Purpose: To improve radiation safety of medical staff and patients during diagnostic and treatment procedures with using of radionuclide radiation sources. Materials and methods: Staff working conditions have been analyzed and the potential for using of various personal protective equipment has been assessed based on the developed framework of personal protection of medical staff and patients at nuclear medicine centers. In accordance with methods described in the current Russian standards, specimens of personal protective equipment manufactured at Russian industrial plants and suitable for use by medical staff and patients at nuclear medicine centers have been tested. Results: Results of laboratory tests of new advanced high-performance personal protective equipment of various purposes for protection of medical staff and patients during diagnostic and treatment procedures with using of radionuclide radiation sources are provided. Training and information documents and guidelines have been developed, including “Study guide on personal protection during diagnostic and treatment procedures with using of radionuclides and ionizing radiation sources” for various departments of the Institute of Continuing Vocational Education, State Research Center – Burnasyan Federal Medical Biophysical Center of the FMBA of Russia, and “Guidelines on personal protection of medical staff and patients during diagnostic and treatment procedures with using of radionuclides and ionizing radiation sources”. Conclusion: Study guide and Guidelines on personal protection of medical staff and patients at nuclear medicine centers during diagnostic and treatment procedures with using of radionuclide radiation sources, as well as the draft of the Standard guidelines on delivery of free personal protective equipment to medical staff have been developed based on the results of work and studies.

Background: We investigated the contact status of medical staff with confirmed or suspected patients with COVID-19 in Sichuan Province, China, as well as the use of personal protective equipment at the time of contact, and we explored the factors affecting the effective use of personal protective equipment. Methods: We performed a cross-sectional study by distributing a questionnaire on occupational protection and exposure of medical staff in the diagnosis and treatment of COVID-19 using a convenience sampling method for online surveys from February 23 to February 29, 2020. Results: In total, 13,829 valid questionnaires from 644 hospitals in Sichuan Province were retrieved, and 802 people were exposed to confirmed or suspected patients with COVID-19, accounting for 5.80%. 688 (85.79%) of 802 people who reported that they had taken effective personal protection measures for each exposure. Sex, work department, and length of service were the independent factors influencing the effective use of personal protective equipment in multivariate analysis (P < .05). Conclusions: Medical institutions need to continue to strengthen the training regarding standard precautions and personal protection, especially for general departments other than fever clinics and isolation wards, as well as medical staff with few working years, to ensure the occupational safety of medical staff.Funding: NoDisclosures: None

Abstract Introduction Preparations of health systems to accommodate large number of severely ill COVID-19 patients in March/April 2020 has a significant impact on nuclear medicine departments. Materials and Methods A web-based questionnaire was designed to differentiate the impact of the pandemic on inpatient and outpatient nuclear medicine operations and on public versus private health systems, respectively. Questions were addressing the following issues: impact on nuclear medicine diagnostics and therapy, use of recommendations, personal protective equipment, and organizational adaptations. The survey was available for 6 days and closed on April 20, 2020. Results 113 complete responses were recorded. Nearly all participants (97 %) report a decline of nuclear medicine diagnostic procedures. The mean reduction in the last three weeks for PET/CT, scintigraphies of bone, myocardium, lung thyroid, sentinel lymph-node are –14.4 %, –47.2 %, –47.5 %, –40.7 %, –58.4 %, and –25.2 % respectively. Furthermore, 76 % of the participants report a reduction in therapies especially for benign thyroid disease (-41.8 %) and radiosynoviorthesis (–53.8 %) while tumor therapies remained mainly stable. 48 % of the participants report a shortage of personal protective equipment. Conclusions Nuclear medicine services are notably reduced 3 weeks after the SARS-CoV-2 pandemic reached Germany, Austria and Switzerland on a large scale. We must be aware that the current crisis will also have a significant economic impact on the healthcare system. As the survey cannot adapt to daily dynamic changes in priorities, it serves as a first snapshot requiring follow-up studies and comparisons with other countries and regions.

Today, there are personal protective equipment, which due to their weight and size characteristics and design features can aggravate professional activity. This is characterized by an increase in the load on several major systems of the employee's body, including the cardiorespiratory system, which can cause the development of occupational pathology. The study of the functions of the cardiorespiratory system of humans in the process of his work is an important task in biomedical tests such as PPE, aggravating their professional activities and future means of individual protection from heavy labor and industrial exoskeletons. The aim of the study is to determine the possibility of using ergospirometry for the physiological assessment of personal protective equipment and industrial exoskeletons. Main part. In medical practice and high-performance sports, ergospirometry is considered the "gold standard" for functional assessment of the cardiorespiratory system. There are very few studies showing the use of ergospirometry for the purpose of physiological assessment of personal protective equipment. Results of research works in which ergospirometry was used in medicine, sports, as well as on the physiological and hygienic assessment of combat equipment, various types of personal protective equipment, including new promising personal protective equipment of the musculoskeletal system-industrial exoskeletons. The use of ergospirometry made it possible to register the main indicators of the human cardiorespiratory system both in field and laboratory conditions and allowed forming an objective conclusion about the dynamics of the degree of stress of the human cardiorespiratory system when using personal protective equipment and industrial exoskeletons with various weight and size characteristics. Thus, the use of ergospirometry both in isolation and in combination with other methods allows us to solve problems related to biomedical testing of personal protective equipment and industrial exoskeletons.

Objectives: Endoscopy confers high risk for acquiring coronavirus disease 2019. Although guidelines recommend that medical staff use personal protective equipment, no infection control equipment have been established for patients. This study aimed to clarify the usefulness of two face masks we had designed for transnasal and transoral endoscopy. Methods: The efficacy of the masks was evaluated by simulating coughing in a mannequin with fluorescent dyes and mapping the droplet trajectory and number. The number of aerosols generated during endoscopy was clinically evaluated in the endoscopy room. Overall, 4356 screening endoscopies were performed with the patients wearing our masks at Medcity21, a health checkup facility, between June and December 2020; the effects of the masks on the patient’s condition were evaluated retrospectively. An 11-item paper-based survey was performed by the endoscopy staff 6 months after the adoption of the mask-based infection control method. Results: Use of both masks reduced the number of droplets released during the simulation. Clinically, the use of both masks did not affect the patients’ conditions during endoscopy and prevented an increase in the aerosols in the endoscopy room. This mask-based infection control method was favorably received, and all staff indicated that understanding the efficacy of our mask-based infection control reduced their anxiety regarding infection. Until December 2020, none of our staff had contracted SARS-CoV-2. Conclusion: Our mask-based infection control method is easy to adopt, inexpensive, and effective; understanding its effectiveness may help ease the fear of infection among endoscopy staff.

Objetivo: relatar a experiência da realização de atividades educativas sobre Paramentação e Desparamentação de Equipamentos de Proteção Individual (EPI) em um hospital da rede federal situado no município do Rio de Janeiro. Método: estudo descritivo, do tipo relato de experiência, realizado em um hospital a partir de ações educativas. Resultados: foram desenvolvidas ações educativas com 894 colaboradores da instituição onde a maioria era do sexo feminino (80,6%), 70% das categorias de enfermagem e mais de 48% dos serviços de internação de adultos. Houve demonstração de insegurança quanto às técnicas, especialmente a de desparamentação. Abstract: Dentists are at risk of experiencing a number of occupational hazards including physical hazards. Potential physical hazards include noise, lighting, ionizing and non-ionizing radiation, percutaneous exposure incident (PEI), and extreme temperatures. The COVID-19 pandemic necessitated unprecedented change within the NHS. Some medical staff have been deployed into unfamiliar roles, while others have been exposed to innovative ways of working. The embedded military Trauma and Orthopaedic (T&O) cadre have been integral to this change. Many of these new skills and ways of working learnt will be transferable to deployed environments. Feedback from the T&O military cadre highlighted key areas of learning as changes in T&O services, use of technology, personal protective equipment, redeployment and training. This paper aims to discuss how these changes were implement and how they could be used within future military roles. The T&O cadre played important roles within their NHS trusts and the skills they learnt will broaden their skills and knowledge for future deployments.

This study aims to analyze the MUI fatwa no. 17 of 2020 concerning the guidelines for prayer for health workers who wear personal protective equipment (PPE) when handling and caring for COVID-19 patients. This study tries to describe the ijtihad method used by MUI in establishing a law. This study uses a type of library research with descriptive analysis. The results of this study state that health workers (PPE) must continue to pray even though it’s not possible to purify themselves (ablution or tayamum). With consideration, 1) if the body (lahiriyah) in hadas condition, it is not obligate his pray (I'adah), 2) if the PPE is unclean then it is necessary to repeat the prayer. The fatwa cannot be separated from the syar'i argument which is the basis for issuing the fatwa, particularly using the talfiq manhaji method. The fatwa can be used as an alternative to confusion over the law, and is intended to create benefits, especially for health workers who treat and handle Covid-19 patients.
