Focus on BMI and subclinical hypothyroidism in adolescent girls first examined for amenorrhea or oligomenorrhea. The emerging role of polycystic ovary syndrome

AbstractPrimary amenorrhea, oligomenorrhea and secondary amenorrhea are diagnosed commonly during adolescence. Weight aberrations are associated with menstrual disorders. Autoimmune thyroiditis is frequent during adolescence. In this study, the commonest clinical and hormonal characteristics of amenorrhea or oligomenorrhea during adolescence were investigated.In this cross-sectional study, one hundred and thirty-eight consecutive young patients presenting with amenorrhea or oligomenorrhea referred to an adolescent endocrinology and gynecology university clinic were studied. Clinical examination and an abdominal ultrasound were performed. Testosterone, free-testosterone, estradiol (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH), prolactin (PRL), dehydroepiandrosterone sulfate (DHEA-S), 17-OH progesterone, sex hormone binding globulin (SHBG), ΔPatients with primary and secondary amenorrhea presented more often with body mass index (BMI) <18.5 and BMI >25 kg/mAmong adolescent patients presenting with amenorrhea or oligomenorrhea for the first time those with low and high BMI present more often with primary and secondary amenorrhea, respectively. Obesity is involved in the development of hyperandrogenemia and hyperinsulinemia, particularly in PCOS patients. In these patients, subclinical hypothyroidism may be concealed and it should be investigated. These patients should be treated for abnormally increased or decreased BMI and be investigated for autoimmune thyroiditis.

Polycystic ovary syndrome (PCOS) affects up to 18% of reproductive-aged women with reproductive and metabolic complications. While lipidomics can identify associations between lipid species and metabolic diseases, no research has examined the association of lipid species with the pathophysiological features of PCOS. The aim of this study was to examine the lipidomic profile in women with and without PCOS. This study was a cross-sectional study in 156 age-matched pre-menopausal women (18–45 years, BMI >20 kg/m2; n = 92 with PCOS, n = 64 without PCOS). Outcomes included the association between the plasma lipidomic profile (325 lipid species (24 classes) using liquid chromatography mass spectrometry) and PCOS, adiposity, homeostasis assessment of insulin resistance (HOMA), sex hormone-binding globulin (SHBG) and free androgen index (FAI). There were no associations of the lipidomic profile with PCOS or testosterone. HOMA was positively associated with 2 classes (dihydroceramide and triacylglycerol), SHBG was inversely associated with 2 classes (diacylglycerol and triacylglycerol), FAI was positively associated with 8 classes (ceramide, phosphatidylcholine, lysophosphatidylcholine, phosphatidylethanolamine, lysophosphatidylethanolamine, phosphatidylinositol, diacylglycerol and triacylglycerol) and waist circumference was associated with 8 classes (4 positively (dihydroceramide, phosphatidylglycerol, diacylglycerol and triacylglycerol) and 4 inversely (trihexosylceramide, GM3 ganglioside, alkenylphosphatidylcholine and alkylphosphatidylethanolamine)). The lipidomic profile was primarily related to central adiposity and FAI in women with or without PCOS. This supports prior findings that adiposity is a key driver of dyslipidaemia in PCOS and highlights the need for weight management through lifestyle interventions.

Oxidative stress seems to be present in patients with polycystic ovary syndrome (PCOS). The aim of this study was to evaluate the correlation between characteristics of PCOS and serum concentrations of afamin, a novel binding protein for the antioxidant vitamin E. A total of 85 patients with PCOS and 76 control subjects were investigated in a pilot cross-sectional study design between 2009 and 2013 in the University Hospital of Essen, Germany. Patients with PCOS were diagnosed according to the Rotterdam ESHRE/ASRM-sponsored PCOS Consensus Workshop Group. Afamin and diagnostic parameters of PCOS were determined at early follicular phase. Afamin concentrations were significantly higher in patients with PCOS than in controls (odds ratio (OR) for a 10 mg/ml increase in afamin=1.3, 95% CI=1.08–1.58). This difference vanished in a model adjusting for age, BMI, free testosterone index (FTI), and sex hormone-binding globulin (SHBG) (OR=1.05, 95% CI=0.80–1.38). In patients with PCOS, afamin correlated significantly with homeostatic model assessment-insulin resistance (HOMA-IR), fasting glucose, BMI, FTI, and SHBG (P<0.001), but in a multivariate linear model, only HOMA-IR remained significantly associated with afamin (P=0.001). No correlation was observed between afamin and androgens, LH, FSH, LH/FSH ratio, antral follicle count, ovarian volume, or anti-Müllerian hormone. In conclusion, elevated afamin values may indicate a state of oxidative stress and inflammation, strongly associated with IR and offering an indicator of impaired glucose tolerance in patients with PCOS irrespective of obesity.

Data concerning metabolic consequences in women with polycystic ovary syndrome (PCOS) are delivered mainly by cross-sectional studies. In this research, we re-examined 31 Caucasian PCOS women after a median period of 120.9 months to evaluate the changes in metabolic syndrome components. Clinical examination, oral glucose tolerance test with estimations of glucose and insulin, lipids, sex hormone-binding globulin (SHBG) and sex hormones assessments were performed on two occasions. Additionally, the euglycaemic hyperinsulinaemic clamp technique was used at the baseline to assess insulin sensitivity (M-clamp value). In the end, the median age of participants was 35. We observed an increase in glucose concentrations, a decrease in insulin concentrations and no changes in insulin resistance markers. Final mean glucose, mean insulin, Matsuda index and body mass index (BMI) were correlated with baseline M-clamp value and SHBG (p < 0.01). During the follow-up, no one in the sample developed diabetes. The annualised incidence rate for conversion from normoglycaemia to prediabetes totalled 4.5%. Baseline BMI, free androgen index, fasting glucose and M-clamp value were identified as prediabetes predictors in young PCOS women (respectively, OR = 1.17, OR = 1.42, OR = 1.2, OR = 0.73, p < 0.05). Prediabetes appeared in 76.47% of the women with a final BMI of ≥ 25 kg/m2 and in 7.14% of the normal-weight women (p = 0.0001). In conclusion, we report a high rate of adverse change in glucose metabolism in overweight and obese participants, a deterioration in β-cell function and strong correlations between metabolic parameters assessed in the third and the fourth decade in PCOS women, emphasising the role of early intervention to prevent cardiometabolic diseases.

Background: Polycystic ovary syndrome (PCOS) increases the risk of metabolic syndrome (MetS). Insulin resistance (IR) plays a major role in the pathophysiology of both PCOS and MetS. Objective: This study was designed to compare the prevalence of MetS among different phenotypes of PCOS and its relationship with androgenic components. Materials and Methods: 182 participants eligible for this five-group comparative study were selected by convenience sampling method. They were classified according to the Rotterdam criteria: clinical and/or biochemical hyperandrogenism (H) + PCOS on ultrasound (P) + ovulation disorders (O) (n = 41), clinical and/or biochemical H + PCOS on P (n = 33), PCOS on P + O (n = 40), clinical and/or biochemical H + O (n = 37), and control (without PCOS) (n = 31). MetS was measured based on the National Cholesterol Education Program Adult Treatment Panel III criteria. Androgenic components included free androgen- index (FAI), total-testosterone (TT) level and sex-hormone-binding-globulin (SHBG). Results: A significant difference was observed between the study groups in terms of MetS prevalence (p = 0.01). In phenotype H+P+O, there was a statistically significant positive association between TG and TT, and a significant negative association between SBP and DBP with SHBG. In phenotype O+P, WC was inversely associated with SHBG. In phenotype H+O, FBS and TG were positively associated with FAI but HDL was inversely associated with FAI. Moreover, WC and DBP were positively associated with TT in phenotype H+O. No associations were detected between MetS parameters and androgenic components in other PCOS subjects (phenotype H+P) and in the control group. TT was significantly higher in the PCOS group suffering from MetS (p = 0.04). Conclusion: According to the research results, hyperandrogenic components are potent predictors of metabolic disorders. Thus, we suggest that MetS screening is required for the prevention of MetS and its related complications in PCOS women. Key words: Polycystic ovary syndrome, Metabolic syndrome, Hyperandrogenism.

Abstract: Women or female involved in sport usually become athlete, and athlete usually have high levels of physical exercise, but excess of physical exercise can cause disturbances in female reproductive organ that affects menstrual cycle. Several studies have been conducted to determine the relationship between excess energy consumption due to high level of physical exercise with impaired reproductive function in female athletes such as primary amenorrhea, secondary amenorrhea, and oligomenorrhea. This study aimed to obtain the description of menstruation in female basketball athletes on SMAN 9 Manado. This was a descriptive study using a prospective cross-sectional design. Subjects taken were female students from SMAN 9 Manado who met the inclusion criteria. Instrument that used are physical exercise and menstruation questionnaire. The result showed that 16 subjects had normal menstrual cycle, 12 subjects experienced polymenorrhea, 4 subjects experienced oligomenorrhea, and 1 subject had experienced secondary amenorrhea. Conclusion: Most of the female basketball athletes of SMAN 9 Manado experienced polymenorrhea, some experienced oligomenorrhea, and only one athlete had experienced secondary amenorrhea.Keywords: menstrual disorder, physical exercise, amenorrhea, oligomenorrheaAbstrak: Wanita yang terlibat dalam dunia olahraga banyak yang menjadi atlet sehingga memiliki tingkat latihan fisik yang cukup tinggi. Latihan fisik yang tinggi dapat menyebabkan gangguan pada wanita, diantaranya dapat menimbulkan gangguan pada siklus menstruasi. Beberapa penelitian telah dilakukan untuk mengetahui hubungan antara pemakaian energi berlebih akibat latihan fisik yang tinggi dengan timbulnya gangguan fungsi reproduksi pada atlet wanita berupa amenorea primer, sekunder, dan oligomenorea. Tujuan penelitian ini untuk mengetahui gambaran menstruasi pada atlet basket siswi di SMAN 9 Manado. Metode penelitian ini deskriptif prospektif dengan desain potong lintang. Subjek penelitian ialah siswi-siswi SMAN 9 Manado yang memenuhi kriteria inklusi. Instrumen yang digunakan ialah kuesioner latihan fisik dan menstruasi. Hasil penelitian didapatkan 16 subjek memiliki siklus haid normal, 12 subjek mengalami polimenorea, 4 subjek mengalami oligomenorea, dan 1 subjek pernah mengalami amenorea sekunder. Simpulan: Sebagian besar atlet basket di SMAN 9 Manado mengalami polimenorea, beberapa mengalami oligomenorea, dan hanya 1 atlit yang pernah mengalami amenorea sekunder.Kata kunci: gangguan menstruasi, latihan fisik, amenorea, oligomenorea.

ObjectiveTo assess whetherTCF7L2single nucleotide polymorphisms rs7903146 C/T and rs11196236 C/T are associated with polycystic ovary syndrome (PCOS) in South Brazilian women.DesignCross-sectional study.MethodsTwo hundred PCOS patients and 102 non-hirsute, ovulatory controls were genotyped by real-time PCR. Haplotypes were constructed from the combination of both polymorphisms. Frequencies were inferred using the PHASE 2.1.1 software.Results and conclusionsThe distribution of rs7903146 (PCOS, 54.4% CC; 28.5% CT; 17.1% TT; controls, 51.0% CC; 37.0% CT; 12.0% TT) and rs11196236 (PCOS, 4.3% CC; 33.5% CT; 62.2% TT; controls, 3.2% CC; 35.5% CT; 61.3% TT) was similar between the groups. rs7903146 and rs11196236 were not in linkage disequilibrium (|D′|=0.34;r2=0.07). PCOS participants were younger, with higher age-adjusted BMI, waist circumference, blood pressure, triglycerides, insulin, homeostasis model assessment index to estimate insulin resistance and total testosterone, and lower HDL-C and sex hormone binding globulin vs controls. In PCOS, no differences between genotypes and haplotypes were found for clinical and metabolic variables. However, for each T (rs7903146) and T (rs11196236) allele added to the haplotypes, a variation of 5.87 cm in waist (Ptrend=0.01), 10.7 mg/dl in total cholesterol (Ptrend=0.03), and 10.3 mg/dl in LDL-C (Ptrend=0.01) was recorded.TCF7L2variants are probably not implicated in PCOS development in South Brazilian women.

Aim. The aim of this work was to study the effectiveness of the piracetam electrophoresis application in the complex sanatoriumresorttreatment of girls with amenorrhea. Material and methods. 56 girls aged 12 to 17 years were under observation, who were treated in the gynecological department ofthe children’s clinical sanatorium “Zdravnitsa”. Primary amenorrhea was recorded in 15 girls, secondary amenorrhea in 41. Long-termresults of sanatorium-resort treatment were studied in 8 girls with secondary amenorrhea. The control group consisted of 20 healthygirls, comparable in age, who underwent hormonal studies. Results. Upon admission to the sanatorium, the main complaint of all girls was the absence of menstruation, many had chronicinflammatory pathology of the ENT organs and pathology of the musculoskeletal system. There was a decrease in ovarian steroidogenesis,while hypoestrogenism was observed in the majority of patients. Girls with amenorrhea are characterized by a high levelof vertigo, an average level of anxiety, fatigue and a fairly low level of irritability. After the sanatorium-resort treatment, an improvementin the general condition of the girls was noted; during their stay in the sanatorium, menstruation was observed in 4 girls withsecondary amenorrhea. After the treatment, a significant positive dynamics of steroid and peptide hormones was revealed in girlswith primary and secondary amenorrhea. In girls with secondary amenorrhea, the ratio of LH/FSH approached the physiological one,but remained significantly higher than normal indicators. Under the influence of sanatorium-resort treatment, the level of adrenalinein the urine normalized in all girls with primary amenorrhea, and in the majority with secondary amenorrhea. There was a positivedynamics of the girls’ psycho-emotional state. According to the long-term results data of sanatorium-resort treatment, the restorationof menstrual function was observed within a year in half of the girls with secondary amenorrhea. Conclusion. The use of the 5% piracetam solution electrophoresis in a complex sanatorium-resort treatment of girls with amenorrheaimproved the general health of girls, normalized hormonal levels and restored menstrual function in half of the girls with secondaryamenorrhea within a year after a course of treatment in the sanatorium.
