Background: Immunization is a necessary aspect of health care of children and injections are there for unavoidable. Many factors affect injection pain during immunization in infants. This study aims to see the effect of use of local anesthetics delivered by various modes for attenuation of vaccine related injection pain in infants and to compare them.Methods: An Open Label Four-Arm Randomized Control Trial of 300 healthy infants of age group 6 weeks to 6 months reported to immunization clinic for immunization with DPT-HiB-Hepatitis B combination vaccine were taken for study. The enrolled subjects were allocated into control group and intervention group (who were applied some form of local anesthesia).Results: Among the four groups of the patients studied we observed a statistical difference in the mean pain scores of the patients recorded at 15 second, 60 second and 5 min after vaccine injection (p value 0.0024 - 0.000). Group A (Infants with topical occlusive LA cream) showed minimum pain scores values at 15 second, 60 second and 5 min after vaccine injection, followed by Group C (Infants with topical LA spray with vapocoolant) whereas Control group (Infants not received any local anaesthesia) and Group B (Infants with topical LA spray without vapocoolant) exhibited the maximum pain scores.Conclusions: Topical occlusive local anesthetic cream and topical LA spray with vapocoolent, were found to be better than topical LA spray without vapocoolant or no topical anesthetic. Use of topical occlusive LA cream led to a lowest pain score. There was no significant difference in the profile of side effects following injection in the four group.

Background: Juxta-anastomotic stenosis (JAS) is one of the predominant causes of arteriovenous fistula (AVF) failure, with the reported incidence of 65%, so that technical modification to alter the outflow vein configuration using the modified technique has been applied to prevent JAS and improve AVF maturation. The aim of the study to evaluate the modified technique of end-to-side distal radiocephalic A-V fistula regarding maturation, patency rate and the resultant juxta-anastomotic stenosis.Methods: This prospective study was carried out on 80 patients with end stage renal disease (ESRD) at vascular surgery unit in general surgery department, Menoufia university hospital that prepared for dialysis. 40 patients "intervention group" underwent the modified technique to establish a functioning radiocephalic fistula; the other group (40 patients, control group) had the conventional technique of end to side radiocephalic fistula. Follow up of patients was over 6 months regarding function, patency rate and development of juxta-anastomotic stenosis.Results: There was statistically significant difference between 2 groups regarding primary failure, patency and JAS. Primary failure was detected in 2 patients in group A and in 5 patients in group B (p-value is 0.04). Considering patency rate, after 3 months the ratio between group A and group B was 37:34 with significant P value of 0.02, and after 6 months the ratio was 35:33 with P value of 0.03. Regarding JAS, by the end of follow up period, 4 patients diagnosed with JAS in group A, while group B had 8 patients, with p value of 0.01.Conclusions: Modified technique of end to side anastomosis for primary radio-cephalic fistula creation has better patency rate and low incidence of JAS than conventional method.

                The evolution of information technology has exerted great influence on nursing education via new pedagogy of knowledge delivery without time and place restriction. Mobile technology revolutionises nursing education and clinical practice via empowering skills of critical thinking and clinical decision-making through learning. The aim of this study is to evaluate the effectiveness of using mobile messenger (Whatsapp) as an educational supporting tool among nursing students. The study design used is a Cluster Randomized Control Trail. Two nursing colleges were selected. Sample size was 93 participants, 48 from the Kuala Terengganu Nursing College Kuala Terengganu as the intervention group while the control group were recruited among 45 participants from UniSZA Nursing College. There is a significant difference in the level of knowledge between pre and posttest among intervention group (mean difference was -8.70 with a standard deviation 8.42, p-value< 0.001) and 93.8 percent of the respondents perceived the usefulness of using WhatsApp mobile messenger to enhance learning. This demonstrates that learning through mobile messenger (WhatsApp) enhances learning and is well received as a new method of learning by almost all students.   Keywords: Mobile learning, WhatsApp messenger, Social Interaction

<p><span>Baby Massage and Spa was a traditional effort that uses a holistic approach through comprehensive care using a combination of massage and water therapy methods that are carried out in an integrated manner to balance the body, mind, and feelings. Age 3-6 months is the right time for babies to do massage and spa. The purpose of this study was to determine the effect of baby massage and spa on the growth of infants aged 3-5 months in Pringsewu District in 2018. This study uses a quasi-experimental approach with research design pre and post-test with control group design. The purposive sampling technique was taken with a sample of 30 infants as the intervention group and 30 infants as the control group. Data were processed using computerization and analyzed using the T-Test. The results showed that there were significant differences between body weight, body length and baby head circumference in the pre and post-treatment. There was no significant difference in the average increase in infant weight between babies who were carried out by baby massage and spa with babies who were not carried out by baby massage and spa with p-value 0.116. There is a significant difference in the baby's body length and head circumference performed by baby massage and spa with those not carried out by baby massage and spa with p-value 0.000. Baby Massage and Spa has a significant effect on increasing the growth of babies aged 3-5 months.</span></p><p><span> </span></p>

Introduction: Shwasa is said as Shigrapranahara Roga. It occurs as the main disease and also a symptom in various diseases. Shwasakruchrata is a common symptom that occurs in Hrudroga. Acharya Charaka mentioned the unique classification of drugs based on their action. Shwasahara Dashemani is one among them. It is containing 10 herbal drugs which are specially indicated in Shwasa Roga. Hence to evaluate the efficacy of Shwasahara Dashemani in Lakshana Roopi Shwasa in L.V.F (Cardiac Asthma) has taken for the study. Aim and Objective: The objective is to assess the efficacy of Shwasahara Dashemani in L.V.F with dyspnea (Cardiac Asthma). Method: The present study is a controlled comparative, open-label, clinical trial with pre and post-test design. A total of 40 subjects of a diagnosed case of L.V.F with dyspnea (Cardiac Asthma) were selected by using a simple random sampling method. Control group subjects were intervened with standard treatment of L.V.F and intervention group subjects were intervened with standard treatment of L.V.F along with Shwasahara Dashemani Ghana Vati, for the duration of 30 days. Its efficacy was assessed before treatment (0th day) and after treatment (31st day) by using BDI (Baseline Dyspnea Index Scale). Results: The P-value of dyspnea of the control group is 1.000 and the P-value of dyspnea of the intervention group is 0.105. This shows that the results of both groups are statistically not significant. But as compared to the control group, the intervention group is clinically significant because after the intervention 35% of subjects had shown improvement in the intervention group. Conclusion: As compared to the control group, in the intervention group Shwasahara Dashemani Ghanavati is clinically significant in relieving cardiac asthma when used with standard treatment of L.V.F. Anemia is one of the public health problems that need more attention of the female because they experience menstruation and they are in the growth phase. From the data of Semarang City Health Office  in 2011, it is shown the incidence of anemia in adolescents aged 10-19 years old are 68.24%. The prevention effort of the case is by measuring hemoglobin and prescribing blood supplement. The objective of the study is to find out the effect of Fe tablets on Hemoglobin in Semarang Merchant Marine Polytechnique students. The objective of the study is to find out the effect of Fe tablets on Hemoglobin in Semarang Merchant Marine Polytechnique students. This research used pre-experimental design with the two group pretest posttest. The population in this Study were all Merchant Marine Polytechnique Semarang at the 4th semester of female students  amounting to 50 with the total respondents 34 respondents taken with purposive sampling technique. The data were collected by performing Hemoglobin examination. The results of the study show that the average Hemoglobin before being prescribed Fe on the intervention and control group was 11.29 g/dl. After being prescribed Fe to the intervention group, it is found the average increased to 13.69 g/dl, while in the control group it is found averaged 11.72 g/dl. Bivariate Analysis uses an independent t test, the value of t is 6.136 with a p-value of   0.000. It shows that the p-value is 0,000 <((0.05), this indicates that there is a significant difference in the decrease in Hemoglobin levels in the population after being prescribed blood supplement between the intervention and control groups in The Semarang Merchant Marine Politechnique Students. It is suggested that the female adolescents to consume more iron-containing foods or consume Fe supplements such as Fe, during menstruation. ABSTRAK  Anemia merupakan salah satu masalah kesehatan masyarakat yang perlu khusus nya pada remaja  putri yang perlu mendapatkan perhatian khusus karena remaja putri mengalami mestruasi tiap bulan dan dalam masa pertumbuhan . Hasil Data Dinas Kesehatan Kota Semarang tahun 2011.  Kejadian anemia pada remaja usia 10-19 sebanyak 68,24%. Untuk menangani masalah tersebut pencegahan yang dilakukan yaitu dengan cara mengukur hemoglobin dan pemberian suplemen tablet tambah darah . Penelitian ini bertujuan untuk melihat Pengaruh Pemberian Tablet Fe Terhadap Hemoglobin Pada Taruni Politeknik Ilmu Pelayaran Semarang. Penelitian ini bertujuan untuk melihat Pengaruh Pemberian Tablet Fe Terhadap Hemoglobin Pada Taruni Politeknik Ilmu Pelayaran Semarang. Jenis penelitian ini menggunakan  pre-eksperimen design dengan rancangan Two group pretest posttest. Populasi dalam penelitian iniadalah seluruh taruni politeknik ilmu pelayaran semarang semester 4 berjumlah 50 taruni dengan jumlah sempel 34 responden dengan tehnik pengambilan sempel purposive sampling. Pengambilan data dengan melakukan pemeriksaan Hemoglobin.  Penelitian menunjukan bahwa rata-rata Hemoglobin sebelum diberikan pada taruni kelompok intervensi  dan  kontrol sama yaitu  11,29 g/dl.  Dan setelah diberikan pada kelompok intervensi  rata-rata meningkat menjadi 13,69 g/dl, sedangkan  kelompok kontrol rata-rata sebesar 11,72 g/dl. Analisis bivariate Menggunakan uji t independen, didapatkan nilai t hitung 6,136 dengan p-value sebesar 0,000. Terlihat bahwa p-value 0,000 <a (0,05), ini menunjukkan bahwa ada perbedaan secara signifikan penurunan (selisih) kadar Hemoglobin b pada taruni sesudah diberikan tablet penambah darah antara kelompok intervensi dan kontrol pada taruni Politeknik Ilmu Pelayaran Semarang.  Hendaknya bagi remaja putri lebih banyak mengomsumsi makanan yang mengandung zat besi atau mengomsumsi suplemen yang mengandung besi seperti Fe terutama saat mestruasi

Introduction: Thyroidectomy is a common surgery in the neck area, in which the application of platysma muscle suture after thyroidectomy is still being discussed. This study was conducted to compare the application (currently common) or non-application of suture for platysma muscle. Methods: In this retrospective cross-sectional study, 117 patients underwent thyroidectomy, among which 63 cases without suturing platysma (control group) and 54 subjects with suturing platysma (Intervention group ) were examined in terms of postoperative pain based on visual analogue scale score measured 24 h post-operation. The samples were also investigated regarding hematoma and seroma, wound infection, length of hospitalization, scarring (1 year after surgery), duration of surgery, and the number of cases using opioids during the hospitalization. Patients with diabetes, previous neck surgery, coagulopathy, and radiation history were excluded from the study. The gathered data were analyzed statistically in SPSS software (version 18) using the Chi-square test and the Mann–Whitney U test. A p-value of less than (0.05) was considered significant. Results: Based on the findings, the mean age of the patients in the Intervention group was calculated at 51 years, of which 41 and 13 cases were females and males, respectively. In the Intervention group, 34 patients underwent complete thyroidectomy and 20 patients had hemithyroidectomy. The mean age score of subjects in the control group was calculated at 50 years, of which 44 and 19 patients were respectively female and male. No significant difference was revealed considering wound infection, length of hospitalization, created scarring, the amount of opioid use (opioids), and postoperative pain. However, only the length of surgery was different between the groups (P-value<0.05). Conclusions: There was no difference between wound and surgical complications and cosmetic results between both groups; nevertheless, due to the duration of the surgery and other benefits, such as consuming less thread, not suturing the platysma is recommended.

This study aimed to determine the effect of the development of horticultural therapy to improve the social skills of mental disorders patients in the community. This research's method is quantitative research with a pre-posttest queasy experimental research design with a control group. The study results the social skills score before and after treatment in the intervention group given horticultural therapy was marked with a p-value of 0.01 (<0.05). In conclusion, there is a significant difference in the mean difference in social skills scores between the intervention and control groups.   Keywords: Mental Disorders, Social Skills, Horticultural Therapy

e24021 Background: The Geriatric 8 (G8) is a simplified screening tool to select the appropriate elderly patients for chemotherapy. Vulnerable patients with impaired G8 score might need additional comprehensive geriatric assessment (CGA) with intervention for individual problem. However, the impact of CGA and therapeutic intervention on rate of complete chemotherapy among these patients is rarely addressed. This study aims to evaluate the benefit of CGA guided intervention. Methods: A single center, randomized, open-label study which included newly diagnosed elderly cancer patients (age ≥ 65) with impaired G8 score (≤ 14) who were designated for chemotherapy. After the enrollment, patients were randomized to 1:1 ratio to receive CGA guided intervention (intervention group) or usual care (control group). The primary end point was the rate of complete chemotherapy at 90-day. Associated factors for complete chemotherapy were evaluated. Results: Between June 2019 and December 2019, 52 patients were randomized (26 patients for intervention group and 26 patients for control group). Mean age was 72 years, 59.6% was female, 40.4% had breast cancer and 51.9% had early stage cancer. With G8 assessment, 55.8% had intermediate (score 11-14) and 44.2% had low (score < 11) impaired G8 score. All baseline characteristics were balanced. Using per protocol analysis, there was no significant difference in rate of complete chemotherapy between groups (61.9% vs 50%, OR 1.63; 95%Cl 0.51-5.23; p = 0.42). Considering subgroup analysis in the intermediate G8 score patients, the intervention group had a significant higher rate of complete chemotherapy than control group (81.8% vs 66.7%, OR 2.71; p = 0.02), but no significant difference in low G8 score group (40% vs 27.3%, OR 1.78; p = 0.58). In univariate analysis, age below 75 years, BMI > 20 kg/m2 and intermediate G8 score showed significant factors for improving rate of complete chemotherapy. Conclusions: This is the first study in south-east Asia using CGA and intervention to improve rate of completion in chemotherapy. Although the CGA and intervention had no significant difference but had tendency to be better in completion rate of chemotherapy than usual care. The intermediate-impaired G8 score subgroup is more likely to benefit from CGA guided intervention for complete chemotherapy as planned.

Abstract Background: Considering the increase in drug resistance over time to Helicobacter pylori treatment relying on the anti-inflammatory and antibacterial effects of atorvastatin to increase the success rate of H. pylori eradication, we examined the effect of adding atorvastatin to standard treatment of H. pylori eradication.Results: A total of 186 symptomatic patients who had been diagnosed with Helicobacter pylori infection and tested for H. pylori eradication were examined by a pathological response or positive urea breath test. Patients who received atorvastatin in addition to standard treatment were also identified based on a table of random numbers. Standard treatment included a 240mg bismuth subcitrate tablet, a 40mg pantoprazole tablet, a 500mg metronidazole tablet, and 2 capsules of 500mg amoxicillin, all taken BID for 14 days. After 4 weeks of treatment, all patients underwent stool testing for H. pylori fecal antigen. If the test was positive, the request was considered a failure of treatment, and if the test was negative, it was considered a successful eradication of H. pylori. The clinical trial registration code for this study is IRCT20190823044589N1. The eradication rate of H. pylori was 80% in the control group and 80.9% in the intervention group, which did not show a statistically significant difference between the two groups (P-value=0.971).Conclusion: Adding atorvastatin to 4-drug regimen of PPI, bismuth subcitrate, amoxicillin, and metronidazole as the first line of treatment for H. pylori eradication is ineffective.Trial registration: IRCT, IRCT20190823044589N1. Fatigue is a widespread clinical complaint among adults with type 2 diabetes. Fluctuating glucose levels can cause fatigue. Several factors are associated with fatigue in diabetic patients, including physiological factors such as hypoglycemia or hyperglycemia, psychological factors such as depression associated with diabetes. Progressive muscle relaxation is a procedure to get relaxation in the muscles through two steps, namely by applying tension to a muscle group and stopping the tension then focusing on how the muscle relaxes, feeling the sensation of relaxation and fatigue is reduced. The aim of this study was to identify the effect of progressive muscle relaxation on fatigue symptoms in type II DM clients in 15 control groups and 15 intervention groups. The research method used a quasi-experimental design with a pretest-posttest control group design approach, consisting of one treatment (in the intervention group) and a control group. Data analysis was performed using univariate and bivariate analysis using t-independent and t-dependent tests. Wilcoxon test results in the intervention group and the control group showed a significant p-value of 0.002 (<0.05), meaning that there was a significant difference in fatigue symptoms in the intervention group before and after the intervention. This shows that there was a decrease in fatigue symptoms in the intervention group before and after combination OHO therapy and progressive muscle relaxation exercises (previously the average respondent in the intervention group experienced symptoms of severe fatigue, after intervention, the average value of fatigue symptoms turned into mild fatigue ). Conclusion: Progressive muscle relaxation combined with OHO therapy can reduce symptoms of fatigue compared to only OHO therapy.
