Thirty eight patients with posttraumatic reflex sympathetic dystrophy (RSD) of upper extremities and 28 healthy individuals of the same age (control group) were examined using computer laser doppler flowmetry (LDF) and wavelet-analysis of amplitude-frequency spectrum of fluctuations of microvascular circulation. RDS prevailed in patients with primary warm forms, increase ofportion of shunt circulation, decrease of sympathetic activity and its breakdown at activation. Patients with primary cold forms were characterized by inconstant increase of neurogenic tonus of microvessels at rest. Prolong activation of peptidergenic function of sensory fibres supporting neurogenic inflammation, edema and pain syndrome were typical. One could not exclude that sympathetic vasomotor function and sympathetic support of pain may innervated by different groups of sympathetic neurons. Objective assessment of perivascular innervation and tissue microcirculation is of great clinical importance for individual RSD treatment protocol. Use of LDF is reasonable for early diagnosis of RSD, determination the role of sympathetic and peptidergenic mechanisms in RSD pathogenesis and indications for sympathetic blockades and capsaicin.

Relevance. Occlusal splints, made for the patients with temporomandibular disorders (TMD), influence proprioceptive sensitivity, sensory characteristics of pain, parameters of microcirculation of the pulp and periodontium of the healthy teeth. Condition of the pulp and periodontium of the healthy teeth should be assessed in patients with TMD associated with connective tissue dysplasia (CTD).Materials and methods. The study examined 36 TMD patients aged 26.3 ± 1.3 лет (М ± m) with CTD (n = 20, main group) and without CTD (n = 16, control group); with natural teeth, without complaints of TMD, masticatory muscles and periodontal condition. Laser doppler flowmetry (LDF) helped to analyze blood flow parameters in the periodontium and pulp of the healthy teeth. Received results were statistically analyzed.Results. Wavelet analysis revealed different microcirculatory flowmotion: high-amplitude irregular type was in patients of the main group, low-amplitude regular type was in controls. In the main group, perfusion is supported by high values of flux and vasomotion, ergotropic sympathetic effects (controlled by catecholamines) prevail, myogenic tone is reduced, elevated microvascular tone reflects possible changes in rheologic blood properties, microcirculatory signs of pain syndrome and relatively elevated ischemic index in pulp microcirculation of healthy teeth.Conclusion. Sensory and regulatory mechanisms, reflected by the condition of periodontium and pulp blood flow, can be regarded as early signs of inflammation and chronic pain in the diagnosis of TMD in patients without complaints.

<b><i>Introduction:</i></b> The role of <i>Ureaplasma</i> spp. (UPs) in the pathogenesis of chronic prostatitis is debated. The lithogenic potential of UPs could be a risk factor for the development of chronic prostatitis. <b><i>Methods:</i></b> A total of 143 patients with identification of UPs were retrospectively selected from a database including patients with prostatitis-like symptoms who were studied according to the same protocol including clinical, microbiological and microscopic evaluation, and transrectal prostate ultrasound. A control group of patients with negative UPs was considered including 393 with chronic bacterial prostatitis (CBP), 42 patients with <i>Chlamydia trachomatis</i> (CT), and 781 patients with chronic pelvic pain syndrome. UPs and <i>Mycoplasma hominis</i> (MH) were identified using a semiquantitative assay. <b><i>Results:</i></b> Calcifications were observed more frequently in patients with UPs (64%) than in patients with CBP without UPs (39%), CT infection (37%), and chronic pelvic pain syndrome (29%) (<i>p</i> &#x3c; 0.0001). UPs were isolated in VB1 alone in 35 patients (urethral UPs), in expressed prostatic secretion (EPS) or post-massage urine (VB3) or sperm in 77 patients (prostatic UPs) and associated with other pathogens in 31 patients (associated UPs). Calcifications were more frequent in prostatic UPs (71%) and associated UPs (73%) than in urethral UPs (34%). Mean NIH-CPSI scores were not significantly different between groups, although mean WBC counts of sperm of patients with urethral UPs were significantly lower than in patients with prostatic UPs (<i>p</i> = 0.000) and associated UPs (<i>p</i> = 0.002). <b><i>Conclusions:</i></b> UPs identification in the urogenital fluids is related to higher rates of prostate calcifications. The ability of UPs to promote the formation of calcifications could be related to the chronicization of prostate infection. In particular, the presence of UPs in VB3/EPS/sperm is associated with higher rates of calcifications and high WBC sperm counts, suggesting a partial or full causative role of UPs in the pathogenesis of this disease.

The main purpose of this paper was to identify the role of ‘Udana vata’ to increase immunity. The study was divided into Literary Study and Clinical Study. References were studied for different keywords like – Saar, Oja, Bala, Rasayana, Udana vata, etc. So, to prove the concept and for the applied aspect of ‘Udana Vata for Immunity’ a clinical study was performed on patients at OPD level. Patients were selected keeping in view, the ‘desire for maintaining health’ as mentioned in Ayurvedic texts. Selected patients for Rasayana were randomly divided into two groups of 25 patients each. The efficacy of the therapy was assessed on the basis of subjective as well as objective criteria. Traditionally there is no objective assessment criteria is given in texts. Thus, a need aroused to do the same, therefore with the help of modern and traditional knowledge following criteria were developed by me and all patients were examined under following criteria. 1. Saarta scale 2. Bala scale 3. Swara scale 4. Varna scale 5. Smriti scale An attempt was made to find out the best condition for getting rasayana effects. This study was done only for ‘vata- tapik rasayana’ (In routine life). There was 72 % marked improved in udana vata group vis a vis only 36% marked improvement in control group. Rasayana are best effective in lehiya form. Oja / Immunity can be improved if lehiya form of rasayana is given in Udana kaal and at udana sthana. Currently the study of the role of peripheral nervous system in control of inflammation in endometriosis and its involvement in the development of the chronic pain syndrome in this pathology is of relevance. The purpose of this work was to assess the reciprocal location of sympathetic and parasympathetic nerve fibers with external genital endometriosis at various stages of progression. The samples for the study were the endometrial biopsies of patients of reproductive age with external genital endometriosis of I-IV stage of the disease, with primary sterility (n = 20). The control group consisted of 3 patients (n = 5) who did not have any signs of endometriosis. Molecular markers were visualized using the immunohistochemical method. Antibodies to tyrosine hydroxylase (TH, 1:200, Abcam, USA) and PGP 9.5 (PGP 9.5, 1:1000, Abcam, USA) were used as primary antibodies. Alexa Fluor 488 (1:1000, Abcam, USA) and Alexa Fluor 647 (1:1000, Abcam) were used as secondary antibodies. The results were visualized with a confocal microscope (FluoView1000 (Olympus)). 3D shooting was used to assess the reciprocal location of the studied markers. As a result of this work, it was found that the expression of both of studied markers was detected in all samples. The obtained data can significantly increase the understanding of the functioning mechanisms of the neoneurogenesis processes in ectopic and eutopic tissue, thereby allowing an increase in the accuracy of the endometriosis diagnostic methods.

Abstract. The Aim of study is the impact of course application of quercetin on the functional state of the endothelium of the microvessels in elderly people with metabolic syndrome. Materials and methods. We examined 110 patients (55 primary and 55 control patients) with metabolic syndrome (MS). Patients in the main group (n = 55) received quercetin ("Kvertin", chewing tablets manufactured by PJSC "Borshchagovsky KFZ", 80 mg 3 times a day) during 3 months. The control group patients (n = 55) received placebo tablets. The volume velocity of skin blood flow in the region of the middle third of the inner surface of the forearm was determined using the laser doppler flowmetry (LDP) on the apparatus BLF-21D (Transonic Systems Inc, USA). The functional state of the endothelium of microvessels was examined in a sample with post-exclusive hyperemia by clamping shoulder vessels of with a cuff for 3 minutes, in which the pressure exceeded the systolic blood pressure of the subject by 50 mm Hg. Blood pressure was measured by the Erkameter 3000 (Germany) mercury sphygmomanometer on the shoulder artery in accordance with ESH recommendations. Results. In the group of patients receiving quercetin, when performing a test with post-occlusive hyperemia, there was a statistically significant increase in the maximum volumetric velocity of the skin blood flow and the duration of the recovery period, the volumetric velocity of the skin blood flow to the initial level, which indicates an improvement in the vasomotor function of the microvessel endothelium. Improvement of the functional state of the endothelium occurred in 38 of 55 patients (69%) examined and was accompanied by a significant decrease in the level of systolic and diastolic blood pressure. Patients in the control group showed no improvement in the indices of vasomotor function of the microvessel endothelium. Conclusions. Course application of quercetin improves the functional state of the endothelium of microvessels, which helps to lower blood pressure in patients with metabolic syndrome. Experiments were performed in unanesthetized rats to determine responses to 48 h water restriction of the renal regional microcirculation (cortex, outer medulla, and inner medulla) using implanted optical fibers and laser-Doppler flowmetry. The role of vasopressin (AVP) as a mediator of renal regional blood low changes and its contribution to urinary concentrating ability were assessed by continuous intramedullary interstitial infusion of specific V1 receptor antagonist d(CH2)5 [Tyr-(Me)2, Ala-NH2]AVP (2ng . kg-1 . min-1). Inner medullary blood flow decreased 34% at the end of 48 h of water restriction, whereas cortical and outer medullary flow did not change. This fall in inner medullary blood flow was substantially attenuated (18%) by the continuous interstitial infusion of the antagonist. Plasma AVP levels increased from control levels of 3.4 +/- 1.1 to 20.5 +/- 5.4 pg/ml (P < 0.05) by the end of the 48-h period of water restriction. Arterial pressure increased slightly but significantly during water restriction in the control rats. Infusion of antagonist impaired the maximal urinary concentrating ability, as demonstrated by the lower urine osmolality in this group than in the control group (1,893 +/- 49 vs. 2,419 +/- 225 mosmol/kg H2O; P < 0.05) measured during the second day of water restriction. Sodium and urea concentration decreased 20 and 22%, respectively, indicating that both contributed to the lower urine osmolality observed in the group of rats receiving the antagonist. We conclude that water restriction induces a selective decrease in inner medullary blood flow, which is mediated almost completely by endogenously released AVP. This vascular effect of AVP contributes to the maximum concentrating ability of the kidney.

Thirty four patients with delayed consolidating fractures and long bones pseudarthrosis and 30 healthy individuals (control group) were examined using laser Doppler flowmetry and computed thermography. It was shown that during the osteogenesis process no isolated changes in bone circulation took place but the potentialities of blood flow in the extremity segment as a whole were mobilized. Important role of bone surrounding soft tissue circulation for provision of adequate osteogenesis was confirmed. In case of pseudarthrosis formation microcirculation system, especially its nutrient part, responded «keenly» to the changes in regional metabolism and bone regeneration. That stipulated diagnostic and prognostic importance of soft tissue microhemocirculation study in the zone of pseudarthrosis. Preoperative functional examination of soft tissue circulation enabled to determine risk group for nonconsolidation ordelayed consolidation of fractures. For better result special treatment tactics can be used (osteogenesis stimulators, free vascularized autografts, etc.).

Abstract Complex regional pain syndrome (CRPS) is a controversial, ambiguous, unreliable, and unvalidated concept that, for these very reasons, has been justifiably ignored in the “AMA Guides Library” that includes the AMAGuides to the Evaluation of Permanent Impairment (AMA Guides), the AMA Guides Newsletter, and other publications in this suite. But because of the surge of CRPS-related medicolegal claims and the mission of the AMA Guides to assist those who adjudicate such claims, a discussion of CRPS is warranted, especially because of what some believe to be confusing recommendations regarding causation. In 1994, the International Association for the Study of Pain (IASP) introduced a newly invented concept, CRPS, to replace the concepts of reflex sympathetic dystrophy (replaced by CRPS I) and causalgia (replaced by CRPS II). An article in the November/December 1997 issue of The Guides Newsletter introduced CRPS and presciently recommended that evaluators avoid the IASP protocol in favor of extensive differential diagnosis based on objective findings. A series of articles in The Guides Newsletter in 2006 extensively discussed the shortcomings of CRPS. The AMA Guides, Sixth Edition, notes that the inherent lack of injury-relatedness for the nonvalidated concept of CRPS creates a dilemma for impairment evaluators. Focusing on impairment evaluation and not on injury-relatedness would greatly simplify use of the AMA Guides.
