ASSESSMENT OF THYROID FUNCTION IN PREGNANT WOMEN OF THE AMUR REGION

A special attention is paid to thyroid gland functioning during pregnancy. It is explained by the fact that pregnancy is a powerful factor which can lead to a manifestation of diseases of the thyroid gland. Cells of the thyroid gland are capable to acquire iodine from food and to synthesize the thyroid hormones participating in all exchange processes both of a pregnant woman and the fetus. For the purpose of studying the features of functioning of the thyroid gland and a number of metabolic processes from ecological positions (in the conditions of city and rural areas) and terms of gestation, 945 pregnant women of the Amur region were examined. Insufficiency of iodine in blood at 95.3% of inhabitants of the city area and at 98.7% of women of rural areas was revealed. It was mainly very severely expressed (47.9 and 55.5%, respectively). Taking into account these data, the assessment of a functional state of the thyroid status was done. Some indicators of lipid profile and carbohydrate ranges were defined. Disturbance of functional activity of mitochondrial dehydrogenases of immunocompetent cells (lymphocytes) in pregnant women of the Amur region in the form of oxidation-reduction reactions imbalance were revealed, which as a whole can badly influence the course of pregnancy and its outcome.

Background: Pregnancy is the period from conception to birth during which there is the requirements of diet that provide the required essential nutrients to keep the mother healthy and allow the fetus to grow and develop in favorable conditions. The objective of the study was to identify the knowledge on dietary pattern among pregnant women attending ANC checkup in Narayani sub-regional hospital. Methods: The study was of descriptive, cross sectional type. The random sampling technique was applied. The study design was conducted in Narayani sub-regional hospital in Parsa district, Nepal. Participants included 96 pregnant women using semi structured questionnaire through face to face interview. Data analysis was done both manually and with the help of SPSS. Results: The study showed that there were more women 56(58.3%) from rural areas than that of urban. About seven out of 10 pregnant women were between the ages of 20 to 35 years of age and the study showed that one-fifth of the pregnant women were married before legal age. The mean age of pregnant women was 24. 1 year. There were 6.2% of pregnant women who were illiterate and six out of 10(59.4%) pregnant women had agriculture as their main occupation. The total sampled 96 pregnant women interviewed all knew about nutrition and mostly the source of information was family, neighbors and friends (34.4%). Almost two-fourth (45.8%) of pregnant women still have the wrong perception regarding consumption of green vegetables and coldness (61.4%) is the main reason behind not consumption of green vegetables. The reason for consumption of processed food was because of easy availability and nine in ten pregnant women replied media influenced them to eat processed food. Two-third pregnant women replied right answer for reason of taking iron tabs. In the study found that there is no association between women literacy and knowledge on nutrition. About 6 in 10 (59.3%) sampled women had moderate knowledge on nutrition whereas only one forth-pregnant woman had high knowledge on nutrition. One-sixth pregnant women had low knowledge. Conclusions: The study revealed that, although every pregnant woman had some sorts of knowledge on nutrition, still there is lack of sufficient knowledge on dietary patterns during pregnancy and some wrong perceptions following diet consumption during pregnancy was found. Health education and awareness programs that influences to develop positive attitude can only ultimately brings changes in the behavior.

Previous work from this laboratory has shown that the thyroid gland of the fetal pig begins to function at about day 46-47 (0.40-0.415 fraction of gestational age). Sera from fetuses contain lower thyroxine (T4), 3,3',5-triiodothyronine (T3) and 3,3',5'-triiodothyronine (rT3) concentrations than maternal sera, except for about 2 weeks before term. The fetal T4 metabolism is dominated by the 5'-monodeiodinating activity (5'-MD). In the present study we measured the iodothyronines content, and the outer (5'-MD) and inner (5-MD) monodeiodinases activity, in homogenates of the placenta. The pig placenta, which is of the epitheliochorial type, was separated into the fetal and the maternal part. The concentrations of T4, T3 and rT3 were lower, and the deiodinating activity of 5'-MD and 5-MD higher, in the fetal than in the maternal placenta. The fetal placenta not only deiodinated more actively T4 to T3 and T4 to rT3, but degraded T3 to 3,3'-diiodothyronine (3,3'-T2) more actively than rT3 to 3,3'-T2. Such divergent deiodinating activity of T4 to T3, T3 to 3,3'-T2 and rT3 to 3,3'-T2 might favor establishing a relatively high and constant rT3 concentrations in fetal and maternal placentas, and a lower T3 in the fetal placenta. The inner ring deiodinating activity (excluding a day before parturition) was always more active in the fetal placenta, while the outer ring deiodinations varied in this respect, depending on the gestation stage. These results support the hypothesis that in the fetal pig, enzymatic deiodination of thyroid hormones forms a barrier which reduces transplacental passage of the hormones and that the fetal part of the placenta is the primary factor in the mechanism regulating the hormonal transfer. In spite of the presence of the barrier, there is an adequate maternal supply of thyroid hormones to the fetus in early gestation, which suggests that the enzymatic mechanism is influenced in some way by the thyroid status of the fetus.

Toxoplasmosis is a zoonosis caused by the Toxoplasma gondii protozoa. The congenital form happens when the parasite reaches the fetus through the placenta and causes damages of different intensities, which can be consequence of the strain virulence, of the immune response capability of the pregnant woman or even of the pregnancy stage, and can result in fetal death or severe clinical symptomatology. The aim of this paper was to determine the prevalence of antibodies to Toxoplasma gondii antibodies in pregnant woman seen at the Basic Health Units (BHU) in the city of Umuarama (PR) and check for possible toxoplasmosis associations with social-demographic characteristics, behavior and environmental variables. In this study, a total of 813 pregnant women who had their prenatal exams at the BHU in the city of Umuarama (PR) were included. Blood samples collected from these subjects were submitted to IgG and IgM anti-T. gondii antibody survey. Each pregnant woman answered an epidemiological questionnaire for the detection of variables, which were later analyzed by the EpiInfo program. The prevalence detected for IgG and IgM anti-T. gondii antibodies were 56.8% and 1.0%, respectively. The variables age group, education level, per capita income, number of pregnancies, ingestion of fresh sausage, presence of peridomiciliary cats in the backyard and the habit of ingesting farm milk showed association to the infection by T. gondii.

In order to study the seasonal effects on the energy supply of immunocompetent blood cells (IBC) in pregnant women living in different conditions of the Amur region, and to substantiate additional methods of diagnosis, prevention and treatment of the identified disorders, a study of the mitochondrial membrane potential (MMP) by flow laser cytometry was conducted in 198 subjects. The data of different types of IBC (lymphocytes, granulocytes and monocytes) at early gestation of women living in urban (Khabarovsk) and rural (Jewish Autonomous region) areas of the Amur region were analyzed. Eight (8) groups were formed according to seasons: winter, spring, summer and autumn. The results of a comprehensive survey indicate some features of intracellular bioenergy processes of IBC depending on the environmental conditions in different seasonal periods. Conditions of women from urban areas are characterized by a significant increase in lymphocytes with reduced MMP which was more pronounced in winter, and by a tendency to increase monocytes in spring-and-summer season. The state of low energy supply of IBC in rural residents differs significantly by means of granulocytes in all presented periods of a year, combinations of various types of IBC in winters and tendency to simultaneous increase in the number of all IBCs with the reduced MMP. The number of IBC with optimal MMP content in urban environment was observed in pregnant women in autumn (23.3%), in rural areas in summer (57.1%). Thus, the study has shown the need to expand the range of diagnostic methods for the formation of pregnancy complications risk groups. Determination of the substrate-energy value in the cells of the immune system is an indication for the individual selection of complex vitamin and mineral medications taking into account seasonal changes. MMP studies allow to monitor the dynamics and evaluate the effectiveness of preventive and therapeutic prescriptions.

Taking into account new approaches in medicine of the 21st century (personalization, prediction, prevention and participation), biogeochemical features of the Amur region, and inhomogeneity of ethnic makeup, the purpose of the work was to determine the occurrence frequency of genes of detoxification system predisposition (glutathione S-transferases – GST) among the population of the Amur region. Ethnic and environmental components were taken into account. Risk groups of pregnant women were formed. Substantiation of a personalized approach to a set of preventive measures for the development of pathological conditions was made. Evaluation of gene polymorphism of phase II xenobiotic biotransformation (GSTT1, GSTM1) was carried out by polymerase chain reaction using diagnostic kits produced by the Center of Molecular Genetics (Moscow). 255 pregnant women of newly-arrived and indigenous population from urban areas (Khabarovsk city) and rural areas of the Amur region (Nanai district) were examined. All examined women were divided into 3 groups: 1 group – urban newcomers, representatives of the (European) Slavic race (n=128); 2 group – rural newcomers, representatives of the (European) Slavic race (n=67); 3 group – rural indigenous of the Mongoloid race (the Nanais), who represent small-numbered peoples of the North, Siberia and the Far East of the Russian Federation from places of traditional residence (n=60). A comparative analysis of the obtained results was carried out in the observed groups. The results were also compared with the data provided by the authors of other regions of the Russian Federation similar in climatic and geographical characteristics (Eastern, Western and Northern Siberia). An ethnic feature of the detoxification system (GSTT1 and GSTM1) of the Amur region indigenous women (nanaian women) was revealed, which is a high frequency of combined deletion polymorphism. The study not only contributes to the scientific and theoretical knowledge, but it is also the basis for the development of additional methods of prevention and correction of disorders associated with exo- and endogenous influence on women in preconception and gestational periods.

Pathologies of the thyroid gland are one of the most common diseases in endocrinology today. Hormones of the thyroid gland carry out hormonal regulation of the cell cycle, the physiological and post-traumatic repair of cells. The hormones of the thyroid gland secrete and enhance the oxidative processes and control the heat production, which can affect the mental state and the ability of the body to adapt to adverse environmental factors. In the case of a lack or almost complete absence of thyroid hormones, normal development and function of any functional system in the body is not possible. In the context of an increase in the incidence of thyroid gland pathology inUkraine, this article presents an analysis of the thyroid status of the conditionally healthy population of Prydniprovia, related to age and gender. For the study, 120 patients of both sexes aged between 20 to 70 were selected. All patients were divided into 3 age groups of 40 people. Each age group was divided into 2 subgroups – male and female, with 20 people in each subgroup. Patients who, in the past and during the experiment, had no cases of thyroid gland disease were selected for accurate and reliable results. The study of thyroid status of the population showed that most of the adult and healthy population of Prydniprovia was not severely affected. All indicators were included in the age and gender limits of the norm, which are established by the preanalytical requirements of the independent laboratory Invitro (Dnipro,Ukraine). In women aged 50–70 years, there is an increased risk of hypothyroidism, which is confirmed by low levels of free thyroxine and triiodothyronine in the blood against the background of increased thyroid stimulating hormone and thyroglobulin levels. In the adult, conditionally healthy population of the Dnipro region aged between 20–70 years, the following correlation relations were observed between thyroid stimulating and thyroid hormones: at a younger age for men, the relationship between TSH and fT4 was –0.97, for TSH and fT3 –0.96, women also had a negative interdependence between these hormones (–0.95 and -0.98 respectively). The age of the men who underwent the test did not change this dependence but in women after 50 years, the Pearson correlation coefficient decreased between the studied hormones TTG and fT4 and fТ3 to –0.94 and –0.93, respectively. Indicators of total thyroxine and total triiodothyronine in the blood serum are not indicative for determining the risk of hypothyroidism.

To maintain thyroid homeostasis, a key role is played not only by the adequate provision of a body with iodine, but also with other essential micronutrients, iron in particular. Iron is involved in the conversion of the amino acid L-phenylalanine into L-tyrosine, which is the part of active center of thyroid peroxidase which catalyzes the reactions of oxidation and organification of iodine during the biosynthesis of thyroid hormones. At the same time, insufficient secretion of thyroid hormones leads to mucinous edema of the gastric mucosa and closure of its ductless glands and reduction in both gastric juice secretion and iron absorption. The objective of the research was to determine the influence of latent iron deficiency onto the thyroid status of schoolchildren. Materials and methods. The study involved 68 boys and 65 girls at the age 6-11 years (Group I) and 12-18 years (Group II). To assess iron metabolism, the levels of hemoglobin in capillary blood, serum iron and ferritin, serum iron binding capacity were determined. To clarify the thyroid status in blood serum samples, there were determined the content of free triiodothyronine and thyroxine, thyroid-stimulating hormone of adenohypophysis, urinary iodine concentration; the dimension of the thyroid gland was determined and evaluated considering gender as well as body surface area according to the standards of thyroid volume (97 percentile). Results. It was established that latent iron deficiency can cause thyroid homeostasis disorders, as indicated by the growth of TSH in blood serum of girls by 37.5% (p<0.01) as compared to the control data. The probability of the formation of thyroid dysfunction significantly increases in case of combined iodine and iron deficiency. The development of subclinical hypothyroidism was established in 90.0% of boys and 89.0% of girls at the age of 6-11 years and 100.0% of boys and 75.0% of girls at the age of 12-18 years with mild iodine deficiency and latent iron deficiency. In schoolchildren with microelementosis, an increase in the thyroid gland was revealed in 66.6% of boys and 75.0% of girls of Group I and in half of boys and girls of Group II.Conclusions. Iron deficiency can cause changes in thyroid homeostasis even at the stage of preclinical disorders. Latent iron deficiency potentiates the development of subclinical hypothyroidism in case of mild iodine deficiency.

One of the most serious problems of modern medicine is sepsis. The number of patients undergoing this complication is 20–30 million (WHO) annually and has no tendency to decrease. Sepsis is characterized by severe multiple organ failure due to a violation of the response of the macroorganism to an infectious agent. Moreover, it is dangerous with high mortality. Sepsis often develops in patients with immunodeficiency conditions, which primarily include pregnant women. The article presents a clinical observation of a case of periostitis in a pregnant woman complicated by a septic state.

The objective: to evaluate the results of planned open surgical interventions (POSI) in pregnant women with primary symptomatic chronic vein disease (PSCVD). Materials and methods. The study included 457 pregnant women operated on a routine basis with PSCVD. The patients underwent ultrasound duplex angioscanning (USDA) of the veins of the lower extremities, inguinal canals, and iliac veins. The severity of the clinical manifestations of PSCVD was evaluated on a VCSS scale. With the help of the circadian visual-analog scale, the dynamics after the operative pain were studied. The effect of POSI on uterine tone and cardiac function of the fetus was investigated using cardiotocography. When forming the results for POSI, data from USDA, the severity of clinical and cosmetic manifestations of PCVD, the need for active prevention of thrombophlebetic, thromboembolic and hemorrhagic complications, minimization of surgical childbirth were taken into account. The results of treatment were studied in 2 groups of patients. Patients of the first group were treated with conservative therapy, the second group – POSI. Parametric indicators, which were presented as mean and standard error M(SD), were used for statistical data processing of the study, and their reliability was estimated using Student’s t-test. The species was determined to be significant at p<0.05. Results. 495 POSI were performed at 28-38 weeks gestation in an obstetric hospital (perinatal center), by a vascular surgeon from the staff of the center, who was familiarized with the peculiarities of working with this contingent of patients. Surgery was performed under local anesthesia in 346 (75.49%) pregnant women at one extremity, in 111 (24.51%) – at two in one session. According to the results of the study, it was noted that in the first group 33.7% of patients had a positive clinical result due to the reduction of pain syndrome, while 67.5% of patients had an increase and spread of varicose transformation in the area of saphenous and/or non-saphenous veins with spread of pathological venous reflux in the distal direction. 78% of patients had the need to administer prophylactic doses of low molecular weight heparins, both during pregnancy and in postpartum period. Out of 126 pregnant women with pronounced varicose veins of the external genital organs and perineum in 36 (28.6%) deliveries were performed by caesarean section. In 9 (3%) patients there was an acute thrombophlebitis of the subcutaneous veins of the lower extremities, which required 5 pregnant women to undergo urgent surgery when the inflammatory process had spread to the middle and above along the femoral vein of the large subcutaneous vein. Pregnant women of the second group with PSCVD on the basis of obstetric hospital (perinatal center) POSI were performed in the organization, which laid the ideology of the FTS program, strictly individual indications for intervention in the optimal terms of pregnancy, multidisciplinary management of patients, due to this in 93% of operated patients regression of clinical manifestations of the disease was noted, whch contributed to the correct and safe delivery of pregnancy with 100% absence of preterm birth, abnormalities in fetal development, pregnancy course, negative impact on obstetric and somatic condition of the pregnant woman. No hemorrhagic, thrombophlebic, thromboembolic complications were noted. Patients in the postoperative period did not require medical support, as during the period of pregnancy, during delivery and in the postpartum period. There were no indications for surgical delivery. 2.4% of patients experienced complications of post-operative wounds in the form of cheese, which had no effect on pregnancy and was eliminated before delivery. In the postoperative period, if necessary, it was recommended to use elastic compression class garments 1–2. Conclusion. POSI made at PSCVD in optimal terms of pregnancy in a specialized obstetric hospital by a vascular surgeon in strictly individual indications is safe, both for the fetus and the pregnant woman. It is promising to further study the results of POSI in pregnant women with PSCVD to introduce it into the arsenal of treatment of this pathology. Disturbance of the thyroid function is often detected in patients with different profiles. A special feature of patients with chronic kidney  disease is the higher incidence of various thyroid function  disturbances, especially hypothyroidism. It is known that in patients  with chronic kidney disease (CKD) iodine excretion from the body is  violated, since normally 90% of iodine is excreted in urine.  Accumulation of high concentrations of inorganic iodine leads to the  formation of the Wolf-Chaikoff effect: suppression of iodine  organization in the thyroid gland and disruption of the thyroid  hormones synthesis. Peripheral metabolism of thyroid hormones is  also disturbed, namely, deiodinase type I activity is suppressed and  peripheral conversion of T4 into T3 is inhibited (so-called low T3  syndrome). Therefore, patients with CKD are often diagnosed with  hypothyroidism, and the origin of hypothyroidism is not always  associated with the outcome of autoimmune thyroiditis. The article  presents an overview of a large number of population studies of  thyroid gland dysfunction in patients with CKD, as well as  experimental data specifying the pathogenetic mechanisms of  thyroid dysfunction in patients with CKD. Therapeutic tactics are still  not regulated. However, in a number of studies, replacement therapy with thyroid hormones in patients with CKD had some advantages.
