Background: Cystic echinococcosis (CE) is one of the most important helminthic parasitic diseases in Iran. The current study aimed to assess the seroprevalence of CE in North Khorasan Province, Northeast of Iran in 2018. Methods: The study was carried out in seven cities of North Khorasan Province. Venous blood samples were collected from 932 individuals referring to health centers of those seven cities. A questionnaire was used to obtain the data regarding the subject's gender, age, residence and risk factors linked to the hydatid cyst. Sera samples were evaluated for anti-hydatid cyst antibodies in an ELISA system, using a recombinant B8/1 antigen of E. granulosus. Results: Of the 932 recruited subjects, 496 (53.2%) were male and 436 (46.8%) were female. The range of participants’ age was between 11 to 83 yr old and the mean age of the subjects was 35.4 (±12.7) years. Anti-hydatid cyst antibodies were detected in the sera of 37 out of 932 subjects, corresponding to a seroprevalence rate of 3.96%. From these, 20 (54.05%) were male and 17 (45.95%) were female. There were no associations between seropositivity to hydatid cyst and age, the gender of the participants, residential areas and having contact with dogs (P>0.05). Conclusion: CE is relatively prevalent throughout the North Khorasan Province in the Northeast of Iran. Rate of CE infection in this Province is somewhat similar to the rate of infection in other parts of the country.

Mentally retarded individuals are more likely to become infected with soil-transmitted infections including toxocariasis. The current study aimed to determine the serostatus of toxocariasis among institutionalized mentally retarded individuals in Hormozgan Province, in the south of Iran. Subjects of the study were 117 mentally disabled individuals, including children and adults, maintained in a charity-based institution. Three millilitres of venous blood was taken from each subject. While sampling, demographic features of the subject were documented in a questionnaire. An ELISA system based on Toxocara larvae excretory-secretory antigens (TES) was utilized to detect anti-Toxocara IgG antibodies in the sera of the patients. The mean age of the subject was 27.6 (±12.31) years and consisted of 55 (47%) males and 62 (53%) females. Out of 117 cases, 33 (28.2%) were seropositive for toxocariasis. The seroprevalence was higher in females (37.1%) than males (18.2%), and the difference was statistically significant ( p  < 0.05). Out of 117 subjects, 76 (64.9%) had severe and 41 (35.1%) had profound mental retardation. Anti-Toxocara antibodies were detected in 18 (23.7%) patients with severe and 15 (35.6%) patients with profound mental retardation. The highest seroprevalence rate of toxocariasis (44.4%) was observed in the age group of ≤10 years followed by 21–30 years old (36.7%). No statistically significant association was seen between the age and also the duration of residency in the care center and seropositivity of toxocariasis ( p  > 0.05). The current study represents the high prevalence of toxocariasis in mentally retarded patients in Hormozgan Province, southern Iran. The elevated seroprevalence rate of toxocariasis in the current study indicates that these subjects constitute a high-risk group for Toxocara infection, which may be attributed to their behavioral patterns.

Background: The relationship between SLE and traditional risk factors for cardiovascular events was evaluated. Methods: The data regarding sixty patients with SLE and 30 healthy controls (age and sex matched) were gathered using SLEDAI forms. Venous blood (10mL) from all the participants was examined for hs-CRP, homocysteine, VCAM1, CBC, anti-DNA antibody, C3, C4, low-density lipoprotein (LDL), cholesterol, FBS and triglyceride. : The IMT of carotid arteries was determined bilaterally by ultrasound. Other measurements included insulin levels via Elisa (Linco/Millipore Corp) and the HOMA-IR index for insulin resistance. Results: The mean age (in years) in the test and control groups was 28.8±10.3 (18-52) and 33.8±9.13 (18-48), respectively. Results: The mean age (in years) in the test and control groups was 28.8±10.3 (18-52) and 33.8±9.13 (18-48), respectively. : The average IMT in the test group was directly related to serum levels of VCAM1 (p<0.001), homocysteine (p<0.001), cholesterol (p<0.009), LDL (p<0.001), TG (p<0.001), and FPG (p=0.004). The association between other risk factors, insulin resistance, carotid IMT and SLEDAI, was nonexistent. Mean insulin and insulin resistance levels in all the participants were 0.43±2.06 µU/mL and 0.09±0.44, respectively. There was no significant difference between the test and control groups regarding serum insulin and insulin resistance levels (p=0.42 and p=0.9, respectively). None of the risk factors, such as hsCRP, VCAM1, or homocysteine, were shown to be related to insulin resistance (p=0.6, p=0.6, p=0.09, respectively). Conclusion:: Our findings did not show an increase in the prevalence of atherosclerosis in patients with SLE. There was no association between IMT and insulin resistance. However, the former was associated with FPG, total cholesterol, LDL, TG, homocystein and VCAM1.

AbstractEvidence indicates that glucose variation (GV) plays an important role in mortality of critically ill patients. We aimed to investigate the relationship between the coefficient of variation of 24-h venous blood glucose (24-hVBGCV) and mortality among patients with acute respiratory failure. The records of 1625 patients in the Multiparameter Intelligent Monitoring in Intensive Care II (MIMIC II) database were extracted. The 24-hVBGCV was calculated as the ratio of the standard deviation (SD) to the mean venous blood glucose level, expressed as a percentage. The outcomes included ICU mortality and in-hospital mortality. Participants were divided into three subgroups based on tertiles of 24-hVBGCV. Multivariable logistic regression models were used to evaluate the relationship between 24-hVBGCV and mortality. Sensitivity analyses were also performed in groups of patients with and without diabetes mellitus. Taking the lowest tertile as a reference, after adjustment for all the covariates, the highest tertile was significantly associated with ICU mortality [odds ratio (OR), 1.353; 95% confidence interval (CI), 1.018–1.797] and in-hospital mortality (OR, 1.319; 95% CI, 1.003–1.735), especially in the population without diabetes. The 24-hVBGCV may be associated with ICU and in-hospital mortality in patients with acute respiratory failure in the ICU, especially in those without diabetes.

Abstract Introduction Globally around 47.4% of children and in India, 58% of children aged 6–59 months are anemic. Diagnosis of anemia in children using accurate technologies and providing adequate treatment is essential to reduce the burden of anemia. Point-of-care testing (POCT) devices is a potential option for estimation of hemoglobin in peripheral and field settings were the hematology analyzer and laboratory services are not available. Objectives To access the validity of the POCTs (invasive and non-invasive devices) for estimation of hemoglobin among children aged 6–59 months compared with hematology analyzer. Methods The study participants were enrolled from the pediatric outpatient department in Haryana, India, from November 2019 to January 2020. Hemoglobin levels of the study participants were estimated in Sahli’s hemoglobinometer and invasive digital hemoglobinometers (DHs) using capillary blood samples. Hemoglobin levels in non-invasive DH were assessed from the finger/toe of the children. Hemoglobin levels measured in POCTs were compared against the venous blood hemoglobin estimated in the hematology analyzer. Results A total of 120 children were enrolled. The mean (SD) of hemoglobin (g/dl) estimated in auto-analyzer was 9.4 (1.8), Sahli’s hemoglobinometer was 9.2 (1.9), invasive DH was 9.7 (1.9), and non-invasive DH was 11.9 (1.5). Sahli’s hemoglobinometer (95.5%) and invasive DH (92.2%) had high sensitivity for the diagnosis of anemia compared with non-invasive DH (24.4%). In contrast, non-invasive DH had higher specificity (96.7%) compared with invasive DH (83.3%) and Sahli’s hemoglobinometer (70%). Invasive DH took the least time (2–3 min) for estimation of hemoglobin per participant, followed by Sahli’s (4–5 min) and non-invasive DH (5–7 min). Conclusion All three POCT devices used in this study are reasonable and feasible for estimating hemoglobin in under-5 children. Invasive DHs are potential POCT devices for diagnosis of anemia among under-5 children, while Sahli’s can be considered as a possible option, where trained and skilled technicians are available. Further research and development are required in non-invasive DH to improve accuracy. Lay summary In India, anemia is a serious public health problem, where 58% of the children aged 6–59 months are anemic. Point-of-care testing (POCT) using digital hemoglobinometers (DHs) has been recommended as one of the key interventions by the Anemia Mukt Bharat program since 2018 in India. These POCT devices are easy to use, less invasive, can be carried to field, require minimal training and results are available immediately. Therefore this study assessed the validity of POCT devices—invasive DH, non-invasive DH and Sahli’s hemoglobinometer among 6–59 months children in facility setting compared with the gold standard hematology analyzer. A total of 120 children under 6–59 months of age were enrolled from the pediatric outpatient department in Haryana, India, from November 2019 to January 2020. The (mean hemoglobin in g/dl) invasive (9.7) and non-invasive DH (11.9) overestimated hemoglobin value, while Sahli’s (9.2) underestimated hemoglobin compared with hematology analyzer (9.4). Invasive DH (92.2%) and Sahli’s hemoglobinometer (95.5%) reported high ability to correctly identify those with anemia compared with non-invasive DH (24.4%). In contrast, non-invasive DH (96.73%) had higher ability to correctly identify those without the anemia compared with invasive DH (83.3%) and Sahli’s (70%).

AbstractFour years (September 2012 to August 2016) of simultaneous current observations across the Yucatan Channel (~21.5°N) and the Straits of Florida (~81°W) have permitted us to investigate the characteristics of the flow through the Gulf of Mexico. The average transport in both channels is 27.6 Sv (1 Sv = 106 m3 s−1), in accordance with previous estimates. At the Straits of Florida section, the transport related to the astronomical tide explains 55% of the observed variance with a mixed semidiurnal/diurnal character, while in the Yucatan Channel tides contribute 82% of the total variance and present a dominant diurnal character. At periods longer than a week the transports in the Yucatan and Florida sections have a correlation of 0.83 without any appreciable lag. The yearly running means of the transport time series in both channels are well correlated (0.98) and present a 3-Sv range variation in the 4 years analyzed. This long-term variability is well related to the convergence of the Sverdrup transport in the North Atlantic between 14.25° and 18.75°N. Using 2 years (July 2014–July 2016) of simultaneous currents observations in the Florida section, the Florida Cable section (~26.7°N), and a section across the Old Bahama Channel (~78.4°W), a mean northward transport of 28.4, 31.1, and 1.6 Sv, respectively, is obtained, implying that only 1.1 Sv is contributed by the Northwest Providence Channel to the mean transport observed at the Cable section during this 2-yr period.

Purpose This paper aims to analyze the situation of the financial system in the Spanish-governed cities of Melilla and Ceuta, Christian and Muslim cities located on the north coast of Africa, and compared it with the mean bankarization level in the rest of Spain in 2000-2015. Design/methodology/approach Although different calculation methods have been proposed, most authors agree that the bankarization level of a country or a territory reflects the development of the society as a whole and has a positive correlation with economic growth. The indicators of financial depth proposed by these researchers are not only the ratio between variables such as loans, deposits, etc., but also the ratios of these variables to the population and the gross domestic product (GDP) of the country or territory. Findings The results obtained revealed that there are differences between these two North African Spanish cities. Furthermore, the financing gap between the mean bankarization levels of these cities and those of mainland Spain was found to be even larger than most of the other economic indicators (GDP per capita and the unemployment rate). Practical implications The authors are convinced that the manuscript is a contribution of great interest for serving pilot experience in cities wishing to offer a development of traditional banking and Islamic banking. The paper should be of interest to readers in the areas of finance systems and commercial banks where two different cultures coexist. Originality/value This is the first research study on the financial framework of European cities whose populations have an approximately equal percentage of Christians and Muslims. The data reflected the existence of savings and loan methods parallel to conventional banking. The conclusion was that in the near future, it would be advisable for European banks to take into account the cultural customs and religious practices of potential Muslim clients.


In this paper the author investigates the periodical variations of the winds, rain and temperature, corresponding to the conditions of the moon’s declination, in a manner similar to that he has already followed in the case of the barometrical variations, on a period of years extending from 1815 to 1832 inclusive. In each case he gives tables of the average quantities for each week, at the middle of which the moon is in the equator, or else has either attained its maximum north or south declination. He thus finds that a north-east wind is most promoted by the constant solar influence which causes it, when the moon is about the equator, going from north to south; that a south-east wind, in like manner, prevails most when the moon is proceeding to acquire a southern declination ; that winds from the south and west blow more when the moon is in her mean degrees of declination, going either way, than with a full north or south declination ; and that a north-west wind, the common summer and fair weather wind of the climate, affects, in like manner, the mean declination, in either direction, in preference to the north or south, and most when the moon is coming north. He finds the average annual depth of rain, falling in the neighbourhood of London, is 25’17 inches.

ABSTRACT Differences in the secretion of pregnane compounds by follicular polycystic ovaries of androgen-sterilized rats and by normal preovulatory ovaries of early prooestrous rats were compared. Some rats were injected i.v. with LH 30 min before bleeding, in order to stimulate the secretion of steroids. This injection of LH greatly increased the secretion of progesterone, 5α-pregnane-3,20-dione and 3α-hydroxy-5α-pregnan-20-one by both types of ovaries. The response of the two progesterone metabolites in the polycystic ovaries was low, suggesting low 5α-reductase activity. Because it is known that the preovulatory LH surge is absent in androgen-sterilized rats, a classical approach was taken to circumvent the probable deficit in cyclic release of LH by giving an i.v. injection of LH (25 μg) every 4 days for 16 days. Ovarian venous blood was collected 4 days after the last injection. The mean secretion of 5α-pregnane-3,20-dione and 3α-hydroxy-5α-pregnan-20-one from the ovaries of such androgen-sterilized rats became much (P <0·01) higher than that of multiple saline-treated controls. These results suggest that low 5α-reductase activity of polycystic ovaries in androgen-sterilized rats may be due to the absence of cyclic release of LH from the pituitary gland. J. Endocr.
