Evaluation of a Health Education Intervention to Improve Parental Knowledge and Attitudes About Chronic Stress and Depression Among Head Start Families

Background. Chronic stress and depression disproportionately affect families experiencing poverty, and likely contribute to disparities in early childhood developmental outcomes. Developing strategies to address chronic stress and depression may help mitigate these disparities. Early Head Start (EHS) and Head Start (HS) programs provide an important platform to address the disproportionate burden of stress and mental health issues experienced by EHS/HS families. However, few low-literacy, broad, scalable interventions improve parents’ knowledge and attitudes around these topics. Objectives. We examined parents’ knowledge and attitudes regarding stress and depression before and after a train-the-trainer (TTT) intervention delivered to 28 EHS/HS agencies across the United States. Methods. Following a TTT workshop, 18 agencies chose to deliver the stress training to 1,089 parents and 5 chose to deliver the depression training to 670 parents. Participating parents completed paper assessments at baseline and 3 months following the training. Paired T-tests and chi-square analyses tested whether responses significantly improved over time. Results. At baseline, 37.2% of parents reported feeling stressed most of the time and 13.4% reported feeling depressed most of the time. Following the trainings and reinforcement activities, parents’ knowledge, attitudes and self-reported behaviors significantly improved, including willingness to seek help for depression, avoidance of negative health-risk behaviors and utilization of healthy stress management practices. At follow up, 18.6% of parents reported feeling stressed most of the time and 11% reported feeling depressed. Conclusion. Findings suggest this low-literacy TTT approach is potentially a promising health promotion intervention with broad dissemination potential.

ABSTRACTObjective: The aim of this study was to examine parental knowledge and attitudes about avulsed permanent teeth and their emergency treatment in children.Methods: A total of 289 parents of children aged 6-12 years receiving care at the Ondokuz Mayis University Pediatric Dentistry Clinic were included in the study. Questionnaires were used to collect data on parents’ demographic characteristics, previous training in traumatic dental injury and treatment and knowledge of avulsed permanent teeth and their emergency management. Chi-square tests were used to identify differences in responses for different variables.Results: More than half of the participating parents were female (69.6%) and between the age of 30-39 years (64.4%). Most (90.7%) reported that they would not replant the avulsed tooth in its socket, and most (68.2%) gave correct answers regarding the optimum time for which an avulsed tooth should be left out of the mouth prior to replantation. However, most parents were not sufficiently informed about the appropriate method for cleaning a dirty avulsed tooth or transporting it to a dentist or doctor. None of the demographic characteristics or training status was found to have a statistically significant effect on the number of correct responses regarding appropriate management of avulsion injuries (P>.05)Conclusion: This study revealed that the parental level of knowledge about the emergency treatment of avulsion in children is inadequate and that educational campaigns are necessary to improve proper emergency management of dental avulsion. Associations between community violence exposure, family management practices, and substance use were compared in a sample of early adolescents in low-income communities from the United States ( N = 151; M age = 12.71 years, standard deviation = 0.65; 50.3% female) and South Africa ( N = 175; M age = 12.55 years, standard deviation = 0.85; 64.6% female) using home interviews with youth and their maternal caregivers. Past year victimization was associated with recent youth substance use. The moderating role of family management practices varied by type of practice (e.g., parental knowledge, control, solicitation, or child disclosure), reporter, and country. High parental knowledge reported by caregiver was protective against substance use only for South African youth. In youth reports, parental knowledge was protective across the United States and South Africa. Youth reports of their disclosure to parents were negatively associated with substance use in the United States but not South Africa. These data highlight the importance of considering both ecological context and reporter in the links between violence exposure, parenting, and substance use in youth.

One of the ideals of a nation is to produce a healthy and smart generation. Good generations in the future are formed from the first 1000 days of life to the age of 5 years. An important stimulus is to foster a child's independence in urinating and defecating in the toilet. The strategy that can be done by parents in teaching their children BAB and BAK is through potty training. The purpose of this study was to determine the relationship of knowledge and attitudes with potty training in under three years child’s. This research is analytic with cross sectional design. A sample of 40 toddler parents were taken using the total population technique. Chi Square test the relationship of parental knowledge with potty training obtained P Value 0.002 <0.05 so that parental knowledge with the implementation of potty training has a significant relationship. Chi Square test the relationship between parental attitudes with potty training obtained P Value 0.004 <0.05 so that parental attitudes with the implementation of potty training have a significant relationship. The conclusion of this study is that knowledge and attitudes have a significant relationship with potty training in under three years child’s. It is recommended to the Puskesmas to make an introduction to potty training programs for parents so that the implementation of potty training can be achieved optimally.

Foreign aid has been the subject of much examination and research ever since it entered the economic armamentarium approximately 45 years ago. This was the time when the Second World War had successfully ended for the Allies in the defeat of Germany and Japan. However, a new enemy, the Soviet Union, had materialized at the end of the conflict. To counter the threat from the East, the United States undertook the implementation of the Marshal Plan, which was extremely successful in rebuilding and revitalizing a shattered Western Europe. Aid had made its impact. The book under review is by three well-known economists and is the outcome of a study sponsored by the Department of State and the United States Agency for International Development. The major objective of this study was to evaluate the impact of assistance, i.e., aid, on economic development. This evaluation however, was to be based on the existing literature on the subject. The book has five major parts: Part One deals with development thought and development assistance; Part Two looks at the relationship between donors and recipients; Part Three evaluates the use of aid by sector; Part Four presents country case-studies; and Part Five synthesizes the lessons from development assistance. Part One of the book is very informative in that it summarises very concisely the theoretical underpinnings of the aid process. In the beginning, aid was thought to be the answer to underdevelopment which could be achieved by a transfer of capital from the rich to the poor. This approach, however, did not succeed as it was simplistic. Capital transfers were not sufficient in themselves to bring about development, as research in this area came to reveal. The development process is a complicated one, with inputs from all sectors of the economy. Thus, it came to be recognized that factors such as low literacy rates, poor health facilities, and lack of social infrastructure are also responsible for economic backwardness. Part One of the book, therefore, sums up appropriately the various trends in development thought. This is important because the book deals primarily with the issue of the effectiveness of aid as a catalyst to further economic development.

Natural resources of coastal Louisiana support the economies of Louisiana and the whole of the United States. However, future conditions of coastal Louisiana are highly uncertain due to the dynamic processes of the Mississippi River delta, unpredictable storm events, subsidence, sea level rise, increasing temperatures, and extensive historic management actions that have altered natural coastal processes. To address these concerns, a centralized state agency was formed to coordinate coastal protection and restoration effort, the Coastal Protection and Restoration Authority (CPRA). This promoted knowledge centralization and supported informal adaptive management for restoration efforts, at that time mostly funded through the Coastal Wetlands Planning, Protection and Restoration Act (CWPPRA). Since the Deepwater Horizon (DWH) oil spill in 2010 and the subsequent settlement, the majority of restoration funding for the next 15 years will come through one of the DWH mechanisms; Natural Resource and Damage Assessment (NRDA), the RESTORE Council, or National Fish and Wildlife Foundation –Gulf Environmental Benefit Fund (NFWF-GEBF). This has greatly increased restoration effort and increased governance complexity associated with project funding, implementation, and reporting. As a result, there is enhanced impetus to formalize and unify adaptive management processes for coastal restoration in Louisiana. Through synthesis of input from local coastal managers, historical and current processes for project and programmatic implementation and adaptive management were summarized. Key gaps and needs to specifically increase implementation of adaptive management within the Louisiana coastal restoration community were identified and developed into eight tangible and specific recommendations. These were to streamline governance through increased coordination amongst implementing entities, develop a discoverable and practical lessons learned and decision database, coordinate ecosystem reporting, identify commonality of restoration goals, develop a common cross-agency adaptive management handbook for all personnel, improve communication (both in-reach and outreach), have a common repository and clearing house for numerical models used for restoration planning and assessment, and expand approaches for two-way stakeholder engagement throughout the restoration process. A common vision and maximizing synergies between entities can improve adaptive management implementation to maximize ecosystem and community benefits of restoration effort in coastal Louisiana. This work adds to current knowledge by providing specific strategies and recommendations, based upon extensive engagement with restoration practitioners from multiple state and federal agencies. Addressing these practitioner-identified gaps and needs will improve engagement in adaptive management in coastal Louisiana, a large geographic area with high restoration implementation within a complex governance framework.

Several authors have suggested that a particular managerial component was needed before cost accounting could be fully used for accountability and disciplinary purposes. They argue that the marriage of managerialism and accounting first occurred in the United States at the Springfield Armory after 1840. They generally downplay the quality and usefulness of cost accounting at the New England textile mills before that time and call for a re-examination of original mill records from a disciplinary perspective. This paper reports the results of such a re-examination. It initially describes the social and economic environment of U.S. textile manufacturing in New England in the early nineteenth century. Selected cost memos and reports are described and analyzed to indicate the nature and scope of costing undertaken at the mills in Lowell, Massachusetts, in the late 1820s and early 1830s. The paper discusses how particular cost information was used and speculates why certain more modern procedures were not adopted. Its major finding is that cost management practices fully measured up to the business complexities, economic pressures, and social forces of the day.

Abstract Delirium is a serious and potentially life-threatening problem, but it remains clinically under-recognized. Various factors contribute to this under-recognition, including limited understanding of delirium, insufficient training and application of delirium assessments, potential stigma for the patient and increased workload for the clinician. As a part of an NIH funded study testing a rapid two-step delirium identification protocol at two hospitals in the U.S. (one urban and one rural), clinicians completed a 12-item survey to assess their knowledge and attitudes about delirium and their confidence in preventing and managing delirium. Survey response options followed a 5-point rating scale (strongly disagree, disagree, undecided, agree, strongly agree). The sample for this analysis included 399 clinicians (MDs=53; RNs=235; CNAs=111). Chi-square was used to test for group differences between clinician types. Less than half of the clinicians reported agreeing with the statement, “delirium is largely preventable” (MDs: 47%; RN: 44%; CNA: 41%, p-value=0.021). MDs and RNs indicated a high level of confidence in recognizing delirium while CNAs endorsed lower levels of confidence (MDs: 87%; RN: 81%; CNA: 65%, p-value=0.001). All types of clinicians reported lower confidence in managing delirium (MDs: 29%; RN: 36%; CNA: 44%, p-value=0.117). 47% of CNAs and 37% of RNs agreed there is a need for additional training in caring for persons with delirium while only 21% of MDs agreed (p = 0.031). Understanding how different types of clinicians think and feel about delirium will inform training and communication initiatives, clinical implementation, and research on best practices for delirium identification and management.

Professional development has been identified as a critical component for school nutrition professionals (SNPs) to successfully implement school meal standards in the United States. However, training needs may vary based on different factors. This study examined (1) the topics of highest priority for SNPs; (2) preferred learning methods; (3) where and when trainings should be conducted; and (4) whether responses differ according to important factors including position type, school locale (urban vs. rural), or job experience. Participants completed surveys that included questions on demographics and preferences for learning methods and training topics (n = 492). Descriptive statistics characterized survey responses. Chi square tests assessed differences in learning method and training topic preferences by participant role, locale, and job experience; Cramer’s V assessed the strength of association for each chi square result. Qualitative responses to open-ended questions were analyzed using an inductive thematic analysis method. Nearly all training topic preferences were significantly different (p < 0.001 using Bonferroni method) when stratified by role. Significant differences were also observed for school locale and years of experience, but to a lesser degree. There was less variation in learning method preferences across staff role. Qualitative results (n = 93) identified three key themes related to training needs: role-specific trainings, innovative learning methods, and geographic access. The combination of quantitative and qualitative analysis indicate that professional development for SNPs should mostly be conducted in-person, be easily accessible, and include hands-on activities. Further, training should be tailored by job role and address situational barriers unique to the geographic area.

Gastric cancer is the third most common cause of cancer deaths worldwide. Despite evidence-based recommendation for treatment, the current treatment patterns for all stages of gastric cancer remain largely unexplored. This study investigates trends in the treatments and survival of gastric cancer. The National Cancer Database was used to identify gastric adenocarcinoma patients from 2004–2016. Chi-square tests were used to examine subgroup differences between disease stages: Stage I, II/III and IV. Multivariate analyses identified factors associated with the receipt of guideline concordant care. The Kaplan–Meier method was used to assess three-year overall survival. The final cohort included 108,150 patients: 23,584 Stage I, 40,216 Stage II/III, and 44,350 Stage IV. Stage specific guideline concordant care was received in only 73% of patients with Stage I disease and 51% of patients with Stage II/III disease. Patients who received guideline consistent care had significantly improved survival compared to those who did not. Overall, we found only moderate improvement in guideline adherence and three-year overall survival during the 13-year study time period. This study showed underutilization of stage specific guideline concordant care for stage I and II/III disease.

Abstract Colorectal cancer (CRC) is one of the most common cancers in the USA. In 2017, an estimated 135,420 people were diagnosed with CRC and 50,260 people died from CRC. Several screening modalities are recommended by the United States Preventive Services Task Force (USPSTF), including annual stool tests that are usually completed at home and under-used compared with colonoscopy despite stated patient preferences for an alternative to colonoscopy. The Community Preventive Services Task Force recommends use of small media interventions (SMIs) to increase CRC screening and calls for a greater understanding of its independent impact on screening participation. This study tested whether a SMI increased the likelihood of participant return of a USPSTF recommended Fecal Immunochemical Test (FIT). In total, 804 individuals participated in a two-group, prospective randomized controlled trial. Descriptive statistics with chi-square tests compared differences in participant characteristics and return rates. Multivariable log-binomial modeling estimated combined effects of patient characteristics with FIT return rates. No differences in return rates were observed overall or by participant characteristics other than the year of enrollment. A multivariable model controlling for all covariates, found gender, insurance type, and regular place for healthcare to be significantly associated with return rates. Receipt of the SMI did not independently increase overall return rates but it may have improved the ease of completing the FIT by some participants, particularly women, those with insurance, and those with a regular place for healthcare.
