CONTEXT: Anticoagulation with warfarin is considered the appropriate treatment for venous thromboembolism and other thrombotic pathologies. Regular INR control is required for dosage adjustment and therapeutic control. Use of portable monitoring systems optimizes management of these patients. OBJECTIVE: To compare INR measurements taken using the portable Coaguchek XS system in capillary blood with the standard laboratory method using venous blood. METHOD: Fifty-two samples each of venous and capillary blood were collected from nineteen patients on warfarin, who had been admitted to the Hospital da Beneficência Portuguesa de São Paulo, and analyzed using the conventional method and the Coaguchek XS system, respectively. RESULTS: Spearman's correlation coefficient ® for the overall performance of the two methods was 0.978 (p<0.0001; 95%CI 0.961-0.988). The Kappa measure of agreement for all patients was 76.8% (p<0.001; IC: 95% 0.975-0.561). Mean INR according to the Coaguchek XS system underestimated the values provided by the conventional method by -0.01 INR points, with a standard error of 0.342. Results for INR values greater than 3.5 were satisfactory with a correlation coefficient of 0.71, but without statistical significance (p>0.714). CONCLUSIONS: The Coaguchek XS system can be used to monitor prothrombin time in patients on oral anticoagulants, provided INR values greater than 3.5 are confirmed using the conventional laboratory method.

Background: The use of clozapine demands regular monitoring of clozapine plasma concentrations and of white blood cell parameters. The delay between sending blood samples for analysis and receiving the results hinders clinical care. Point-of-care testing (POCT) can provide drug assay results within a few minutes. Aim: This study aimed to investigate the utility of a novel point-of-care device that can measure clozapine concentrations using capillary blood samples collected via a finger stick. Method: During a five-week period starting in June 2019 eligible patients were asked to provide a finger-stick capillary sample in addition to their usual venous blood sample. Samples were analysed by the novel point-of-care device and by the standard laboratory method. Capillary blood samples were tested by the MyCare™ Insite POCT analyser, and a quantitative measurement of clozapine concentration was provided within six minutes. Results: A total of 309 patients agreed to measurements by the two methods. Analysis revealed clozapine concentrations in venous blood as determined by the laboratory method ranged from 20 to 1310 ng/mL and by POCT from 7 to 1425 ng/mL. There was a strong positive correlation ( R = 0.89) between the results from the venous and the capillary sample methods. The slope of the association between standard assay and MyCare™ Insite was 1.0 with an intercept of –21 ng/mL, indicating minimal bias. Conclusion: Clozapine concentrations can be accurately measured at the point of care using capillary blood samples collected via a finger stick. This approach may be more acceptable than venous sampling to patients and, with almost instant results available, more useful to clinicians.

SummaryA new simple method is described for the determination of thromboplastin time on 0.02 ml of capillary blood. It employs an automatic pipette with a disposable mouthpiece (sections of a plastic capillary tube), which is replaced by a new one after each determination. The automatic pipette may be used in the same way for pipetting the thromboplastin suspension into the test tube. The latter is the only utensil, which has to be cleaned after the determination.In comparison with other procedures of thromboplastin time determinations, the new micro-method has been found as reliable as “macro”-methods using venous blood.

Introduction: In the Russian Federation, much attention is traditionally paid to military education and training. A special place in its structure is occupied by the system of cadet classes and corps. A distinctive feature of the learning mode in such institutions is a combined effect of standard and specific factors of indoor school environment and intensive physical activity owing to sports, applied military and drill training. No evidence-based methods of establishing nutrient requirements of children in modern conditions of cadet corps have been developed so far, which predetermines the potential of transforming nutrition from a health-saving factor into a health risk factor. Our objective was to provide a scientific substantiation of the model of healthy nutrition for students of cadet-type educational establishments. Methods: The statistical significance of the correlation was evaluated using the Student’s t-test. Correlation and regression analyses were used to assess cause-and-effect relationships. The Pearson correlation coefficient (rxy) was used as an indicator of the strength of the relationship between quantitative indicators x and y, both having a normal distribution. Correlation coefficient (rxy) values were interpreted in accordance with the Chaddock scale. For the purpose of statistical modeling, the method of multiple linear regressions was used. Conclusions: We substantiated the innovative model of organizing healthy nutrition for students of cadet-type schools based on the correlation and regression analyses with determination of statistical significance of the studied characteristics. Its efficiency indicators include an increase in average functional capabilities of students by more than 10 % and a reduction in the probability of developmental disorders by more than 25 %.

Abstract Background Both capillary and venous blood samples have been interchangeably used for the diagnosis of malaria in Ethiopia. However, Plasmodium parasites are thought to be more concentrated in capillary than in venous blood. Hence, selecting a sample source where parasites are more concentrated is indispensable approach in order to maximize the accuracy of blood film microscopy. Therefore, the present study aimed to compare the detection rate and the parasitemia level of Plasmodium species from conventional capillary and venous blood films, and buffy coat preparations. Methods A facility based cross-sectional study was conducted from Feburary to March 2020 among 210 febrile patients attending Hamusite health center, northwest Ethiopia. Capillary and venous blood samples were collected and buffy coat was prepared from each sample. Thin and thick blood films were prepared, stained, and examined microscopically following standard protocol. Data were analysed using Statistical Package for Social Sciences Software version 20 and Med-Calc software version 19.3. Results Capillary blood buffy coat (61/210, 29.0%) had significantly higher detection rate as compared to capillary (48/210, 22.9%) and venous (42/210, 20.0%) blood films (p < 0.001). However, no significant difference was observed between capillary and venous blood films (p = 0.070) in detecting Plasmodium species. The highest and the lowest mean asexual stage parasite counts were found in capillary blood buffy coat (4692.88) and venous blood (631.43) films, respectively showing significant variations (p < 0.001). Mean gametocyte count was also highest in capillary blood buffy coat (3958.44). As compared to capillary blood buffy coat, the sensitivity of venous blood buffy coat, capillary blood film and venous blood film were 73.8, 78.7, 68.9%, respectively. Conclusion Capillary blood buffy coat samples showed the highest sensitivity in detecting and quantitating malaria parasites that its use should be promoted in clinical settings. However, conventional capillary and venous blood films could be used interchangeably.

The purpose of this study is to compare whether or not there is a difference between venous and capillary blood samples in blood glucose measurements and investigate the effects of different aseptic methods used in skin cleaning before collecting blood samples on measurement results. This quasi-experimental study was conducted with 109 patients. The capillary first and second blood drop values taken from the patients after fasting and at 2 hours following 75 g oral glucose tolerance test (OGTT) and capillary and venous blood glucose values were compared. There was no significant difference between the median venous blood glucose value and the capillary second blood drop value taken after wiping the finger with alcohol. There was no significant difference between the first and second blood drop values of capillary blood glucose 2 hours after OGTT.

The ‘Dextrostix’-reflectance meter system for blood glucose analysis has been evaluated using a blood glucose reference method. A high degree of concordance between the two methods was obtained when analyses were performed by skilled laboratory staff on venous blood samples containing fluoride, with a 75 s contact time. Skilled laboratory staff performed significantly better than unskilled staff. Capillary blood glucose concentrations correlated poorly with concentrations in venous blood samples taken at the same time as the capillary blood.

AbstractSeveral cytokines and adipokines are related to clinical severity and progression in knee osteoarthritis. The aim of this study was to evaluate the associations of IL-8 with clinical severity and with local and systemic adipokines and cytokines. This is a Cross-sectional study including 115 women with symptomatic primary knee osteoarthritis with ultrasound-confirmed joint effusion. Age, symptoms duration and body mass index were collected. Radiographic severity was evaluated according to Kellgren–Lawrence. Pain and disability were assessed by Lequesne and Knee injury and Osteoarthritis Outcome Score pain, symptoms and function scales. Three inflammatory markers and five adipokines were measured by ELISA in serum and synovial fluid. Partial correlation coefficient (PCC) and corresponding 95% confidence interval were used to evaluate association. Synovial fluid IL-8 was significantly associated with clinical severity scales. After controlling for potential confounders, associations measured by a Partial Correlation Coefficient (PCC) remained essentially unaltered for Lequesne (PCC = 0.237), KOOS pain (PCC = − 0.201) and KOOS symptoms (PCC = − 0.209), KOOS function (PCC = − 0.185), although the later did not reach statistical significance. Also in synovial fluid samples, associations were found between IL-8 and TNF (PCC = 0.334), IL6 (PCC = 0.461), osteopontin (PCC = 0.575), visfatin (PCC = 0.194) and resistin (PCC = 0.182), although significance was not achieved for the later after statistical control for confounders. None of these associations were detected in serum. In conclusion, IL-8 was associated with clinical severity, inflammatory markers and adipokines in synovial fluid, but not in blood. Although the reported associations are weak to moderate in magnitude, these findings reinforce the notion that local and not systemic inflammation is more relevant to clinical severity in knee OA women with joint effusion.

We introduce a new test to study the cosmological principle with galaxy clusters. Galaxy clusters exhibit a tight correlation between the luminosity and temperature of the X-ray-emitting intracluster medium. While the luminosity measurement depends on cosmological parameters through the luminosity distance, the temperature determination is cosmology-independent. We exploit this property to test the isotropy of the luminosity distance over the full extragalactic sky, through the normalization a of the LX–T scaling relation and the cosmological parameters Ωm and H0. To this end, we use two almost independent galaxy cluster samples: the ASCA Cluster Catalog (ACC) and the XMM Cluster Survey (XCS-DR1). Interestingly enough, these two samples appear to have the same pattern for a with respect to the Galactic longitude. More specifically, we identify one sky region within l ~ (−15°, 90°) (Group A) that shares very different best-fit values for the normalization of the LX–T relation for both ACC and XCS-DR1 samples. We use the Bootstrap and Jackknife methods to assess the statistical significance of these results. We find the deviation of Group A, compared to the rest of the sky in terms of a, to be ~2.7σ for ACC and ~3.1σ for XCS-DR1. This tension is not significantly relieved after excluding possible outliers and is not attributed to different redshift (z), temperature (T), or distributions of observable uncertainties. Moreover, a redshift conversion to the cosmic microwave background (CMB) frame does not have an important impact on our results. Using also the HIFLUGCS sample, we show that a possible excess of cool-core clusters in this region, is not able to explain the obtained deviations. Furthermore, we tested for a dependence of the results on supercluster environment, where the fraction of disturbed clusters might be enhanced, possibly affecting the LX–T relation. We indeed find a trend in the XCS-DR1 sample for supercluster members to be underluminous compared to field clusters. However, the fraction of supercluster members is similar in the different sky regions, so this cannot explain the observed differences, either. Constraining Ωm and H0 via the redshift evolution of LX–T and the luminosity distance via the flux–luminosity conversion, we obtain approximately the same deviation amplitudes as for a. It is interesting that the general observed behavior of Ωm for the sky regions that coincide with the CMB dipole is similar to what was found with other cosmological probes such as supernovae Ia. The reason for this behavior remains to be identified.

Background: In military environment, soldiers regularly practice or undergo different types of extreme training activities. However, globally the literatures available on the physiological and biochemical demand of different extreme military training activities are very scanty and less reported. Aims and Objective: The present study was undertaken to quantify the cardio-respiratory and biochemical responses of military training event in jungle environment. Materials and Methods: Mathew’s Mad Mile (MMM) activity is a type of specialized run of 1.5 mile in jungle environment. This training activity was conducted on rugged jungle terrain comprised of undulated uphill, downhill, muddy surface. Twenty-five SHAPE-1 healthy soldiers were volunteered into training event. Cardiorespiratory data was recorded continuously throughout the event and venous blood sample was drawn before and immediately after completion of the event. Statistical significance was considered at p<0.05. Results: There was no significant difference observed in heart rate and breathing rate while core body temperature was significantly (p = 0.02) higher in slow finisher as compared to fast finisher. In fast finisher, post exercise level of BDNF, BNP, SDH, cortisol and UCP1 increased significantly (p<0.05), whereas, BHB (p<0.01) decreased significantly in comparison to pre-exercise. In slow finishers, post exercise level of cortisol, KYNA and UCP1 increased significantly (p<0.05), whereas, BDNF, BNP and SDH decreased significantly (p<0.05), in comparison to pre-exercise. Conclusion: The outcome of this study indicated that the slow finishers were more susceptible to risk of injury due to higher exercise induced thermogenesis and mental stress in comparison to fast finisher.
