Abstract -BACKGROUND: Genu recurvatum in stroke patient hemiplegia causes readily cumulative damage and degenerative changes of knee cartilage. It is important to detect early lesions of cartilage for appropriate treatment and rehabilitation.-PURPOSE: The purpose is to provide theoretical basis for early rehabilitation of hemiplegia patients. -STUDY TYPE: Cross-sectional study. -POPULATION: 39 Stroke patients with genu recurvatum and 9 healthy volunteers. -SEQUENCE: We used zero TE double echo imaging sequence. -ASSESSMENT: Analyze the water content in knee joint cartilage at 12 different sites of stroke patients with genu recurvatum using a method similar to porosity index. -STATISTICAL TESTS: Statistical analysis was performed using SPSS 17.0 statistical software. The mean ± standard deviation was used to represent the mean. The independent sample t test was used for all mean comparisons. When the data did not conform to the normal distribution or variance heterogeneity, the non-parametric test was used. P< 0.05 was considered statistically significant.-RESULTS: When compared hemiplegia limb vs. non-hemiplegia limb in patients, the ratio of deep/shallow free water content of the cartilages at the junction of the femur and anterior horn (1.16 vs. 1.06) and posterior horn (1.13 vs. 1.25) of lateral meniscus were significant differences (P<0.05). -DATA CONCLUSION: Conclusion is that Genu recurvatum in stroke patients with hemiplegia can cause changes in moisture content of knee cartilage, and the changes of knee cartilage are more obvious with the increase of genu recurvatum. The so-called "healthy limb" is no longer the real meaning of healthy limb, and should be considered simultaneously with the affected limb in the development of rehabilitation treatment plan.

The coronavirus disease 2019 (COVID-19) pandemic has caused millions of deaths worldwide. Acute ischemic stroke is a life-threatening risk factor for COVID-19 infection. Neutrophil-to-lymphocyte ratio (NLR) is one of the predictors of poor prognosis in acute ischemic stroke. The aim of this study was to assess the correlation between NLR values and the clinical outcome of acute thrombotic stroke patients with COVID-19 that was measured using the National Institutes of Health Stroke Scale (NIHSS). A cross-sectional hospital-based study was conducted in Dr. Soetomo General Hospital Surabaya, Indonesia. Patients with acute thrombotic stroke and COVID-19 admitted between 1 March 2020 and 31 May 2021 were recruited. The NLR values and the NIHSS scores were assessed during the admission and the correlation between NLR and NIHSS scores was calculated. This study included 21 patients with acute thrombotic stroke and COVID-19, consisting of 12 males and 9 females. The mean age was 57.6 years old. The mean NLR values was 8.33±6.7 and the NIHSS scores ranging from 1 to 33. Our data suggested a positive correlation between NLR values and NIHSS scores, r=0.45 with p=0.041. In conclusion, the NLR value is potentially to be used as a predictor of the clinical outcome in acute thrombotic stroke patients with COVID-19. However, further study is warranted to validate this finding.

BACKGROUND Odontogenic infections are recognized as one of the most common diseases in the world. Organisms that cause dental infection and are a part of the oral normal flora include dental plaque bacteria, mucosal surface bacteria, and gingival bacteria. This study aimed to investigate the prevalence of odontogenic infections by involving the facial and cervical spaces in patients referred to Shahid Rahnemoon Hospital from 2014 to 2018. METHODS This was a descriptive cross-sectional study. The sampling method was census and 308 patients with all types of odontogenic infections associated with the involvement of the facial and cervical spaces referred to Shahid Rahnemoon Hospital from 2014 to 2018 were included in the study. Questionnaire was used for data collection. The data was entered into SPSS version 23 software and analysed using statistical tests. RESULTS The mean age of participants was 31.10 ± 14.48; the mean number of hospitalization days was 4.83 ± 2.52 days; the mean body temperature was 37.45 ± 0.54. Of the 308 patients studied, 221 (71.8 %) had no history of systemic disease. Also, 294 (5 %) had no airway involvement. The most commonly prescribed antibiotic was penicillin + metronidazole with a frequency of 54.9 %, The most common tooth that was the source of infection was mandibular tooth no. 6 with a frequency of 22.7 %, the most common type of treatment was incision + drainage + antibiotic therapy with a frequency of 44.2 %; the most common involved area was the submandibular + buccal space with a frequency of 20.1 % and the most common age range of involvement was 20 - 29 years. CONCLUSIONS In odontogenic infections, penicillin + metronidazole is the most commonly used antibiotic and buccal + submandibular space is the most common area involved. Objective: Marital satisfaction is considered as satisfaction with a marital relationship on which the presence of a child has different effects. Concerns about a childfree life and its effect on marital satisfaction in infertile couples are very critical. Therefore, this study was intended to characterize and compare concerns about a childfree lifestyle and the need for parenthood and their relationship with marital satisfaction in infertile couples. Method: A total of 200 men and 200 women who referred to fertility centers in Tehran participated in this cross-sectional study. Convenience sampling method was used for sampling. Demographic survey, ENRICH Marital Satisfaction Scale, and Fertility Problem Inventory were used for data collection. The resulting data were analyzed using descriptive and analytical statistical tests (Pearson Correlation Coefficient and Stepwise Regression). Results: The mean scores for concern about a childfree lifestyle and the need for parenthood in women were significantly higher than in men. The variables rejection of a childfree lifestyle and the need for parenthood were respectively predictors of marital satisfaction in women and men. Conclusion: Since marital satisfaction in infertile couples is affected by their feelings about having a child and becoming a parent, it is therefore suggested that appropriate counseling be provided in supportive healthcare programs for infertile couples to promote their marital satisfaction.

Introduction: Limits of movement in knees is one of the most common complications following trauma and surgery. With early diagnosis and appropriate treatment choices, knee replacement and performance improvement are expected in many patients. However, limits of movement in knees is unpleasant and should be corrected as soon as possible. In this study, we decided to evaluate the results of arthroscopic release in motion limits of knees. Methods: This was a descriptive cross-sectional study.14 patients who referred to the Shahid Sadoughi Hospital of Yazd from 2014 to 2016 and diagnosed with knee limitation (flexion or extension restriction of more than 10 degrees) were selected. Patients were undergone arthroscopic release and followed-up for 6 months. Lysholm knee score, degrees of flexion and extension, and patient satisfaction were evaluated. The results were analyzed by SPSS-18 and statistical tests Paired T-test as well as Spearman correlation. Results: The mean age of the patients was 28.92±10.99 years. The most common type of trauma in the patients was femoral distal fracture (46.7%). The frequency of the patients' satisfaction with arthroscopy was as followed: 50% (7 patients), had moderate satisfaction, 28.6% (4 ones), high satisfaction, and 21.4% (3 ones) had low satisfaction.The mean difference in Lysholm Knee Score, improvement in degree of extension, improvement in degree of flexion and pain score before and after arthroscopy were 26.21±14.11,64.78±26.13,3.57±2.92 and 4.13±1.65,respectively.There was a significant difference between the mean values of these factors before and after arthroscopy. Conclusion: According to the results obtained, we can conclude that arthroscopic release is an effective and low-complication method in motion limits of knees.

Breast feeding (BF) is known to have many advantages but our Arab region has a pattern similar to western countries; where exclusive breastfeeding less than 35%. Knowledge given to women during her antenatal visits plays an important influential role in her attitude and practice of BF later on.  Objective: To explore knowledge and attitude of female medical students at Taibah University about breastfeeding.  Methods: A Cross sectional study was carried out from October to November 2012. It included female medical students. Stratified sampling allocation technique was used; with a total sample of 149 students. A specialty designed self-administrated questionnaire in Arabic form was used. It included socio-demographic data, obstetric history, data about the knowledge and attitude of female medical students towards breastfeeding value and guidelines. Mean percent knowledge score for value of BF, mean percent score for BF guidelines and mean percent score for attitude were calculated. Appropriate statistical tests for qualitative and quantitative data were used accordingly. Results: Majority (91.9%) (of the studied sample) was never married and 37.6% got their knowledge about BF via books. Regarding ever married women, 58.3% only were gravid (mean =2.2±1.94). Only 25.0% were family planning users; 66.7% of them used hormonal contraceptives. Only 20% breast fed their infants with a mean duration for exclusive BF of 3.8 ± 2.87 months and mean age of weaning of 2.3 ±0.63 months. The mean knowledge percent score for guidelines of BF was 64.7±8.45. The mean attitude percent score for concepts related to BF was 76.9±7.91. Conclusion: Media and internet have minor roles in getting knowledge about BF. The mean knowledge score about advantages of BF, guidelines of BF and attitude towards BF of all female medical students; especially academic years were unsatisfactory.

Introduction: Mental workload is one of the effective factors on the performance and efficiency of people working in an organization that put their health at risk. The aim of this study was to determine the mental workload of staffa working in a bank in Hamadan City of Iran  by NASA-TLX method. Methods: In this cross-sectional study, 110 bank employees were surveyed. The data were collected using demographic and NASA-TLX questionnaires. Finally, collected data were analyzed using SPSS version 21 by running descriptive statistics and statistical tests. Results: According to the participants, the mean and standard deviation of effort and mental demand, as the most important dimensions were 81.04 ± 14.51 and 79.50 ± 18.05, respectively. The performance dimension was selected as the least important dimension with the mean and standard deviation of 64.37 ± 10.17. The results of statistical analysis showed no significant relationship between the participants' total score of mental workload and their demographic information such as age, gender, educational level, marital status, experience, and work overtime(P>0.05). The Pearson correlation coefficient showed that age (P<0.05) and work experience (P<0.01) had a significant and inverse relationship with temporal demand dimension . Conclusion: Based on the findings, the mean of perceived mental workload of bank employees was high. Therefore, effective programs should be conducted to mitigate and reduce the mental workload and to promote mental health of bank employees.

Introduction: Biopsy is the most accurate way to make a definitive diagnosis, based on which the prognosis and treatment plan can be determined. This study aimed to evaluate the knowledge, attitude, and practice of general dentists and dental students about the principles of biopsy. Materials & Methods: This descriptive-analytical and cross-sectional study conducted on 188 general dentists and 93 senior dental students of Isfahan University of Medical Sciences using a questionnaire. Then data were analyzed using Pearson correlation coefficient, T-test) in all tests, the significance level is considered 0.05). Results: The mean score of dentists’ knowledge was 4.62 ± 1.42 and, the mean score of students’ knowledge was 4.96 ± 1.77 out of 8 points. The knowledge of dentists who had participated in retraining courses was higher than that of dentists who had not attended any biopsy retraining and was statistically significant (p value > 0.001).The mean score of dentists’ attitude (39.6 ± 4.06) was higher than the students’ (37.05 ± 3.06). Conclusion: Level of knowledge and attitude of dentists and students about the principles of biopsy was moderate. Their performance was also poor and most of the dentists expressed a lack of necessary skills for not doing a biopsy. This study shows the need for educational programs related to the principles of biopsy, in the form of retraining courses and more emphasis on the student curriculum.

Background Recent evidence supports that most non-lacunar cryptogenic strokes are embolic. Accordingly, these strokes have been designated as embolic strokes of undetermined source (ESUS). Aims We undertook an international survey to characterize the frequency and clinical features of ESUS patients across global regions. Methods Consecutive patients hospitalized for ischemic stroke were retrospectively surveyed from 19 stroke research centers in 19 different countries to collect patients meeting criteria for ESUS. Results Of 2144 patients with recent ischemic stroke, 351 (16%, 95% CI 15% to 18%) met ESUS criteria, similar across global regions (range 16% to 21%), and an additional 308 (14%) patients had incomplete evaluation required for ESUS diagnosis. The mean age of ESUS patients (62 years; SD = 15) was significantly lower than the 1793 non-ESUS ischemic stroke patients (68 years, p ≤ 0.001). Excluding patients with atrial fibrillation ( n = 590, mean age = 75 years), the mean age of the remaining 1203 non-ESUS ischemic stroke patients was 64 years ( p = 0.02 vs. ESUS patients). Among ESUS patients, hypertension, diabetes, and prior stroke were present in 64%, 25%, and 17%, respectively. Median NIHSS score was 4 (interquartile range 2–8). At discharge, 90% of ESUS patients received antiplatelet therapy and 7% received anticoagulation. Conclusions This cross-sectional global sample of patients with recent ischemic stroke shows that one-sixth met criteria for ESUS, with additional ESUS patients likely among those with incomplete diagnostic investigation. ESUS patients were relatively young with mild strokes. Antiplatelet therapy was the standard antithrombotic therapy for secondary stroke prevention in all global regions.

Background: Coronavirus disease 2019 (COVID-19) pandemic can lead to psychological distress among clinicians and medical students due to the possibility of disease transmission and closure of educational centers. Objectives: This study aimed to investigate the extent of psychological distress and coping strategies among clinicians and medical students during the COVID-19 pandemic in Rasht, Iran. Methods: Using the Kessler Psychological Distress scale (K10), this cross-sectional analytical study evaluated psychological distress and coping strategies among 109 clinicians and medical students working in Razi Hospital of Rasht, Iran during the COVID-19 pandemic. The K10 scale has 10 items with a 5-point Likert scale ranging from 0 (never) to 5 (always); the Cronbach’s alpha coefficient was calculated as 0.771. Also, to examine the coping strategies, a 10-item researcher-made checklist was used (CVI = 0.79; CVR = 0.82). The questionnaires were distributed by e-mail from May to June, 2020. After entering the data into SPSS software (version 20), the data were analyzed using appropriate statistical tests. Results: The mean age of the responders was 36.14 years (SD = 10.68). The mean K10 total scores was 12.94 (SD = 7.64). There was a significant negative correlation between age and K10 total scores (r = -0.397, P < 0.001). Single participants had a higher K10 total score compared with married ones (P = 0.003). Furthermore, interns had a higher K10 total score compared with residents (P = 0.012) and faculty members (P < 0.001). Conclusions: Since interns are at a higher risk of psychological distress due to less work experience during the COVID-19 pandemic, continuous monitoring of psychological distress and teaching coping strategies in crisis situations should be included in the training curriculum and educational courses of this group.

BACKGROUND Over the last three decades, prevalence of stroke has been high in India, while the Western countries have witnessed a downward slide. Epidemiological studies suggest that hyperhomocysteinaemia might be a potentially modifiable nonlipid risk factor associated with stroke, in addition to the long-recognized factors like hypertension, diabetes mellitus, hyperlipidaemia and smoking. Hyperhomocysteinaemia occurs due to deviation in the metabolic pathway of methionine, attributed by deficiency of vitamins, enzymes and other factors. The present study was undertaken to assess the proportion of hyperhomocysteinaemia in patients with acute ischemic stroke. We also compared the risk factors associated with stroke and serum levels of homocysteine. METHODS This is a cross sectional observational study conducted in a tertiary care hospital. The sample size was 140. Both male and female consecutive patients of age more than 18 years, with first attack of acute ischaemic stroke admitted in the Department of Neurology were selected. Baseline fasting serum samples were obtained for testing serum homocysteine levels. Statistical tests used were proportion, chi square and logistic regression. RESULTS Among 140 acute ischaemic stroke patients, total homocysteine level was raised in 83.6 % cases. The prevalence of moderate hyperhomocysteinaemia in our study was 65.4 % and intermediate hyperhomocysteinaemia was 17.9 % among stroke patients. The mean (± SD) homocysteine level was 22.75 (± 8.19). CONCLUSIONS A strong association was found between hyperhomocysteinaemia and acute ischaemic stroke. We could not find any significant correlation between total homocysteine level and most risk factors of stroke.
