Vaccination Coverage: Vaccine-Related Determinants & Anthropometric Measures in Children Resident in a Rural Community in Nigeria

Background: Vaccination of children has played a significant role in reducing early childhood morbidity and mortality from vaccine-preventable diseases; however, some factors act as deterrents in achieving adequate coverage in this susceptible population. Aims & Objectives: The study, therefore, aimed to identify vaccine-related determinants of childhood vaccination as well as determine the relationship between childhood vaccination status and body weight, height, and a child’s body mass index (BMI). Methods: The study was conducted using a cross-sectional design in which 608 caregiver-child pair was recruited sequentially by using a two-stage sampling technique. Structured questionnaires based on the SAGE vaccine hesitancy model were used to interview the participants. Elicited data was analyzed and categorical variables were presented in tables and charts as frequencies, while a chi-square test was used to test the association between the independent and dependent variables. Pearson’s correlation analysis was also done to determine the correlation between vaccination status and weight, height, and BMI of children. Result: The study showed that vaccination coverage was suboptimal (70.56%) in children and was below the expected target of 80%. Although a few (183, 30.10%) of the respondents claimed they would prevent the vaccination of their children due to the fear of needles, the majority (87.50%) will be willing to accept more vaccine doses for their children if there were no pain. While factors such as the experience of adverse reaction (X2 = 13.22, df = 2, p<0.001), crying from pain (X2 = 11.33, df = 2, p<0.001) and the scientific evidence of safety (X2 = 34.63, df = 2, p<0.001) were significantly associated with a complete vaccination status, vaccination status was positively correlated with the weight (r=0.160, p<0.001), height (r=0.081, p=0.023) and BMI (r=0.214, p<0.001) of children in the rural community. Conclusion: Vaccination uptake and coverage can be significantly improved in children by designing and implementing interventional programs that target pharmaceutical and vaccine-specific factors acting as barriers in these rural communities.

Tetanus Toxoid (TT) is a routine vaccination activity under Expanded Program on Immunization (EPI) in Bangladesh. This was a descriptive type of cross-sectional study with a sample size of 110, selected purposively by non-probability sampling technique. Data were collected through a pretested questionnaire. The aim of this study was to assess TT vaccination coverage among the women of child bearing age, Rangamati Pourashava. As per survey, among 110 respondents, 29 (26.4%) were of 15-25 years, 52 (47.3%) were of 26-35 years, 25(22.7%) were of 36-45 years & only 4(3.6%) were of >45years. In this study, the major group, 86(78.2%) were married, 17(15.4%) were unmarried & only 7 (6.4%) were widow. Most of the respondents considered vaccination as the way of disease prevention, so that the coverage being 92 (83.6%) in case of TT vaccination. It was found that 70% women received TT vaccine from Government EPI centers followed by 23.6% from NGO facilities and only6.4% from private clinics. Among the respondents, 56.3% respondents had completed their dose schedule of TT vaccine respectively. Most of the respondents had their vaccination card. The percentage was 70 for TT respectively. Among others, 20% failed to follow it because of forgetfulness of doses and 3.34% because of losing their card. So, TT vaccination coverage among the women of child bearing age was satisfactory but it also suggests that more awareness campaign for vaccination is sensational among the mass people of our country. This cross sectional study of fifteen days’ duration was conducted among Married women attending in OPD at Dhamrai Dhaka to know Reproductive Health Practice of Married Women in the Rural Community with a sample size of 100 following purposive sampling technique using pretested semi-structured questionnaire by face to face interview method after taking informed consent. Data were analyzed manually and by using computer. Only 39% respondents were found within age group 20-30 years and 33% completed primary education. Majority the of respondents (79%) were married. This study also found that 39% were housewives and majority (56%) had poor monthly income (TK <10,000). Majority (78%) of respondent’s duration of married life less than or equal to 15 years. This study also revealed that (72%) respondents were found having their first issue less than or equal to 25 years of age. About 40% respondents were found having two children. About 47%, 52% & 25% respondents received advices on healthy diet, personal hygiene and cautions about drug uses respectively during antenatal period. Majority (84%) of respondents were done USG on their routine antenatal care investigation. About 65% respondents delivered their last issue at hospital among them 55% respondents last delivery conducted by qualified doctors. Majority (85%) respondent received TT immunization and 71% respondents were not faced any complication in last delivery and 53% respondents were suffering from problems related to reproductive system and among them abortion (35%), PPH (30%) and perineal tear (20%). Reproductive health practice was still worse among the rural community Effective strategies must be taken to improve reproductive health status of the rural women.

Objective This study describes the prevalence of vaccine hesitancy associated with the Catalan systematic childhood vaccination calendar and some related psychosocial determinants among paediatric primary care nurses in Barcelona (Spain). Methods Cross-sectional descriptive study. In 2017 we invited the paediatric nurses (N = 165) working in Barcelona public primary health centres with paediatric departments (N = 41) to participate. They answered a questionnaire with sociodemographic and behavioural variables: severity and perceived probability of contracting the diseases of the vaccines in the vaccination schedule; safety and protection offered by each vaccine; and beliefs, social norms, and knowledge about vaccines. Outcome variable was vaccine hesitancy, dichotomized into not hesitant (nurses who would vaccinate their own offspring), and hesitant (including those who would not vaccinate them, those who had doubts and those who would delay the administration of one or more vaccines). We performed bivariate analysis and adjusted logistic regression models. Results 83% of paediatric nurses (N = 137) agreed to participate. 67.9% had the intention to vaccinate their children of all the vaccines in the systematic schedule. 32.1% of nurses experienced vaccine hesitancy, especially about the HPV (21.9%) and varicella (17.5%) vaccines. The multivariate analysis suggests associations between hesitancy and low perception of the severity of whooping cough (aOR: 3.88; 95%CI:1.32–11.4), low perception of safety of the HPV vaccine (aOR:8.5;95%CI:1.24–57.8), the belief that vaccines are administered too early (aOR:6.09;95%CI:1.98–18.8), and not having children (aOR:4.05;95%CI:1.22–13.3). Conclusions Although most paediatric nurses had the intention to vaccinate their own children, almost one-third reported some kind of vaccine hesitancy, mainly related to doubts about HPV and varicella vaccines, as well as some misconceptions. These factors should be addressed to enhance nurses’ fundamental role in promoting vaccination to families.

Background: Malnutrition (under and overnutrition) presents significant threats to child health. The co-existence of under and overnutrition in a population is increasingly being described in the literature .Objective: To identify the prevalence of malnutrition among under-five children in Enugu metropolis.Methods: A cross-sectional study of pre-primary school children conducted from January to May 2016. using stratified sampling technique. Caregiver-administered questionnaire was used to obtain relevant information. Weights and heights were measured using a standard weight scale and stadiometer/ length mat respectively. Wasting, stunting, overweight and obesity were determined based on the recommended WHO Growth Standard. Data analysis was done with SPSS version 20.0. The associations between nutritional status on one hand, and categorical variables such as age grouping, sex, socio-economic status, and maternal educationwere determined using chi square. . P-value < 0.05 were reported as statistically significant.Results: Eighteen (2.4%) and 26 (3.5%) subjects were wasted and stunted respectively. Eleven (1.5%) subjects were overweight while another 11(1.5%) subjects were obese. Risk factors for undernutrition were maternal education and low socioeconomic class while risk factor for overnutrition was upper socioeconomic class..Conclusion: There is a low rate of malnutrition in the area of study. However, sustained efforts must continue to prevent further rise and possibly eliminate the scourge of malnutrition.Keywords: Malnutrition, Nigeria.

 Introduction: Acute Respiratory Infection (ARI) is one of the leading causes of morbidity and mortality in under five-year children. Risk factors include age, sex, socio‐economic status, indoor air pollution, passive smoking, lack of basic health services, and lack of awareness. In this study, we aimed to determine the associated risk factors of ARI in children under five-years of age. Methods: We carried out a hospital-based descriptive cross-sectional study in the peak period of of November 2017 to February 2018. The study was conducted in Fishtail Hospital and Research Center (FHRC), Pokhara, Nepal. Non probability, purposive sampling technique was used and a structural interview was taken for data collection. The tool comprised of two main parts. Part I- questions related to socio-demographic variables of mother and baby. Part II- questions related to risk factors of ARI. The collected data was analyzed using Statistical Package for Social Science (SPSS) software, version 16. The Chi Square statistic is used for testing relationships between categorical variables. Results: In total of 302 children visited to FHRC with the symptoms of ARI, only 188 (63%) were diagnosed as ARI. The most common symptoms were fever (42.2%), cough (35.7%), running nose (34.1%), difficulty in breathing (28.5%) and chest in drawing (11.6%). The age of children with ARI ranged from one to sixty month with the mean age of 21.46 ±13.52. Among 188 children, 51% were males whereas 49%were females. There were no statistically differences of children weight at birth among ARI. Our results revealed environmental and social factors associated with ARI. Conclusions: The risk factors significantly associated with ARI were malnutrition, exposure to wood smoke and mosquito coil and contact with person having ARI. Reducing these conditions may reduce the morbidity and mortality associated with ARI in children.

Abstract Background Vaccines are one of our most important tools for preventing outbreaks and keeping the world safe. Most unvaccinated children live in the poorest countries including Ethiopia. Therefore, this study aimed to identify the determinants of vaccination coverage among children aged12–23 months in Ethiopia. Methods A cross-sectional secondary data were obtained from the 2016 Ethiopian Demographic and Health Survey data (EDHS). A total of 1929 children were included. A Multilevel Proportional Odds Model was used to identify the individual and community-level factors associated with child vaccination. Result Among 1, 929 children, only 48.6% (95% CI: 46.3 to 50.8%) were fully vaccinated while 37.8% (95% CI: 35.7 to 40.1%) were partially vaccinated.. The multilevel ordinal logistic regression model reveled that housewife mother (AOR =1.522, 95%CI: 1.139, 2.034), institutional delivery (AOR =2.345, 95%CI: 1.766, 3.114),four or above antenatal care visits (AOR = 2.657; 95% CI: 1.906, 3.704), children of mothers with secondary or higher education (AOR = 2.008; 95% CI: 1.209, 3.334),Children whose fathers primary education (AOR = 1.596; 95% CI: 1.215, 2.096), from the rich households (AOR = 1.679; 95% CI: 1.233, 2.287) were significantly associated with childhood vaccination. Conclusion Child vaccination coverage in Ethiopia remains low. Therefore, there is a need to increase child vaccination coverage by promoting institutional delivery and prenatal care visits, as well as maternal tetanus immunization. Besides, public initiatives needed to improve child vaccination coverage, women’s and husband’s education, poor women, and further advancement of health care services for poor women, housewife women, women living in remote areas should be made to maintain further improvements in child vaccination. Furthermore, policies and programs aimed at addressing cluster variations in child vaccination need to be formulated and their implementation must be strongly pursued.

AbstractObjectiveTo compare mothers’ perceptions of their own infants’ nutritional status with anthropometric indicators of undernutrition.DesignA qualitative study and cross-sectional quantitative survey. The qualitative study involved developing tools to assess mother’s perception. Two methods of verbal description and a pictorial scale were developed. The quantitative survey involved measuring maternal perception and comparing it with the anthropometric measures of weight-for-age Z-score (WAZ) and mid-upper arm circumference-for-age Z-score (MUACZ).SettingA rural community setting in Kenya.SubjectsSeventy-four infants aged between 4 and 6 months, and their mothers, living in rural Kenya were enrolled.ResultsUsing verbal description, the positive and negative likelihood ratios were 3·57 (95 % CI 1·44, 9·98) and 0·69 (95 % CI 0·50, 0·96) respectively for MUACZ<−2; and 4·60 (95 % CI 1·60, 13·3) and 0·67 (95 % CI 0·49, 0·92) respectively for WAZ<−2. Using the pictorial scale, the positive and negative likelihood ratios were 8·30 (95 % CI 1·91, 36·3) and 0·69 (95 % CI 0·52, 0·93) respectively for MUACZ<−2; and 4·31 (95 % CI 1·22, 15·0) and 0·78 (95 % CI 0·61, 1·00) respectively for WAZ<−2.ConclusionsIn a rural community, mothers better identify undernutrition in their infants using a pictorial scale than verbal description. However, neither can replace formal anthropometric assessment. Objective anthropometric tools should be validated for identification of severe acute malnutrition among infants aged less than 6 months.

Background: Hepatitis B, which is caused by the Hepatitis B virus (HBV), is a global health problem that has resulted in high morbidity and mortality with knowledge and awareness about the occurrence and mode of transmissions relatively low among the populace. Therefore, this study examined knowledge on the mode of transmission and preventions of hepatitis B among undergraduate students in Lagos, Nigeria. Methods: The cross-sectional descriptive research was used, and a multi-stage sampling technique was adopted to select three hundred respondents which comprised male and female from three tertiary institutions in Lagos state in 2019. Hepatitis B Knowledge Questionnaire with a reliability index of 0.74 was used for data collection. The descriptive statistics of frequency count and percentages was used to analyse the demographic characteristics of respondents while the chi-square test was used to determine associations between categorical variables Results: The results revealed that a high proportion of the respondents had poor knowledge about the transmission and prevention of hepatitis B.. The result revealed that the p-value ≤ 0.05 was considered statistically insignificant. Therefore, the study revealed that there is no significant relationship between gender and hepatitis B Conclusion: A critical level of public awareness and vaccination coverage, particularly among students, is essential to decrease Lagos' burden.

Background Vaccination policy in France was previously characterised by the coexistence of eight recommended and three mandatory vaccinations for children younger than 2 years old. These 11 vaccines are now mandatory for all children born after 1 January 2018. Aim To study the French population’s opinion about this new policy and to assess factors associated with a positive opinion during this changing phase. Methods A cross-sectional survey about vaccination was conducted from 16 November–19 December 2017 among the GrippeNet.fr cohort. Data were weighted for age, sex and education according to the French population. Univariate and multivariate analyses were performed to identify factors associated with a favourable opinion on mandatory vaccines’ extension and defined in the ‘3Cs’ model by the World Health Organization Strategic Advisory Group of Experts working group on vaccine hesitancy. Results Of the 3,222 participants (response rate 50.5%) and after adjustment, 64.5% agreed with the extension of mandatory vaccines. It was considered a necessary step by 68.7% of the study population, while 33.8% considered it unsafe for children and 56.9% saw it as authoritarian. Factors associated with a positive opinion about the extension of mandatory vaccines were components of the confidence, complacency and convenience dimensions of the ‘3Cs’ model. Conclusions In our sample, two thirds of the French population was in favour of the extension of mandatory vaccines for children. Perception of vaccine safety and benefits were major predictors for positive and negative opinions about this new policy.

Background: Although vaccination coverage against a disease is not exactly the same as the community immunity against that disease, it is undoubtedly directly related to it and provides an estimate of the coherence and efficacy of community health infrastructure. Aims: To evaluate the vaccination coverage of children throughout the Islamic Republic of Iran in 2019. Methods: This was a cross-sectional study. A probability proportional to size cluster sampling method was used and the vaccination data of 8682 children aged 24–35 months were collected in the form of 1447 clusters, each composed of 6 participants. Only valid data, including vaccination card or electronic health files, were used. The results were reported in the form of descriptive tables. Results: Overall, 97.82% of Iranian participants (8068 of 8248) and 90.32% of non-Iranian participants (392 of 434) had received all necessary childhood vaccination by the time of interview. In total, 93.02% of all participants had presentable vaccination cards,; and the immunization history of 535 (6.16%) children was retrieved using their electronic health files. The dropout rate between receiving pentavalent vaccine 1 and pentavalent vaccine 3 was 0.01%. In 29 provinces, vaccination coverage was ≥ 95%. In the other 2 provinces, the figure was 93.30%. Conclusion: Immunization coverage of children aged 24–35 months fully complied with eradication/elimination goals of vaccine-preventable diseases. In 2019, measles and rubella elimination was certified in the Islamic Republic of Iran. However, non-Iranian people with immunization coverage < 95% constitute a high-risk group for possible outbreaks.
