Redes de Atenção à Saúde: Movimentos do Ensino e Serviço Rumo à Efetivação do Modelo Assistencial

Revisão integrativa da literatura, que objetivou caracterizar as produções bibliográficas sobre Redes de Atenção à Saúde no Brasil entre o período de 2005 e 2015. Na busca dos trabalhos, ocorrida em janeiro de 2016, utilizaram-se os termos: “Redes de Atenção à Saúde” e “Assistência à Saúde”, no portal Biblioteca Virtual em Saúde. Um total de 16 produções, que se relacionavam ao tema proposto foram selecionadas e divididas, em duas temáticas de discussão: “As Redes e sua implicação no modelo de atenção à saúde” e “A participação dos diferentes setores e sujeitos para a efetivação das Redes”. Os artigos discutem acerca das potencialidades, que um sistema operacionalizado na dinâmica de Redes pode trazer para a atenção em saúde, bem como revelam a importância do envolvimento do setor ensino no processo de construção e consolidação destas, em decorrência de sua função de notável importância nos processos formativos de futuros profissionais da saúde. As publicações também demonstram diversas dificuldades na concretização desse modelo de atenção, o que advém da falta de conscientização e adesão ao modelo, por parte dos sujeitos, que atuam diretamente ou indiretamente no Sistema de Saúde, incluindo-se profissionais, estudantes, professores, usuários e gestores. Foi encontrado um número limitado de artigos científicos, o que demonstra necessidade de ampliar as pesquisas na área.Palavras-chave: Redes de Atenção à Saúde. Assistência à Saúde. Educação em Saúde.AbstractIntegrative review of literature that aims to characterize the bibliographic productions about Health Care Networks in Brazil between the period of 2005 and 2015. The search of the articles, collected in January of 2016, included the terms “Health Care Networks” and “Delivery of Health Care”, on Health Library Virtual portal. An entire of 16 publications that were related to the proposed theme were selected and divided into two thematic discussions: “The impact produced by Networks on health care system” and “The participation of different sectors and people to the Networks establishment”. The articles discuss about the potential that an operationalized system in Networks dynamics can bring to health care as well as reveal the importance of the education sector involvement concerning the construction and consolidation of the same, due to their remarkable importance when it comes to theformation process of future health professionals. The publication also demonstrates several difficulties achieving this care model, which arise from the lack of awareness and accession to the model by the subjects that act directly or indirectly on the Health System, including professional, students, professors and managers. A limited number of scientific articles were found which shows the need to expand the researches in this area.Keywords: Health Care Networks. Health Delivery. Health Education.

Background: Future health-care professionals face stress both during education and in later professional life. Next to educational trainings, many students are forced to assume part-time employment. Objective: Applying the Job Demands-Resources Model to the educational context, we investigate which role part-time employment plays next to health-care professional students’ education-specific demands and resources in the prediction of perceived stress. Method: In this cross-sectional study, data from N = 161 health-care students were analysed, testing moderation models. Results: Education-specific demands were associated with higher and education-specific resources with lower amounts of perceived stress. Part-time employment functioned as moderator, i.e. demands were less associated with stress experiences in students who were employed part-time. Conclusion: Identifying part-time employment as a resource rather than a demand illustrates the need to understand students’ individual influences on stress. Both educators and students will benefit from reflecting these resources to support students’ stress management.

El duelo es un proceso natural que han de pasar las personas tras la pérdida de un ser querido. Durante la actual pandemia COVID-19 los profesionales sanitarios han sido víctimas, pues se han tenido que enfrentar a una situación sin precedentes en la que se ha puesto en juego su propia salud mental. El objetivo principal del estudio ha sido analizar el duelo en los profesionales sanitarios durante la actual pandemia COVID-19. Se realizó una revisión de la literatura, no sistemática, en las diferentes bases de datos científicas para analizar el duelo en el ámbito de la medicina ligado al duelo vivido por los profesionales sanitarios, centrado en la situación actual de crisis sanitaria, así como los problemas que añadió la pandemia COVID-19. La pandemia COVID-19 ha puesto de manifiesto la importancia del duelo y la despedida justa, la cual no se pudo dar en la mayoría de los casos. Es imperiosa la preparación que han de recibir los profesionales sanitarios para enfrentar dicho proceso e incluso, los futuros profesionales sanitarios. Queda de manifiesto la importancia de la capacitación y cuidado de la salud mental, tanto en pacientes, familiares, personal sanitario y futuros profesionales de la salud. A la vez que, se ha de garantizar la resiliencia enfocada a los aspectos relacionados con el duelo. Grief is a natural process that people go through after the loss of a loved one. During the current COVID-19 pandemic, health professionals have been victims, as they have had to face an unprecedented situation in which their own mental health has been put at stake. The main objective of the study has been to analyze the grief in health professionals during the current COVID-19 pandemic. A literatura review, non-systematic was carried out in the different scientific databases to analyze the grief in the field of medicine linked to the grief experienced by health professionals, focused on the current situation of health crisis, as well as the problems added by the COVID-pandemic. 19. The COVID-19 pandemic has highlighted the importance of mourning and a fair farewell, which could not be given in most cases. The preparation that health professionals must receive to face this process and even future health professionals is imperative. The importance of training and mental health care is evident, both in patients, relatives, health personnel and future health professionals. At the same time, resilience focused on aspects related to grief must be guaranteed.

Background: During the coronavirus disease 2019 (COVID-19) pandemic, health professionals were pushed to the front line of a global health crisis unprepared and resource constrained, which affected their mental well-being.Aim: This study aimed to investigate the effectiveness of a brief online mindfulness-based intervention (MBI) on stress and burnout for health professionals training and working in South Africa during the COVID-19 crisis.Setting: The context of the study is the overburdened, under-resourced health care system in South Africa during a global pandemic.Methods: A mixed method framework was adopted for this study. The quantitative data was analysed using descriptive analysis and the participants’ qualitative experiences were interpreted using interpretative phenomenological analysis.Results: Forty-seven participants took part in this study. The study found a statistically significant (p 0.05) reduction in stress levels and emotional exhaustion as well as an increase in mindful awareness and feelings of personal accomplishment after the intervention. The participants’ shared experiences were analysed in two parts. The pre-intervention analysis presented with central themes of loss of control and a sense of powerlessness because of COVID-19. The post-intervention analysis comprised themes of a sense of acquired control and empowerment through increased mindfulness.Conclusions: The study found that a brief online MBI can be associated with reduced levels of stress and burnout as well as an increased sense of control and empowerment, felt both personally and professionally, during a global crisis.Contribution: The impact of an online MBI for health care professionals amidst a pandemic has not been previously documented.

ABSTRACT Objective: to analyze the perceptions of Spanish nurses regarding the country’s economic crisis situation, and its impacts on nursing work, health system and population’s health. Methods: qualitative approach, with data collection using an internet-based questionnaire and individual in-depth interviews. Data were analyzed according to Thematic-Categorical Content Analysis, supported by Historical and Dialectical Materialism perspective. Results: the categories produced discuss themes as: cutbacks in health care and the consequences of workforce non-replacement and work overload; salary impact; care model changes; negative impacts on population health. The impact on population health and work was discussed, especially regarding vulnerable groups, as well as in assistance model reconfiguration, reinforcing the biomedical and assistance perspective.

Abstract Background Anti–tumor necrosis factor (anti-TNF) drugs are highly effective in the treatment of moderate-to-severe Crohn’s disease (CD) and ulcerative colitis (UC), but they are very costly. Due to their effectiveness, they could potentially reduce future health care spending on other medical therapies, hospitalization, and surgery. The impact of downstream costs has not previously been quantified in a real-world population-based setting. Methods We used the University of Manitoba IBD Database to identify all persons in a Canadian province with CD or UC who received anti-TNF therapy between 2004 and 2016. All inpatient, outpatient, and drug costs were enumerated both in the year before anti-TNF initiation and for up to 5 years after anti-TNF initiation. Costs before and after anti-TNF initiation were compared, and multivariate linear regression analyses were performed to look for predictors of higher costs after anti-TNF initiation. Results A total of 928 people with IBD (676 CD, 252 UC) were included for analyses. The median cost of health care in the year before anti-TNF therapy was $4698 for CD vs $6364 for UC. The median cost rose to $39,749 and $49,327, respectively, in the year after anti-TNF initiation, and to $210,956 and $245,260 in the 5 years after initiation for continuous anti-TNF users. Inpatient and outpatient costs decreased in the year after anti-TNF initiation by 12% and 7%, respectively, when excluding the cost of anti-TNFs. Conclusions Direct health care expenditures markedly increase after anti-TNF initiation and continue to stay elevated over pre-initiation costs for up to 5 years, with only small reductions in the direct costs of non-drug-related health care.

Competing demands for resources within the health care system require health care providers to ensure the most effective and efficient use of resources. The evidence from the United States, the United Kingdom and other jurisdictions suggests that integrated health delivery systems (IDS) may be a cost-effective way to meet the health care needs of a population. This article introduces a framework for use in monitoring and evaluating the performance of an integrated delivery system. The establishment of a consistently used evaluation framework for integrated delivery systems will provide the government, governing bodies and other evaluators with an effective assessment tool that will enable greater understanding of the impact of the IDS on the health care system. It will also provide information to enable ongoing performance improvements within the system.

Practice of hand hygiene is a very ancient concept of personal cleanliness which reflects in several religious and practiced in numerous cultures. It is worth to consider the grounds of religious and cultural issues in promoting hand hygiene in healthcare services in order to be more effective. Objectives: To study the review of literature of religious and cultural aspects of hand hygiene in healthcare accreditations. Methods: It is a descriptive study in which the several literatures on religious and cultural aspects of hand hygiene in healthcare accreditations were studied. Significance of Research: Healthcare Associated Infections (HCAI) are major threat to patient safety which increases the length of stay, long-term disability, increased resistance of microorganisms to antimicrobials, massive additional financial burden, high costs for patients and their families, and excess deaths. Data Collections: The data was collected from relevant published journals, articles, research papers, academic literature and web portals. Conclusion: Community behaviour influences health care worker’s professional behaviour has been supported by insufficient scientific facts until now. There are no data existing on the impact of religious customs on hand hygiene observance in health-care settings where religion is very deep-seated.

Introduction: Interdisciplinary collaboration (IDC) is important in health care settings as the complex nature and demands of the health care work environment requires the expertise and knowledge of different individuals or specialists working together to solve multifaceted and complex patient care problems. Objective: To assess the health professionals' attitude towards the development of an interdisciplinary collaborative approach to patient care in health institutions and to systematically review the impact of IDC as a panacea for effective health outcomes in Nigeria. Methodology: The research is a systematic review that provides various approaches for studying interdisciplinary teams. Fifty articles were selected from different search engines such as Google, google scholar, science direct and research gate with the search term Interdisciplinary collaboration among health care professionals. Articles were arranged based on most relevant, relevant and closely related articles. Result: The study revealed that IDC is pivotal in evidence-based care and contributes immensely to effective and efficient health outcomes. It puts the patient at the centre of the healthcare team's focus and allows all health professionals, with the patient, to collaboratively provide input, be part of the decision making, and improve outcomes. Although there are several obstacles to IDC, adopting this team-based culture of mutual respect and understanding is possible and, in fact, necessary. Conclusion: This study reveals that there are many benefits to IDC. It can improve safety and healthcare delivery, as well as reduce costs. The interprofessional team supports patient and personnel engagement, organizational efficiency and innovation.

Dynamics of ageing of Latvia’s population is faster than in other European Union countries, which has been caused by the social economic situation and immigration of society members. Ageing of society will have a considerable impact on health care in the future. Hence, it is essential to invest resources in the process of the development of health care by educating health care specialists who will be motivated to solve social wellbeing problems in the country in a professional way.Students and lecturers of Riga Stradins University Liepaja branch have been taking part in European Later Life Active Network (ELLAN) project since 2013. In the research done within the framework of the project, Kogan’s attitude scale to elderly people has been made use of, as well as Nolan’s questionnaire was used to learn about the future health care specialists’ attitude and their expectations concerning work with elderly people. Data collected in Latvia in comparison with the rest of the four countries (Ireland, Germany, Italy and Finland) taking part in the project present the lowest level of attitude indicators. The article focuses on the issue of future health care specialists’ motivation and attitude in their professional work with elderly people, as well as, the impact of the environment on promoting the development of positive attitude towards elderly people and work with them during the study process.
