Aim. To study the opinion of mothers of newborns and neonatologists about the organization of inpatient care for newborns in St. Petersburg. Methods. An anonymous survey of 2123 mothers of newborns and 172 neonatologists of St. Petersburg was conducted. The representativeness of the sample was checked by using the method of Professor A.M. Merkov, with the errors do not exceed 4 and 3%, respectively. Mathematical processing and data analysis were carried out by using the statistical software package PASW STATISTICS. Results. 28.9% of mothers were partially or completely unsatisfied with the organization of inpatient care for newborns, in most cases due to the unfriendly and inattentive attitude by the medical staff (22.4%), the unsatisfactory provision by child care products (12.8%) and poor material and technical equipment of a medical organization (12.5%). On average, the human qualities of neonatologists were rated quite high by mothers at 4.630.09 (out of 5) points, and in mid-level practitioners were significantly lower 3.910.08 points (p 0.05). In general, staying a newborn in a medical organization required financial costs for 37.3% of women, and 13.9% of respondents who received help through compulsory medical insurance paid for individual laboratory and other tests on their own. Mothers rated the organization of inpatient care for newborns on average 4.010.09 points. Compared to mothers, neonatologists rated it higher in the medical organization they work for and lower overall in St. Petersburg (M=4.110.10 points and M=3.780.12 points; p 0.05). According to the doctors, the main disadvantages of organizing inpatient care for newborns are poor material and technical equipment and problems associated with the staffing and qualifications of medical personnel. Conclusion. There is a need to improve the process of providing inpatient care to newborns.

Bevezetés: A neonatális szepszis mortalitása magas, és koraszülöttek esetén a szepszis rizikója emelkedik a születési súly csökkenésével. Anyag és módszer: A szerzők gyorsteszt segítségével 12 érett újszülött és 60 igen kis súlyú (VLBW) koraszülött interleukin-6-vérszintjét határozták meg a C-reaktív protein vizsgálatával egyidejűleg, akiket a Semmelweis Egyetem I. Számú Szülészeti és Nőgyógyászati Klinikájának Neonatális Intenzív Centrumában kezeltek 2005-ben és 2006-ban. A szérum-IL-6 meghatározása gyorsteszt segítségével történt (Milenia Quickline IL-6, PicoScan system). Eredmények: Vizsgálataikban a szérum-interleukin-6- és CRP-meghatározás korrelált. A szérum-IL-6-meghatározás szenzitivitása a CRP-értékkel együtt az általuk vizsgált betegcsoportban 100% volt, álnegatív eset nem fordult elő. A vizsgálat pozitív prediktív értéke 93%. Adataikat Mann–Whitney-teszt segítségével analizálták (SPSS Statistical Software Package, Chicago, IL, USA). Szignifikáns különbség volt az igazolt szepszis és az infekciós betegcsoport IL-6-értékei között ( p = 0,048), valamint az infekciós és nem infekciós csoport interleukin-6-értékei között ( p < 0,005). Következtetések: Az igen kis súlyúak nem specifikus infekciós tünetekhez hasonló reakcióit értékelve, a szeptikus és nem infekciós esetek elkülönítése a neonatális morbiditás és mortalitás csökkentése érdekében is fontos szempont, a költségek csökkentésével egyidejűleg.

AbstractObjectivesThe mortality rate associated with community-acquired pneumonia is still considered high as it occupies a leading position among all infectious diseases. The primary purpose of this study is to evaluate the implementation of the respiratory bacterial culture sampling in the clinical practice of the Russian Federation’s therapeutic departments and its impact on inpatient care.MethodsThe study included 1,344 patients with community-acquired pneumonia. The analysis of the obtained data was performed using the statistical software package SPSS 23.0 (IBM SPSS Statistics). The description of quantitative indicators was carried out with an indication of the median (25; 75 percentile). Qualitative with an indication of the absolute and relative frequencies n (%).ResultsIt was noted that a third of the sample did not pass through the first stage (bacterioscopy). We noticed the predominance of oropharyngeal flora in the results. The sputum collection was carried out on average 3 (1; 5) days after the start of hospitalization, while antibiotic therapy was prescribed to all patients on the first day, the results of the bacteriological study were ready on average 6 (4; 8th) calendar days after hospitalization, while in 20 (1.5%) cases, the patient was already discharged. Due to the inefficiency of the starting regimen, antimicrobial agents have changed in 84 (6.3%) patients and averaged 5.5 (4.0; 10.0) days from the start of hospitalization.ConclusionsThe violation of the technique and the time of sputum collection resulted in a reduction in the method’s value.

The main purpose of this study is to examine the effect of corporate social responsibility on the organizational performance of the Dangote cement factory in Ethiopia. In order to achieve the research objectives, the authors adopted both a descriptive and explanatory research design. Structured questionnaires were used to gather data from the targeted group of the study, where the probability and non-probability sampling technique was employed to draw a sample of 179. The collected data were presented and analyzed using the SPSS (version 20) statistical software package. A multiple linear regression model was applied to see the effect of CSR on organizational performance. The study showed that philanthropic activities, ethical activities, and legal activities affect organizational performance, have a positive relationship, and are statistically significant at a 1% confidence interval. It was concluded that for the existence of business industries, the community relation is key to ensuring a conducive atmosphere for the business to succeed.

The purpose of the study is to improve the adaptation system for personnel in a multidisciplinary hospital. Results: the problems of organizing the adaptation process were identified, the methods of analysis and assessment of the management of the adaptation process in a medical organization were adapted, the effectiveness of the existing system of adaptation of medical workers in the organization was assessed. Conclusion: the study proved the need for a more complete and high-quality adaptation process; the process is not static, a creative approach is needed, taking into account the characteristics of the organization (team) and the individual abilities of a specialist; to improve the organization, for the effective adaptation of medical personnel, it is necessary to develop the institution of mentoring.

Aim. To assess the correspondence between the results of the European Social Survey (ESS) “The Impact of Medical Activity Bureaucratization on Professional Values of Personnel” and the results of sociological surveys of personnel of medical organizations such as Federal State Budgetary Institution Research Institute for Complex Issues of Cardiovascular Diseases (NII KPSSZ) and Federal State Budgetary Institution “Kuzbass Clinical Cardiology Dispensary named after academician L.S. Barbarash” (GBUZ “KKKD”).Methods. Comparative analysis of the results of ESS (3271 medical workers from 29 countries were questioned) and the results of sociological surveys of the medical personnel at NII KPSSZ (204 respondents in 2017) and at GBUZ “KKKD” (385 respondents in 2021) was carried out.Results. According to the survey data of employees of NII KPSSZ the dominant values are: professionalism and conscientious attitude to work, respectful attitude to colleagues and patients, strict compliance with law and regulatory requirements. Among the main values of GBUZ “KKKD” staff the following were named: discipline and order in fulfilling responsibilities, professionalism and conscientious attitude to work, and respectful attitude to patients.Conclusion. On the basis of the comparative analysis, the authors proved the validity of the conclusions and results of an international study devoted to the problem of the influence of bureaucratization of a medical organization on the structure of personnel values in medical organizations focused only on the treatment and diagnostic process. If an institution functions as a scientific and medical organization, then its employees tend to strive for professional self-development, innovative and creative thinking, which is not consistent with the conclusions of international researchers.

The article discusses the particular conditions of holding medical organizations accountable for causing harm to the health of patients. The conditions of civil liability, as well as the degree of responsibility of a medical organization in the provision of medical services, are analyzed. The authors are of the opinion that medical care should be organized in accordance with the procedures, conditions and standards for the provision of such care, however, the standards cannot cover all the options that may arise during the provision of medical care, therefore they are aimed at creating an average “sample”, to determine the approximate order of possible actions carried out by medical personnel. Attention is also paid to the consideration of the features of causing harm to the patient’s health, depending on whether the harm was caused in the provision of medical care or medical services. In conclusion, it was concluded that the conditions for bringing medical organizations to justice should include: unlawful action (inaction), harm, a causal relationship between unlawful action and harm, as well as the fault of the injurer.

Aim. Evaluation of the awareness of health workers of vaccine prevention at the present stage. Methods. To achieve the aim, survey of health workers of the Republic of Tatarstan was conducted using the developed questionnaire. The coverage of survey was 3 250 people. Statistical processing of the results was carried out using Microsoft Excel 2010 programs. Results. The conducted study revealed that a significant proportion (95 %) of health workers were positive about vaccine prevention, were vaccinated themselves and vaccinated their children. The most prevalent forms of informing parents of vaccinated children about adverse reactions after vaccination are oral recommendations by the doctor and information sheets about adverse reactions. The majority of interviewed health workers (89.9 %) support the initiative to cancel the right of citizens to refuse vaccination. In the opinion of health workers, the main components of successful organization of vaccine prevention in the medical organization are availability and diversity of vaccines, the level of qualification of medical personnel, and equipment of the vaccination room. Less than half of health workers were trained in specialized training on immunoprevention. The majority of health workers (86.3 %) consider it necessary to expand the national schedule of preventive vaccinations of the Russian Federation. Conclusion. Organizational measures to inform health workers in various areas of vaccine prevention are needed to be taken; promising directions are organizing specialized courses on immunoprevention and discussion of relevant aspects of immunization of the population and adverse reactions in post-vaccination period.

The article examines the trends in the development of the medical services market in the Republic of Crimea for the period 2015–2020. The analysis of the number of medical organizations by form of ownership, size and types of activity, capacity and technical equipment, provision of medical personnel, the volume of medical services in the context of public and commercial segments was carried out. In the period under study, there was an increase in the medical services market, due to an increase in the number of private medical organizations, the volume of mandatory medical insurance funds under the basic program. The expansion of the base and capacity of the sanatorium-resort complex, the growth of domestic medical tourism, laboratory services and telemedicine were identified as promising areas of market development.

Currently, work in any medical organization carries a risk of coronavirus infection, and, first of all, this applies to medical organizations dealing with the treatment of patients infected with coronavirus. Medical workers are a group at increased risk of infection with the SARS-CoV-2 virus in the provision of medical care in modern conditions, which determines their incidence of COVID-19 [1-3]. The aim of the study is to explore the prevalence of cases of COVID-19 infection in medical workers providing medical care on an outpatient basis. Materials and methods. The analysis of the incidence of COVID-19 medical workers in one of the large medical organizations of the Samara region during the period of the pandemic was carried out. The medical staff of the medical organization includes 207 doctors. There are 11 subdivisions in the structure of the medical and prophylactic institution. Research methods: content analysis of the modern regulatory framework (2020-2021), statistical, expert. For the period from May 2020 to March 2021 71 cases of infection of medical workers on an outpatient basis with SARS-CoV-2 were identified, of which 32 doctors (45%) and 39 people (55%) nurses. Based on the results of the expert assessment, the insurance nature of the infection case was established in 50 people (70%). Among the medical and nursing categories of medical workers, the largest number of cases was made by doctors and nurses of primary contact - specialists of the district service: general practitioners (40%), pediatricians (32%), doctors - obstetricians-gynecologists (12%), nurses adults (76%) and children (20%) polyclinic departments. A third of health workers received inpatient treatment for severe COVID-19, an average of 60 days. Conclusion. Medical workers are a risk group of COVID-19 who need support measures, since the possibility of occupational infection has been sufficiently proven, and the risk probably depends on the work performed and the conditions of direct contact of medical personnel, which requires further study in the current persisting epidemic conditions.

At present, the quality and availability of medical care are the aspects to which close attention is paid in the system of organizing medical care. One of the ways to achieve a high level of quality and accessibility of medical care is building up human resources, i.e. the presence of highly qualified general practitioners, sub-specialties and middle medical personnel in sufficient numbers in a medical organization. Currently, the oncological service of Russia has been assigned tasks to fulfill the instructions from the State Program for the Development of Health Care. The results of the work should lead to a decrease in mortality and an increase in the quality of life of the population. To address the issue of reducing mortality from malignant neoplasms, in particular from prostate cancer, a three-level system of oncological care has been created on the territory of the Nizhny Novgorod Region, including 1 regional center, 2 interdistrict cancer centers, 88 primary oncology rooms and 96 examination rooms. The tasks set to reduce morbidity and mortality from oncological diseases can be realized only with close interaction of the oncological service with the primary health care sector, in which the prevention should be the priority direction of work.
