Background The burden of cardiovascular diseases is rising in the developing world including Sub-Saharan Africa. The rapid rise of cardiovascular disease burden is in part due to undetected and uncontrolled cardiovascular risk factors. The clustering of metabolic syndrome (MetS) components is associated with a high risk of cardiovascular diseases. This complex biochemical disorder is still poorly studied in western Africa. In this study, we aimed to determine the prevalence of metabolic syndrome and its determinants among the adult population in Burkina Faso. Methods We performed a secondary analysis of the data from the first national survey on non-communicable diseases risk factors using the World Health Organization (WHO) Stepwise approach. We included 4019 study participants aged 25 to 64 years. The metabolic syndrome prevalence was estimated using six different definitions. Results The mean age was 38.6±11.1 years. Women represented 52.4% and three quarters (75%) lived in rural areas. The overall prevalence of metabolic syndrome according to the different definitions was 1.6% (95%CI:1.1–2.2) for the American College of Endocrinology, 1.8% (95%CI: 1.3–2.4) for the WHO, 4.3% (95%CI:3.5–5.2) for the National Cholesterol Education Program Adult Treatment Panel III, 6.2% (95%CI: 5.1–7.6) for the AAH/NHBI, 9.6%(95%CI: 8.1–11.3) for the International Diabetes Federation and 10.9% (95%: 9.2–12.7) for the Joint Interim Statement. The metabolic syndrome components with the highest prevalence were low High density lipoprotein (63.3%), abdominal obesity (22.3%) and hypertension (20.6%). People living in urban areas and those with older age have higher prevalence of metabolic syndrome regardless of the definition used. Conclusion Our findings suggest various levels of prevalence of MetS according to the definition used. Identifying the most appropriate criteria for MetS among the adult population is important to early detect and treat this syndrome and its components at the primary health care level to control the rising burden of cardiovascular diseases in the context of ongoing epidemiological transition in the country.

Abstract This study aimed to determine anthropometric cut-points for screening diabetes and the metabolic syndrome (MetS) in Arab and South Asian ethnic groups in Kuwait and to compare the prevalence of the MetS based on the ethnic-specific waist circumference (WC) cut-point and the International Diabetes Federation (IDF) and American Heart Association/National Heart, Lung, and Blood Institute WC criteria. The national population-based survey data set of diabetes and obesity in Kuwait adults aged 18–60 years was analysed. Age-adjusted logistic regression and receiver operating characteristic (ROC) analyses were conducted to evaluate for 3589 individuals the utility of WC, waist:height ratio (WHtR) and BMI to discriminate both diabetes and ≥3 CVD risk factors. Areas under the ROC curve were similar for WC, WHtR and BMI. In Arab men, WC, WHtR and BMI cut-offs for diabetes were 106 cm, 0·55 and 28 kg/m2 and for ≥3 CVD risk factors, 97 cm, 0·55 and 28 kg/m2, respectively. In Arab women, cut-offs for diabetes were 107 cm, 0·65 and 33 kg/m2 and for ≥3 CVD risk factors, 93 cm, 0·60 and 30 kg/m2, respectively. WC cut-offs were higher for South Asian women than men. IDF-based WC cut-offs corresponded to a higher prevalence of the MetS across sex and ethnic groups, compared with Kuwait-specific cut-offs. Any of the assessed anthropometric indices can be used in screening of diabetes and ≥3 CVD risk factors in Kuwaiti Arab and Asian populations. ROC values were similar. The WC threshold for screening the MetS in Kuwaiti Arabs and South Asians is higher for women.

Background: Risk factors can lead to clinical conditions, like metabolic syndrome, that predisposes the development of cardiovascular diseases. Objective: The goal of this population-based, prospective and non-randomised cohort study was to study the association between patients with metabolic syndrome and other various factors defining metabolic syndrome. Methods: All the patients referred to the department of Medicine, Al-Ameen Medical college hospital and District Hospital, Vijayapur, Karnataka, India over a period of twenty-two months extending from December 2013 to September 2015 were considered in this study. Results: In the current study, out of 100 patients, 62.9% patients had metabolic syndrome with positive family history of hypertension, diabetes mellitus. 70.8% patients had metabolic syndrome with positive history of smoking. 64.3% patients had metabolic syndrome with positive history of alcohol .73.9% patients had metabolic syndrome with positive history of IHD. 87% of the patients with metabolic syndrome had SBP> 130 mmHg, and 78.85% patients had DBP>85 mmHg. And metabolic syndrome was observed in 71.8% patients on anti hypertensive drugs. The mean level of total cholesterol, LDL cholesterol, triglyceride is increased whereas the mean level of anti-atherogenic HDL cholesterol is low in subjects with MS. At least one lipid abnormality was present in > 95 % of cases. Around 81% subjects with BMI <25 (out of 38) had metabolic syndrome and 58% subjects with BMI>25(out of 62) had metabolic syndrome. Conclusion: All the components defining the metabolic syndrome correlated positively with the abdominal obesity. Systolic blood pressure values were significantly higher than diastolic blood pressure in subjects with abdominal obesity. Metabolic syndrome has multiple risk factors determined by various aspects like the race, the life style, geographical factors larger study is needed to understand the correlation between various components defining it. A healthy lifestyle, that includes avoiding tobacco exposure and proper weight control, must be encouraged in this high-risk population. Abstract Background/Introduction L-carnitine (L-C) has been investigated as a potential therapy for cardiovascular (CV) disease, but its direct effects on human atherosclerosis are unknown. Epidemiological studies suggest a possible reduction of CV risk factors following treatment, whereas animal studies have shown that L-C may increase pro-atherogenic metabolites. Purpose The purpose of this study was to determine whether L-C therapy led to atherosclerosis progression or regression by direct quantification of carotid atherosclerotic lesions in patients with metabolic syndrome (MetS). Methods This study was a Phase 2, prospective, parallel, double blinded, randomized, placebo-controlled, two-center trial. MetS was defined according to the International Diabetes Federation harmonized definition, where presence of any 3 of the 5 following risk factors constituted a diagnosis: elevated waist circumference; elevated triglycerides; reduced HDL or treated; elevated blood pressure or treated; elevated glucose or HbA1c or treated. Participants with a baseline carotid total plaque volume (TPV) ≥50 mm3 were randomized to placebo or 2 g L-C daily for 6 months. Plaque progression was quantified by 3D carotid ultrasound for change in TPV and reduction in vessel lumen area (% area stenosis, Fig. 1). Absolute differences were assessed on the raw scale, while percent change on the log scale. Analysis of covariance (ANCOVA) was used to assess within- and between-arm differences. Results Of the 177 participants randomized, 157 completed the study (L-C n=76, placebo n=81). No statistically significant difference between arms was found in the primary outcome (TPV). However, there was progression of plaque stenosis in the treatment arm: the L-C group had an increase in stenosis of 9.8% (p=0.01) higher than the placebo arm, and a 2.7% (p=0.03) greater absolute change. Total cholesterol and LDL levels (0.10 mmol/L and 0.05 mmol/L, respectively) were higher in the intervention arm compared to the placebo arm (−0.06 mmol/L and −0.07 mmol/L). Figure 1 Conclusions We observed progression of atherosclerosis with L-C therapy compared to placebo in patients with MetS. The clear lack of benefit of L-C therapy in this population raises serious concerns for its further use as a potential therapy. Although the clustering of cardiovascular risk factors is unquestionable, the clinical significance of the metabolic syndrome as a distinct entity has been debated in the past years. Recently, the term ‘metabolic syndrome’ has been replaced by ‘global cardiometabolic risk’ which implies cardiovascular risk factors beyond the metabolic syndrome. The metabolic syndrome can be frequently detected among people in western and developing countries affecting 25-30% of adult population, and its prevalence rate is increasing. Prospective studies show that the metabolic syndrome is a significant predictor of incident diabetes but has a weaker association with cardiovascular morbidity and mortality. At the same time the metabolic syndrome is inferior to established predicting models for either type 2 diabetes or cardiovascular disease.The underlying pathomechanism of the metabolic syndrome is still poorly understood. The role of insulin resistance – although not as a single factor – is still considered as a key component. In the last decade the importance of abdominal obesity has received increased attention but some studies, mainly in the Asian population, showed that central obesity is not an essential component of the syndrome. Regardless of the theoretical debates the practical implications are indisputable. The frequent clustering of hypertension, dyslipidaemia and glucose intolerance, that often accompanies central obesity, can not be ignored. Following the detection of one risk factor, the presence of other, traditional and non-traditional factors should be searched for, as the beneficial effect of intensive, target oriented, continuous treatment of metabolic and cardiovascular risk factors has been proven in both the short and long term.

The reversal rate from clustering of cardiovascular disease (CVD) risk factors—components of the metabolic syndrome (MetS) is not known.Methods and Results. Among 35,534 subjects who received the annual health examinations at the NiigataHealth Foundation (Niigata, Japan), 4,911 subjects had clustering of 3 or more of the following CVD risk factors: (1) body mass index (BMI) ≥25 Kg/m2, (2) blood pressure ≥130 mm Hg in systolic and/or ≥85 mm Hg in diastolic, (3) triglycerides ≥150 mg/dL, (4) high-density lipoprotein cholesterol ≤40 mg/dL in men, ≤50 mg/dL in women, and (5) fasting blood glucose ≥100 mg/dL. After 5 years 1,929 subjects had a reversal of clustering (39.4%). A reversal occurred more often in males. The subjects with a reversal of clustering had milder level of each risk factor and a smaller number of risk factors, while BMI was associated with the least chance of a reversal.Conclusion. We concluded that a reversal of clustering CVD risk factors is possible in 4/10 subjects over a 5-year period by habitual or medical interventions. Gender and each CVD risk factor affected the reversal rate adversely, and BMI was associated with the least chance of a reversal.

ABSTRACTObjectiveThe objective of this study is to determine the prevalence of abdominal obesity and its associated factors in Burkina Faso. We hypothesize that there is a high burden of abdominal obesity and it is significantly associated with sociodemographic and cardiovascular risk factors.DesignWe performed secondary analysis of the survey conducted in Burkina Faso using the World Health Organization (WHO) STEPwise approach.SettingThe study was conducted in Burkina Faso with all 13 regions of the country included.ParticipantsOur study involved 4308 adults of both sexes aged 25 to 64 years.Main outcomeOur primary outcome was the abdominal obesity which was could defined using a cut-off point of waist circumference (WC) of ≥94 cm for men and ≥80 cm for women.ResultsThe overall age-standardized prevalence of abdominal obesity was 22.5% (95% CI: 21.3–23.7). This age-standardized prevalence was 35.9% (95% CI: 33.9–37.9) among women and 5.2% (95% CI: 4.3–6.2) among men (p < 0.001). In urban areas, the age-standardized prevalence of abdominal obesity was 42.8% (95% CI: 39.9–45.7) and 17.0% (95%CI: 15.7–18.2) in rural areas (p < 0.001). The overall age-standardized prevalence of very high WC (WC ≥102 cm for men and ≥88 cm for women) was 10.2% (95%CI: 9.3–11.1). According to the National Institute for Health and Clinical Excellence (NICE) BMI–WC matrix, which combines the body mass index (BMI) and WC to define different levels of cardiovascular health risk, 14.6% of adult Burkinabè had an increased cardiovascular health risk.ConclusionOur study shows a high prevalence of abdominal obesity among the adult population in Burkina Faso. These findings suggest that the measurement of WC should be systematically incorporated in Burkina Faso primary healthcare centers for the early detection of high cardiovascular risk in order to reduce levels of premature death.STRENGTHS AND LIMITATIONS OF THIS STUDY➢This is the first national representative study on abdominal obesity in the context of an emerging epidemiological transition in Burkina Faso.➢A recommended cut-off point was used to define abdominal obesity among the adult population in Burkina Faso, which we found to be associated with “intermediate” cardiovascular risk factors.➢The waist circumference and risk factors used in this study were measured using the standard approach proposed by the WHO [1]. Background: Urbanization, surplus energy uptake, decreased physical activities are general risk factors of metabolic syndrome However, it’s status, and associated components remain unexplored in the Terai region of Nepal. This study evaluated the prevalence of metabolic syndrome and its components among adults with central obesity of Terai region of Nepal using International Diabetes Federation criteria.Methods: Community based cross-sectional study was conducted in three Terai districts of Janakpur Zone, Nepal. A total of 378 adults having central obesity were selected using cluster sampling by camp approach. Interview, physical and clinical examination, measurement of fasting blood sugar, and lipid profile were conducted for all participants. The prevalence of metabolic syndrome and its components with 95% CI were estimated.Results: The metabolic syndrome prevalence was 74.9% (95% CI:70.2-79.2%), with no significant differences between male (77.7%, 95% CI:71.0-83.5%) and female (72.2%, 95% CI: 65.2-78.3%). The most common factors observed were low high density lipoproteins with highly significant differences between male (77.7%, 95% CI:71.0-83.5%)) and female (90.2%, 95% CI: 85.094.0%-; p=0.001) and hypertriglyceridemia with significant differences between male (57.6%, 95% CI: 50.1-64.5%) and female (46.9%, 95% CI: 39.7-54.2%; p=0.037). Conclusions: Higher prevalence of metabolic syndrome and its risk factors in Janakpur of Nepal likely suggest lack of awareness and health promotion activities for metabolic syndrome and indicate an urgency for a public health program to maintain quality of life. Background: Metabolic syndrome is a cluster of metabolic risk factors for type 2 diabetes mellitus and cardiovascular diseases. Physiological variations occurring in women during menopause are thought to be a predisposing factor for the metabolic syndrome. The aim of this study was to evaluate the prevalence of metabolic syndrome and its individual components according to menopausal status in a sample of Moroccan women. Methods: We analyzed data of 653 women aged 19 years and older. We used the recently published joint interim statement criteria to classify subjects as having metabolic syndrome. Results: Out of the total subjects, 262 (40.12%) were syndromic. The metabolic syndrome and abdominal obesity were more common in postmenopausal than in premenopausal women. The highest prevalence of metabolic syndrome was observed in menopausal transition especially among women aged 49-52 years (62.59%). During this period, half of women have at least three metabolic syndrome risk factors. Conclusion: Abdominal obesity associated with metabolic changes occurring in menopause was a risk factor for the development of metabolic syndrome in women. There is a need to adopt a healthy lifestyle to prevent weight gain in women. This can minimize the incidence of metabolic syndrome and its consequences as type 2 diabetes and cardiovascular diseases.

Метаболический синдром - это комплекс взаимосвязанных нарушений углеводного и жирового обмена, а также механизмов регуляции АД и функции эндотелия, в основе развития которых лежит снижение чувствительности тканей к инсулину. Следовательно, ведущим компонентом, патофизиологической основой и объединяющим фактором большинства симптомов, описываемых в рамках метаболического синдрома, является резистентность периферических тканей к действию инсулина, тесно коррелирующая с большинством метаболических нарушений. Необходимо подчеркнуть, что практически все составляющие метаболического синдрома являются установленными факторами риска развития сердечно-сосудистых заболеваний. Вероятность развития метаболического синдрома увеличивается с возрастом. В статье обсуждается возможность представления метаболического синдрома как геронтологической проблемы. Metabolic syndrome is a complex of interrelated disorders of carbohydrate and fat metabolism, as well as mechanisms for regulating blood pressure and endothelial function, which are based on a decrease in tissue sensitivity to insulin. Therefore, the leading component, pathophysiological basis and uniting factor of most of the symptoms described in metabolic syndrome is the resistance of peripheral tissues to the action of insulin, which is closely correlated with most metabolic disorders. It should be emphasized that almost all components of metabolic syndrome are established risk factors for developing cardiovascular diseases. The likelihood of developing metabolic syndrome increases with age. The article discusses the possibility of presenting the metabolic syndrome as a gerontological problem.
