A complex approach to the early diagnosis of skin melanoma has been proposed. The approach involves a step-by-step examination of pigment tumors using several imaging systems. The features of morphometry of clinical images of pigmented skin neoplasms, features of imaging systems, the main stages of automated image processing and pattern recognition in the melanoma diagnosis has been considered. The metrological features of the proposed approach has been shown: the measurement errors of the clinical parameters of skin neoplasms by the proposed methods do not exceed the allowable errors. The approbation of the offered approach has been showed that sensitivity and specificity of the used methods exceeds 90%

This article presents the results of the observation of cases of skin melanomas in Poland. Dermatologists raise the problem of increased sunlight penetration and the unreasonable use of solar radiation. This causes numerous cases of skin cancer. The aim of the article is to signal the threat posed by the depletion of the ozone layer in Poland. The article uses a research method based on an analysis of statistical data on skin cancers, supported by the available research in this area. Studies show that the incidence of skin cancer in Poland in 2007-2016 nearly doubled. Women are more prone than men to skin neoplasms. The mortality rate due to skin melanoma also increased dramatically. In the article, we point to the need of raising awareness of the hazards, and consider the coverage of treatment costs by life insurance as one of the options to protect the health of insured persons. We also raise the problem of prophylaxis on the basis of sunscreen cosmetic products.

The article is devoted to the technological and metrological features of creating a measurement standard for calibration and verification of the system of automated morphometry of skin neoplasms images in the diagnose skin melanoma. The requirements for the measurement standard has been formulated. The technological process of manufacturing the measurement standard based on thermal-transfer printing has been developed. The main technological parameters of printing has been defined: the temperature should be 180ºC, the image transfer speed should be 2.0 sec/inch. The color difference in the manufacture of the standard is not more than ΔE*ab=2.5 (CIELAB 1976), the image size tolerance is not more than 0.02%.

Introduction. Currently, numerous studies are published by authors of different countries to demonstrate the effectiveness of noninvasive methods in the diagnosis of melanoma. Methods. A systematic search was conducted independently in the databases PubMed and Cochrane Central Register of Controlled Trials (CENTRAL) until April 2020 according to certain inclusion criteria. Data extraction was carried out independently, followed by generalization using descriptive tables. Due to the heterogeneity of the included studies and the impossibility of carrying out a meta-analysis in view of this, we performed a narrative description. Results. A total of 765 potential publications for inclusion were found and checked, of which 53 were included. By design, the studies were assigned to studies of simultaneous design – 40, to randomized clinical trials – 7, to meta-analysis – 6. Data in the included publications on 76802 skin neoplasms were reported, of which 9070 were melanomas. The extracted data were summarized in descriptive tables. Conclusion. With continuing technological progress, the development of noninvasive imaging technologies in the diagnosis of skin melanoma should follow the path of cost-effective, simple and accurate diagnosis.

The problem of skin melanoma diagnostics from digital images of the tumor is considered. Clinical algorithms for detecting skin melanoma are briefly described. An overview of the works devoted to the automated assessment of the asymmetry of the distribution of shape, color, area of globules – important signs of melanoma – is given. A model for estimating the heterogeneity of the distribution of the characteristics of globules on digital images in the skin neoplasms diagnosis is developed and models of signs of heterogeneity of this distribution are proposed. The comparative evaluation of the proposed models was carried out experimentally using a software system developed in C++. The most informative features are identified. The greatest accuracy 93 % in estimating the heterogeneity of the distribution of the characteristics of globules was shown by the sign “the reduced inverse of the greatest frequency of occurrence of the measured areas of globules”. The results obtained can be applied in the development of systems to support medical decision-making in the diagnosis of melanoma.

The problem of early diagnosis of one of the most dangerous malignant neoplasms of the skin – melanoma is considered. A model for identifying the structural elements of lines in digital images of skin neoplasms in oncodermatology has been developed. The model is based on adaptive binarization of the original digital dermatoscopic image of skin neoplasms and subsequent operations of dilation, erosion, skeletonization and filtering of false fragments of lines. Test dermatoscopic images of skin neoplasms are visually divided into four groups to conduct the experiment. The optimal parameters of image processing of four groups for the model of selection of structural elements – lines – are experimentally established. The experimentally determined accuracy of the selection of lines was 95 %. The work is the result of interdisciplinary cooperation between dermatologists of the Central Medical Academy of the Presidential Administration of the Russian Federation, the Medical Institute of the Peoples' Friendship University of Russia and specialists in the field of information and measurement systems of the Engineering and Physical Institute of Biomedicine of the National Research Nuclear University “MEPhI”. The proposed model can be used in the development of computer systems to support medical decision – making in the diagnosis of skin melanoma – a dangerous malignant neoplasm.

A method for differentiation of pigmented skin neoplasms based on digital processing of optical images has been proposed. The optical images are detected using a digital camera and an etalon for its color and spatial calibration. The method implies segmentation and automatic differentiation of neoplasms based on 5 parameters –the diameter, area, color, shape and smoothness of margins of the neoplasm. It reveals clinical features of the neoplasm, required for diagnosis, and allows for calculating a probability of the neoplasms malignizaion. The proposed method has been verified using 360 imaging of pigmented neoplasms of the skin in vivo; among them: ordinary, dysplastic nevi of the skin and skin melanoma. The observed sensitivity and specificity of the proposed technique are 97% and 95%, correspondingly.

Early diagnosis of skin melanoma is one of the most reliable ways to improve the prognosis for the life of patients with this tumor. Organization of medical care for patients with a high risk of developing melanoma, together with the use of non-invasive diagnostic methods and teaching the patient the principles of prevention and early diagnosis of skin malignancies should improve the survival rates of patients with melanoma. The article discusses the experience of implementing the Organizational model of medical care for patients with skin neoplasms in Moscow, as well as the key rules for managing patients at risk of developing skin melanoma with special emphasis on recommendations to patients for regular self-examination of the skin, lifestyle correction and the use of photoprotective agents.

We studied the solar radiation effect on the morbidity of malignant skin neoplasms and mortality from them in the Irkutsk region population as one of risk factors. It is established that population’s morbidity rates of skin melanoma from 3.3 ± 0.3 to 5.0 ± 0.4 per 100 thousand and other malignant skin neoplasms from 26.0 ± 0.9 to 32.4 ± 0.9 per 100 thousand increased; the mortality rates from them became respectively from 1.6 ± 0.2 to 1.8 ± 0.2 per 100 thousand and from 0.2 ± 0.1 to 0.5 ± 0.1 per 100 thousand in the region during the 2007–2017 period. According to the results of the correlation-regression analysis we found the effect on the morbidity of malignant skin neoplasms for such parameters as direct solar radiation on a horizontal surface, total solar radiation, radiation balance and longwave radiation balance; direct and measured solar radiation influenced mortality from malignant skin neoplasms. To reduce the morbidity of malignant skin neoplasms of the Irkutsk region population, it is necessary to widely educate the population about the dangers of excessive exposure to ultraviolet radiation and inform about local levels of solar radiation according to quality monitoring data.
