Objectives: The radiographic crossover sign (COS) is an indicator of acetabular retroversion, classically representing anterosuperior overcoverage often treated with acetabuloplasty which could exacerbate acetabular hypovolemia in dysplasia. No prior study has investigated the prevalence of acetabular retroversion in patients with dysplasia undergoing hip arthroscopy, acetabuloplasty rates, and outcomes in this setting. Study objectives are to report the prevalence of acetabular retroversion in dysplasia, the influence of the COS on arthroscopic acetabuloplasty rates, and relative outcomes compared with control groups. Methods: A retrospective cohort study was performed from a prospectively collected multi-center database. Patients undergoing isolated hip arthroscopic surgery were assigned to dysplasia (lateral center-edge angle (LCEA) ≤ 25°) or one of two control groups: normal(LCEA 26-38°), or pincer femoroacetabular impingement (FAI)(LCEA ≥ 39°). The prevalence of COS and acetabuloplasty rates between and within study and control groups were compared (Chi square analysis). Minimum 2 year outcomes utilizing iHOT-12 were compared (analysis of variance). Results: Of 401 study patients, 64 (16%) comprised the dysplasia group, 273 (68%) normal group, and 64 (16%) pincer group. Mean LCEAs were 22.3°, 31.5°, and 42.4°, respectively (p<0.001). COS prevalence was similar between groups at 31%, 26%, and 33%, respectively (p=0.873). Acetabuloplasty rates were dependent on the amount of acetabular coverage with lowered rate in the dysplasia group(40%) and increasing rates in the normal(52%) and the pincer(90%) groups (p=0.013). Post-operative iHOT-12 scores improved in all groups (68, 74, and 77, respectively, p=0.222). Patients with a COS plus rendered acetabuloplasty reported similar scores that improved to 77, 74, and 79, respectively,(p=0.949). Patients with dysplasia with COS averaged 73.1(SD 23.3) iHOT-12 score compared to 67.1(SD: 28.7) for the patients with dysplasia without COS (p=0.466). Within the dysplasia with COS subgroup, there was no significant difference in iHOT-12 scores between the group with acetabuloplasty (77.0;SD:17.1) and without acetabuloplasty (70.4;SD:27.4,p=0.580). Conclusion: Acetabular retroversion occurs with similar prevalence in borderline dysplasia compared with non-dysplastic hips undergoing hip arthroscopy but with lower acetabuloplasty rates not influenced by the COS. Arthroscopic outcomes of dysplasia with retroversion are similarly successful and comparable with those of dysplasia and non-dysplastic hips. Although infrequently performed, acetabuloplasty did not compromise outcomes in dysplasia with retroversion.

AbstrakIbu nifas sering mengalami masalah menyusui diantaranya karena kurangnya produksi ASI. Penelitian ini bertujuan melihat keefektifan pemberian paket "SUKSES ASI" ibu menyusui dengan seksio sesarea terhadap produksi ASI di wilayah Depok. Penelitian ini menggunakan desain kuasi eksperimen dengan post test only design. Jumlah sampel adalah 29 orang kelompok intervensi dan 31 orang kelompok kontrol. Uji kesetaraan karakteristik didapatkan hasil tidak ada perbedaan bermakna antara kelompok intervensi dan kelompok kontrol (p> α, α< 0,05). Hasil uji analisis dengan Chi-Square didapatkan perbedaan yang bermakna antara kepuasan produksi ASI (p= 0,002, OR 95% CI 9,244), kelancaran produksi ASI dari indikator bayi (p= 0,000, OR 95 % CI 9,000) dan kelancaran produksi ASI dari indikator ibu (p= 0,004, OR 95 % CI 0,181) antara kelompok intervensi dan kontrol. Diharapkan paket "SUKSES ASI" ini dapat digunakan dalam intervensi keperawatan maternitas, serta pengembangan paket yang lebih sempurna melalui penelitian lanjutan dengan survei pengkajian kebutuhan ibu post seksio sesarea untuk kelancaran produksi ASI. AbstractPostpartum mothers often have feeding problems include lack of milk production. This study purposed to look at the effectiveness of the package "SUKSES ASI" nursing mother with caesarean section on milk production in the area of Depok. This study used quasi-experimental design with a post test only design. Number of samples was 29 intervention group and 31 control group. Equivalence test characteristics obtained results no significant difference between the intervention and control groups (p> α, α< 0,05). The results with the Chi-Square analysis found significant differences between milk production satisfaction (p= 0,002, OR 95% CI 9,244), the smooth production of the indicators of infant milk (p= 0,000, OR 95% CI 9,000) and lactation of indicators mothers (p= 0,004, OR 95% CI 0,181) between the intervention and control groups. Expected package "SUKSES ASI" can be used in maternity nursing interventions, as well as the development of a more complete package through continued research with the needs assessment survey of post-caesarean section for maternal lactation.

Objective: Left-handedness reportedly has been more common in persons with neurological afflictions (e.g., stroke) and malformations (e.g., cleft lip with or without cleft palate) that demonstrate marked unilateral involvement. Coronal synostosis is also more frequently unilateral, affecting the right side more commonly than the left. We sought to compare left-handedness in patients with unilateral coronal synostosis versus healthy controls. Subjects: All patients aged 3 years or older with nonsyndromic unilateral coronal synostosis and healthy controls recruited by pediatricians blinded to the study. Main Outcome Measures: Prospective data obtained for all participants included age, gender, and handedness. In patients with unilateral coronal synostosis, the side of synostosis and age at surgery were documented. Left-handedness in the study and control groups was compared using chi-square analysis. Left-handedness also was analyzed in the study group according to side of fusion. Results: Eighty-six patients with nonsyndromic unilateral coronal synostosis comprised the study group; there were 96 controls. The mean ages of the study (8.8 years) and control groups (9.8 years) were not statistically different (p > .05). There were more girls in the study group (67%) than in the control group (56%), but this difference was not statistically significant (p > .05). Left-handedness was documented in 30.2% of the study group and 11.4% of the control group (p < .005). Left-handedness was twice as common in patients with left versus right unilateral coronal synostosis (44.4% versus 20.4%; p < .05). Conclusions: Left-handedness is nearly three times more common in patients with unilateral coronal synostosis than in controls and four times more likely in patients with left-sided fusion.

BackgroundHypertension is known as one of the most important non-communicable pervasive diseases.ObjectiveThe purpose of the present study was to determine the effect of a mobile-based educational app on the blood pressure (BP) of patients with hypertension.MethodsThis clinical trial was conducted on 66 military personnel who were definitively diagnosed with hypertension by a physician, and then assigned randomly into two groups as intervention (receiving mobile-based educational app) and control (receiving standard medical management but no app). Before the intervention, BP levels of both groups were measured with a calibrated sphygmomanometer. After 6 weeks, the BPs of both groups were remeasured using the same sphygmomanometer. Thereafter, descriptive and inferential statistics, including paired t-test, Mann-Whitney, Chi-square and Wilcoxon tests, were used. The data obtained were analysed using SPSS-21 software at a significance level of p<0.05.ResultsComparison of the intervention and control groups showed no statistically significant difference between the groups in systolic BP (p=0.479) and diastolic BP (p=0.851) in the pre-intervention phase, but after the intervention, systolic and diastolic BP levels were significantly lower in the intervention group than in the control group (p=0.0001).ConclusionThe results suggested that the mobile-based educational app had a significant effect on reducing BP in patients with hypertension. Therefore, using this app is recommended for those military personnel with hypertension.

The research method used is a quasi-experimental method which consists of experimental class and class II controlkelas SDN 79 Pekanbaru, which illustrates a significant difference in the ability to calistung between the two. Research on the data pretest seen from arithmetic average of the experimental 70.66 and control 65.9, experimental writing skills 77, and controls 71.6, experimental ability to read 76, and controls 77.5. To find significant differences in the ability of the chi square analysis was used to calculate the ability Xhitung <Xtabel (1.67 <3.841), there was no significant difference, the ability to write with Xhitung <Xtabel (0.6 <3.841), there was no significant difference on writing skills, and the ability to read with Xhitung <Xtabel (1.12 <3.841), there was no significant difference in reading ability. Posttest data seen from the average value of the experimental class numeracy 75.6 and 71.7 control, the ability to write an experimental class 81.9 and control 74.6, literacy class experimental 82.1 and control 78.5. To determine differences in the ability to use the chi square analysis on numeracy Xhitung <Xtabel (3.51 <3.841), there was no significant difference in the experimental class and the control class numeracy skills. The ability to write with Xhitung <Xtabel (3.51 <3.841), there was no significant difference, while the ability to read Xhitung <Xtabel (0.71 <3.841), there was no significant difference. Improved results in the experimental class capabilities calistung numeracy skills of 0.15, and control 0.16, the ability to write of 0.26, and control 0.11, the ability to read the experimental class of 0.31, and controls 0.07, it is seen larger in capacity building calistung the experimental class.Key words: calistung, scientific thematics, numeracy skills, reading skills, and writing skills

Background: Hypospadias is the most common congenital malformation of the urethra with the prevalence of 1 in 200-300 live male births. The objective of this trial was to compare the frequency of urethrocutaneous fistula between Snodgrass and two staged Aivar Bracka repair of distal penile hypospadias in male children. Material & Methods: This RCT was conducted in Department of Plastic & Reconstructive Surgery, Hayatabad Medical Complex, Peshawar, Pakistan from 1st June 2013 to 31st May 2014. 64 male patients were randomly allocated to two surgical procedures, 32 to experimental (Snodgrass) group and 32 to control (two stage Aivar Bracka) group. Age in years, age groups and presence of urethrocutaneous fistula were variables. Age in years was analyzed by mean, SD and range and other variables by count and percentage. McNemar chi-square test was applied to see the significance of difference between the experimental and control groups in terms of frequency of urethrocutaneous fistula at alpha 0.05. Results: Mean age in experimental (Snodgross) group was 6.74 years±1.26 & in control (Aivar Bracka) group 6.71 years ±1.29. The urethrocutaneous fistula was present in three (9.38%, 80% CI 2.77-15.99%) cases in experimental (Snodgross) group and in six (18.75%, 80% CI 9.91-27.59%) cases in control (Aivar Bracka) group. McNemar chi-square test showed no significant difference between the experimental and control groups in terms of frequency of urethrocutaneous fistula. Conclusion: The results are comparable for Snodgrass repair and two stage Aivar Bracka repair for distal penile hypospadias in male children in terms of frequency of urethrocutaneous fistula in our population.

Abstract Background and aims: Nutrition disorders are a common risk factor for many clinical disorders, and effective methods for resolving it are motivational interviewing methods that can be useful in encouraging people to promote lifestyle. In recent years, motivational interviews have come up with community health and often people in need of a change in nutritional habits are so vast that they can maintain their health. Material and method: This interventional study was carried out on all housewives referring to Mazdak Health home. They were selected by simple sampling method. The sample size was 35 in each subgroup. Food intake was assessed by a feed frequency questionnaire (FFQ) and the energy consumption in kilocalories per day was calculated using Nutritionist 4 software. Using SPSS18 and Chi-square tests, the exact test Fisher, T and Independent Tests, Mann-Whitney, and Wilcoxon were analyzed. Results: The mean score of healthy nutrition index after intervention was significantly different in intervention and control groups (p-value = 0.004). Also, in whole grain (p-value = 0.001), dairy (p-value = 0.01) Fruits (p-value = 0.01), vegetables (p-value = 0.003) had a significant difference between the mean of intervention and control groups after intervention. There were no significant differences between the other indices after the intervention in the intervention and control groups. Conclusions: The intervention has been effective and generally increases the index of healthy nutrition index after intervention. But on components of dairy, sodium, refined grains, absent calories, proteins have not had any effect.

Introduction: The role of virgin coconut oil in the treatment of allergic rhinitis is controversial. Thus, the aim of the present study is to determine the effects of virgin coconut oil ingestion, in addition to standard medications, on allergic rhinitis. We also studied the side effects of consumption of virgin coconut oil. Methods: Fifty two subjects were equally divided into test and control groups. All subjects received a daily dose of 10mg of loratadine for 28 days. The test group was given 10ml of virgin coconut oil three times a day in addition to loratadine. The symptoms of allergic rhinitis were scored at the beginning and end of the study. Results:, the symptom score were divided into nasal and non-nasal symptom scores. Sneezing score showed a significant difference, however the score was more in control group than test group, indicating that improvement in symptom was more in control group. The rest of the nasal symptom and non-nasal symptom score showed no significant difference between test and control groups. Approximately 58% of the test subjects developed side effects from consumption of virgin coconut oil, mainly gastrointestinal side effects. Conclusion: In the present study, ingestion of virgin coconut oil does not improve the overall and individual symptoms of allergic rhinitis, furthermore it has side effects.

Abstract This study attempts to see the effect of Think, Talk, and Write strategy on the students’ narrative writing competence. Considering the importance of the use of teaching media, this study tries to integrate Stick Figure as a teaching media in Think, Talk, and Write Strategy. A quasi experimental study was conducted to see the improvement of the students’ narrative writing competence. It involved 42 students who were selected on the basis of convenience sampling and assigned into two groups; experimental and control groups. The statistical analyses of paired sample t-test in experimental group showed that there was significant improvement on the students’ writing competence before (M=5.77, SD= 2.342) and after (M= 11.79, SD= 2.342), t(21)=12.059, p<0.05.Moreover, the result of independent t-test between experimental and control groups showed a significant difference. It can be seen that the mean differences was 3.79545 and the significance value is lower than 0.05, 0.000<0.05.

OBJECTIVESurgical site infection (SSI) after posterior spinal surgery is one of the severe complications that may occur despite administration of prophylactic antibiotics and the use of intraoperative aseptic precautions. The use of intrawound vancomycin powder for SSI prevention is still controversial, with a lack of high-quality and large-scale studies. The purpose of this retrospective study using a propensity score–matched analysis was to clarify whether intrawound vancomycin powder prevents SSI occurrence after spinal surgery.METHODSThe authors analyzed 1261 adult patients who underwent posterior spinal surgery between 2010 and 2018 (mean age 62.3 years; 506 men, 755 women; follow-up period at least 1 year). Baseline and surgical data were assessed. After a preliminary analysis, a propensity score model was established with adjustments for age, sex, type of disease, and previously reported risk factors for SSI. The SSI rates were compared between patients with intrawound vancomycin powder treatment (vancomycin group) and those without (control group).RESULTSIn a preliminary analysis of 1261 unmatched patients (623 patients in the vancomycin group and 638 patients in the control group), there were significant differences between the groups in age (p = 0.041), body mass index (p = 0.013), American Society of Anesthesiologists classification (p < 0.001), malnutrition (p = 0.001), revision status (p < 0.001), use of steroids (p = 0.019), use of anticoagulation (p = 0.033), length of surgery (p = 0.003), estimated blood loss (p < 0.001), and use of instrumentation (p < 0.001). There was no significant difference in SSI rates between the vancomycin and control groups (21 SSIs [3.4%] vs 33 SSIs [5.2%]; OR 0.640, 95% CI 0.368–1.111; p = 0.114). Using a one-to-one propensity score–matched analysis, 444 pairs of patients from the vancomycin and control groups were selected. There was no significant difference in the baseline and surgical data, except for height (p = 0.046), between both groups. The C-statistic for the propensity score model was 0.702. In the score-matched analysis, 12 (2.7%) and 24 (5.4%) patients in the vancomycin and control groups, respectively, developed SSIs (OR 0.486, 95% CI 0.243–0.972; p = 0.041). There were no systemic complications related to the use of vancomycin.CONCLUSIONSThe current study showed that intrawound vancomycin powder was useful in reducing the risk of SSI after posterior spinal surgery by half, without adverse events. Intrawound vancomycin powder use is a safe and effective procedure for SSI prevention.

Background: The relationship between SLE and traditional risk factors for cardiovascular events was evaluated. Methods: The data regarding sixty patients with SLE and 30 healthy controls (age and sex matched) were gathered using SLEDAI forms. Venous blood (10mL) from all the participants was examined for hs-CRP, homocysteine, VCAM1, CBC, anti-DNA antibody, C3, C4, low-density lipoprotein (LDL), cholesterol, FBS and triglyceride. : The IMT of carotid arteries was determined bilaterally by ultrasound. Other measurements included insulin levels via Elisa (Linco/Millipore Corp) and the HOMA-IR index for insulin resistance. Results: The mean age (in years) in the test and control groups was 28.8±10.3 (18-52) and 33.8±9.13 (18-48), respectively. Results: The mean age (in years) in the test and control groups was 28.8±10.3 (18-52) and 33.8±9.13 (18-48), respectively. : The average IMT in the test group was directly related to serum levels of VCAM1 (p<0.001), homocysteine (p<0.001), cholesterol (p<0.009), LDL (p<0.001), TG (p<0.001), and FPG (p=0.004). The association between other risk factors, insulin resistance, carotid IMT and SLEDAI, was nonexistent. Mean insulin and insulin resistance levels in all the participants were 0.43±2.06 µU/mL and 0.09±0.44, respectively. There was no significant difference between the test and control groups regarding serum insulin and insulin resistance levels (p=0.42 and p=0.9, respectively). None of the risk factors, such as hsCRP, VCAM1, or homocysteine, were shown to be related to insulin resistance (p=0.6, p=0.6, p=0.09, respectively). Conclusion:: Our findings did not show an increase in the prevalence of atherosclerosis in patients with SLE. There was no association between IMT and insulin resistance. However, the former was associated with FPG, total cholesterol, LDL, TG, homocystein and VCAM1.
