Abstract Background: Vincristine is a vesicant drug which is normally given intravenously. Intrathecal administration of vincristine is a therapeutic misadventure, causing chemical leptomeningitis and focal ventriculitis. While 55 cases of intrathecal vincristine administration have been reported worldwide, only 32 cases have been documented in the literature, 13 have been reported to the FDA Med Watch, and many more cases are believed to be unreported. Of the documented cases, 84.3% resulted in death and 15.6% of those who survived had serious neurological consequences such as quadriplegia and paraplegia. Methods: The Research on Adverse Events and Reports (RADAR) project conducted a review of literature published between 1968 and June 2006. Sources included Med Watch reports, published abstracts and journal articles, online newsletters, and letters from pharmacists. Findings: The reports reviewed showed that intrathecal administration of vincristine occurred most often because of inadequate communication between pharmacy and medical staff (22 of 32 cases; 68.7%). In these cases, the pharmacy mistakenly delivered vincristine syringes together with syringes containing intrathecal medications and physicians or nurses wrongly administered vincristine intrathecally. Pharmacy error alone, such as the mislabeling of syringes, accounted for 6 of 32 cases of intrathecal administration of vincristine (18.7%) while physician/nurse error alone (failure to read syringe labeling or check physician’s orders) accounted for 4 of 32 cases (12.5%). Conclusions: Since vincristine is lethal when given intrathecally, its administration should be executed with the precautionary measures employed with other potentially lethal substances, such as blood products. Carefully reviewing physician orders before drug administration and dispensing vincristine in syringes incompatible with spinal needles can also curb fatal error. Other preventive measures include properly labeling vincristine syringes for intravenous use only and diluting vincristine in intravenous mini-infusion bags. Abstract Background-The sharp uptick in the cases of mucormycosis in the background of the COVID19 pandemic is a cause of concern and the reasons and it’s impact remains to be seen. We studied the clinical characteristics in patients with mucormycosis and COVID19 co-infection and performed a literature review.Methods-This retrospective study was conducted at tertiary centre in India. All patients admitted with COVID19 and mucormycosis were included, clinical details were obtained from hospital records. We did review of literatures using the terms “SARS-CoV2” OR “COVID19” AND “Mucormycosis” AND “co-infection” on Pubmed published before February 20, 2021.Results-Sixteen cases (M:F–13:3), mean age 46·5 years (24-75years), were included. Fourteen had known risk factors for mucormycosis, the most common being diabetes mellitus. Most patients (n=14) were symptomatic with mucormycosis before diagnosis of COVID19. There was delay in surgery by 22.5 days (IQR–>17.75–29.5), pending SARS-CoV-2 RT-PCR negativity. There were six deaths in this cohort, unrelated to the COVID19 severity. The literature review revealed eleven case reports on co-infection. Patients who had developed mucormycosis were found to have history of mechanical ventilation.Conclusion-The apparent increase in the incidence of mucormycosis may be due to decompensation of underlying comorbidities (decreased access to healthcare), and increased use of immunosuppressants in COVID19. Patients with co-infection were noted to have poorer outcomes.

AbstractIntroductionThe prognosis of preterms and ill newborns has improved substantially during the last decades. Besides conventional methods, additional interventions play an important role in this development. Managing parents' needs under the circumstances of intensive care can be a crucial aspect of dealing with this difficult situation. But data on this group has not been systematically evaluated so far.MethodWe performed a systematic literature review following the PRISMA criteria. Keywords were neonates, neonatology, newborn, preterm, parents, and spirituality. All study types in English and German were included.ResultsAfter evaluating the abstracts of the initial search results, we considered 33 studies eligible for analysis. Study types are surveys, interviews (qualitative studies), case reports, and reviews. Topics were parents, ethnical differences, medical staff, and palliative care.DiscussionThere is a variety of studies on spirituality in neonatology with a broad spectrum of study types. A meta-analysis is difficult, especially because of the heterogeneity of the study designs, patient numbers and questionnaires used. Although the studies are promising, short-term and long-term outcomes have not been evaluated yet. Interesting endpoints such as the influence on mortality, morbidity and duration of hospital stay cannot be stated. Thus it is too early to derive practical recommendations for the medical staff from these studies.ConclusionAlthough there is a body of data on spirituality in neonatology that addresses several important aspects, even more studies on the impact of parents' needs and the consequences for the medical staff are required.

The vast majority of infants in the NICU receive peripheral intravenous (PIV) therapy for administration of fluids, nutrition, medications, and blood products. The potential complications of infiltration and extravasation are common in this population. Consequences of inf.ltration and extravasation may be prevented or mitigated by early detection and prompt treatment. In addition, innovative therapies for wound care are constantly evolving. In order to improve outcomes, a practice guideline for intravenous (IV) infiltration prevention, management, and treatment is presented based on literature review and consultation with wound care experts. The guideline includes preventive measures, standardized IV assessment, staging, an algorithm outlining injury, and wound care recommendations.

This study aims to review the existing literature on organisational sustainability to understand diverse concepts related to organisational sustainability and its evolution. To achieve these objectives, the researcher has reviewed more than one hundred and fifty peer reviewed journal articles related to organisational sustainability that were published between 1987 and 2020. Findings of the literature review confirmed that there were diverse definitions of organisational sustainability and that organisational sustainability is in the process of evolution. Further, this review of literature confirmed the importance of stakeholder engagement in sustainability related decision making at the organisational level. The long term perspective has become a key concern when implementing sustainability in organisations; it has, however, become one of the biggest sources of worry for organisational leadership in this millennium as well. National cultural values and organisational culture are also identified in the literature as influencing leadership decisions related to organisational sustainability. Hence, this literature review confirmed that there are no best solutions or strategies in achieving organisational sustainability, though managers should adopt holistic perspectives and develop innovative solutions to deal with sustainability issues unique to their own organisations. Therefore, it can be concluded that organisational leadership implemented according to a holistic perspective would facilitate excellent results in establishing organisational sustainability. Future research should be directed towards understanding organisational sustainability in a context specific manner rather than using a generalised approach.

Here we present a case of a 38-year-old Indian man with a history of extramarital relationships who presented with pleurisy, skin rash, and radiological findings of pleural effusion. After thorough investigation of the etiology of his acute illness, he was found to be positive for syphilis. Review of literature revealed a small number of case reports of pleural effusion as a manifestation of secondary syphilis. The review of criteria proposed in the literature was utilized to diagnose this patient as a case of pulmonary syphilis.

This study explored urgently needed research issues on COVID-19 infection spreading across animals and humans and vice versa.FindingsEssential and much needed precautionary measures and necessary preventive steps to protect pet animals from infection were highlighted.Originality/valueEssential investigation and experiment results were highlighted. The necessary measure to prevent the spread of infection among and across species and people was analyzed.

Foreign body in the ureter is not common. Most cases of eroded foreign body are preceded by an endoscopic or laparoscopic procedure. For most cases, ureteroscopy, holmium laser fragmentation of encrusted or calcified foreign body followed by extraction of foreign body using grasping forceps have provided optimal outcome. The literature review was conducted to assess the challenges in the management of ureteral foreign body in 13 case reports and compared the outcome using a clinical vignette of a 48-year-old female with metallic clip in the left proximal ureters following laparoscopic left hemicolectomy managed successfully with ureteroscopic holmium laser fragmentation and extraction with grasping forceps.

Background:It is well recognised that men with unilateral testicular cancer may go on to develop metachronous cancer in the contralateral testis. Here, we present two cases of metachronous bilateral testicular cancer and a literature review.Clinical cases:In both cases, the second testicular cancer occurred several years after the initial cancer, and both cases were not screened for presence of contralateral germ cell neoplasia in situ upon the first diagnosis.Discussion:We also present a literature review on the need for screening biopsies of contralateral testis for germ cell neoplasia in situ and the risk factors which should encourage screening. Furthermore, we also explore the effect of chemotherapy on the incidence of contralateral testicular cancer and the effectiveness of radiotherapy in the treatment of germ cell neoplasia in situ.Conclusion:We believe that screening biopsies of the contralateral testis upon diagnosis of unilateral testicular cancer should be encouraged as it can lead to better management of the condition and a relatively favourable outcome on functional fronts.
