In this paper, we examine how parental caregiving affects women’s employment in Japan. Drawing on the 2005–2014 Longitudinal Survey of Middle-Aged and Elderly Persons, we estimate logistic regression models for the employment status of middle-aged women in various types of employment as a function of caregiving intensity to examine when and in what context caregivers’ employment may be at risk for Japanese women. The results showed that working women who began providing 5 or more hours of care per week were significantly more likely to leave their jobs than non-caregiving women; those who began providing fewer than 5 hours of care per week did not show this likelihood. Among women in regular employment, those who began to provide 5 or more hours of care per week and those who provided care in the previous year were more likely to stop working or change jobs than their non-caregiving counterparts.

Abstract Background Obesity and homocysteine (Hcy) are two important risk factors for cardiovascular disease (CVD); however, there were conflicting results for the relationship between them. Our study is to explore the associations of general and central obesity with hyperhomocysteinemia (HHcy) in middle-aged women. Methods The current analysis was based on data from 11007 women aged 40-60 years. Height, weight, and waist circumference (WC) were measured and serum homocysteine was determined. Multiple logistic regression models were used to assess the associations of the risk of hyperhomocysteinemia (HHcy, Hcy>15μmol/L) with BMI and WC. Results 13.71% women had HHcy. The prevalences of BMI-based general obesity and WC-based central obesity were 11.17% and 22.88%, respectively. Compared with non-obese women, the mean serum Hcy concentration was significantly higher in WC-based central obese women (P = 0.002), but not in BMI-based general obese women (P > 0.05). In the multiple logistic regression models, central obesity was positively related to the risk of HHcy (OR = 1.30, 95%CI=1.10 to 1.52), while general obesity was inversely related to the risk of HHcy (OR = 0.82, 95%CI=0.72 to 0.93 and OR = 0.71,95% CI = 0.57 to 0.89). Conclusions Central obesity was positively, while general obesity was negatively related to the risk of HHcy. Menopause showed no effect modification on these associations. This paper describes linear regression models with parametrically weighted explanatory variables and related logistic regression models that estimate parameters characterizing (1) the effects of weighted variables on the dependent variable and (2) weights for the components of weighted variables. The models also characterize parsimoniously the interaction effects between weighted variables and covariates on the dependent variable by the use of various constraints on parameters. In particular, the models are concerned with testing the significance of variation with covariates in the weights of weighted variables separately from the significance of variation with those covariates in the effects of weighted variables. The usefulness of these models in sociological research is demonstrated by an illustrative analysis of the class identifications of married working women using education, occupational prestige, and income as three variables weighted between own and spousal attributes, and using year, age, race, part-time–full-time distinction, and employment status as covariates.

Objective: This study aims to investigate the association of sleep duration with vision impairment (VI) in middle-aged and elderly adults.Methods: This cross-sectional study used the data from the baseline survey of the China Health and Retirement Longitudinal Study (CHARLS) 2011–2012, a national survey of adults aged 45 years or older. Weighted multilevel logistic regression models were used to evaluate the association between self-reported sleep duration and VI.Results: Of the 13,959 survey respondents, a total of 4,776 (34.2%) reported VI. The prevalence of short (≤6 h/night) and long (>8 h/night) sleep durations was higher among respondents with VI than those without VI (P < 0.001). Multilevel logistic regression models showed that compared with a sleep duration of 6–8 h/night, a sleep duration of ≤6 h/night was associated with a 1.45-fold [95% confidence interval (CI) = 1.34–1.56] higher VI risk, and a sleep duration of >8 h/night was associated with a 1.18-fold (95% CI = 1.03–1.34) higher VI risk, after adjusting for sociodemographic data, lifestyle factors, and health conditions. Vision impairment was associated with short sleep duration in respondents from all age or gender categories. However, VI was associated with long sleep duration in respondents from the elderly or female categories. The association between VI and long sleep duration disappeared in respondents of middle-aged or male categories.Conclusions: The potential impact of sleep on the risk of visual functions requires further attention. A more comprehensive and integrated health care and rehabilitation system covering vision and sleep is also needed.

Abstract Introduction Napping, episodes of short daytime sleep separated from the main sleep period, can compensate for the detrimental effects of inadequate night-time sleep or, as ‘siestas’, may form an integral part of personal sleep-wake schedules. Napping may also precipitate or perpetuate insomnia symptoms through the erosion of homeostatic sleep pressure at night. Using longitudinal data, these analyses were designed to evaluate links between daytime napping styles and the incidence of persistent insomnia symptoms, in a sample of middle aged and older adult good sleepers at baseline. Methods 567 participants (65% female; >35 years old) were sub-sampled from the NITES cohort (Perlis et al, 2019). All were self-rated good sleepers, reporting typical sleep latencies (5 nights/week) and WASO durations of = 1 nap/baseline period) or habitual (>= 2 naps/week). ‘Persistent insomnia symptoms’ cases reported sleep initiation and/or maintenance complaints on >= 3 nights/week for at least two consecutive weeks. Relationships between nap categories and the incidence of persistent insomnia symptoms arising in months 4–12 of the study were assessed in logistic regression models adjusted for age, gender, employment status and depression. Risk is expressed as the odds ratio (OR) with 95% confidence interval (CI). Results Prevalence rates for ever/habitual napping were 57% and 10%. Unemployed status and aged 50+ significantly increased habitual nap probability. The 1-year incidence of persistent insomnia symptoms was 5.6%. In logistic regression models, ‘ever’ napping was associated with an almost 3-fold increase in the risk of developing persistent insomnia symptoms (OR=2.994; 95% CI 1.244–6.969; p=.014). When the logistic regression model was rerun with habitual napping, the risk attenuated substantially (OR = 1.4, 95% CI .592–3.163; p=.463). Conclusion These results suggest that napping patterns serve as a marker for insomnia symptom development, with haphazard non-habitual napping patterns associated with significantly greater risk. Objective: While the use of intraoperative laser angiography (SPY) is increasing in mastectomy patients, its impact in the operating room to change the type of reconstruction performed has not been well described. The purpose of this study is to investigate whether SPY angiography influences post-mastectomy reconstruction decisions and outcomes. Methods and materials: A retrospective analysis of mastectomy patients with reconstruction at a single institution was performed from 2015-2017.All patients underwent intraoperative SPY after mastectomy but prior to reconstruction. SPY results were defined as ‘good’, ‘questionable’, ‘bad’, or ‘had skin excised’. Complications within 60 days of surgery were compared between those whose SPY results did not change the type of reconstruction done versus those who did. Preoperative and intraoperative variables were entered into multivariable logistic regression models if significant at the univariate level. A p-value <0.05 was considered significant. Results: 267 mastectomies were identified, 42 underwent a change in the type of planned reconstruction due to intraoperative SPY results. Of the 42 breasts that underwent a change in reconstruction, 6 had a ‘good’ SPY result, 10 ‘questionable’, 25 ‘bad’, and 2 ‘had areas excised’ (p<0.01). After multivariable analysis, predictors of skin necrosis included patients with ‘questionable’ SPY results (p<0.01, OR: 8.1, 95%CI: 2.06 – 32.2) and smokers (p<0.01, OR:5.7, 95%CI: 1.5 – 21.2). Predictors of any complication included a change in reconstruction (p<0.05, OR:4.5, 95%CI: 1.4-14.9) and ‘questionable’ SPY result (p<0.01, OR: 4.4, 95%CI: 1.6-14.9). Conclusion: SPY angiography results strongly influence intraoperative surgical decisions regarding the type of reconstruction performed. Patients most at risk for flap necrosis and complication post-mastectomy are those with questionable SPY results.

Abstract Previous studies show that working people are less likely to experience unmet health care needs than non-workers. Also, employment and health are located in gendered social contexts. The present study aims to examine the moderating effect of gender on the association between employment status and unmet health care needs among middle-aged Koreans. We conducted logistic regression using the Korean Health Panel data (in 2016 and 2017; N=2,573, age range=45-64). Having experiences unmet health care needs in 2017 was the binary dependent variable. Employment status in 2016 was the binary independent variable and gender was the moderating variable. Age, education level, marital status, annual income, household type, type of medical security, disability, self-rated health, the number of chronic diseases, and stress level in 2016 were also in the analytic model based on the Andersen’s health behavioral model. The percentages of middle-aged people experiencing unmet health care needs were 18% for working men, 11% for non-working men, 13% for working women, and 16% for non-working women. The result showed there was significant moderating effect of gender (B= .72, p&lt; .05). Specifically, working men were less likely to experience unmet health care needs than non-working men. On the contrary, there was not the significant difference in experiencing unmet health care needs between working and non-working women. It indicates that it is necessary to supplement medical services for especially for middle-aged men who are not employed because they might experience considerable amounts of unmet health care needs.

Abstract Purpose To test the effect of anatomic variants of the prostatic apex overlapping the membranous urethra (Lee type classification), as well as median urethral sphincter length (USL) in preoperative multiparametric magnetic resonance imaging (mpMRI) on the very early continence in open (ORP) and robotic-assisted radical prostatectomy (RARP) patients. Methods In 128 consecutive patients (01/2018–12/2019), USL and the prostatic apex classified according to Lee types A–D in mpMRI prior to ORP or RARP were retrospectively analyzed. Uni- and multivariable logistic regression models were used to identify anatomic characteristics for very early continence rates, defined as urine loss of ≤ 1 g in the PAD-test. Results Of 128 patients with mpMRI prior to surgery, 76 (59.4%) underwent RARP vs. 52 (40.6%) ORP. In total, median USL was 15, 15 and 10 mm in the sagittal, coronal and axial dimensions. After stratification according to very early continence in the PAD-test (≤ 1 g vs. > 1 g), continent patients had significantly more frequently Lee type D (71.4 vs. 54.4%) and C (14.3 vs. 7.6%, p = 0.03). In multivariable logistic regression models, the sagittal median USL (odds ratio [OR] 1.03) and Lee type C (OR: 7.0) and D (OR: 4.9) were independent predictors for achieving very early continence in the PAD-test. Conclusion Patients’ individual anatomical characteristics in mpMRI prior to radical prostatectomy can be used to predict very early continence. Lee type C and D suggest being the most favorable anatomical characteristics. Moreover, longer sagittal median USL in mpMRI seems to improve very early continence rates.
