Small and medium-sized enterprises (SME) play an important role in the economic mechanisms of many countries. The article considers the main parameters of the situation that developed in Russia at the beginning of the implementation of the national project (NP). The purpose of the study is to assess the relevance of this project for the country and the validity of its indicators, taking into account the specifics of financingin the context of managing the social and economic systems of Russia. The analysis of the published research results on this topic in economic and instrumental aspects is carried out. The article analyzes the representation of small (including micro) and medium-sized enterprises in the main sectors of the national economy, identifies more «successful» sectors in this regard. The regional (by subjects of the Russian Federation) structure of such enterprises in the country is estimated on the basis of frequen-cy distributions, and quantitative estimates of their main parameters are made. The dynamics of general and state budget financing of NP in comparison with the number of employees in this area is considered. The change in the cost of creating one new workplace and one SME subject within the NP over time is analyzed; the dynamics of the number of employees per one SME subject is estimated. The structure of the distribution of financial resources of the NP between its constituent federal projects is studied. It is concluded that the national project is relevant for the country and its regions, and it is advisable to continue improving market management mechanisms and management for the successful development of SME. Proposals are formulated to supplement the list of NP indicators and to account for inflation, as well as to reduce discrepancies in information about the SME sector of the Russian Federation according to different sources.

The article considers the concept of “digital economy” and its relationship with the concept of “knowledge economy”. It is concluded that the digital economy is officially considered the previous stage before the development of the knowledge society (knowledge economy). A review of the opinions of specialists from different fields of knowledge about the positive and negative effects, as well as the possible risks of the development of the digital economy, is presented. It is proved that despite the objectiveness of the process of moving towards the information society, and further towards the knowledge society, humanity must make efforts that this path will not be destructive for its previous achievements in various fields. From the perspective of system economic theory, an analysis of the system effects and risks of the development of the digital economy in Russia is made. For this purpose, an assessment of the system balance of the distribution of funds in the national project “National Program “Digital Economy of the Russian Federation” is performed. The results of federal projects are classified by system types and the system balance indices are calculated for assessment the financing volumes of the four basic types of economic systems (object, environment, process, and project). The obtained values of the indices of system balance of distribution of funds of the national project “National Program “Digital Economy of the Russian Federation” showed the presence of significant imbalances in the volume of financing between systems of various types. It was revealed that priority support was given to the systems of environment and process types responsible for the development of infrastructure and protocols for collecting, processing, storing and disseminating data. This circumstance should be taken into account in the formation of the country’s economic policy since both the economic systems of the four basic types and the conditions and factors of economic growth should develop proportionally. Application of the presented approach to the development of the digital economy in Russia will make it possible to efficiently distribute financial resources between subsystems, to avoid both duplication of functions by systems of various levels and excessive strengthening or weakening of one or several types of systems.

We analyzed the data of the Federal Information Fund for Social and Hygienic Monitoring on the obesity-associated morbidity of the population of the Russian Federation during the 2013-2017 period(children, adolescents, adults).Data assessment was carried out by the Federal Hygienic and Epidemiological Center of Rospotrebnadzor. About 40 subjects of the Russian Federation are included into the «risk areas» due to obesity-associated morbidity. An increase in the obesity-associated morbidity in adolescents by 1.3 times was noted, an increase in this indicator is predicted in this age category of the population, and the obesity-associated morbidity among children including aged 0 to 14 years with the first-ever diagnosis decreased by 2% in 2017. The implementation of the main directions of the national project «Demography» will allow us to determine the list of basic indicators for determining the dependence of the influence of adverse factors on the development of alimentary-dependent diseases, as well as form analytical and information panels of the Federal Information Fund for Social and Hygienic Monitoring on this issue.

At the present time, the Government of the Russian Federation is engaged in the development and preparation of a number of national projects aimed at accelerating the social and economic development of the country. In the course of this undertaking, the Government should pay special attention to the technology involved in the projects, which contributes to the relevance of the study. The study aims is to improve the methodology of project technology as a part of public management in the aspect of the implementation of national projects. The main tasks within the study were: reviewing the organizational and management structures of a national project, clarification of the definition of a national project from the perspective of project management. In order to solve the problems stated in the study, empirical and theoretical methods were used, including an analysis of the national regulatory framework and methodology in the field of project management; systematization of organizational and management structures; comparison of project management models for the implementation of comprehensive planning documents as adopted in the Russian Federation. As a result, we have identified a number of features and problems arising in the process of integration of projects at the regional and municipal level, elaborated the concept of a national project, and proposed to introduce it to the Russian legislation.

This article lists regulatory and legal documents regulating paid veterinary services Governmental, regional. Recommended list of paid and free veterinary services. The objects of state veterinary supervision, methods and techniques are listed. An analysis of extrabudgetary revenues from the provision of paid veterinary services by specialists of the Irkutsk region based on the results of the activities of 15 regional state budget institutions is presented. The analysis shows that 35.5% of the provision of paid veterinary services was for personnel. Table 2 shows the use of extrabudgetary revenues of state veterinary institutions of the Irkutsk region from the provision of paid veterinary services. An analysis of the financial activities of animal disease control stations in the Irkutsk region shows that 56.3% of extrabudgetary funding is due to the provision of paid veterinary services. Due to special climatic conditions, it is planned to increase the cost of paid veterinary services by 50% for cities and areas equated to the regions of the Far North. The cost of services provided at night, increased by 50%. The list of prices is supplemented and expanded by types of services not taken into account in previously valid price lists. The price list is revised with the introduction of VAT in the amount of 20%. The Ministry of Agriculture of the Russian Federation, in agreement with the Ministry of Economy and Finance of the Russian Federation on January 28, 1992, approved a list of paid and free veterinary services. The free veterinary services performed at the expense of the budget (vaccination, diagnosis, treatment) include events for especially dangerous diseases of animals, birds, fish. The provision of paid veterinary services is regulated by the Rules for the provision of paid veterinary services, approved by Decree of the Government of Russia dated August 06, 1998 No. 898. Paid veterinary services are provided to productive and unproductive animals in farms and enterprises, regardless of subordination and forms of ownership. In total, prices have been developed for 2186 different types of paid veterinary services.

At present, the problem of employment of the population still occupies one of the first places in the social policy and economy of both individual constituent entities of the Russian Federation and the country as a whole. The scientific novelty of the work is associated with the study of an urgent and requiring constant improvement of the development of the problem and methodological foundations of unemployment and employment in relation to the Russian Federation and the innovative economy. The article offers recommendations for improving the regulation of employment and the labor market at the appropriate levels. The conclusion is made that only well-coordinated work of all parties of social partnership will make it possible to successfully solve problems in the sphere of labor and employment of the population, thereby ensuring social stability in the constituent entities of the Russian Federation and in Russia as a whole.

Generalized forms of invasive meningococcal desease (IMD) are very dangerous because they have a high mortality rate.The aim of the work was to assess the cost-effectiveness of meningococcal vaccination of infants with the 4-valent MenACWY-D conjugate vaccine in the Russian Federation.Material and methods. Cost-effectiveness analysis based on epidemiological data for the Russian Federation was carried out by a modeling method with a horizon of 80 years from the position of the healthcare system and taking into account the social perspective. Vaccination costs were calculated on the basis of the registered price of the vaccine, including VAT, the costs of GFMI therapy and patient rehabilitation – based on the compulsory medical insurance tariffs in St. Petersburg for 2021. Indirect costs due to temporary disability of patients’ parents, disability of patients and premature death were estimated by the lost GDP. In the base case, costs were discounted by 3,5% per year, life expectancy – by 1,5% per year.Results. Given the assumptions made, the average lifetime burden of a child’s illness is 17,556 million rubles. (no discounting). In the base case, the incremental cost per LYG from the payer perspective – 7,821 million rubles, and from the social perspective – 3,328 million rubles. Incremental cost per QALY – 5,350 million rubles and 2,277 million rubles, respectively. The most important factors that have a significant impact on the cost-effectiveness of vaccination are the incidence of IMD, the price of the vaccine, and the value of the discounting of costs and life expectancy.Conclusions. Given the assumptions made, meningococcal vaccination of infants with 4-valent meningococcal vaccine can be considered as a viable option.

Purpose of the study. Healthcare budget impact analysis in the Russian Federation of providing adult patients with chronic obstructive pulmonary disease (COPD) with basic therapy for outpatient treatment. Material and methods. The hypothesis of reducing costs for the treatment of exacerbations of COPD was tested while providing all patients with basic therapy on an outpatient basis. The model calculates the direct medical costs of drug provision and treatment of exacerbations in patients with COPD in the current situation and when all patients with COPD are covered with basic therapy as part of a program of preferential drug provision. The simulation period is 1 year. The analysis does not take into account social losses associated with disability, payment of disability certificates, reduced labor productivity and mortality. Results. The cost of providing all patients with COPD with the recommended basic drug therapy on an outpatient basis is substantially offset by a decrease in the cost of treating exacerbations: drug costs should increase by 14.1 billion rubles compared with the current situation, while the cost of providing medical care to patients with exacerbations is reduced by 9.7 billion rubles. As a result, the difference in direct medical costs, or the necessary additional investments, is 4.4 billion rubles. The price of drugs used for basic therapy has the greatest impact on the size of the required additional investments. Conclusion. Providing all patients with COPD with the recommended basic drug therapy on an outpatient basis, we can expect a decrease in the number of moderate exacerbations per year by 14.7% and severe exacerbations by 31.2%. Taking into account not only medical, but also socio-economic costs can not only offset additional investments, but also demonstrate significant savings. When implementing a pilot project of preferential drug provision for patients with COPD in a specific region of the Russian Federation, it is possible to reduce the cost of purchasing medicines when forming a municipal order.

The article presents main properties of the federal projects of the National project «Health care», as well as the main activities of those projects, in which Roszdravnadzor has a particular interest. The article depicts the control and monitoring system applied by the Department of state control of implementation of state healthcare programs of Rosdravnadzor, which allows to detect in real time the subjects of the Russian Federation that have the highest risk of falling short of the targeted indicator of the regional projects and to take this information into account during the development of the plan for the control and surveillance activities.
