users' and carers' experiences of CRHT using a standardised questionnaire. We also assessed the impact of CRHT on psychiatric admissions and readmissions by analysing routinely collected data from November 2003 to November 2009.ResultsThere was a 24% decrease in acute psychiatric admissions in the year after CRHT began operating, whereas the previous 5 years saw an 8% reduction in the admission rate. The mean duration of in-patient stay fell by 6.5 days (22% decrease) in the 12 months following CRHT introduction, alongside a 4% decrease in readmissions and a 17% reduction in Mental Health Act 1983 admissions. Although the mean response rate was low (29%), 93% of patients reported clinical improvement during CRHT care, 27% of patients felt totally recovered at discharge from CRHT, 90% of patients felt safe during CRHT treatment, and 94% of carers said their friend or relative got better with CRHT input.Clinical implicationsCrisis resolution and home treatment service in Edinburgh had a positive impact during the first 12 months in terms of reduced admissions, reduced duration of in-patient stay and reduced use of the Mental Health Act. The service can catalyse a more efficient use of in-patient care. Service users and carers report high rates of improvement and satisfaction with CRHT.

Abstract Background Studies have shown increased anxiety, depression, and stress levels among different populations during the coronavirus disease 2019 (COVID-19) pandemic. However, the impact of the pandemic on the mental health of dialysis patients remains unknown. The aim of this study was to investigate the mental health of dialysis patients during the COVID-19 pandemic compared to the period preceding the pandemic. Methods Data originate from the ongoing multicentre observational Dutch nOcturnal and hoME dialysis Study To Improve Clinical Outcomes (DOMESTICO). Patients who filled in a health-related quality of life (HRQoL) questionnaire during the pandemic and six to three months prior were included. The mean difference in Mental Component Summary (MCS) score of the Short Form 12 (SF-12) was analysed with multilevel linear regression. A McNemar test was used to compare presence of mental health-related symptoms during and prior to the COVID-19 pandemic. Results A total of 177 patients were included. The mean MCS score prior to COVID-19 was 48.08 ± 10.15, and 49.00 ± 10.04 during the COVID-19 pandemic. The adjusted mean MCS score was 0.93 point (95% CI − 0.57 to 2.42) higher during the COVID-19 pandemic than during the period prior to the pandemic. Furthermore, no difference in the presence of the following mental health-related symptoms was found during the COVID-19 pandemic: feeling anxious, feeling sad, worrying, feeling nervous, trouble falling asleep, and trouble staying asleep. Conclusions The mental health of dialysis patients appears to be unaffected by the COVID-19 pandemic. Dialysis patients may be better able to cope with the pandemic, since they have high resilience and are less impacted by social distancing measures. Objective: In any struggling healthcare system, it is always the frontline workers—doctors, nurses, and paramedical staff – that are affected first. This study aimed to assess the psychological impact—anxiety, stress, depression, and post-traumatic stress disorder (PTSD) —among doctors working in COVID-19 wards and the underlying triggers in a public hospital in Karachi. Methods: Pre- and post-survey methodologies were adopted. Post-graduate trainees working in COVID-19 isolation facilities from April till September 2020 were interviewed before starting their one-month rotation and after completing it. The psychological impact was assessed using two standard instruments—Depression Anxiety and Stress 21 (DASS-21) and the Impact of Event Scale-Revised (IES-R). Data were entered and analyzed through SPSS version 22.0. Results: For 100 doctors, the mean score for stress, anxiety, stress, and depression on DASS-21 and PTSD on IES-R significantly increased after the rotation (p 0.01). The mean score of depression and IES-R was higher in younger (28 years) doctors (p ≤ 0.05). Married doctors scored higher on anxiety and IES-R (p ≤ 0.05). Doctors who were not willfully performing their duties scored worse on all measures of DASS-21 and IES-R (p 0.01). Doctors facing family resist­ance scored significantly higher on stress, anxiety, and IES-R (p 0.01). Conclusion: Working in COVID-19 facilities is imparting a negative impact on the psychological health of doctors. Healthcare administration should be aware of this psychological distress and should introduce mental health aid at administrative levels to prevent a mental health epidemic among care providers.

Abstract Background: The impact of COVID-19 pandemic on mental health of pregnant and lactating women is unclear. This study aimed to assess the impact of COVID-19 on psychological health, sexual function, and quality of life (QoL) in Iranian pregnant and lactating women and compare the results with non-pregnant / non-lactating women.Method: This comparative cross-sectional study was carried out on pregnant and lactating women, with non-pregnant / non-lactating women from May to Jun 2020. Patients were asked to complete three questionnaires: Hospital Anxiety and Depression Scale (HADS), Female Sexual Function Index (FSFI), and Short-Form Health Survey (SF-12). One-way ANOVA was used to reveal the statistical differences between the three groups.Result: The mean age of patients was 20.81±5.92 years old. The mean (SD) score of HADS in pregnant, lactating and non-pregnant / non-lactating women were 12.11 (6.72), 11.98 (8.44) and 9.38 (6.2) respectively, and the results showed that the scores in pregnant, lactating women were higher than non-pregnant / non-lactating women (P<0.001). Also the mean (SD) score of QOL and FSFI was 68.29 (9.47), 74.18 (12.65), 79.03 (10.48) and 22.71 (8.16), 22.72 (8.16), 26.19 (3.93) in three groups and the scores in pregnant, lactating women were lower than non-pregnant / non-lactating women (P<0.001).Conclusion: The COVID-19 epidemic increases the risk of depression, anxiety, FSD, and lowers QoL in pregnant and lactating women, with the general population. This suggests the urgent need for psychological intervention in the maternal population during the epidemic.

Abstract Background and objectives: Researchers are exploring the epidemiology, clinical characteristics, treatment, vaccination and the challenges faced by healthcare authorities. However less focus is being paid towards the impact of COVID-19 on mental health of the patients. This study is a cross-sectional study, measuring the prevalence of emotional distress among patients with COVID-19 in the Maldivian population.Methods: This study was conducted in Maldivian nations above 18 of age with COVID-19 who were admitted in isolation facilities. Patients who were on treatment for any other chronic medical conditions, severe and critical COVID-19 disease were excluded. This study was conducted over a period of 2months by administering a local translated version of DASS21 questionnaire.Results: The total of 195 patients were included in this study. The mean age of the patients was 40 (CI at 95% 38 - 42) years. The respondents were 48.7% men and 51.3% women. Overall, 9% of patients with COVID-19 had depression while 23% of patients had anxiety and 12% of the patients had stress. There was a statistically significant relationship between gender and depression, anxiety and stress (p<0.01). Symptomatic cases had a significantly higher level of stress than asymptomatic patients (p<0.05), but no significant association was observed with symptomatic status and anxiety or depression.Interpretations & conclusion: The management of patients with COVID-19 should be multi-disciplinary with special focus on the mental wellbeing of our patients. We should aim to establish proper communication with the patients in order to identify emotional distress and provide appropriate mental health care.

ABSTRACT Objective This study sought to analyze the impact of oral and mental health on the quality of life of older people enrolled in a Family Health Unit, as well as its associated factors. Methods This was a cross-sectional, descriptive, exploratory study, conducted through interviews. The probabilistic sample consisted of 86 older people, users of the unit, aged 60 and over, dependent or independent, from both sexes. The methodological frameworks adopted for data collection were the evaluation of oral health conditions (edentulism) and the structured interview, employing oral health perception questionnaires (SB-Brazil), the OHIP – 14 (Oral Health Impact Profile), and the PHQ-2 (Patient Health Questionnaire – Two Items) screening tool for depressive disorders. Secondary data was collected from e-SUS individual records (e-SUS Primary Care). Results The majority of the sample (64%) was female, between 60 and 90 years old (mean of 72.4 years). Most frequently occurring OHIP-14 dimensions were pain (16.3%) and psychological disability (11.6%). The variables age, income, schooling, need for and use of prostheses, activities of daily living (ADL), self-reported diseases (diabetes, hypertension and cancer) had no significant association with quality of life measures related to oral health. A good correlation and association (p < 0.001) between PHQ-2 (depressive disorders) and OHIP-14 scores was observed. Conclusion In conclusion, an association between oral and mental health can be observed in older people; thus, for this population-especially in the case of older women oral and mental health care requires an extensive and multiprofessional approach, aimed at improving quality of life.

AbstractBackgroundFor young people, just as in the general population, COVID-19 caused many changes in their lives, including an increased risk for mental illness symptoms. We aimed to study the impact of the COVID-19 pandemic in anxiety and depression symptoms in a cohort of university students.MethodsThis study is part of broader longitudinal research on university students’ mental health with data of the Portuguese version of The Patient Health Questionnaire (PHQ-9) and the Portuguese version of the Generalised Anxiety Disorder (GAD-7) with evaluations on January, May and October 2019 and June 2020, as well as socio-demographic information.Results341 university students (257 females and 84 males) were included, with a mean age of 19.91 (SD=1.58). In June 2020, the mean for perceived wellbeing loss was 60.47% (SD=26.56) and 59.54% (SD=28.95) for mental health loss. The proportion of students with scores equal to or above 15 in the PHQ-9 ranged between 22.6% and 25.5% in 2019 and 37.0% in June 2020. The proportion of GAD-7 scores above cut-off ten ranged between 46.0% and 47.8% in 2019 and 64.5% in 2020. Compared with preceding trends, PHQ-9 scores were 3.11 (CI=2.40-3.83) higher than expected, and GAD-7 scores were 3.56 (CI=2.75-5.37) higher.DiscussionCOVID-19 impacted negatively depressive and anxiety symptoms, confirming previous studies and young people’s vulnerability in such uncertain times.
