The analysis of the current state of the Russian system of psychiatric care has been carried out; its main shortcomings have been highlighted: the arbitrariness of diagnosing mental disorders, unduly widespread use of antipsychotics, the poor condition of clinical guidelines, the low quality of professional education. Ways of reforming clinical psychiatry are proposed.

Для методологического обоснования использования новых подходов к организации психиатрической помощи ветеранам боевых действий в ведомственных общесоматических учреждениях здравоохранения проведен анализ существующих проблем при оказании психиатрической помощи пенсионерам комбатантам с психическими расстройствами системы Министерства внутренних дел (МВД) России. Показано, что существующие организационные подходы имеют ряд существенных недостатков и не обеспечивают в полном объеме качественного оказания терапии и медикопсихологической реабилитации данному контингенту. Предложено в рамках Концепции развития психиатрической службы МВД России предусмотреть проведение целенаправленной профилактической работы с комбатантами - действующими сотрудниками силовых структур, включающей раннюю диагностику начальных форм психических расстройств, своевременное оказание лечебнореабилитационной помощи с последующим системным мониторингом состояния их психического здоровья в ведомственных учреждениях здравоохранения. Это позволит не только повысить качество диагностики психических расстройств, но и улучшить терапию психопатологических нарушений, уменьшить стигматизацию психиатрической помощи и систематизировать направления работы с данным контингентом на этапах вторичной профилактики изменений ментальных функций. For the purpose of methodological justification of the use of new approaches to the organization of psychiatric care for veterans of military operations in departmental general hospital health care institutions, the analysis of existing problems in providing psychiatric care to retired combatants with mental disorders of the system of the Ministry of internal Affairs (MIA) of Russia was carried out. It is shown that the existing organizational approaches have a number of significant drawbacks and do not fully provide quality therapy and medical and psychological rehabilitation to this contingent. Proposed in the development Concept of the psychiatric service of MIA to provide targeted prevention work with ex-combatants, current employees of power structures, including early diagnosis of early forms of mental disorders, timely provision of treatment & rehabilitation help with subsequent systematic monitoring of their mental health in government health facilities. This will not only improve the quality of diagnosis of mental disorders, but also improve the treatment of psychopathological disorders, reduce the stigma of psychiatric care and systematize the directions of work with this contingent at the stages of secondary prevention of changes in mental functions.

The article highlights the current state and problems of vocational health professional education of physicians and nurses in the specialty «cosmetology». Legal documents: orders, instructions, regulations on the activities of medical practitioners and nurses in the specialty «cosmetology» are described. The authors noted that availability of qualified staff, regularly and systematically improving their professional skills, not only the student´s individual technologies, largely determines the quality and safety of services provided on cosmetology. The authors believe that cosmetologist must have higher professional education in one of the specialties: «General medicine», «Pediatrics», he should complete his residency training on the specialty «Cosmetology» or to complete a professional retraining on specialization «Cosmetology» in the presence of higher education on the specialty «Dermatovenereology». During labour activity a doctor is obliged to obtain additional professional education at least once in five years, what determines the quality of his activities.The authors note, there are the need for greater involvement in the training of private health organizations, which have modern material and technical base and highly qualified personnel.

AbstractThis paper presents a discussion of the historical development of deinstitutionalization in the United States, including the political, ideological, and social origins of that policy. The outcomes of deinstitutionalization are considered, including the impact of the policy on the quality of life of people with severe mental disorders. The authors also provide a brief review of the literature on the efficacy and cost-effectiveness of hospitalization versus outpatient (community) care. Recent developments in the use of involuntary hospitalization are discussed, along with a number of important trends in the delivery and financing of services for persons with severe mental disorders.

Presented in this article is a review, which evaluates the current state of Russia’s secondary professional education system (SPE), and the degree to which it is prepared to train specialists who would be in demand by an innovative economy. Unlike the higher professional education system, during the last decade SPE has not only ceased to be a government priority, but has also “suffered” to a certain degree due to increased attention on behalf of the federal government towards universities, which have attained special statuses – such as “federal”, “research”, “supporting”. Limited financial resources have been redistributed for the benefit of higher educational facilities, this leading to a decrease in funding for training personnel within SPE, while also harming the resource base. This took a serious toll on those technical colleges which trained specialists in new, promising professions. Given such a situation, communicating with employers becomes one of the key factors. However, the cooperation between SPE facilities and enterprises has been experiencing crisis due to the general decline of a number of manufacturing plants during the post-Soviet period. The study is based on the analysis of official statistical data, materials from sociological surveys, as well as expert opinions. This article consistently analyzes the current state of the SPE system, evaluates the training level of teachers, students’ motivation for receiving secondary education, as well as the most popular professions. Special attention is devoted to corporate training acting as the equivalent of SPE, in the form of additional professional training. The article is concluded by examining possible directions for the development of the SPE system, in order to facilitate its adaptation to the demands of an innovative economy. Three possible directions are offered, which might help increase the quality of education within the SPE system, given the current conditions of financial limitations and issues associated with the quality of human resources. This includes cooperation with higher educational facilities, transferring a certain part of educational programs to the premises of employers, and conducting personnel training and advanced training by means of participation in independent programs such as WorldSkills and other such initiatives.

Psychiatry today is mainly practised within a curative framework. However, many mental disorders are persistent and negatively affect quality of life as well as life expectancy. This tension between treatment goals and the actual illness trajectory has evoked a growing academic interest in ‘palliative psychiatry’, namely the application of a palliative care approach in patients with severe persistent mental illness. Recently, Trachsel et al presented a working definition of palliative psychiatry. This first official attempt to capture the concept is based on WHO’s widely accepted definition of palliative care but modified and limited to include only severe persistent psychiatric illness. While this is a welcome step in the discussion on palliative care approaches in psychiatry, it also opens up for new questions. One of the most evident is whether psychiatry actually needs its own definition of palliative care or, put differently, whether there is something about mental disorders that differs so radically from other medical conditions that it calls for a separate definition. We acknowledge the need to discuss the goals of psychiatric care in patients with severe persistent psychiatric illness. However, we question whether a separate definition of palliative care exclusive to psychiatry is the right way to go. In this paper, we discuss why.

  Penempatan peran yang baik bagi Family Caregiver sangatlah membantu lansia dalam meningkatkah qualitas hidupnya, meningkatkan motivasi dalam menjalankan hidup Penelitian ini bertujuan Mengetahui hubungan peran Family Caregiver dalam pemenuhan qualitas hidup bagi lansia di Kelurahan Langenharjo Kabupaten Kendal. DesainPenelitianDeskriptifKorelasional menggunakan pendekatan Krosectional,tehnikSamplingStratified Simple Random Sampling dengan karakteristik heterogen, dari populasi mempunyai hak yang sama untuk diseleksi sebagai sampel teknik undianPengambilan data dengan menggunakan kuesioner yang telah diuji validitas dan reliabilitasnya. Uji statistik Chi-square, dengan taraf signifikasi 5%jumlah sampel pada penelitian ini 70 sampel pada Family Caregiver dari 213 populasi yang ada. Hasil penelitian dari 70 responden didapatkan Peran Family Caregiver tidak baik dengan qualitas hidup tidak baik 33 (47,1%), sedangkan Peran Family Caregiver kurang baik dengan qualitas hidup lansia baik 3 (4,3%). Untuk distribusi Peran Family Caregiver kurang baik dengan qualitas hidup lansia tidak baik sebanyak 6 responden (8,6%) sedangkan untuk distribusi Peran Family Caregiver kurang baik dengan qualitas hidup lansia baik sebanyak 23 responden (32,9%). Terakhir, untuk distribusi Peran Family Caregiver baik dengan qualitas hidup lansia tidak baik didapatkan hasil 2 responden (2, 9%) sedangkan untuk distribusi Peran Family Caregiver baik dengan qualitas hidup lansia baik didapatkan hasil 3 responden (4,3%)Menunjukkan nilai ρ value 0,001 (ρ < 0,05) berarti ada hubungan antara dukungan keluarga dengan kepatuhan lansia dalam keikutsertaan posyandu lansia. Disarankan kepada semua Family Cregiver lansia untuk mampu memahami pentingnya perhatian, dukungan bagi lansia dalammeningkatkan qualitas hidup yang lebih baik bagi lansia.   Kata kunci : Peran family caregiver, qualitas hidup, lansia.   ABSTRACT Placement of a good role for Family Caregiver is very helpful for the elderly to improve their quality of life, increase motivation in living life Research Objective: To know the relationship between the role of Family Caregiver in fulfilling quality of life for the elderly in Langenharjo Village, Kendal Regency. Descriptive Correlational Research Design uses a cross sectional approach, Sampling Stratified Simple Random Sampling technique with heterogeneous characteristics, from the population has the same right to be selected as a sample lottery technique Retrieving data using a questionnaire that has been tested for validity and reliability. Test Chi-square statistics, with a significance level of 5% the number of samples in this study 70 samples on the Family Caregiver from 213 populations. Results of the Study Of 70 respondents found the role of Family Caregiver was not good with poor quality of life 33 (47.1%) , while the role of the Family Caregiver is not good with the quality of life of a good elderly 3 (4.3%). For the distribution of the role of Family Caregiver is not good with the quality of life of the poor family as many as 6 respondents (8.6%) while for the distribution of the Role of Family Caregiver is not good with the quality of life of good elderly as many as 23 respondents (32.9%). Finally, the distribution of the Role of Family Caregiver with good quality of life for the poor is obtained by 2 respondents (2, 9%), while the distribution of the Role of Family Caregiver with good quality of life for the elderly is obtained by 3 respondents (4.3%). 0.001 (ρ <0.05) means that there is a relationship between family support and the compliance of the elderly in the participation of the elderly posyandu. It is recommended to all elderly Cregiver families to be able to understand the importance of attention, support for the elderly in improving the quality of life better for the elderly   Keywords: Role of Family Caregiver, Quality of Life, Elderly

The quality of programs in library and information activity offered by various advanced professional training centers are discussed along with the issues and vectors of advanced training of the RNPLS&T staff. The RNPLS&T has to choose between expensive courses when new knowledge and skills are guaranteed, and online express courses (fast, easy, cheaply) where no new competences are guaranteed though employees get standard certificates. This trend facilitates establishing more and more online training centers that are just making money due to the demand for standardized certification. The double standards of education programs evaluation influence the quality of knowledge, though documented and certified. The authors argue that the above mentioned educational organizations have to be controlled, the more so, as the libraries are to accomplish their staff appraisal.
