Pain Perception in Clarinetists with Playing-Related Pain After Implementing a Specific Exercise Program

BACKGROUND: Wind musicians suffer injuries resulting from muscle overuse and poor postural habits, often due to the lack of required physical fitness. For this reason, it is important to study and analyze the characteristics of their activity in order to select appropriate preventive exercises. METHODS: 10 clarinetists, who were experiencing joint pain due to instrumental practice, followed a specific program of physical activity, 3 times a week for 2 months. To assess postural changes after its implementation, the Langlade test and muscle pain scale were used at the beginning and the end of the program. RESULTS: The results show a significant decrease in perceived pain (p<0.001) and changes in the dorsal spine (p=0.001). Given the relationship between the improvement in the Langlade item, which refers to the correct position of the shoulder blades, and muscle pain, with a correlation level of 0.582, it can be said that a change in the shoulder girdle position leads to a decrease of pain in that area. CONCLUSIONS: In this pilot study of 10 clarinetists, a regular program of physical activity for 9 weeks led to an observable change in posture and a reduction in pain using self-report measures.

Abstract. Background: Acute suicidal affective disturbance (ASAD) has been proposed as a suicide-specific entity that confers risk for imminent suicidal behavior. Preliminary evidence suggests that ASAD is associated with suicidal behavior beyond a number of factors; however, no study to date has examined potential moderating variables.  Aims: The present study tested the hypotheses that physical pain persistence would moderate the relationship between ASAD and (1) lifetime suicide attempts and (2) attempt lethality. Method: Students ( N = 167) with a history of suicidality completed self-report measures assessing the lifetime worst-point ASAD episode and the presence of a lifetime suicide attempt, a clinical interview about attempt lethality, and a physical pain tolerance task. Results: Physical pain persistence was a significant moderator of the association between ASAD and lifetime suicide attempts ( B = 0.00001, SE = 0.000004, p = .032), such that the relationship between ASAD and suicide attempts strengthened at increasing levels of pain persistence. The interaction between ASAD and pain persistence in relation to attempt lethality was nonsignificant ( B = 0.000004, SE = 0.00001, p = .765). Limitations: This study included a cross-sectional/retrospective analysis of worst-point ASAD symptoms, current physical pain perception, and lifetime suicide attempts. Conclusion: ASAD may confer risk for suicidal behavior most strongly at higher levels of pain persistence, whereas ASAD and pain perception do not influence attempt lethality.

This study investigated the relationship of a gap between the intent to be physically active and actual participation in physical activity (‘intention–behavior gap’) and self-efficacy for physical activity during childhood. A self-report questionnaire was used to collect information from 946 children from the fourth and sixth grades in Japan on self-efficacy, intention, and physical activity. Children with an intention–behavior gap (high intent–low activity or low intent–high activity) had higher self-efficacy scores than those with low intent and low activity (27.66 or 27.65 vs. 21.69; p < .001). They had lower self-efficacy scores than those with high intent and high activity (27.66 or 27.65 vs. 30.56; p < .001). Children with an intention–behavior gap had lower self-efficacy for physical activity than those who intended to be and were physically active. Such children may benefit from education interventions that focus on improving self-efficacy.

The purpose of this study was to examine if offspring physical activity may affect the relationship between maternal overweight and offspring fatness and blood pressure (BP). Subjects included 144 maternal-child pairs (n = 74 boys and 70 girls, mean age = 7.3 yrs). Maternal prepregnancy BMI was determined by self-report. Offspring characteristics included resting systolic and diastolic BP, body fatness by dual energy x-ray absorbtiometry, and moderate-to-vigorous physical activity (MVPA) using the Actigraph accelerometer. Children whose mothers were overweight or obese prepregnancy (Prepreg OW) were significantly larger and fatter than children from mothers with a normal prepregnancy BMI (Prepreg NORM). Prepreg OW children also had higher mean arterial pressure than Prepreg NORM children. BP values were not different across maternal Prepreg BMI/MVPA groups. Percent fat was significantly different across Prepreg BMI/MVPA groups. Prepreg OW children that did not meet the daily recommended value of MVPA were the fattest. Prepreg OW children that attained 360 min of MVPA/day had a mean percent body fat that was similar to Prepreg NORM children of either MVPA group.

Lutein is a carotenoid that reduces the risk of some chronic diseases, possibly by altering physical activity behavior. The objective of this study was to conduct a systematic review of studies examining the relationship between lutein status (dietary intake/blood concentration) and physical activity. Peer-reviewed studies published in Medline, Web of Science, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Scopus, and Embase were included if they reported a measure of association between lutein status and physical activity. Seventeen studies met the inclusion criteria. Eleven reported positive associations, three reported mixed results, and three reported no association. Two studies used objective measures of lutein status (blood concentration) and physical activity (accelerometry) and reported positive associations, with correlations of ≥0.36 and differences of ≥57% in physical activity between upper and lower tertiles. Studies using self-report measures reported weaker correlations (r = 0.06 to 0.25), but still more physical activity (18% to ≥600% higher) in those with the highest compared with the lowest lutein status. Higher lutein status may be associated with higher levels of physical activity, which may contribute to a reduced risk of chronic disease.

Background: Positive associations have been observed between acculturation and body mass index (BMI), but the mediators of this relationship are not well established. Acculturation researchers have called for investigating the influence of socio-contextual variables as mediators. The objective of this study was to test the mediating effects of salty snacks, sweet snacks, physical activity, and sedentary behavior on the relationship between acculturation and BMI among Latino adolescents. Methods: Adolescents who self-identified as Latino ( n = 431) at public high schools in Southern California were recruited and completed a self-report survey. A bootstrapped multiple mediation model was used to test mediation pathways. Results: Acculturation was positively associated with physical activity ( B = 0.09, p < .05). The indirect effects of salty snacks, sweet snacks, physical activity, and sedentary behavior were not associated with BMI, suggesting no mediation. Conclusions: Future research should examine additional mediating variables on the relationship between acculturation and BMI.

Abstract. Background: Most people are comfortable asserting the beneficial effects of physical exercise on mental health and well-being. However, little research has examined how different types of physical activity affect these outcomes. Aims: The current study sought to provide a comprehensive understanding of the differential relationships between different types of physical activity and various aspects of health and well-being. In addition, we sought to understand the role of self-connection in these relationships. Method: One hundred forty-three participants completed a questionnaire designed to measure their current weekly activity as well as their current health and well-being. Specifically, we examined three intensities of activity (walking, moderate, and vigorous) and three types of activity (team-based, community-based, and not team nor community-based) on self-reported health, anxiety, depression, affect, flourishing, job satisfaction, life satisfaction, and meaning in life. In addition, we examined self-connection as a possible moderator of these relationships. Results: Results suggested that physical activity was inconsistently related to health and well-being, and activity intensity and type were important to understanding these relationships. In contrast, self-connection reliably related to health and well-being and moderated the relationship between activity type and the presence of meaning. Limitations: The cross-sectional, self-report nature of the study limits its contribution. In addition, we only examined a subset of all physical activities that people engage in. Conclusion: In all, results suggest that the relationships between physical activity, mental health, and well-being are tenuous, at best. Future research needs to examine these relationships further and continue to examine self-connection to determine how to best increase health and well-being through physical activity.

Studies show an inverse relationship between breakfast frequency and weight gain. This may reflect poor eating habits generally and associated low physical activity (PA) or direct impacts of breakfast on mechanisms leading to lethargy and reduced PA. The relationship between breakfast frequency and PA is inconclusive. We aimed to determine whether breakfast frequency is associated with PA levels in British adolescents independent of body composition and socio-economic status (SES). Habitual breakfast frequency (self-report questionnaire) was assessed in 877 adolescents (43 % male, age 14·5 (sd 0·5) years old). PA was measured over 5 d (accelerometry, average counts/min; cpm). Associations between daily PA and breakfast frequency were assessed using linear regression adjusted for body fat percentage and SES. Effect modification by sex and associations with PA during the morning (06.00–12.00 hours) were explored. For boys, there were no significant associations between breakfast frequency and PA. For girls, less frequent breakfast consumption was significantly associated with lower PA (cpm) during the morning (occasional v. frequent β − 6·1 (95 % CI − 11·1, − 1·1), P = 0·017) when adjusted for body fat percentage and SES. There were no associations between PA and breakfast consumption over the whole day; however, for girls, less frequent breakfast consumption may be associated with lower PA levels during the morning, suggesting that breakfast consumption should perhaps be taken into consideration when aiming to promote PA in adolescent girls.

Objectives: This study sought to evaluate the relationship between baseline depression and prospective engagement in walking and exercise behavior after enrollment in an exercise intervention. Methods: The study used baseline, 1-month, 12-month, and 24-month in-person interview and pedometer data collected from Latinos aged >60 years participating in an exercise intervention (¡ Caminemos!) at 27 senior centers ( n = 572). Results: After joining an exercise intervention, and when using continuous pedometer data and scores from the Yale Physical Activity Survey (YPAS) as the outcomes of interest, older adults with baseline depression exhibited comparable levels of physical activity across time when compared to their nondepressed counterparts. Significant difference in physical activity levels between the depressed and nondepressed subgroups no longer existed within one month of initiating the exercise intervention. Discussion: Among sedentary older Latino adults, having depression may not delay exercise initiation nor does it appear to prevent achievement or maintenance of an exercise program.

Objective: Musculoskeletal pain is a common health problem in women, especially during pregnancy and the postpartum period, a possible reason for which is the lack of adequate physical activity. Nonetheless, very few studies have determined the relationship between physical activity level and musculoskeletal pain. Therefore, the aim of this study was to evaluate the relationship between physical activity level and musculoskeletal pain in physician mothers within two years after childbirth. Material and Methods: This study was an online cross-sectional self-report survey. An online questionnaire was administered to physician mothers via the online "Physician Mothers" social group in Facebook, with 3,787 members. A total of 127 participants responded to the questionnaire, which consisted of three parts, namely, sociodemographic information, the International Physical Activity Questionnaire-Short Form, and the Cornell Musculoskeletal Discomfort Questionnaire. Results: The average age of the participants was 32.5±3.5 years. The physical activity level turned out to be inactive in 91 participants (71%), moderate in 30 (24%), and vigorous in six (5%). The most frequent musculoskeletal complaint after childbirth was back pain (n=112, 88.2%), followed by lower back pain (n=109, 85.8%), neck pain (n=70, 55.1%), and hip pain (n=52, 40.9%). There was no significant relationship between physical activity level and musculoskeletal pain in physician mothers (p=0.441). Conclusion: In this study, physician mothers were found to be mostly physically inactive, and their activity level was not observed to be related to their musculoskeletal pain.

AbstractThe purpose of this study was to examine the effect of Dynamic Neuromuscular Stabilization (DNS) exercise on maximum paddling force (PF) and self-reported pain perception in the shoulder girdle area in flatwater kayakers. Twenty male flatwater kayakers from a local club (age = 21.9 ± 2.4 years, body height = 185.1 ± 7.9 cm, body mass = 83.9 ± 9.1 kg) were randomly assigned to the intervention or control groups. During the 6-week study, subjects from both groups performed standard off-season training. Additionally, the intervention group engaged in a DNS-based core stabilization exercise program (quadruped exercise, side sitting exercise, sitting exercise and squat exercise) after each standard training session. Using a kayak ergometer, the maximum PF stroke was measured four times during the six weeks. All subjects completed the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire before and after the 6-week interval to evaluate subjective pain perception in the shoulder girdle area. Initially, no significant differences in maximum PF and the DASH questionnaire were identified between the two groups. Repeated measures analysis of variance indicated that the experimental group improved significantly compared to the control group on maximum PF (p = .004; Cohen’s d = .85), but not on the DASH questionnaire score (p = .731) during the study. Integration of DNS with traditional flatwater kayak training may significantly increase maximum PF, but may not affect pain perception to the same extent.
