Patient safety incidents type of clinic administration are incidents that occur in the process of patient identification, handover, agreement, informed consent, queuing, referral, admission, discharge of patients from inpatient, transfer of care, division of tasks, response of emergency. Based on patient safety incidents reports of RSU Haji, there were 12 patient safety incidents type of clinic administration (standard 0% or zero accident) during January 2014-June 2016 in inpatient unit. This research aimed to analyze the correlation between nurse’s individual characteristics with patient safety incidents type of clinic administration. This research was an observational descriptive study with cross sectional design. Questionnaires were distributed to 48 nurses that divided into 6 inpatient units. The results of this research showed that the lower the nurse's knowledge about patient safety and patient safety incidents type of clinic administration, the higher the nurse’s work fatigue, the lower the nurse’s motivation toward to the application of patient safety program, the higher the tendency of patient safety incidents type of clinic administration occured in inpatient unit of RSU Haji Surabaya. The conclusion of this research are knowledge, work fatigue, motivation of nurse have a correlation with patient safety incidents type of clinic administration in inpatient unit of RSU Haji Surabaya.  Keywords: patient safety incidents, clinic administration, nurse, hospital

The patient safety mission at the hospital is to prevent patient safety incidents from occurring, the number of incidents is said to be like an iceberg phenomenon. The incidence rate in Indonesia based on data from the Joint Commission International (2012) shows that as many as 13% due to surgical errors and 68% due to blood transfusion errors. This study aims to analyze the relationship between leadership support and compliance with nurses in identifying patients in the implementation of patient safety in the Datu Beru Aceh Hospital. Tengah. The research design used a quantitative analytic survey with a cross-sectional design. The population of all nurses working in the Women's Internal Medicine Room at Datu Beru Hospital, Central Aceh was 34 people. The sampling technique used total sampling. Data analysis was carried out by univariate, bivariate and multivariate with chi_square. The results of the research variable Leadership Support with Nurse Adherence obtained p-value = 0,000, meaning that there is a relationship between leadership support and nurse compliance to identify patients in the application of patient safety. Multivariate analysis of leadership support variables obtained sig value of 0.000 <p = 0.05, which means that there is a significant influence of the leadership support variable on nurse compliance. There is a relationship between leadership support and the compliance of nurses to identify patients in the application of patient safety and there is a significant effect.

Nurses’ knowledge regarding patient safety affects nurses’ behaviors in managing drugs related to patient safety. A preliminary study at Sultan Agung Islamic Hospital Semarang showed that nurses’ knowledge regarding patient safety was still low, especially about drug management, including storing, ordering/prescribing and recording, administering, and monitoring drugs. This study aimed to determine the correlation between nurses’ knowledge regarding patient safety and nurses’ behaviors in drug management among nurses in the inpatient units of Sultan Agung Islamic Hospital, Semarang. This research was an observational study with a cross-sectional design. The population was 178 nurses in the inpatient units of the specified hospital with a total sample of 123 respondents. The data were collected using a questionnaire and analyzed using Spearman’s rank correlation test. The results showed a correlation between nurses’ knowledge regarding patient safety and nurses’ behaviors in storing drugs (p=0.001 <ά=0.05), ordering and recording drugs (p=0.000 <ά=0.05), administering drugs (p=0.001 <ά=0.05), and monitoring drugs (p=0.003 <ά=0.05). It is expected that nurses evaluate the drug management implementation in the inpatient units so that patient safety is guaranteed as well as plan and manage their behaviors in managing drugs in the hospital to ensure patient safety.

Insiden keselamatan pasien masih menjadi masalah utama yang mengancam keselamatan pasien di rumah sakit. Pelaporan insiden keselamatan pasien merupakan   salah satu bentuk upaya yang dapat diterapkan untuk  mengendalikan tingginya insiden keselamatan pasien. Penelitian ini bertujuan untuk mengetahui hubungan antara pengetahuan perawat dan fungsi pengawasan kepala ruangan dengan motivasi perawat dalam pelaporan insiden keselamatan pasien. Penelitian ini menggunakan desain penelitian cross-sectional dan analisis kolerasi. Populasi dalam penelitian ini merupakan perawat pelaksana yang bertugas di ruang rawat inap RSD Mangusada yang berjumlah 93 orang. Penelitian ini menggunakan proportional random sampling dengan sampel berjumlah 76 orang. Teknik analisis yang digunakan yaitu uji kolerasi Pearson dengan tingkat kepercayaan 95%. Hasil penelitian yang dilakukan di RSD Mangusada menunjukan bahwa ada hubungan antara pengetahuan perawat dan fungsi pengawasan kepala ruangan dengan motivasi perawat dalam pelaporan insiden keselamatan pasien (p<0,05). Nilai correlation coefficient menunjukan bahwa adanya hubungan yang sangat kuat dan positif, sehingga dapat disimpulkan bahwa semakin baik pengetahuan perawat dan fungsi pengawasan kepala ruangan maka semakin tinggi motivasi perawat dalam pelaporan insiden keselamatan pasien.   Kata kunci : fungsi pengawasan, motivasi, pengetahuan, perawat   THE CORRELATION OF NURSING KNOWLEDGE AND SUPERVISION FUNCTION OF HEAD NURSE WITH NURSING MOTIVATION IN PATIENT SAFETY INSIDENT REPORTING   ABSTRACT The incidence of patient safety is a major problem that threatens patient safety at the hospital. Reporting incidents of patient safety is one form of effort that can be applied to control the high incidence of patient safety. This study aimed to determine the correlation  between nurse knowledge and the supervisory function of the head of the room with nurses' motivation in reporting patient safety incidents. This research is a quantitative recearch with descriptive correlation method and using cross-sectional design. The population in this study were 93 nurses who served in the treatment room of Mangusada Hospital. the sample was 76 people selected through proportional random sampling technique. Pearson product moment was used to analyzed the data with confidence lovel of 95%. The results of research conducted at Mangusada Hospital showed that there was a relationship between nurses 'knowledge and the supervisory function of the room head with nurses' motivation in reporting patient safety incidents (p<0.05). The value of the correlation coefficient shows that there is a very strong and positive relationship, so it can be concluded that the better the nurse's knowledge and supervisory function of the head of the room the higher the nurse's motivation in reporting patient safety incidents. AbstrakKeselamatan pasien merupakan dasar dari pelayanan kesehatan yang baik. Pengetahuan tenaga kesehatan dalam sasaran keselamatan pasien terdiri dari ketepatan identifikasi pasien, peningkatan komunikasi yang efektif, peningkatan keamanan obat yang perlu diwaspadai, kepastian tepat lokasi, prosedur, dan tepat pasien operasi, pengurangan risiko infeksi, pengurangan risiko pasien jatuh. Tujuan penelitian untuk mengetahui hubungan antara pengetahuan dengan penerapan keselamatan pasien pada petugas kesehatan di Puskesmas Kedaung Wetan Kota Tangerang. Metode Penelitian menggunakan deskriptif korelasi menggunakan pendekatan cross sectional. Populasi sebanyak 50 responden. Teknik pengambilan sampel menggunakan total sampling. Instrumen yang digunakan berupa lembar kuesioner. Teknik analisa diatas menggunakan analisa Univariat dan Bivariat. Hasil Penelitian ada Hubungan Pengetahuan dengan Penerapan Keselamatan Pasien pada Petugas Kesehatan, dengan hasil, p value sebesar 0,013 &lt; 0,05 maka dapat disimpulkan bahwa ada Hubungan Pengetahuan dengan Penerapa Keselamatan Pasien pada Petugas Kesehatan. Knowledge of health personnel in patient safety targets consists of accurate patient identification, increased effective communication, increased safety of the drug that needs to be watched, certainty in the right location, procedure, and precise patient surgery, reduction in risk of infection, reduction in risk of falling patients. The purpose of this study was to determine the relationship between knowledge and the application of patient safety to health workers in the Kedaung Wetan Health Center, Tangerang City. The research method uses descriptive correlation using cross sectional approach. The population is 50 respondents. The sampling technique uses total sampling. The instrument used was a questionnaire sheet. The analysis technique above uses Univariate and Bivariate analysis. The results of the study there is a Relationship of Knowledge with the Implementation of Patient Safety in Health Officers, with the result, p value of 0.013 &lt;0.05, it can be concluded that there is a Relationship between Knowledge and Patient Safety Implementation in Health Officers. Abstract Background When there is a gap in professionals’ adherence to safe practices during cancer treatment, the consequences can be serious. Identifying these gaps in order to enable improvements in patient safety can be a challenge. This study aimed to assess if cancer patients and their relatives can be given the skills to audit reliably four safe practices, and to explore whether they are willing to play this new role. Methods We recruited 136 participants in 2018, from the oncology and haematology day hospital of a tertiary hospital in Spain. Patient identification, hand hygiene, blood or chemotherapy identification, and side effects related to transfusion and chemotherapy, were the safe practices selected for evaluation. The study comprised two parts: an interventional educational program and a cross-sectional design to collect data and assess to what degree participants are able and willing to be auditors depending on their characteristics using multivariate logistic regression models. A participant’s auditing skill were assessed pre and post the educational intervention. Results The model was seeking predictors of being a good auditor. 63 participants (46.3%) were classified as good auditors after the training. To have younger age, higher educational level and to have had an experience of an adverse event were associated with a higher probability of being a good auditor. Additionally, 106 (77.9%) participants said that they would like to audit anonymously the professionals’ compliance of at least three of four safe practices. The willingness to audit safe practices differed depending on the safe practice but these differences did not reach statistical significance. Conclusions The data gathered by patients and relatives acting as auditors can provide healthcare organizations with valuable information about safety and quality of care that is not accessible otherwise. This new role provides an innovative way to engage patients and their families’ in healthcare safety where other methods have not had success. The paper sets out the methods that healthcare organizations need to undertake to enrol and train patients and relatives in an auditor role.

Introduction. Chronic energy deficieny (CED) is a condition of a body characterized by low body weight and low energy stores, possibly limited physical capacity due to deprivation of food over a long period time. Ministry of Health ( Kemenkes) showed that in 2015, 305 out of 100.000 death of pregnant women is realated to malnutriotion and CED. Objective. The aim of this study is to determine the overview of pregnant women nutritional status based on mid-upper arm circumference (MUAC) in Sundari Medan General Hospital. Method. The study was an observational descriptive study with a cross sectional design. The samples of this study consists of pregnant women from Sundari Medan Genaral Hospital, who had fulilled the inclusion and esclusion criteria by total sampling. Results.. Among 60 samples, the prevalance of Non-CED woman (85%) was found higher than the mild malnutrition (15%). Conclusion. Prevalance of CED pregnant women was found higher in risky age, middle educated and high income family.

Background: Patient satisfaction has always been an area of focus for hospitals, but gained particular importance due to the changes in the Centers for Medicare and Medicaid reimbursement policies. Hospital managers and clinicians interact with patients in different ways and have different perspectives on safety culture, yet little is known about how that impacts patient satisfaction.Objective: To examine how the agreement between clinicians and management perspectives on patient safety culture is related to patient satisfaction by employing cross-sectional design with linear regressions.Methods: Two data sets were used: 2012 Hospital Survey on Patient Safety Culture and 2012 Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS). The dependent variables were: overall rating of the hospital experience and willingness to recommend a hospital. The independent variables were four safety culture domains: communication openness, feedback, and communication about errors, teamwork within units, and teamwork between units.Results: The results suggest that manager and clinician agreement on high levels of communication openness, feedback and communication about errors, teamwork between units, and teamwork across units were positively and significantly associated with overall patient satisfaction and willingness to recommend. Additionally, more favorable perceptions about patient safety culture by only clinicians yielded similar findings.Conclusions: For policymakers, measuring managers and clinicians’ perceptions on patient safety culture may provide a valuable indicator of patient satisfaction throughout the country. While managers are more likely to have the power to devote resources to patient safety initiatives, they may not adequately judge culture in their unit and should take into account the perspectives of clinicians who have a more frontline perspective.

Pemberi layanan kesehatan harus mengutamakan keamanan pasien sebagai perioritas. Sebagai tenagakesehatan dengan jumlah terbesar, perawat mempunyai andil besar dalam meningkatkan budayakeselamatan pasien di rumah sakit. Penting untuk mengkaji penerapan budaya keselamatan pasien padaperawat, sehingga hasilnya dapat dijadikan acuan dalam melakukan perbaikan.Penelitian descriptivecomparative cross sectional design dilakukan kepada 50 responden perawat dari instalasi gawat darurat,Hemodialisa, Instalasi pasien rawat jalan dan ruang operasi dengan menggunakan instrument HospitalSurvey of Patient safety Culture (HSOPCS) yang terdiri dari 12 dimensi. Uji statistic Mann Whitneydigunakan untuk mengetahui apakah terdapat perbedaan penerapan antara staff pelaksana dan incharge,antara head nurse dan kepala ruangan.Hasil penelitian menunjukkan nilai rata-rata respon positif dari 12dimensi yang diberikan oleh staf pelaksana (74,6%). Ada 5 dimensi yang perlu di tingkatkan yaitupersepsi tentang keselamatan pasien secara menyeluruh (70,5%), harapan dan tindakan manajer dalammeningkatkan keselamatan pasien (65,7%), respon tidak menghukum terhadap kesalahan (48,6%),staffing (70,5%) serta overan dan transisi (65,7%). Sementara incharge mempunyai nilai rata-rata (79,4%). Ada 4 dimensi yang perlu ditingkatkan yaitu frekuensi pelaporan insiden(66,7%), persepsi tentangkeselamatan pasien secara menyeluruh (55,6%), harapan dan tindakan manajer dalam meningkatkankeselamatan pasien (41,7%),dan staffing (66,7%). Pada head nurse nilai rata-rata respon positif dari 12dimensi (76,7%). Ada 5 dimensi yang perlu ditingkatkan yaitu persepsi tentang keselamatan pasien secaramenyeluruh (62,5%), harapan dan tindakan manajer dalam meningkatkan keselamatan pasien (65,6%),respon tidak menghukum terhadap kesalahan (50%), kerjasama tim antar unit (68,8%) serta overran dantransisi (56,3%). Pada kepala ruangan nilai rata-rata respon positif dalam 12 dimensi (88,6%). Ada 1dimensi yang perlu ditingkatkan yaitu respon tidak menghukum terhadap kesalahan (66,7%). Terdapatperbedaan yang signifikan antara staf pelaksana dengan incharge, antara head nurse dengan kepalaruangan dalam penerapan budaya keselamatan pasien. Dengan meningkatkan penerapan budayakeselamatan pasien diharapkan perawat dapat memberikan perawatan yang komprehensif, berbasisevidence dan berpusat pada kebutuhan pasien sehingga keselamatan pasien dirumah sakit dapat tercapai.Kata Kunci: Budaya keselamatan pasien, Perawat, HSOPSC
