Development of Horizontal Tooth Wear in Maxillary Anterior Teeth from Five to 18 Years of Age

Sizes of horizontal wear facets of maxillary anterior teeth were studied longitudinally from the primary dentition at age five to the young adult dentition at the age of 18 years. By a planimetric method, we calculated the wear areas on dental casts taken at the ages of five, ten, 14, and 18 years from the dentition of 39 healthy, orthodontically untreated subjects with good morphological occlusion. For young adults, we also studied the association between the amount of wear and reported parafunctions, maximal bite force, salivary buffer capacity, salivary flow rate, and some cephalometric variables. Size of wear facets on all anterior teeth increased with age. Significant correlations were found between the total wear areas of the six anterior primary teeth at five years of age and those of their permanent successors at age 14 (r = 0.44) and 18 (r = 0.39). For an individual, tooth wear at five years of age was, however, of low predictive value for tooth wear in young adulthood, whereas tooth wear at 14 years of age predicted it well (r = 0.89). Highest correlations between tooth wear and background factors at 18 years of age were found for maximal anterior bite force (r = 0.44) and for the size of the gonial angle (r = -0.31). Wear of anterior teeth was not associated with reported parafunctions in young adulthood.

Saliva have significant roles in maintaining the health of oral cavity. Salivary flow and composition of saliva play a critical role. Saliva also serves as buffer system. Increasing the salivary flow will increase salivary buffer capacity and pH of saliva. Salivary buffer capacity serves to protect the teeth from the acid that comes from food which is formed by acidogenic microorganisms. Milk and dairy are good diet for children that recommended by American Heart Association (AHA). Cheese as one of dairy mild can increase the salivary flow, that help keep the dental hygiene, and prevent bacteria sticking to the teeth. The aim of the study was to identify the effect of cheese and milk consumption on salivary buffer capacity of students aged 10-12 year at Sekolah Dasar Negeri 57 Banda Aceh. Salivary buffer capacity after consume cheese measured after chewing cheese 4 gram about 32 times whereas salivary buffer capacity after consume milk consumption was measured after gargling 25 ml milk 34 times. Salivary buffer capacity measured with Saliva-check buffer (GC). Results of Wilcoxon test showed significant differences (p<0.05) salivary buffer capacity after consumption of milk compared with cheese. Capacity buffer salivary after consumption of cheese higher than milk. In conclusion, cheese and milk consumption have effect on salivary buffer capacity. Salivary buffer capacity after cheese consumption is higher than after milk consumption.

Abstract Objective: To determine the physiologic changes of salivary flow rate, pH, and buffer capacity and the levels of Streptococcus mutans and Lactobacillus spp in patients undergoing fixed orthodontic treatment. Materials and Methods: The study included 23 patients scheduled for fixed orthodontic therapy. All subjects received equal braces, bands, and brackets, bonded with the same material. Stimulated saliva samples were taken before placement of the appliance, and at weeks 6, 12, and 18 during the therapy. Salivary flow rate and salivary pH were measured, and the salivary buffer capacity was determined. Saliva samples were cultivated on selective microbial agar for microorganism detection. Results: A significant (P &lt; .05) increase in stimulated salivary flow rate and salivary pH was found. The salivary levels of S mutans and Lactobacillus spp also inscreased significantly (P &lt; .05), and the major peak was at week 12 of fixed orthodontic therapy. Conclusion: The 6th to 12th week of orthodontic therapy is the period of the most intensive intraoral growth of S mutans and Lactobacillus spp and a time of very intensive salivary functions and physiologic response.

Aim: To determine the severity and distribution of occlusal tooth wear among young North Indian adults and to evaluate the correlation of occlusal tooth wear with bite force. Materials and Methods: A total of 164 subjects were enrolled in the present study. Inclusion criteria included subjects with age range of 25-40 years having a full complement of natural dentition (excluding third molars), with no history of orthodontic treatment, FPD and trauma. Maxillary and mandibular casts of each subject were taken. Tooth wear score of anterior and posterior teeth of both the arches was calculated using a five-point (0 to 4) ordinal scoring system. The calculated tooth wear scores were then compared with data concerning age, sex, number of daily meals, vegetarian/non-vegetarian diet, Group function/Canine guided occlusion and bite force. Nonparametric (Mann-Whitney) test was used to determine the relationship between various factors and occlusal tooth wear. Spearman’s correlation coefficient was used to determine the correlation between tooth wear and bite force. Results: After applying statistical analysis to the data collected, total tooth wear score of the whole sample was 30.07 ± 6.39. Anterior teeth had significantly higher wear score than posteriors (P < 0.01). Males showed significantly higher (P < 0.001) tooth wear in both arches factors such as bite force and age showed significant correlation with tooth wear (P=0.000), however, the number of meals taken per day did not show any significant correlation. Higher tooth wear loss was seen in non vegetarian dietary pattern but it was statistically insignificant. It was also found that Group function occlusion showed significantly higher mean tooth wear loss 45.76 ± 9.19 as compared to Canine guided occlusion 26.37 ± 10.68 (P=0.000).

The dentifrices currently available in the marketplace contain many anticariogenic substances, fluoride and abrasives aimed to better clean the dental surface, remove dental plaque, improve salivary flow and its buffer capacity and reduce colonies of bacteria such as S. mutans, the causative agent of dental caries. The objective of this study was to evaluate the possibility of adequately removing dental plaque using an experimental almond oil dentifrice (Titoil) with no abrasives or antiplaque agents. This study was carried out with 80 volunteers, all of them 18-year-old recruits from the military training school of Araçatuba - SP. Saliva sampling and dental plaque disclosing were undertaken both before and after 28 days of toothbrushing with a low abrasive dentifrice (Group 1: 40 volunteers) or with Titoil (Group 2: 40 volunteers). Statistical analysis of the results revealed that the experimental dentifrice (Titoil) did not interfere with salivary flow and reduced dental plaque more than the low abrasive dentifrice, improved the salivary buffer capacity and decreased salivary S. mutans (Caritest-SM) as much as regular dentifrices. It was concluded that if the dental industry replaces abrasive by vegetable oil in dentifrices, these will be more effective in maintaining oral health and will cause less dental abrasion.

A group of wild moose from Manitoba, Canada, were documented as having excessive tooth wear. Replicas of one incisor tooth were collected from 16 individuals and from 19 controls for comparison by volumetric and scanning electron microscopic analysis. All animals were aged from tooth cementum lines. Tooth replicas were cast from silicone rubber impressions in epoxy resin and sputter coated with gold. Tooth wear with age was compared statistically by measuring crown height, percent tooth loss, interfacet distance, and facet area on the incisors. The unique pattern of wear on the anterior teeth of affected moose was described. In affected animals, crown heights were significantly reduced and percent tooth loss was excessive. Microwear analysis found that an abrasive agent, acting predominantly in a lingual to labial direction, had been responsible for loss of enamel and dentine incisally and interproximally, and on the facial and lingual surfaces to a lesser extent. Interdental facets became involved by vertical abrasion. Control teeth showed fewer but coarser scratches and only approached the pattern of wear found in affected moose in a few individuals in old age. The loss of tooth structure would lead to diminished cropping efficiency and damage to the periodontium.

Objectives: Biopure MTAD™, a new root canal irrigant has shown promising results against the most common resistant microorganism, E. faecalis, in permanent teeth. However, there is lack of studies comparing its antimicrobial effectiveness with NaOCl in primary teeth. The purpose of this study was to compare the in vivo antimicrobial efficacy of NaOCl 2.5% and Biopure MTAD™ against E. faecalis in primary teeth. Study design: Forty non vital single rooted primary maxillary anterior teeth of children aged 4-8 years, were irrigated either with NaOCl 2.5% (n=15), Biopure MTAD™ (n=15) and 0.9% Saline (n=10, control group). Paper point samples were collected at baseline (S1) and after chemomechanical preparation (S2) during the pulpectomy procedure. The presence of E. faecalis in S1 & S2 was evaluated using Real time Polymerase Chain Reaction. Results: Statistical significant difference was found in the antimicrobial efficacy of NaOCl 2.5 % and BioPure MTAD™ when compared to saline (p&gt;0.05). However, no statistical significant difference was found between the efficacies of both the irrigants. Conclusions: NaOCl 2.5% and BioPure MTAD™, both irrigants are equally efficient against E. faecalis in necrotic primary anterior teeth. MTAD is a promising irrigant, however clinical studies are required to establish it as ideal root canal irrigant in clinical practice.

Background: An injury to both the primary and permanent teeth and the supporting structures is one of the most common dental problems seen in children. Splinting is usually difficult or impossible to perform in the primary dentition (due to diminutive room size and lack of patient cooperation). Healing must, therefore, occur despite mobility at the fracture line, usually resulting in interposition of connective tissue. In some instances, infection will occur in the coronal pulp. The present study reported a case of trauma to the anterior primary teeth and alveolar bone in a four year old child. The trauma has caused fracture to the crowns and roots of the primary anterior teeth. The following case was managed in a procedure that may provide primary teeth subjected to trauma a better chance than extraction with a better prognosis. Case presentation: a 4 and a half year old child was subjected to trauma in anterior segment of maxilla. Suturing of the torn soft tissue was the first step followed by pulpotomy for the left primary lateral incisor. Fixation of the right primary central and lateral incisors was done by acid etch wire fixation. Both clinical and radiographic follow up was carried out for 6.4 years. Results: healing of the soft tissue was observed after one week and completed after two months. Fixation of the teeth continued for ten months. The fracture lines in the roots remained in position. Clinically there was no sign of any pulpal inflammation or necrosis. Radiographically, no signs of infection to the surrounding tissues could be seen, no resorption in the alveolar bone, external or internal resorption of the root did not happen also. After ten months fixation ended and the wire was removed. At that time there was normal resorption of the roots of the primary incisors in relation with the normal development of the permanent incisors. After 3 years both permanent central incisors erupted in their normal position. After 6.4 years all four permanent incisors erupted into occlusion in their normal position. Conclusion: primary teeth with root fractures and severely mobile coronal fragments can be treated by a conservative approach. The severity of the sequels is directly related to the degree of permanent tooth formation (child’s age), type of dental trauma and extent of the impact. SUMMARY Tooth wear is a multifactorial condition of growing concern. In clinical practice, it is often a challenge for prevention and treatment since many etiological factors may be involved. This case report describes an esthetic rehabilitation of a young patient presenting tooth wear due to erosion. The etiological factor of this case was the patient sucking on lemons, an acidic fruit. The main complaint of the patient was the appearance of short maxillary incisors. The treatment involved orthodontic and restorative dentistry techniques. First of all, an orthodontic device was used to increase the vertical dimension of occlusion and create an adequate space for the direct restorations. The posterior teeth were restored with two direct composite resin techniques; the anterior teeth were then restored using a balanced occlusion. The seven-year follow-up of the case is presented. Replicas of the restorations were made and visualized under scanning electron microscopy up to the 12-month evaluation. In the clinical follow-up at seven years, maintenance of the results and restorations can be clearly seen.

Esthetic management of extensively decayed primary maxillary anterior teeth requiring full coronal coverage restoration is usually challenging to the pediatric dentists especially in very young children. Many esthetic options have been tried over the years each having its own advantages, disadvantages and associated technical, functional or esthetic limitations. Zirconia crowns have provided a treatment alternative to address the esthetic concerns and ease of placement of extra-coronal restorations on primary anterior teeth. The present article presents a case where grossly decayed maxillary primary incisors were restored esthetically and functionally with readymade zirconia crowns (ZIRKIZ, HASS Corp; Korea). After endodontic treatment the decayed teeth were restored with zirconia crowns. Over a 30 months period, the crowns have demonstrated good retention and esthetic results. Dealing with esthetic needs in children with extensive loss of tooth structure, using Zirconia crowns would be practical and successful. The treatment described is simple and effective and represents a promising alternative for rehabilitation of decayed primary teeth.
