Objective cyclodeviation measurement in normal subjects by means of Cyclocheck® application

Purpose: To evaluate the range of cyclodeviation in normal individuals by means of Cyclocheck® application recently designed by the authors and freely available at www.cyclocheck.com . Methods: Healthy subjects with normal muscle balance, best-corrected visual acuity of ⩾0.8, and stereopsis on Randot charts of ⩽100 s of arc were included in the study. Two separate digital fundus photographs were taken of each eye of every patient. The disk-foveal angle was calculated using the Cyclocheck® application. The average result of the disk-foveal angle measurements were considered for data analysis. Results: A total of 131 patients met inclusion criteria for the study population. The mean value of the disk-foveal angle in the whole study group (both right and left eye) was 6.39° ± 2.72° with 5.26° ± 2.56° (range from −0.4° to 12.55°) in the right eye and 7.52° ± 2.39° (range from 1.25° to 12.76°) in the left eye. The mean value of the disk-foveal angle of the left eye was greater by 2.26° than that of the right eye. Conclusion: Cyclocheck® software allows easy assessment of cyclodeviation. Normal individuals present with a positive value of the disk-foveal angle with a certain spread of the results. The analysis of obtained measurements revealed a significant asymmetry between both eyes with the left eye being more excyclodeviated in an otherwise orthotropic population, which remains a subject for further investigations.

1. To assess the role of changes in lung collagen in pulmonary fibrosis, the content of this protein was measured in biopsy and autopsy lung from patients with cryptogenic fibrosing alveolitis (CFA), a fibrotic lung disorder of unknown cause. 2. The collagen concentration was measured in lung samples from 21 patients with CFA (14 autopsy and seven open-lung biopsy) and 17 normal subjects; total lung collagen was determined in the right lung of 10 patients who died from CFA and the results were compared with those from 10 normal lungs. 3. There was a wide variation in the collagen concentrations but the mean value (±sem) for patients with CFA (217±13mg/g dry weight) was significantly higher (P < 0.02) than that of the controls (155 ± 15mg/g dry weight). The mean collagen concentration of the autopsy samples (243 ± 20 mg/g dry weight) was significantly higher (P < 0.05) than that of the biopsy samples (165 ± 24 mg/g dry weight). 4. The mean total collagen was markedly raised (P < 0.001) in right lungs of patients with CFA (32.5 ± 4.3g) compared with normal lungs (14.0 ± 1.1g). When corrected for the predicted lung volume this difference in total lung collagen remained statistically significant (P < 0.01, mean for patients 4.7 ± 0.7 mg/ml, controls 2.3 ± 0.2 mg/ml). 5. These results demonstrate an increased deposition of lung collagen in this form of pulmonary fibrosis. They also suggest that there is a greater collagen concentration in lungs of patients with later disease, indicating a progressive deposition of collagen during the course of the disease.

Abstract Background This study aims at determining the biological effect of 75/25 w/w nano-hydroxyapatite/chitosan (nHAp/CS) scaffolds on bone regeneration, in terms of fraction of bone regeneration (FBR), total number of osteocytes (Ost), and osteocyte cell density (CD), as well as its biodegradability. Methods Two critical-size defects (CSDs) were bilaterally trephined in the parietal bone of 36 adult Sprague-Dawley rats (18 males and 18 females); the left remained empty (group A), while the right CSD was filled with nHAp/CS scaffold (group B). Two female rats died postoperatively. Twelve, 11, and 11 rats were euthanized at 2, 4, and 8 weeks post-surgery, respectively. Subsequently, 34 specimens were resected containing both CSDs. Histological and histomorphometric analyses were performed to determine the FBR, calculated as [the sum of areas of newly formed bone in lateral and central regions of interest (ROIs)]/area of the original defect, as well as the Ost and the CD (Ost/mm2) in each ROI of both groups (A and B). Moreover, biodegradability of the nHAp/CS scaffolds was estimated via the surface area of the biomaterial (BmA) in the 2nd, 4th, and 8th week post-surgery. Results The FBR of group B increased significantly from 2nd to 8th week compared to group A (P = 0.009). Both the mean CD and the mean Ost values of group B increased compared to group A (P = 0.004 and P < 0.05 respectively). Moreover, the mean value of BmA decreased from 2nd to 8th week (P = 0.001). Conclusions Based on histological and histomorphometric results, we support that 75/25 w/w nHAp/CS scaffolds provide an effective space for new bone formation.

Diffusion-tensor magnetic resonance imaging (DT-MRI) offers great potential for understanding structure-function relationships in human skeletal muscles. The purposes of this study were to demonstrate the feasibility of using in vivo human DT-MRI fiber tracking data for making pennation angle measurements and to test the hypothesis that heterogeneity in the orientation of the tibialis anterior (TA) muscle's aponeurosis would lead to heterogeneity in pennation angle. Eight healthy subjects (5 male) were studied. T1-weighted anatomical MRI and DT-MRI data were acquired of the TA muscle. Fibers were tracked from the TA's aponeurosis by following the principal eigenvector. The orientations of the aponeurosis and muscle fiber tracts in the laboratory frame of reference and the orientation of the fiber tracts with respect to the aponeurosis [i.e., the pennation angle (θ)] were determined. The muscle fiber orientations, when expressed relative to the laboratory frame of reference, did not change as functions of superior-to-inferior position. The sagittal and coronal orientations of the aponeurosis did not change in practically significant manners either, but the aponeurosis′ axial orientation changed by ∼40°. As a result, the mean value for θ decreased from 16.3 (SD 6.9) to 11.4° (SD 5.0) along the muscle's superior-to-inferior direction. The mean value of θ was greater in the deep than in the superficial compartment. We conclude that pennation angle measurements of human muscle made using DT-MRI muscle fiber tracking are feasible and reveal that in the foot-head direction, there is heterogeneity in the pennation properties of the human TA muscle.

Quantitative aspects of the colonization of the gonads by germ cells in the quail embryo (Coturnix coturnix japonica) A quantitative analysis made on quail embryos coming from 13 isolated parent couples reveals some significative variations of a genetic origin, between some of the off spring studied: the differences observed concern both the quantitative importance of the colonization of gonads by germ cells and the asymmetrical distribution of PGCs (primordial germ cells) between the two genital ridges. The chronological study of the colonization in the quail shows, as in both the duck and the chick, two periods of rapid and regular increase of the number of gonadic PGC, at stages from 13 to 18 and from 24 to 30 of Hamburger & Hamilton. The distribution of germ cells between the two genital ridges is, at the beginning of the colonization, not very asymmetrical. Between stages 18 and 24 the asymmetry increases and remains stable so that the mean value of D % (percentage of the number of PGC contained in the right gonad) from that moment on is equal to 29–34 %. This value is specific for the quail embryo.

These studies were performed to see if jejunal malabsorption of magnesium in patients with chronic renal disease was influenced by therapy with 1 alpha, 25-dihydroxyvitamin D3 [1,25-(OH)2D3; 2 microgram/day by mouth for 7 days]. This treatment restored normal serum concentrations of the vitamin D metabolite from 0.9 +/- 0.2 to 4.2 +/- 0.6 ng/dl. Jejunal absorption of magnesium, measured by a triple-lumen constant-perfusion technique, was enhanced in each of the seven patients by this therapy. The mean value rose from 0.04 +/- 0.02 to 0.13 +/- 0.02 mmol . 30 cm-1 . h-1. This last value is similar to the magnesium absorption rate in untreated normal subjects. These results demonstrate that magnesium absorption in the human jejunum is dependent on vitamin D, and they show that 1 alpha,25-dihydroxyvitamin D3 therapy in patients with chronic renal failure is associated with an enhanced jejunal absorption of magnesium.

Purpose : To evaluate the normal distribution of exophthalmometric values in Turkish adult population and the effect of age, gender, refractive status and axial length on globe position. Methods : One hundred and twenty-two males and 114 healthy females with age ranging from 18 to 87 years were included in the study. The study population was recruited from patients presenting to our institution for routine refractive examination. Hertel exophthalmometer was used to measure the degree of ocular protrusion. Effect of age, refractive error, interpupillary distance, and axial length on globe position was detected with linear regression analyses. Results : The mean Hertel exophthalmometric size was 15.7+2.6 mm (range; 11 to 21 mm). The mean value for males was 16.1±2.6 mm (range; 11 to 21 mm), and for females 15.5±2.6 mm (range; 11 to 20 mm). The mean distance between the lateral rims of the orbit was 102 + 5.1 mm (range; 88 to 111mm). The mean exophthalmometric values were not statistically different in males and females. Age and mean spherical equivalents were negatively correlated with exophthalmometric measurements. Axial length was positively correlated with exophthalmometric measurements. Conclusion : The exophthalmometric measurement of the eye is affected by the age, spherical equivalent and the axial length. Standard normative values of the Hertel exophthalmometric measurements should be reevaluated with larger samples.

Abstract We used a whole-cell glucocorticoid receptor assay to examine characteristics of the glucocorticoid receptor in the lymphocytes of normal human donors. We measured binding of [3H]dexamethasone to the lymphocytes of four different donors on several different occasions; the variation about the mean for the assays was +/- 15%. Whole-cell assays in 15 normal subjects showed a mean value of 6.18 fmol/10(6) cells or 3722 sites per cell, with a somewhat higher level in men (7.67 fmol/10(6) cells, or 4620 sites per cell) than women (4.48 fmol/10(6) cells, or 2698 sites per cell). We saw no correlation between donor age and receptor values, in either group. Assays in which we used [3H]prednisolone demonstrated similar binding properties as with [3H]dexamethasone. The mean glucocorticoid receptor value for normal human T-cells from three donors was 2.52 fmol/10(6) cells, or 1518 sites per cell.

1. Free water clearance (CH2O) was measured in sixteen normal subjects and twenty-five patients with uncomplicated hypertension at different levels of salt intake. 2. In normal subjects CH2O and Na+ excretion were related thus: log Na+ excretion = 0·1685 + 0·1508 × CH2O. At a given value for Na+ excretion the standard deviation for CH2O was ±1·5 ml min−1. 3. The mean value for the ratio CH2O/creatinine clearance was higher in the hypertensive patients than in controls at all levels of Na+ excretion. 4. Most previous studies reporting decreased values for CH2O in hypertension can be explained by the exponential relationship between CH2O and Na+ excretion.

Abstract Background: Blood cells are deficient in membrane-bound glycosyl-phosphatidylinositol (GPI)-anchored proteins in paroxysmal nocturnal hemoglobinuria (PNH) because of an acquired disorder of the pluripotent stem cell. CD55 and CD59 antigens are the most common GPI-anchored proteins that are used for the diagnosis of the presence of PNH clone. The association of MDS with PNH is rather controversial. There are few published individual cases of PNH arrived from a previous MDS or MDS following PNH. Aim: Recent studies have demonstrated the existence of PNH clone in some MDS patients. These studies were performed mostly on erythrocytes and to the best of our knowledge they are only 4. Granulocytes appear to be more sensitive markers of PNH clone existence than erythrocytes and therefore we investigate the expression of PNH clone on granulocytes of patients with MDS. Material-Methods: A total number of 95 patients 25–80 yrs old with MDS {A:19 with refractory anaemia (RA), B:9 with refractory anaemia with ring sideroblasts (RARS), C:17 with refractory anaemia with excess blasts (RAEB), D: 12 with refractory anaemia with excess blasts in transformation (RAEB-t) and E: 19 with chronic myelomonocytic leukemia (CMLL)} and 19 healthy donors were included in our study. The presence of GPI-anchored proteins (CD55, CD59)-deficient granulocytes was examined by flow cytometry. For the detection of the PNH clone the commercial kit by Beckman Coulter (cellquant CD55/CD59) was used. Statistical analysis was made by ANOVA, while Robust test was performed because there was no homogeneity of variances by ANOVA. Results: Table 1 shows the percentages of CD55 and CD59 deficient granulocytes in the 5 groups of pts with MDS comparing with the control group. a) Although the mean values of CD55 deficient granulocytes in the groups RAEB, RAEB-t and CMLL were higher enough than those of the normal subjects, there were no statistically significant differences (p>0.10) in the comparison of the mean values of CD55 deficient granulocytes between each group of patients with the control group. b) On the contrary there was a very statistically significant difference in the comparison of the mean value of CD59 deficient granulocytes between the three last groups (C, D and E) of pts with the control group (p< 0.086, p< 0.001 and p<0.001 respectively). On the contrary the two low risk forms (RA, RARS) revealed no such populations. Non-expression of CD59 antigen on granulocytes is a more sensitive marker than that of CD55 antigen for the presence of a possible PNH clone in MDS patients.

Abstract Aim: We decide to study the effect of erythropoietin on the platelet function and glycoproteins expression in patients (pts) with myelodysplastic syndrome (MDS). All patients suffered from primary MDS but none of them received any special treatment. From our study pts that suffered from diseases or they were receiving medication that affect the function of platelets were excluded. Furthermore pts with platelet count&lt;50,000/μl were excluded also because we were unable to check the aggregation of platelets. Material: 41 subjects were studied, 15 normal subject (10 men and 5 women mean age 66,6±14 years old)and 26 pts (17 men and 9 women with mean age70,9±7,4 years old) suffered from all types of MDS according to FAB criteria (9 with RA, 3 with RARS, 7 RAEB, 4 with CMML and 3 with RAEB-t). We divided them in 2 groups: 1st - 7 Pts receiving human recombinant erythropoietin (rEPO) with mean dose 30.000 iu subcutaneous weekly and 2nd - 19 without rEPO. Methods. 1- The platelet function was studied in Platelet Ionized Calcium Aggregometer (PICA) using Ristocetin, ADP, Collagen and Adrenalin as stimulators. 2- The expression of platelet glycoproteins (GPIb, IX, IIb, IIIa and P-selectin) was studied using the flow cytometry and special monoclonal antibodies. This way the percentage of glycoprotein expressed in platelet membrane and MFI were estimated. We performed the statistical analysis of our results using the t-test with common standard deviation.. Results: our results concerning the aggregation test and flow cytometry are presented in tables 1,2 and 3. From the study of our results we can see that while the decrease of aggregation between the patients under EPO and normal subjects is statistically non significant (p&lt;0.1)the decrease of the corresponding values between the pts that did not received EPO and normal subjects was statistically very significant for all stimulators (p &lt;0,001). pts under EPO show an important increase of platelet expressing GPIIb percentage grater than the expressed percentage of patients without EPO (60% vs. 46,1% correspondingly, p&lt;0.001). The difference of expressed MFI was not statistically significant. Conclusion: The findings of our study show us that erythropoietin improves the function of platelets in patients with MDS, probably through the increase of platelet percentage which express glycoproteins.
