B-cell lymphoma presenting as acute pancreatitis symptoms in a child

Lymphoma which has a wide range of manifestations is the third malignancy in pediatrics. Nearly, 50% of patients have extranodal involvement. Pancreas can be affected secondarily more than primarily. A 10-year-old boy with recurrent abdominal pain in the epigastric region for six weeks was referred to Amirkola Children's Hospital, affiliated to Babol University of medical sciences (north of Iran). The patient was icteric with elevated levels of amylase and lipase. A hypoechoic mass near the head of the pancreas was detected by ultrasound examination. Pathology of stomach polyps revealed small blue round-cell tumor compatible with a lymphoma. In children with acute pancreatitis symptoms and palpable abdominal mass, the non-Hodgkin lymphomas (NHL) should be considered as an important, though rare possible cause.

Homogeneous and stable dispersions of functionalized carbon nanotubes (CNTs) in aqueous solutions are imperative for a wide range of applications, especially in life and medical sciences. Various covalent and non-covalent approaches were published to separate the bundles into individual tubes. In this context, this work demonstrates the non-covalent modification and dispersion of pristine multi-walled carbon nanotubes (MWNTs) using two S-layer proteins, namely, SbpA from Lysinibacillus sphaericus CCM2177 and SbsB from Geobacillus stearothermophilus PV72/p2. Both the S-layer proteins coated the MWNTs completely. Furthermore, it was shown that SbpA can form caps at the ends of MWNTs. Reassembly experiments involving a mixture of both S-layer proteins in the same solution showed that the MWNTs were primarily coated with SbsB, whereas SbpA formed self-assembled layers. The dispersibility of the pristine nanotubes coated with SbpA was determined by zeta potential measurements (−24.4 +/− 0.6 mV, pH = 7). Finally, the SbpA-coated MWNTs were silicified with tetramethoxysilane (TMOS) using a mild biogenic approach. As expected, the thickness of the silica layer could be controlled by the reaction time and was 6.3 +/− 1.25 nm after 5 min and 25.0 +/− 5.9 nm after 15 min. Since S-layer proteins have already demonstrated their capability to bind (bio)molecules in dense packing or to act as catalytic sites in biomineralization processes, the successful coating of pristine MWNTs has great potential in the development of new materials, such as biosensor architectures.

Primary hyperparathyroidism (PHPT) due to parathyroid adenoma presenting as recurrent acute pancreatitis is a rare entity. A 17-year-old male presented with recurrent attacks of pancreatitis and was found to have elevated serum calcium and Parathyroid hormone levels, 11.9mg/dL (8.5-10.2 mg/dL) and 396 pg/ml (10-65pg/ml) respectively. USG neck showed a 1.1 x 0.9 cm hypoechoic nodule in the superior aspect of left thyroid lobe. Parathyroid scintigraphy findings were consistent with parathyroid adenoma. After recovery of pancreatitis, surgical excision of the adenoma was done and the histopathological findings confirmed parathyroid adenoma. There were no further recurrence of pancreatitis following the excision.Asian Journal of Medical Sciences Vol.8(1) 2017 98-100

Klebsiella organisms are known to cause a wide range of clinical syndromes in human beings which include pneumonia, urinary tract infection, abdominal infection, surgical site infection, soft tissue infection and infection of intravascular devices. The incidence is higher among immunocompromised individuals and those with chronic debilitating diseases like diabetes, alcoholism etc. This case report is regarding an elderly diabetic male who developed necrotizing fasciitis of leg following local application of steroids. Introduction. Cystic dystrophy of the duodenal wall is a rare complication of the ectopic pancreas that is characterized by cyst/s formation within thickened duodenal wall. Case Outline. A 61?year?old male with recurrent abdominal pain, weight loss (about 25 kg) who had been moderate alcohol abuser and heavy smoker was presented. On ultrasonography, very thickened duodenal wall (2.5 cm), an enlarged head of the pancreas with cyst of 3 cm in diameter as well as dilated pancreatic duct (<6 mm) were seen. Barium meal showed stenosis of the first and second part of the duodenum. CT and endoscopic ultrasound confirmed the ultrasonographic finding. The patient underwent surgery. The pathologic finding was established only on first two portions of the duodenum and limited part of the head of the pancreas along duodenum while the rest of the pancreas was normal. Due to poor general condition, gastrojejunostomy was performed. Although some improvement was evident, the patient did not become asymptomatic, and, therefore, four months later a cephalic duodenopancreatectomy was carried out which made him fully asymptomatic. A year later, the patient was symptom?free and in good health. Histologic examination showed a cystic dystrophy of the duodenal wall in the ectopic pancreas. Conclusion. Unless there are strong contraindications, cephalic duodenopancreatectomy is best treatment of the disease.

Pancreatic B-cell lymphoma is rare; it accounts for 0.2% to 2.0% of extranodal non-Hodgkin lymphoma, and constitutes less than 0.5% of all pancreatic malignancies. Most histologic types of the pancreatic lymphoma are diffuse large B-cell lymphoma, and follicular lymphoma is quite rare. We report here a case of pancreatic follicular lymphoma that was initially detected by acute pancreatitis. This is the first reported case of pancreatic follicular lymphoma presenting with acute pancreatitis. A 71-year-old woman had epigastric and left upper quadrant abdominal pain. Computed tomography (CT) revealed features of acute pancreatitis. After standard therapy for pancreatitis, enhanced CT showed a pancreatic tumor (50 × 35 mm) in the body of the pancreas with gradual enhancement. Endoscopic retrograde cholangiopancreatography and magnetic resonance cholangiopancreatography showed a complete interruption of the pancreatic duct in the body, with mild dilation of the duct in the tail of the pancreas. Endoscopic ultrasonography revealed hypervascularity of the pancreatic tumor. The patient underwent distal pancreatectomy to remove the cause of pancreatitis and to disclose the diagnosis. Histologic examination revealed follicular lymphoma of pancreas. Despite recent improvement in clinical strategies, differential diagnosis between pancreatic lymphoma and pancreatic cancer is still difficult without histologic information. Pancreatic lymphoma should be considered as a differential diagnosis in a patient who initially presents with acute pancreatitis.

Background: Little is known about the etiological risk factors of acute pancreatitis (AP) in Eastern India. Aims and Objective: The aim of this study is to assess current trends etiology of AP in Eastern India. Materials and Methods: A retrospective study with cross-sectional design was undertaken based on available medical records of patients admit between January 2014 and January 2017 with physician-assigned diagnoses of AP at KPC Medical College & Hospital, Kolkata (n = 234). Multivariate analyses were done to identify risk factors, and distribution was compared on the basis gender. Groups were selected on the basis of a working diagnosis. A stratified comparison was done in 3 commonest etiology groups identified: ‘alcohol’, ‘non-alcohol’ and ‘idiopathic’.Results: The commonest etiology was attributed to alcohol (29.4%), idiopathic cause (20.5%), and obstructive cause (14.5%). Prevalence of AP was higher with alcoholism in men (37% vs. 10.8%; p < 00.1), however, other causes like idiopathic cause (12.3% vs. 37.8%), duct obstruction (1.2% vs. 43.2%), obesity (6.1% vs. 24.3%), and hypertriglyceridemia (6.1% vs. 14.8%) were higher among females ( p < 0.001 for all comparisons). Compared to non-alcoholics, patients with idiopathic AP were more likely to be overweight ( p = 0.019) and have T2 DM ( p = 0.021). Moreover, impact assessment of T2 DM status further revealed that the risk of AP was even greater with obesity (Odds Ratio [OR] 1.37; 95% Confidence Interval [CI] 0.57 – 3.26; p = 0.047) and smoking (OR 1.72; 95% CI 1.0 – 2.97; p = 0.049). Eighteen cases (7.7%) of ‘severe’ AP were identified, mostly due to: trauma in 6 (2.5%), idiopathic in 6 (2.5%), gallstones in 4 (1.7%), and alcoholism in 2 (0.8%).Conclusion: Alcohol intake is the predominant etiological risk factor for acute pancreatitis in Eastern India. Gender and type 2 diabetes mellitus are important contributory determinants.Asian Journal of Medical Sciences Vol.8(6) 2017 24-29

: The application of nanotechnology in medicine and pharmaceutical purpose suggested a novel procedure in the nanotechnology terminology as nanomedicine. There is a wide range of applications for nanotechnology in medicine, such as the use of nanocarriers in drug delivery systems. Recently a remarkable attention to DNA has been made through its amazing functionality and its nature as a nanomaterial in biological systems. Since DNA is a biocompatible, the use of DNA as a nanomaterial in medicine has shown a great perspective of rational engineering of DNA nanostructures. According to new approaches in treatment of diseases in gene levels, gene therapy, using DNA as a nanomedicine possesses an important role in the medical sciences as the researchers published enormous papers and patents in the fields, for instance, the applications of DNA and DNA-based nanostructures as drug or gene nanocarriers, DNA-based diagnostics and DNA nanovasccines. Here, some examples of DNA-based nanomedicine in the patent frame were reviewed.

Different biomaterials in the form of ceramics, metal alloys, composites, glasses, polymers, etc. have gained wide-range acceptance in the realm of medical sciences. Bioimplants from such biomaterials have been constructed and used widely for different clinical applications. With the continual progress, biomaterials that may be resorbed inside the body have been developed. These have done away with the major challenge of removal of an implant after it has served its intended function. Important factors are taken into consideration in design and development of implants from such biomaterials are mechanical properties, degradation rate, surface modification, rate of corrosion, biocompatibility, and non-toxicity. Given the importance of such materials in clinical applications, the chapter presents an overview of the bioresorable composites and their implants. The related properties and the functions served have been outlined briefly. Further, the challenges associated and the remedies to overcome them have also been delineated.
