Significance. Due to a widespread use of the «lean manufacturing» concept in health and lack of skills in applying lean technologies in practice among health specialists, educational institutions are challenged to modernize content of the levels of professional training of nurses. Purpose. To identify major directions for modernizing levels of professional training of nurses as a means of forming specialists of the new formation. Material and methods. The study was conducted on the basis of the Arzamas Medical College and health care facilities of Arzamas that provide outpatient health care. Students in Nursing (qualification «nurse» / «medical brother») in the second and third year (312 people) and practicing nurses (123 people) were selected as the respondents. Special questionnaires were developed. Conclusions. The study results substantiate the need to update the training content at all levels of professional education of nurses - both basic and postgraduate. Modernization of the content of the levels of training should incorporate training in innovative lean manufacturing, in improving motivation and readiness for innovations in professional activities, in developing initiative and responsibility for personal performance and organizational performance as a whole into major professional educational programs for nurses and programs of advanced training of nurses. The practical results of the study made it possible to identify major directions for modernizing levels of professional training of nurses and served basis for the development of the «Model for developing competencies in «lean manufacturing» tools on the basis of a medical college». Scope of application. The data obtained in the course of the research can be used as additional information in realizing the basic professional educational program in the specialty 34.02.01 «Nursing», as well as in developing training programs for nurses at primary health care settings.

Abstrak Bidan sebagai tenaga kesehatan strategis yang berperan dalam pelayanan kesehatan ibu dan anak dituntut memiliki kompetensi tinggi untuk dapat menjalankan fungsinya dengan baik. Kompetensi yang tinggi dapat tercapai bila penyelenggara pendidikan profesi bidan memenuhi standar penyelenggaraan pendidikan. Berdasarkan data Majelis Tenaga Kesehatan Indonesia (MTKI) tahun 2016, nilai rata-rata uji kompetensi DIII kebidanan hanya 41,08. Peserta uji kompetensi yang belum lulus sebanyak 46,5%. Hasil yang masih jauh dari harapan juga ditunjukkan dari rerata try out uji kompetensi tenaga kesehatan tahun 2012 hingga tahun 2015 yang cenderung menurun. Kajian ini bertujuan untuk mendapatkan informasi identifikasi kompetensi bidan berdasarkan Kepmenkes 369/MENKES/SK/III/2007 tentang standar profesi bidan pada hasil Risdiknakes tahun 2017. Kajian dilakukan menggunakan observasi, wawancara mendalam dan literatur review. Informan adalah bidan di puskesmas dan pakar kebidanan. Hasil kajian menunjukkan bahwa kompetensi bidan di fasilitas pelayanan kesehatan masih belum sesuai standar. Beberapa faktor dalam penyelenggaraan pendidikan kebidanan turut membentuk kompetensi bidan yang dihasilkan. Proses rekrutmen calon peserta didik, kualitas dosen, dan proses penyelenggaraan pendidikan kebidanan secara keseluruhan merupakan komponen yang harus menjadi fokus untuk menghasilkan bidan yang sesuai dengan standar kompetensi seperti tercantum dalam Kepmenkes Nomor 369/MENKES/SK/III/2007. Kata kunci: kompetensi bidan, kajian kebidanan, pendidikan bidan, kurikulum kebidanan Abstract Midwives are strategic health workers who play an important role in maternal and child health services. They are required to have well competencies to run their tasks properly. Well, competencies can be achieved if the midwife's professional education providers meet the standards. Based on the Indonesian Health Workers' Assembly (MTKI) data in 2016, the average value of the DIII midwifery-competency test was only 41.08. Participants who failed the competency test were as much as 46.5%. It is still far from the expectation as the average value of health workers’ competency tests try out between 2012 to 2015 tends to decline. This study aims to identify midwife competencies based on Minister of Health's decree No. 369/MENKES/SK/III/2007 on midwives' profession standards and the results of the 2017 Research on Health Workers’ Education (Risdiknakes). The study was conducted using observation, in-depth interviews, and literature review. Informants are midwives at primary health care and midwifery experts. The results of the study indicate that midwife competencies in health care facilities are still not up to standard. Several factors in the administration of midwifery education also shape the competence of the midwives produced. The process of recruiting prospective students, the quality of lecturers, and the process of conducting midwifery education as a whole are components that must be the focus to produce midwives that comply with the competency standards in Minister of Health's decree No. 369/MENKES/SK/III/ 2007. In the article on the basis of scientific and pedagogical sources the ways of perfection of the system of professional education with the purpose of preparing the future specialist for the practical realization of the idea of health saving of the nation at all stages of educational activity are analyzed and solved; the general structure of the system of quality assurance of education at the higher educational institution level is substantiated; determined the effective use of didactic principles; new approaches to the development of content, forms and methods of professional training of specialists in the new formation; the content of professional training of future specialists is investigated, covering the set of knowledge, skills and abilities, possession of which enables to work for the chosen type of profession; The set of principles, functions and factors that influence the content of the training is outlined in the curricula and programs of higher education institutions. An analysis of the structure of training of future specialists in higher education institutions is presented, which predicts mastery of a complex of professional and personal components of a future specialist. The peculiarities of preparation of future specialists for the implementation of health-saving technologies in the educational programs offered by educational establishments have been clarified. Types of variety of industrial and research practices are shown, forms and methods of teaching are named. The content of the professional training of future specialists is characterized, the prerequisites for creating the concept of forming the future specialists' willingness to carry out innovative activity are outlined; conditions of effective use of health-saving technologies are formulated, essential characteristics and stages of preparation of future specialists in educational establishments are explained; The basic provisions are substantiated, the observance of which ensures the training of new formation specialists for the implementation of health-saving technologies. It is noted that the capacity for self-organization is the basis of successful professional activity of the future specialist and the educational disciplines of the programs of preparation of domestic higher education institutions are observed, the study of which contributes to the formation of the competence of selforganization. The essence of the problem under consideration and the technology of its solution are revealed.

It is clear that the modern role of both the physicians, in general, and psychiatrists, in particular, requires intensive decision-making which is helped by management principles. Psychiatrists, in addition to their clinical qualifications and skills are asked to occupy positions and undertake responsibilities as clinical executives, directors of health care facilities, administrators of Academic units and even Mental Health Commissioners, all of them requiring managerial knowledge and leadership qualities. Increased pressure by patients for improved quality of services and access to new and innovative treatments needs to be balanced against the expectation of the health care system of the physician to act ‘economically’, following cost containment guidelines and staying within expenditure ceilings. Such decisions require specialized knowledge and a deep understanding of the principles and the functions of management and health economics. Such knowledge is only gained through specialized training by introducing management teaching, either at the undergraduate level or preferably at the residency level, as part of the core curriculum or as an elective which may include items such as administration principles, quality assurance, budgeting, resource allocation, accreditation procedures and what is close to the psychiatrist's clinical background the personnel management. This may be extended to ongoing professional education programmes for psychiatrists who are already active in the field.

Several approaches to organizational renewal have been described, but few are reported for health care institutions in Canada. In contrast, approaches to strategic planning in health care facilities have been well documented. From our experience over the past six years, the theory and practice of organizational renewal complement the focused activities of strategic planning. This combination can be an effective means to enhance organizational performance, employee commitment and a shared vision among the various stakeholders within the hospital and community. This article outlines the process and benefits that can accrue through such efforts. It demonstrates how the investment of organizational renewal strategies can produce sustainable, operational and strategic planning benefits for community hospitals.

Studies on developments in the Brazilian civil construction report there is still a disconnection between the design phase and implementation in addition to a lack of management and coordination of effective design. This frequent dissociation compresses time and cost of project development that usually can be understood as an isolated instrument. In this context, academic education significantly influences the practices exercised by professionals in the labor market and the fragmented form of education currently adopted does not provide a collaborative environment, affecting the integration of the various agents involved in the development of the project and during the execution of the building. Thus, the main objective of this study is to evaluate the influence of vocational training in project management activities by applying a case study of a large project. As a result, it has been found that the company studied could better manage and coordinate the design process. Similarly, it could reduce project costs and project development time if it had applied effective risk management to avoid negative impacts and losses. The study results indicate that management concepts provide important inputs to professional training of construction agents.

Due to high prevalence of rheumatic diseases, which are often characterized by a chronic, progressive course and early patient disability, the problem of health care quality in rheumatology remains extremely relevant. The purpose of the study was to analyze patient satisfaction with availability and quality of rheumatological outpatient care provided by health care facilities of the Republic of Bashkortostan. A survey of 289 adult patients who visited a rheumatologist for outpatient care at health care facilities of the Republic of Bashkortostan was conducted from May,1 to December,1, 2019 The study results show that in general patients expressed high satisfaction with the services provided and conditions of health care facilities (77% and 78%, respectively). At the same time, satisfaction with doctors’ professional skills was moderate (54%), while satisfaction with the organization of making a rheumatological appointment and official website of the facility was low (37% and 29%, respectively). In general, the data obtained indicate that patients highly appreciate quality of rheumatological outpatient care in the Republic of Bashkortostan. However, the need to further improve professional competencies of doctors, organization of scheduling appointments and official websites of health care facilities call for a special attention.

The major purpose of the system of training and continuous professional development is training of competent professionals capable to adequate response to challenges of health care. In health care, the system of training and continuous professional development of administrative and managerial personnel is to be implemented through competence approach. The employer determines working functions and working conditions. The educational organizations determine level of knowledge and skills. The competences are to be a connecting link between them. The successful process of formation and development of competences is needed in designing model of key and professional competences and upgrading actual trajectory of supplementary professional education of health care administrators. The training of health care administrative personnel is to be targeted to mastering integrated standard professional competences which are to be come the basis of educational programs of supplementary professional education in all educational organizations. The development of each competence requires different approaches, means and methods of training which are to be selected by educational organizations independently with observance of main principles of adragogics. The selection of the most effective technologies of education for development of each competence plays an important role in the system of training of health administrative personnel. The practice of common programs of supplementary professional education for health administrators with different set of working functions is to be eradicated. It is appropriate to return to successful national practice of differentiated programs of supplementary professional education addressed to every administrative position. The purpose of concluding qualifying evaluation is to become estimation of level of mastering of competences required in administration activities. The concluding qualifying evaluation is to be carried out by educational organizations conjoint with professional community. The subsequent “monitoring” of administrators is to become as an obligatory component of continuous professional development of health administrators. The consulting by educational organization is an effective form of continuous professional training.

Background: The introduction of Federal state standards of higher education has caused the need to study the quality and condition of specialists postgraduate training in the field of health. In 20172019 at St. Petersburg state pediatric medical university a study aimed at examining the satisfaction of residents with educational training at the faculty of postgraduate and additional professional education was hold, the results of which allowed to identify the problem areas of training in order to further adjust the content and organizational aspects of the quality of training of highly qualified specialists in the field of health protection of citizens of Russia. Aims: The estimation of the quality of postgraduate training of specialists in the field of health care based on the pedagogical reflection of their satisfaction with the process of training during the residency. Methods: The study was conducted by means of a questionnaire consisting of a number of questions stated in the form of appeals aimed at obtaining information illustrating the content, organizational, operational and personal resources of postgraduate education of residents. The respondents were asked to rank their responses according to their satisfaction with the quality of education on the scale that ranged from excellent to very poor. The object of the study was 125 graduates completed their professional training at the stage of postgraduate education in residency at various clinical departments. The duration of the study, including the preparation of analytical materials, the organization, and conduct of the survey, the subsequent analysis of the data, totally amounted to two years. The analysis of the survey results allowed to identify the targets of possible correction of informative, organizational, operational and personal aspects of postgraduate training of doctors. Results: The participants of the study were 125 respondents-resident physicians who completed the educational trajectory in various areas of professional training on the basis of clinical departments of pediatric medical university. It should be noted that the distribution of participants in the questionnaire by graduate clinical departments was characterized as uneven, due to the peculiarities of the plan of admission to the residency, social order from the health care institutions of Russia, and finally, individual educational needs and needs of students. The average age of residents is 26 years. As a result of the study, it was possible to identify informative, organizational and personal problem areas of training of residents. In particular, there were problems in the content of professional education, in the level of pedagogical skills of the teaching staff of clinical departments, in the development of professional competencies of residents, in the content of test tasks in certain specialties. Due to the factor analysis of the results, a semantic space was built that reflects the analysis of the satisfaction of graduates of Spbsmu with the quality of training at the stage of residency. Conclusions: Based on the results of the study, it was possible to identify the targets of correctional work, involving the improvement of organizational, informative, technological and personal resources of the educational program of postgraduate training of residents.

Abstract Utilization of health care facilities for child delivery is associated with improved maternal and neonatal outcomes, but less than half of mothers use these for child delivery in Nigeria. This study investigated the factors associated with facility delivery in Nigeria, and their variation between the Northern and Southern parts of the country – two regions with distinct socio-cultural make-ups. The study included 33,924 mothers aged 15–49 who had given birth in the last 5 years preceding the 2018 Nigeria Demographic and Health Survey. Overall, higher age, being educated, being a Christian, being an urban resident, being exposed to mass media, making joint decisions with partner on health care, beginning antenatal visits in the first trimester and attending antenatal clinics frequently were found to be associated with improved use of a health care facility for child delivery. An average mother in Northern Nigeria had a 38% chance of having a facility-based delivery, whereas the likelihood in the South was 76%. When other factors were adjusted for, age and listening to the radio were significant predictors of facility-based delivery in the South but not in the North. In the North, Christians were more likely than Muslims to have a facility-based delivery, but the reverse was true in the South. Rural women in the South had a 16% greater chance of having a facility-based delivery than urban women in the North. The study results suggest that there is inequality in access to health care facilities in Nigeria, and the differences in the socio-cultural make-up of the two regions suggest that uniform intervention programmes may not yield similar results across the regions. The findings give credence to, and expand on, the Cosmopolitan-Success and Conservative-Failure Hypothesis.

Background: As per the WHO estimates, Slums are home to an estimated 8280 lakh people, representing around one third of the world’s urban population. Health is a major economic issue for slum residents. Objective of the study was to assess utilization of health care facilities by slum dwellers with special reference to maternal and child health.Methods: A list of all slums and the number of households therein was collected from city municipal corporation Shivamogga of which 10 slums were selected by simple random sampling all the households in the selected slums were included in the study. Quantitative data were collected from the households of selected slums after taking informed consent using a pretested semi-structured questionnaire. Information about health care seeking with reference to maternal and child health was obtained by checking the available medical records and as self-reported by the respondents. Data were analyzed by using SPSS software.Results: In this study, among the 339 households studied 93.5% of the women had more than 4 antenatal visits and 99.1% of the deliveries were institutional, the most frequently contacted healthcare facility was government medical college.  Conclusions: The availability of better healthcare facilities like government medical college in the proximity of these slums has led to good health seeking behaviour among them.
