Relationship Among Adherence to The Mediterranean Diet and Anthropometric and Metabolic Parameters in Subjects with Obesity

Background: There is moderate-to-high evidence that the Mediterranean diet prevents increases in body weight and waist circumference in non-obese individuals but less is known about its effects in subjects with overweight and obesity. The present study was focused on exploring the cross-sectional association among the adherence to Mediterranean diet and the most commonly used variables of metabolic and cardiovascular risk factors in a cohort of overweight subjects from a typical Mediterranean region, Apulia, in Southern Italy. Methods:: The study was performed in a cohort of 1214 individuals, all with overweight or obesity but no other clinical condition. We investigated the association among adherence to Mediterranean diet, assessed with the PREDIMED score, and anthropometric parameters [namely body mass index (BMI), WC, waist to height ratio (WHtR) and neck circumference (NC)], fasting serum levels of glucose, insulin, uric acid and lipids (triglycerides, total cholesterol, HDL cholesterol and LDL cholesterol), and blood pressure and insulin resistance, measured by HOMA-IR. Results:: The waist to height ratio was negatively associated to a PREDIMED score ≥7 (p<0.04), whereas HDL cholesterol was positively associated to a PREDIMED score ≥7 (p<0.04) Conclusion: This study suggests that body fat distribution and HDL-cholesterol are the parameters most strongly influenced by MedDiet in Apulian subjects.

Obesity and unhealthy eating habits have been associated with irregular sleep–wake patterns during the week, also known as social jet lag. The Mediterranean diet is a healthy pattern related with a better health and sleep quality. However, potential associations with social jet lag remain unexplored. The aim of this study was to examine whether higher social jet lag is linked to lower adherence to the Mediterranean diet and whether it is associated with BMI (Body Mass Index). This cross-sectional study included 534 young adults (18–25 years). Anthropometric parameters, adherence to the Mediterranean diet, chronotype and social jet lag were studied. Our results revealed that individuals with greater social jet lag showed lower adherence to the Mediterranean diet and had a higher BMI. Among the habits that characterized the Mediterranean dietary pattern, we observed that higher social jet lag was significantly associated with a lower intake of fruits and vegetables, as well as skipping breakfast. Hence, the promotion of regular sleep habits together with healthy dietary patterns should be considered for obesity prevention, especially among young adults.

Chronotype is the attitude of a subject in determining individual circadian preference in behavioral and biological rhythm relative to the external light–dark cycle. Obesity and unhealthy eating habits have been associated with evening chronotype. The Mediterranean diet (MD) is a healthy nutritional pattern that has been reported to be associated with better health and quality of sleep. Thus, the aim of the study was to investigate the association of chronotype categories with adherence to the MD in a population of middle-aged Italian adults. This cross-sectional study included 172 middle-aged adults (71.5% females; 51.8 ± 15.7 years) that were consecutively enrolled in a campaign to prevent obesity called the OPERA (obesity, programs of nutrition, education, research and assessment of the best treatment) Prevention Project that was held in Naples on 11–13 October 2019. Anthropometric parameters, adherence to the MD and chronotype were studied. Chronotype was classified as morning in 58.1% of subjects, evening in 12.8% and intermediate in 28.1%. Our results demonstrated that individuals with evening chronotype, when compared to intermediate (p < 0.001) and morning chronotype (p < 0.001), were more prone to follow unhealthy lifestyle, performing less regular activity and being more frequently smokers. In addition, they showed the lowest adherence to the MD compared to morning (p < 0.001) and intermediate chronotypes (p < 0.001). The lower the chronotype score, the higher body mass index (BMI) values in the whole population (r = −0.158; p = 0.038), thus suggesting that evening chronotype was a common finding in subjects with obesity. In addition, positive correlations of chronotype score with age (r = 0.159; p = 0.037) and PREDIMED score (r = 0.656; p < 0.001) were found. The adherence to the MD, more than the intake of the single food items, was found to predict morning and evening chronotypes. In conclusion, evening chronotype was associated with unhealthy lifestyle and low adherence to the MD. Chronotype score was inversely associated to BMI and positively associated to age and adherence to the MD. Thus, the assessment of chronotype should be taken into account in the management of obesity and in the development of nutritional strategies.

Abstract Background: Obesity has become a global epidemic with a rise in noncommunicable diseases. It is now becoming the problem of low- and middle-income countries such as Nepal. Conventional risk factors are present in a high proportion in the Nepalese population. As a routine surveillance or registry system is absent, the actual burden and trend of obesity and hypertension in Nepal are unknown. Hypertension and other cardiovascular diseases can be prevented by detecting risk factors such as obesity and high blood pressure. A simple anthropometric measurement could be used to determine the risk of hypertension. However, the best predictor of hypertension remains contentious and controversial. We aimed to determine the burden of obesity and hypertension and test the ability to determine hypertension through different anthropometric measurements in hospital outpatients in a low-income setting.Methods: This hospital-based cross-sectional descriptive study was conducted from June to December 2019 among 40-69 year outpatients in a tertiary eye and ENT hospital in a semi-urban area of Nepal among a randomly selected sample of 2,256 participants from 6,769 outpatients visited in Health Promotion and risked factor screening service. We performed a correlation analysis to determine the relationship between anthropometric measurements and blood pressure. The area under the receiver operating characteristic (ROC) curve of body mass index (BMI), waist to height ratio (WHtR) and waist circumference (WC) was calculated and compared.Results: The mean (SD) age of the participants was 51.75 (8.47) years. The overall prevalence of obesity and overweight by BMI was 16.09% and 42.20%, respectively. The overall prevalence of abdominal obesity by waist-to-height ratio was 32.76%, which is higher than obesity by BMI. High waist circumference was observed among 66.76% participants, whereas female participants had a very higher prevalence of high waist circumference (77.46%) and male participants (53.73%) (p<0.001). The prevalence of hypertension among the participants with BMI≥25 kg/m2, WHtR≥0.5 and WC≥ cutoff values was 45.97%, 42.52% and 45.28%, respectively. The overall prevalence of hypertension and prehypertension was 40.67% and 36.77%, respectively. Male participants had a slightly higher prevalence of hypertension (42.72%) than female participants (39.00%). The areas under the curve (AUCs) were significantly higher than 0.5 for BMI (0.570, 95% CI: 0.548-0.592), WC (0.585, 95% CI: 0.563-0.607) and WHtR (0.586, 95% CI: 0.564-0.608). In both genders, the area under the curve was significantly higher than 0.5 (P<0.01). In all age groups, the area under the curve was also significantly higher than 0.5.Conclusion: Waist circumference was both correlated as well as had higher predictive capacity amongst WHtR and BMI and may play a major role in the future diagnosis of HTN in Nepali adults. Regardless of the anthropometric metrics used to measure overweight and obesity, the hospital setting is an opportunity centre to screen for overweight, obesity and hypertension, which are major risk factors for NCDs.

The Mediterranean diet (MD) is a healthy nutritional pattern that has been reported to be associated with better health and sleep quality. Thus, the aim of the study was to investigate whether adherence to the MD is associated with sleep quality in a population of middle-aged Italian adults. This cross-sectional study included 172 middle-aged adults (71.5% females; 51.8 ± 15.7 years) that were consecutively enrolled in a campaign to prevent obesity called the OPERA (Obesity, Programs of Nutrition, Education, Research and Assessment of the best treatment) prevention project that was held in Naples on 11–13 October 2019. Anthropometric parameters, adherence to the MD and sleep quality were studied. Overall, 50.6% of the subjects were good sleepers (the Pittsburgh Sleep Quality Index (PSQI) < 5) while 49.4% were poor sleepers (PSQI ≥ 5). Our results demonstrated that good sleepers, when compared to poor sleepers (p < 0.001) had significantly higher adherence to the MD as assessed by PREDIMED (Prevención con Dieta Mediterránea) score, lower BMI (body mass index) and waist circumference (WC). The higher PSQI, the higher the BMI (p < 0.001) and WC values (p < 0.001), thus suggesting that poor sleep was more common in subjects with obesity. In addition, a negative correlation between PSQI and the PREDIMED score (p < 0.001) was found. to the intake of the cluster of foods enclosed in the MD, rather than the intake of the single food, predicted PSQI. By performing a receiver operator characteristic (ROC) curve analysis, we determined a cut-off value at a PREDIMED score < 9 as the threshold for screening poor sleepers. In conclusion, good sleepers had lower BMI and WC and higher adherence to the MD than poor sleepers. PSQI was positively associated to BMI and WC while it was negatively associated to adherence to the MD. The consumption of the MD dietary pattern rather than the intake of a single nutrient has a beneficial effect on sleep quality. Hence, the assessment of sleep should be taken into account in the management of obesity and promoting adherence to the MD could be a tool to improve SD.

AbstractObjectiveTo examine the association of 24 h urinary Na excretion and Na:K with obesity in Chinese adults.DesignPopulation-based cross-sectional study using a four-stage stratified sampling strategy.SettingShandong Province, China.SubjectsChinese adults (n 1906) aged 18–69 years who provided complete 24 h urine samples.ResultsOdds of obesity increased significantly across increasing quartiles of urinary Na excretion (1·00, 1·54, 1·69 and 2·52, respectively, for overweight; 1·00, 1·20, 1·50, and 2·03, respectively, for obesity; 1·00, 1·44, 1·85 and 2·53, respectively, for abdominal obesity (assessed by waist circumference); and 1·00, 1·28, 1·44 and 1·75, respectively, for abdominal obesity (assessed by waist-to-height ratio); P for linear trend <0·001 for all). In addition, odds of abdominal obesity, but not odds of overweight and obesity, increased significantly with successive Na:K quartiles. Additionally, for each increment in urinary Na excretion of 100 mmol, odds of overweight, obesity, abdominal obesity (by waist circumference) and abdominal obesity (by waist-to-height ratio) increased significantly by 46 %, 39 %, 55 % and 33 %, respectively. Similarly, with a 1 sd increase in Na:K, odds of abdominal obesity (by waist circumference) and abdominal obesity (by waist-to-height ratio) increased significantly by 12 % and 15 %, respectively.ConclusionsThese findings suggest that 24 h urinary Na excretion and Na:K might be important risk factors for obesity in Chinese adults.

Introduction: Central adiposity in children has increased to a higher degree than general adiposity however it is not a routine measurement in clinical practice. We aimed to estimate the prevalence of overweight, obesity, and abdominal fat distribution and observe the prevalence of abdominal obesity among non-obese 6-10-year-old children.Material and Methods: Weight, height, and waist circumference were measured in a sample of 793 children (408 girls). International Obesity Task Force cut-offs were used to define overweight and obesity. Abdominal obesity was defined as waist-to-height ratio ≥ 0.50. Chi-square tests were used to observe the prevalence of the obesity indicators among boys and girls, and the relation between International Obesity Task Force cut-offs and abdominal obesity.Results: The prevalence of overweight, including obesity among children was 21.9% (18.9 – 25.0), 6.1% (4.2 – 8.0) were obese and 21.9% (18.6 – 25.0) had a waist-to-height ratio ≥ 0.50. Girls had significantly higher prevalence of overweight, including obesity compared to boys (χ2 = 4.59, p = 0.03), but no differences were found for abdominal obesity according to children’s gender (χ2 = 3.32, p = 0.07). A proportion of normal (8.2%; 5.9 – 10.6) and overweight children (59.5%; 50.9 – 69.0) were abdominally obese.Discussion: The prevalence of general and abdominal obesity in children living in central Portugal is of concern. Many children with abdominal obesity would not be considered obese with the International Obesity Task Force cut-off points.Conclusion: A high proportion of abdominal obesity was observed in children with normal weight or overweight, suggesting that waist-to-height ratio should be included in routine clinical practice and might be particularly useful to assess the health status of the child.

Much research suggests that Mediterranean eating habits and lifestyle contribute to counteract the risk of chronic diseases while promoting longevity, but little information is available on the effects of the Mediterranean diet (Med-Diet) on thyroid function, particularly among overweight/obese subjects. Nevertheless, consistent data reported a slight increase in serum levels of the thyroid-stimulating hormone (TSH) and a higher rate of conversion of thyroxine (T4) to triiodothyronine (T3) in obesity. This cross-sectional study was aimed at investigating the relationship between adherence to the Med-Diet and circulating thyroid hormones in a cohort of overweight/obese subjects from Apulia (Southern Italy). Methods: We studied 324 consecutive outpatient subjects (228 women and 96 men, age range 14–72 years) taking no drug therapy and showing normal levels of thyroid hormones, but complicated by overweight and obesity (body mass index (BMI) ≥ 25 Kg/m2). The PREDIMED (PREvención con DIeta MEDiterránea) questionnaire was cross-sectionally administered to assess the adherence to the Med-Diet, and hormonal, metabolic, and routine laboratory parameters were collected. Results: Higher adherence to Med-Diet was found to be inversely related to free T3 (p < 0.01) and T4 (p < 0.01) serum levels. Considering each item in the PREDIMED questionnaire, people consuming at least four spoonfuls of extra-virgin olive oil (EVOO) per day, as well as those consuming at least two servings of vegetables per day, had lower free T3 levels (p 0.033 and p 0.021, respectively). Furthermore, consuming at least four spoonfuls of EVOO per day was found to be associated to lower free T4 serum concentrations (p 0.011). Multinomial logistic regression models, performed on tertiles of thyroid hormones to further investigate the relationship with Med-Diet, corroborated the significance only for free T4. Conclusion: Increased adherence to the Med-Diet was independently associated to a slightly reduced thyroid function, but still within the reference range for free T3 and T4 serum levels. This first finding in this field opens up a research line on any underlying biological interplay.

AbstractObjectiveThe waist-to-height ratio (WHtR) assesses abdominal adiposity and has been proposed to be of greater value in predicting obesity-related cardiovascular health risks in children than BMI. The present study aims to develop WHtR cut-offs for overweight and obesity based on the 85th and 95th percentiles for the percentage body fat (%BF) in a cohort of children and adolescents.DesignWaist circumference (WC), height, triceps and subscapular skinfolds were used to calculate WHtR and %BF. Correlations between WHtR and %BF and WHtR/mid-abdominal skinfold were made. Receiver-operating characteristic (ROC) curve analysis was used to select WHtR cut-offs to define overweight and obesity. Subjects were grouped by WHtR cut-offs, and mean values for anthropometry, blood lipids and blood pressure (BP) variables were compared.SettingAustralian primary and secondary schools.SubjectsA total of 2773 male (M) and female (F) subjects of the 1985 Australian Health and Fitness Survey, aged 8–16 years.ResultsCorrelation coefficients between WHtR and %BF were M:r= 0·73, F:r= 0·60,P< 0·01 and WHtR/mid-abdominal skinfold were M:r= 0·78, F:r= 0·65,P< 0·01. WHtR of 0·46(M) and 0·45(F) best identified subjects with ≥85th percentile for %BF and 0·48(M) and 0·47(F) identified subjects with ≥95th percentile for %BF. When comparing the highest WHtR group to the lowest, both sexes had significantly higher means for weight, WC, %BF, TG (male subjects only), systolic BP (female subjects only) and lower means for HDL cholesterol (P< 0·05).ConclusionsWHtR is useful in clinical and population health as it identifies children with higher %BF at greater risk of developing weight-related CVD at an earlier age.

Abstract The aim of this paper was to evaluate the socioeconomic differences in adult anthropometric parameters of young women in Poland. The study was cross-sectional and conducted in the years 2015 to 2018 among 1257 women aged 19–24 years. The heights, weights, wrist widths and waist, hip and chest circumferences of the subjects were measured. Body mass index (BMI), waist-to-hip ratio (WHR), waist-to-height ratio (WHtR), waist-to-chest ratio (WCR) and chest-to-height ratio (CHtR) were calculated. A survey was conducted to collect data on the women’s socioeconomic characteristics. The application of the Generalized Linear Model (GLM) including all socioeconomic indicators (urbanization level of place of residence in childhood, parental education, number of siblings, material conditions) revealed no significant association of these with any of the analysed anthropometric traits. The results of the logistic regression showed no significant differences in the risk of underweight, too low abdominal adiposity or too high abdominal adiposity. However, the risk of overweight and obesity was significantly affected by the degree of urbanization of the women’s place of residence in childhood and by their number of siblings. The results show that the socioeconomic factors that once had a significant influence on anthropometric traits currently do not play such an important role. This change can be explained by the equalization of living conditions and lifestyles of individual social groups in Poland.

Background: Worldwide raising trend in obesity among children is causing serious public health concerns and in developing countries it is threatening the viability of basic health care delivery. The objective of this study was to screen for overweight and obesity among school going children using the established methods as well as the newer screening tools and to compare the efficacy of the various screening toolsMethods: Prospective cross sectional study was conducted various government and non-government schools within city limits. And participants involves 1000 children in the age group of 5 to 15 years who met the predefined criteria were selected by purposive sampling.Results: Using BMI as a screening tool, 6.3% were identified as overweight and 5.8% as obese. Using Waist circumference alone, 5.2% were identified as overweight and 4.5% as obese. 13.8% and 5% were identified as overweight and obese respectively by waist circumference to height ratio. Using augmented BMI, 14.7% were identified as overweight and 4.3% as obese.Conclusions: Using WHTR and Augmented BMI, larger percentage of children were identified as overweight while the percentage of children identified as obese remained similar to using BMI or Waist circumference alone.
