ABSTRACT Background - Several scoring was developed for evaluation of children with fecal retention using plain radiograph. There are controversies about specificity and sensitivity of these scoring system. Objectives - The aim of this study was to evaluate Barr, Blethyn, and Leech score in evaluation of fecal load in plain radiograph. Methods - This case control study was conducted on children aged 2-14 years old with abdominal pain who visited Abuzar children's Hospital of Ahvaz University of Medical Sciences. This study was conducted in fall season. Children with history of previous abdominal surgery, any systemic illness including sickle cell anemia were excluded. Children with constipation were placed in case group. Subjects without constipation were placed in control group. Subjects without exclusion criteria were examined by physician who is blind to aim of the study. Careful history and physical examination was done. Demographic features, history of gastrointestinal problem, duration of abdominal pain, defecation habit, stool consistency (loose, hard), and results of physical examination were recorded. Rome III criteria was used for definition of constipation. Abdominal x-ray was ordered for each patients. Abdominal radiography was reviewed by radiologist. Barr, Leach, and Blethyn scores were calculated for each case. Results - In this study 102 children with functional constipation and 102 children without constipation as a control were included. Mean ±SD for case and control group was 68.39±34.88 and 69.46±32.60 (P=0.82).Leech score (mean ±SD) was 11.05±2.177 and 5.67±3.228 for case and control group respectively (P<0.0001). Barr score (mean ±SD) was 14.86±3.54 and 7.16±5.59 for case and control group respectively (P=<0.0001). Blethyn (mean ±SD) score was 1.97±0.667 and 1.04±0.900 for case and control group respectively (P=0.000). Sensitivity and specificity of Barr score was 83% and 79% respectively. Sensitivity and specificity of Leech score was 92% and 80% respectively. Sensitivity and specificity of Blethyn score was 79% and 92% respectively. Conclusion - Barr, Blethyn and Leech scores were significantly higher in children with abdominal pain and constipation in contrast to children with abdominal pain and without constipation. Sensitivity of Leech score was more than Barr and Blethyn scoring systems. Specificity of Blethyn score was more than Barr and Leech score.

Introduction: The prevalence of multiple symptoms, including mental illness, symptoms that mimic diseases such as pancreatitis, nonspecific abdominal pain and cholecystitis, are high in drug-dependent patients. Objectives: The aim of this study was to consider the clinical signs and serum lead levels of individuals referred to four university hospitals with various complaints. Patients and Methods: In this study, 128 sample cases in two groups, the case group and control group, have been examined. The case group included 64 persons using oral opium who were suffering from various complaints and referred to emergency department of mentioned hospitals. The control group included 64 patients without any history of addiction. Results: Mean of blood serum lead level in opium users and control group was 76.34±17.82 mg/dL and 7.68±3.72 mg/dL respectively, and the difference between these groups was significantly meaningful (P<0.001). The most prevalent complaints were abdominal pain and pulmonary complaints. Conclusion: Findings of the current study showed high levels for an average serum lead level for oral opium addicts among the studied statistical population. In addition, results show that ultimate diagnosis of lead poisoning is a factor justifying symptoms for addicts with nonspecific symptoms, the subject that confirms several previous reports. This finding asserts the necessity of screening of serum lead level for patients addicted to opium compounds in order to prevent more severe side effects.

BACKGROUND: Alopecia areata is a non-scarring hair loss, which typically starts quickly. Atopy is one of the possible predisposing risk factors for this condition.AIM: This study aimed to evaluate the prevalence of thyroid disease, atopic dermatitis and allergic diseases in children with alopecia areata and compare the results with healthy individuals.METHODS: This case-control study was conducted on 50 patients with alopecia areata, diagnosed by a dermatologist, and 150 healthy individuals as the control group. Participants filled the questionnaires, and necessary tests were performed.RESULTS: In this study, the mean age of the participants was 2.55 ± 14.26 and 3.19 ± 11.92 in the case and control groups, respectively. Prevalence of asthma was 22% in the case group and 12.5% in control group (P = 0.109). Also, allergic rhinitis and eczema were observed in 20% and 22% of the subjects of the case group, whereas they were reported to be 8% and 10% in the control group (PV = 0.03 and 0.175, respectively). Moreover, 28% and 8% of the participants in the case and control groups had a family history of atopy and allergic disorders, respectively (P = 0.046). A significant difference was observed between the two groups regarding gender, type of delivery and contact with animals.CONCLUSIONS: According to the results of this study, a significant association was observed between the prevalence of alopecia areata and atopic conditions, such as allergic rhinitis and history of atopic dermatitis.

Background Bronchiolitis peak incidence is in children aged 2 -6months. History of atopy in parents, non-exclusive breastfeeding,exposure to cigarette smoke, and infants living in crowded areasmay be risk factors for bronchiolitis. Gestational of age at birth isalso influences the mortality oflower respiratory tract infection.Objective To evaluate the following conditions as possiblerisk factors for bronchiolitis: history of atopy, non-exclusivebreastfeeding, preterm infants, exposure to cigarette smoke, and2:: 6 persons residing in the home.Methods A sex-matched case-control study was conductedby collecting data from medical records at Sanglah Hospital,Denpasar. The case group subjects met the diagnostic criteriafor bronchiolitis and were aged 1-24 months. The control groupincluded patients with diagnoses unrelated to the respiratorysystem. Data was analyzed using bivariate (Mc.N emar) andmultivariate methods (logistic regression) with 95% confidenceintervals and statistical significance value of P <0 .05.Results There were 96 subjects in our study, consisted of 48subjects in the case group and 48 in the control group. Thecase and control groups were similar in baseline characteristics.The presence of history of atopy (OR 34.7; 95%CI 3 to 367,P=0.003), non-exclusive breastfeeding (OR 4.3; 95%CI 1.4 to13, P=0.010), exposure to cigarette smoke (OR 3; 95%CI 1 to9.2, P=0.047), and 2:: 6 persons living in the home (OR 7.9;95%CI 2.6 to 24, P<0.0001) were found to be significant riskfactors for bronchiolitis, while the preterm infants seem notsignificant as a risk factor of bronchiolitis (OR3; 95%CI 0.31 to78.99, P=0.625).Conclusion History of atopy, non-exclusive breastfeeding,exposure to cigarette smoke, and 2:: 6 persons living in the homeare found to be risk factors, while preterm infants seem not a riskfactor for bronchiolitis.

Background: Psoriasis is multifactorial in origin. Streptococcal infection, sore throat has been claimed to be an  infectious  cause.Objective: The  purpose  of  the  present  study  was  to  observe  the  association  of Streptococcus with guttate psoriasis.Methodology: This was an observational case-control study which was conducted in the Department of Dermatology & Venereology at Bangabandhu Sheikh Mujib Medical University (BSMMU), & Shaheed Suhrawardy Medical College hospital (ShSMCH) in Dhaka city of Bangladesh during the period of February 2012  to October 2012. Patients clinically and histopathologically diagnosed as psoriasis were selected as case group and patients with skin diseases other than psoriasis were selected as control group.Results: A total of 115 patients were included in this study of which 64 patients were in case group and the rest 51 patients were designated as control group. The mean age was 30.73±14.69 years and 26.47±12.64 years in cases and control groups respectively (p>0.05). About 66.6% patients gave a positive history of sore throat. ASO titer was raised (>200 IU/ml) in 25(83.3%) patients of guttate psoriasis and 5(23.8%) in patients of their non-psoriatic controls (p<0.05). Streptococcus pyogenes was found in 9(30.0%) patients of case group and only 2(9.5%) patients were found in their control group.Conclusion: Streptococcal throat infections are associated with guttate psoriasis patients.DOI: http://dx.doi.org/10.3329/jssmc.v5i2.20763J Shaheed Suhrawardy Med Coll, 2013;5(2):91-94


Introduction: Benign Paroxysmal Positional Vertigo (BPPV) is the most common vestibular disorder. Migren also common in general population. Several studies have shown inconsistent result for the relationship between migraine and BPPV.This study aims to determine relationship between migraine and the occurance of BPPV Methode: This case control study selected subjects with consecutive sampling techniques. Sampling was conducted at H. Adam Malik General Hospital Medan and network hospital. Thirty two patients with BPPV were selected as case group and matched with thirty two patients without BPPV as control group. Migraine history was taken in medical record. This study began in March until June 2020. Result: The majority of case and control group were female with eighteen subjects (56.2%) and median age about 60.5 (27-78) years old. We found eight patients (25.0%) with history of migraine in case group and in control group four patients (12,5%) with history of migraine. There is no significance relationship in migraine and the occurance of BPPV with p= 0.33 and OR=2.33 (95% CI = 0.62-8.71) Conclusion: There is no relationship between migraine and the occurance of BPPV.

Introduction: The diagnosis of acute appendicitis is generally clinical and once it is diagnosed, operative management ensues. Abdominal pain is the main presenting complaint of patients with acute appendicitis. The diagnostic sequence of colicky central abdominal pain which is followed by vomiting with migration of the pain to the right iliac fossa. The site of maximal tenderness is often said to be over McBurney's point. Specialist investigations are rarely needed to confirm a diagnosis of acute appendicitis, and the diagnosis is predominantly a clinical one. Judicious use of urine and blood tests, for inflammatory response variables, allow exclusion of other pathologies and provide additional evidence to support a clinical diagnosis of appendicitis. Material and Methods: Every patient with acute onset of right lower quadrant abdominal pain and without previous history of appendectomy was considered as suspected of having acute appendicitis. Every patient with right iliac fossa pain and without history of appendectomy is suspected of having appendicitis until proven otherwise were included in the study. All Clinical and laboratory tests relevant to acute appendicitis were done among study participants. In Clinical parameters appetite, diarrhea, dysuria, vomiting, signs of localized peritonitis i.e. rebound tenderness and or guarding and pain migration was observed.in laboratory parameters, CRP, complete blood count (CBC) were measured and analysed. Patient’s demographic characteristics were noted. Results: A total of 100 patients were included in the study of which 50 were included in the control and 50 in case group. Mean age in control group was 29±7.23 years while in acute appendicetomy group was 29 ±8.77 years. In control group 44% were male and 56 % were female while in acute appendicitis 42% were male and 58 % were female. All clinical presentations were statistically significant in case and control group. CRP was positive in 8 (16%) in control group while in Acute appendicitis group it was 21 (42%)  (P=0.0071). Conclusion: CRP did not contribute to the overall diagnostic accuracy. The successful diagnosis of acute appendicitis can be done through proper clinical examination and adequate evaluation of laboratory parameters Keywords: Acute Appendicitis, CRP, appendicetomy

Angka Kematian Ibu (AKI) di Kabupaten Pati bergerak fluktuatif dan menunjukkan status tertinggi pada tahun 2011 dengan 126 per 100.000 kelahiran hidup. Tujuan penelitian ini adalah menggambarkan kematian ibu di Kabupaten Pati dan menganalisis faktor risiko kematian maternal di Kabupaten Pati Tahun 2011. Penelitian menggunakan pendekatan observasional analitik dengan metode kasus kontrol. Jumlah sampel adalah 24 untuk setiap kelompok kasus dan kelompok kontrol. Analisis data menggunakan uji univariat, bivariat dan multivariat. Hasil dari penelitian adalah tiga penyebab utama kematian ibu di Kabupaten Pati adalah penyakit jantung, preeklamsi/eklamsi, dan perdarahan. Kematian ibu tersebar di 16 kecamatan dari 21 kecamatan yang ada dan sebagian besar kematian terjadi pada masa nifas. Analisis regresi logistik menunjukkan bahwa faktor yang berpengaruh terhadap kematian ibu adalah komplikasi kehamilan (OR = 12,198, nilai p = 0,010), komplikasi persalinan (OR = 9,94, nilai p = 0,020) dan riwayat penyakit (OR = 27,735, nilai p = 0,011). Secara bersama-sama, ketiga variabel tersebut berkontribusi terhadap 64,3% kematian ibu yang terjadi di Kabupaten Pati Tahun 2011.Maternal Mortality Ratio (MMR) of Pati Regency fluctuated and showed the highest state in 2011 with 126 per 100.000 live births. This research aimed to describe maternal mortality cases in Pati Regency and to analyze risk factors of maternal mortality happened in 2011. This research used observational analysis, i.e. cases control study. The number of samples was 24 in each of case group and control group. Data was analyzed with univariate, bivariate, and multivariate. This research resulted that three major causes of maternal mortality were heart disease, preeclampsia/eclampsia, and hemorrhage. Maternal mortality spread on 16 from 21 sub-districts on Pati Regency and the majority of them happened in post delivery period. Logistic regression analysis concluded that risk factors influenced maternal mortality were pregnancy complication (OR = 12.198, p value = 0.010), delivery complication (OR = 9.94, p value = 0.020) and history of illness (OR = 27.735, p value = 0.011). Collectively, those variables contributed to 64.3% of maternal mortality in Pati Regency 2011.

Objective: Though preeclampsia (PE) is an age-old disease, its pathology still remains obscure. Available epidemiological evidences suggest that PE is a disease of multiple theories. Recently serum homocystine level is considered as factor to be associated with preecampsia and the higher the level the greater is the severity of preeclampsia. The present study is aimed at investigating this hypothesized association. Methodology: This case-control study was conducted over a period of 24 months from January 2012 to December 2013 in the Department of Obstetrics & Gynaecology, Dhaka Medical College, Dhaka. Pregnant women admitted with severe preeclampsia were the case, while pregnant women attending at the antenatal clinic without preeclampsia were the controls. Severe preeclampsia was diagnosed by blood pressure > 160/110 mm of Hg with proteinuria 3+ or more in dipstick test. The control group comprised of women of 24 - 40 weeks gestation, with blood pressure (both diastolic and systolic) remaining within normal limits without having any medical or obstetric complications. Data were analysed using software SPSS (Statistical Package for Social Sciences) version 16.0. The test statistics used to analyse the data were descriptive statistics, Chi-square (χ2) Test, Student’s t-Test and Receiver-operating characteristic (ROC) curve analysis. Result: Around two-thirds of the patients in both groups were in the age range of 21-30 years with mean age of the cases and control being 25.8 ± 5.2 and 24.1 ± 3.7 years respectively (p = 0.108). Over three-quarters (77.5%) of the patients in case group and 60.0% in control group belonged to middle class (p = 0.091). Majorities of the cases (85.0%) and controls (90.0%) were preterm (gestational age < 37 weeks) with mean gestational age being 33.2 ± 3.3 and 32.3 ± 3.5 weeks in case and control groups respectively (p > 0.05). The patients in either group were predominantly primigravida. Seven (17.5%) patients in the case group gave the history of past preeclampsia as opposed to none in the control group (p = 0.005). Family history of preeclampsia was reported by the case group alone (p = 0.027). Majority (95.0%) of the cases had 3+ proteinuria. Serum homocystine concentration was significantly raised (15.7 ± 8.3 μmol/L) in case group than that in the control group (6.7 ± 1.3 μmol/L) (p < 0.001). Based on the receiver-operator characteristic (ROC) curve, serum homocysteine levels in pregnant women had the best area under the curve (0.975 or 97.5%) with sensitivity and specificity of the predictor variable being 92.5 and 77.5% respectively. Conclusion: The study concluded that homocysteine levels are significantly elevated in patients with preeclampsia compared to the pregnant women without preeclampsia. Homocysteine may be of value in the monitoring of pregnancies to be complicated by preeclampsia. Ibrahim Card Med J 2015; 5 (1&2): 54-60

In recent years, the prevalence of asthma and allergic rhinitis has increased in developed countries. Helicobacter pylori (H. pylori) infection can exacerbate asthma. The purpose of this study was to investigate the relationship between asthma and H. pylori seroprevalence in children. In this cross-sectional study, 100 children aged 5-15 years hospitalized in Besat hospital in Sanandaj were investigated from 2015 to 2016. Fifty children with asthma were considered as the case group and 50 non-asthmatic children as the control group. The questionnaires were completed, including demographic information, history of asthma, exposure to cigarette smoke, and family history of gastric and duodenal ulcers. Blood samples were collected from the children, and the serum level of specific antibodies (IgG) of H. pylori was measured. There were 42 and 31 boys in the case and control group, respectively. The mean age in the case group was 8.12±2.29 and in the control group was 8.9±2.52 years. In the case group, 48% were exposed to cigarette smoke and in the control group, 18%. There was a statistically significant difference between the groups in terms of gender and exposure to cigarette smoke (P=0.001 and P=0.013, respectively). There was no significant difference between the case and control groups in terms of H. pylori seroprevalence. (P=0.211). There was no significant association between asthma and H. pylori seroprevalence. (P=0.22). Bronchial Asthma generally starts from childhood which is condition where respiration channel experiences constriction because of hyperactivity with some specific stimulation which cause inflammation. Some risk factors are smoking exposure of cigarette smoke, weather changes, mite on house dirt, pet and history of family sickness. The purpose of this research is to know Relation of mite on house dirt, exposure of cigarette smoke  and history of family sickness with bronchial asthma incident to child 5 – 10  years old on working area of Puskesmas Lempake Samarinda City in 2016. Method which used was analytic survey with Case Control approaching. The total sample was 36 children which consisted of 18 case group and 18 control group with matching by using age and gender which submitted with Purposive Sampling technique. Data Analysis used Chi Square with wrong degree α = 0,005. The result showed that there was relation of mite of house dirt (p = 0,006), history of family sickness (p = 0,001) and no relation with exposure of cigarette smoke (p = 0,370) with bronchial asthma incident to child 5 – 10 years old on working area of Puskesmas Lempake Samarinda City in 2016.
